Each ring life buoy on a vessel must... If on a vessel on an oceans or coastwise route, be orange in color. The ring life buoy is sun damaged and is no longer orange. 46 CFR 180.70(b)

Each child size life jacket must be stowed in a location that is appropriately marked and separated from adult life jackets so the child size life jackets are not mistaken for adult life jackets. The child life jackets are not separated from the adult life jackets in the aft life jacket bench. 46 CFR 180.78(a)(5)

All vessels except non-self propelled vessels shall be provided with suitable steering apparatus. The cotter pin on the starboard side of the steering ram is not secured. 46 CFR 182.30-1 (old-t)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure and its appurtenances, including the following: Inspection of all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather decks. The port side wooden hatch frame to the crab hold is rotting. Provide repair proposal and repair prior to next credit dry dock. 46 CFR 176.802

At each inspection for certification and at every other vessel inspection the bilges may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Excessive oil has collected in the lazarette bilge from a previous hose replacement.... Remove oil and properly mount the hoses to the bulkhead to the satisfaction of the MI. 46 CFR 176.830 (b)

Except as otherwise provided in this section, machinery and fuel tank spaces shall be separated from accommodation spaces by watertight and/or vapor tight bulkheads.... Lube oil hose leading from a tank on the starboard side of the pilothouse penetrates the deck, into the engine room, and is not vapor tight. Ensure penetration is vapor tight to the satisfaction of the MI. 46 CFR 182.35-1(a) (Old-T)

Each inlet or discharge pipe that penetrates the hull below a line drawn parallel to and at least 6 inches above the deepest load waterline must have means to prevent water from entering the vessel if the pipe fractures or otherwise fails....2" through hull fitting on the STBD/AFT end of the engine room is no longer in use, and is not capped. Place cap over the fitting to prevent water ingress in the case of a valve failure. 46 CFR 179.350(c)

A seat must be constructed to minimize the possibility of injury and avoid trapping occupants...The aft deck contains three benches for seating that have no means to prevent transverse movement. Install a means to prevent the transverse movement of the benches to the satisfaction of the MI. 46 CFR 177.820(b)

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and an internal structural at least once every two years. The vessel has not completed a drydock examination within the last two years. Conduct satisfactory drydock examination prior to the carriage of passengers. 46 CFR 176.600(c)(1)

A vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. The waterlight was secured to the hand rope instead of the body of the life ring buoy.

For a vessel described by 46 CFR 175.110(c), the training program in paragraph (a) of this section must address firefighting proficiency and must include, but need not be limited to— (1) Training in the use and location of firefighting equipment and general firefighting knowledge, including: (i) Location of firefighting appliances and emergency escape routes; (ii) Types and sources of ignition; (iii) Flammable materials, fire hazards and spread of fire; (iv) The need for constant vigilance; (v) Actions to be taken on board; (vi) Fire and smoke detection and automatic systems on board; and (vii) Classification of fire and applicable extinguishing agents. (2) The drills required by § 185.524, including fire location and fire type; and (3) Emergency egress training for each member of the crew, to occur for all members of the crew— (i) At least monthly while such members are employed on board the vessels; and (ii) Each time a crew member joins the crew of such vessel. Vessel is certified for oceans route. Operator has not been conducting quarterly training or monthly egress training despite being informed on the requirements during the last inspection. Vessel to conduct and log training to the satisfaction of the attending marine inspector.

Every vessel subject to Part III of Title III of the Communications Act must have a detailed inspection of the radio installation by an FCC-licensed technician in accordance with § 80.59 once every five years. The Safety Radiotelephony Certificate was expired. 47 CFR 80.903

Unless other means are provided to ensure that a potential waste receptacle fire would be limited to the receptacle, waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Vessel has plastic trash can on board. Vessel to remove and replace trash can with appropriate material to the satisfaction of the attending marine inspector.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date U.S. Coast Pilot or similar publication. The Coast Pilot on board was not up-to-date.

All cable and wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. The middle support on the stbd side of the generator fuel tank in the LAZ was broken causing the cable to lay on the floor.

