Scantlings, material, and workmanship, subdivision of cargo spaces, fitting of cofferdams, and testing of tanks shall be at least equivalent to the requirements of the American Bureau of Shipping or other recognized classification society. Vessel has incurred damage on both, the port and starboard bow head-log close to the corners; thus, raising the deck and affecting several internal structural members and plating: inside the bow void 5 head-log chords are web crushed and bended and 2 welds between deck longs and vertical stiffeners are cracked, top deck plating on starboard bow side is sharply bended (left side is a little lesser), etc. Vessel has been authorized 45 days to conduct permanent repairs. 1/8" x 2" fracture #1 cargo tank port stiffener penetrating the tank

1" hairline fracture to headlong bottom lap seam

Gaskets on #6 STBD wing void, Port stern and P/S bow weather deck hatches did not seal.

P/V valves Identification labels were painted over and unreadable.

Exhaust pipe on prime mover is open and easily accessible and is located near cargo transfer valve.

Barge has numerous fractures on stbd bow that have been welded and tested at 1.5 psi sat. Once offloaded required to crop and renew area approx 12' x 3' to satisfaction of attending CG marine inspector.

Stern void has an approximately 2" crack in the aft port bilge corner. During damamge survey inspector also noted broken dog on #3 starboard void hatch and a fractured hatch on the starboard side of the stern void.

Starboard side on barge damaged by collision. Damage runs from approx middle of #3 starboard void all the aft to the stern box. Gradual set-in startys aqt the #3 void approx 2" deep and becomes approx 12" deep at the stern box. CG-835 issued for vessel to complete permanent repairs prior to next cargo ops. Provide repair proposal and make permanent repairs to the starboard stern corner (15" x 2") to the cognizant OCMI prior to reloading cargo.

18Aug04: Vessel rep noted damage in way of the #1 stbd wing void tank knuckle and effected repairs.

Make permanent repairs to fracture (starboard stern transom extending from deck down approx 18 inches) prior to loading next cargo, but in no case later than 09Mar97.inspector.

PRIOR TO NEXT DISCHARGE OF CARGO MAKE NECESSARY REPAIRS TO CARGO PUMP TO ENSURE NO LEAKAGE OF PRODUCT, TO THE SATISFACTION OF A USCG MARINE INSPECTOR.
