#1 Void stbd center, aft bulkhead in front of ladder, through hole into void #2; crop and renew.

#2 Void port center, behind ladder, clad weld crop and renew.

In Void 05P, docking plug is suspect. Conduct vacuum box test on docking plug to confirm watertightness.

Specific tests and inspections shall be made by the inspector.For marine engineering equipment see subchapter F, for electrical Engineering equipment see subchapter J. Two new 100KW Generators were installed and a 42KW removed, test installation to satisfaction of attending USCG MI, prove over current protection is satisfactory for downstream conductor(s).

In the event of alterations involving the safety of the vessel, the applicable plans shall be submitted for approval covering the proposed work except as modified by § 91.45–1. The general scope of the plans shall be as noted in § 91.55–5. Added FOSS PAM, changed electrical installation, replaced generators not in kind. Obtain approved; barge electrical one-line plan for the modified section, General Arrangement plan, FCP, & Sea Fastening plan that reflect new PAM and generator(s). The PAM installations have been inspected to plans as submitted. PAM is missing fasteners on the aft inboard corner, weld fasteners to corner as per Fastening Plan. New Generator one-line has not been submitted, generator testing prior to ops is covered by separate 835.

Each hatch exposed to the weather must be watertight. Nuts missing/loose on ballast tank hatches. Tighten/replace nuts to ensure hatches remain watertight.

The installation of all systems of an electrical engineering or interior communication nature, together with the details of design, construction, and installation, shall be in accordance with the requirements of subchapter J. Prove proper operation of the general alarm system in the PAM on the aft port side deck. Lockout/tag out PAM, Correct prior to usage.

There shall be at least two means of escape from all general areas accessible where the crew may be quartered or normally employed. Emergency escape for the upper standard feeeder PAM is held in place with a nylon strap. Provide a suitable fastening device that will not easily fail as a turnbuckle/chain or similar to secure.

Crop and renew aprox. 4'x7' section of side shell and side shell longitudinals #3 and #4 in bay #3 in #2 stbd void where inset and damaged prior to next credit drydock.

A survey, either general or partial, according to the circumstances,shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel. Gas free and make safe for workers certified by a Marine Chemist the #2 stbd void. Make #2 stbd void available for inspection by a USCG Marine Inspector.

All electrical engineering details and installations shall be designed and installed in accordance with Sub-chapter J (Electrical Engineering) of this chapter. Provide MSC approved electrical plans to OCMI.

No repairs or alterations affecting the safety of the vessel may be made without OCMI notification, plans are required for all modification not in-kind. Provide USCG MSC approval for new Barge Master PAM (constructed after 2016), Obtain CG# from local OCMI for existing Barge Master PAM (constructed prior to 2016).

All marine engineering details such as piping, valves, fittings, boilers, pressure vessels, etc., shall be in compliance with Sub-chapter F. Install generator set racor fuel filter switch approved type (metal lower bowl or attached with metal flame impingement shield).

Marine Inspector identified deflected frame and gusset in #3 stbd void forward. Submit plan proposal for repairs.

Marine Inspector identified deflected frame, tripping bracket, and side shell on the aft port and stbd corners of the barge. Submit plan proposal for repairs.

Marine Inspector identified washed out welds in #6 STB tank on the bottom shell between the 3rd and 4th transverse frames on the inboard side of the space, and in the #2 STB void on the bottom shell near the bottom of the aft ladder. Submit plan proposal for repairs.

Coamings and hatchway covers to exposed hatchways on decks above the superstructure deck shall comply with the requirements of the assigning authority. 01 Hatch cover was found to be missing a stud, preventing it from being completely secured to the deck and watertight.

Vessels carrying oil as secondary cargo must carry appropriate equipment and supplies for the containment and removal of on-deck oil cargo spills of at least one-half barrel. Containment kit was wasted and water soaked. Replace containment kit equipment and supplies. Rectify w/in 7days. 33 CFR 155.220 (a)

Before issuing a certificate or placement of load line marks on a vessel, the assigning and issuing authorty shall make an intitial or periodic survey of the vessel as requied by this subchapter. ABS shall complete the load line survey and ensure the load line markings are completed.

The port and stbd fore and aft draft marks are no longer painted below the 9ft mark. Ensure all draft marks are appropriately painted in contrasting color to the hull.

The port side draft mark, draft line, and deck line are no longer painted. Ensure all marks and lines are painted in accordance with 46 CFR 42.13-40.

Several void hatch covers have broken studs. Replace as appropriate: Void 2P, Void 3S (aft), Void 4S (aft), and Void 6S (fwd).

8ft draft mark on the bow is being rubbed off and needs to be repainted.

Navigation lights need a flat black backing.

Hatch cover for 7stbd and 6port need to be resealed and all bolts tightened down.

Rails need to be refitted with 3 courses instead of 2. Replace or repair 3 stanchiopns that were taken off for cargo ops.

Replace or repair stbd watertight door forward line locker.

8ft draft mark on the bow is being rubbed off and needs to be repainted.

Navigation lights need a flat black backing.

Hatch cover for 7 stb and 6 port need to be resealed and all bolts tightened down.

Rails need to be refitted with 3 courses instead of 2. Replace or repair 3 stanchions that were taken off for cargo ops.

Replace or repair starboard watertight door forward line locker.

Remove all debris, standing water, and flammbale material from the forward line locker.

Attended vessel to conduct internal structure examination. During the examination the marine inspector noted sixty deficiencies throughout vessel which is contained in detail in vessel/misle narrative. (see for detatails)