An inspection for renewal of a Certificate of Inspection is conducted to determine if the vessel is in satisfactory condition, fit for the service intended, and complies with all applicable regulations. It normally includes inspection and testing of the structure, machinery, equipment, and on a sailing vessel, rigging and sails. The transverse deck beam above the generator fuel tank in the LAZ was corroding. Fix at next credit dry dock.

It normally includes inspection and testing of the structure, machinery, equipment, and testing of the structure, machinery, equipment, and on a sailing vessel, rigging and sails. The fwd crew accommodation area on the hull stbd side right below lower bunk was delaminated. Fix at next credit dry dock.

For fixed-gas fire extinguishing systems, the inspections and tests required by Table 176.810(b) of this section, in addition to the tests required by 46 CFR 147.60 and 147.65. The owner or managing operator must provide satisfactory evidence of the required servicing to the marine inspector. Service record for fireboy must be provided.

Each vessel that operates on the high seas, or that operates beyond three miles from the coastline of the Great Lakes, must have on board a FCC Type Accepted Category 1, 406 MHz EPIRB, installed to automatically float free and activate. Call sign on EPIRB NOAA registration is not the current callsign for the vessel. Update callsign on NOAA registration.

Alcohol-testing devices must be used according to the procedures specified by the manufacturer of the testing device and by this part. Alcohol test strips are expired and must be replaced.

Each ring life buoy on a vessel must: Be approved in accordance with § 160.050 in subchapter Q of this chapter. Grab line. The grab line shall be 3/8-inch diameter polyethylene, polypropylene, or other suitable buoyant type synthetic material having a minimum breaking strength of 1,350 pounds. Lifebuoy was not of approved type. Replace w/ approved lifebuoy.

Portable fire extinguishers must be located so that they are clearly visible and readily accessible from the space being protected. The installation and location must be to the satisfaction of the Officer in Charge, Marine Inspection. Bracket must be provided for fire extinguisher in the operating station.

During the examination all accessible parts of the vessel's underwater body and all through hull fittings, including the...rudders...shall be made available for examination. Seal on starboard rudder post must be replaced at next credit drydock.

Vessels must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). Vessel VHF radiotelephone did not have DSC capability. 47 CFR 80.905(a)(1)

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Vessel had frayed cable for bridge and outlet dead end wire in engine room overhead forward.

Provision must be made for closing all supply duct cowls or scoops and exhaust duct discharge openings for a space protected by a fixed gas extinguishing system. All closure devices must be readily available and mounted in the vicinity of the vent. Vessel is missing 3 vent closures for engine room.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment. Lazarette bilge pump leaking water at overboard discharge connection.

Abandon ship and man overboard drills and training shall be logged or otherwise documented for review by the Coast Guard upon request. Master and crew were not logging drills.

The EPIRB test required by paragraph (a) shall be logged or otherwise documented, as applicable. Master or crew was not logging tests for EPIRB.

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. COD was not on board the vessel. 46 CFR 67.313

Pre-engineered fixed gas fire extinguishing systems must automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. Pre-engineered fixed gas fire extinguishing systems did not shut down generator engine. 46 CFR 181.420(a)(3)

A submersible electric bilge pump must have positive shutoff valve is installed at the hull penetration. Shutoff valve are not installed on the port and starboard center holds. 46 CFR 182.520(e)(7)

Machinery spaces must be separated from accommodation spaces by boundaries that prevent the passage of vapors. Two holes are located in the deck allowing for the passage of vapors from the engine room to the cabin. Make vapor tight.

The Coast Guard will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners. Master [name withheld], was unable to provide a copy of a valid medical certificate. Provide proof of a valid certificate to marine inspector.

A vessel must carry adequate and up-to-date charts of large enough scale to make safe navigation possible. Chart was from 2012 and had no corrections. Replace chart.

Each survival craft must be secured to the vessel by a painter with a float-free link permanently attached to the vessel. and stowed so that when the vessel sinks the survival craft floats free. The float free link was not installed properly and prevented proper operation. Vessel operator corrected the deficiency on the spot.
