A propulsion engine control system, including pilothouse control, must be designed so that a loss of power to the control system does not result in an increase in shaft speed or propeller pitch. New electronic propulsion control systems were installed during Sep-Oct 2025 Drydock. Newly installed electronic propulsion control systems shall be tested according to MSC accepted and reviewed testing protocol to be witnessed by USCG marine inspectors.

Alterations to equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Repairs or alterations that affect the safety of the vessel include, but are not limited to: propulsion and power supply systems. The owner or managing operator shall submit drawings, sketches, or written specifications describing the details of any proposed alterations. The OCMI may require an inspection and testing whenever a repair or alteration is undertaken. A re-power of the vessel was performed during their Sep-Oct 2025 Drydock. The drawing for “Seawater Cooling Modifications” were submitted to MSC and approved under MSC letter E1-2502749, dated 08/22/25. An onboard verification/inspection of drawing: EBDG Dwg. No. 24099-256-0, Rev. A, was conducted by Sector Puget Sound. SPS found the new installation did not correspond to the approved plan. Multiple discrepancies were identified including bill of materials and clean out deletes were found between the MSC approved plan and the actual onboard installation. Make revisions to plans and/or onboard installations so that the two correspond. Any revised plans shall be submitted to and approved by MSC as follow-up to letter E1-2502749. Follow-on verification by local OCMI may be required once revised approved plans are received.

Make revisions to plans and/or onboard installations so that the two correspond. Any revised plans shall be submitted to and approved by MSC as follow-up to letter E1-2502749. Follow-on verification by local OCMI may be required.

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. After conducting ultrasonic testing, significant corrosion and pinholes were identified in the tank top and structural members of the sewage tank. The tank top serves as the passenger deck in the lounge area. Due to operational limitations, SPS authorized a temporary repair. The vessel is required to permanently repair, to local OCMI satisfaction, the tank top and structural members of the sewage tank no later than 30 September 2026. No further extensions will be permitted.

...DOORS OR PASSAGEWAYS USED SOLELY BY CREW MEMBERS MUST HAVE A CLEAR OPENING NOT LESS THAN 28 INCHES...VESSEL HAS TABLES AND CHAIRS IN THE PASSAGEWAY ON THE MAIN DECK FROM THE CREW ACCOMMODATIONS TO THE DINING AREA, NARROWING THE PASSAGEWAY TO 23 INCHES IN WIDTH. VESSEL TO ALTER TABLE CONFIGURATION TO EXPAND WIDTH OF PASSAGEWAY TO 28 INCHES. 46CFR177.500(f)

ELECTRICAL EQUIPMENT ON A VESSEL MUST BE INSTALLED AND MAINTAINED TO...PROTECT PASSENGERS, CREW, OTHER PERSONS, AND THE VESSEL FROM ELECTRICAL HAZARDS, INCLUDING FIRE, CAUSED BY OR ORIGINATING IN ELECTRICAL EQUIPMENT, AND ELECTRICAL SHOCK. VSL WAS UTILIZING MULTIPLE "DAISY CHAINED" EXTENSION CORDS NEAR CREW BERTHING. IN ADDITION, MULTIPLE DEAD END WIRES WERE FOUND IN THE ENGINE ROOM. VSL TO REMOVE ANY EXCESS DEAD END WIRES AND DISCONNECT ANY DAISY CHAINED EXTENSION CORDS/POWER STRIPS. 46CFR183.200(b)

ALL CABLE AND WIRE MUST...BE INSTALLED WITH METAL SUPPORTS SPACED NOT MORE THAN 24 INCHES APART. MULTIPLE POWER DISTRIBUTION CABLES COMING FROM EACH GENERATOR ARE SUPPORTED BY ONLY PLASTIC ZIP TIES FOR THE FIRST 5'. VESSEL TO ADEQUATELY SUPPORT CABLES WITH APPROPRIATELY SPACED METAL SUPPORTS. 46CFR183.340(b)

Make revisions to plans and/or onboard installations so that the two correspond. Any revised plans shall be submitted to and approved by MSC as follow-up to letter E2- 2303615 . Follow- on verification by local OCMI may be required once revised approved plans are received .

The minimum clear opening of a door or passageway used as a means of escape must not be less than 810 millimeters (32 inches) in width, however, doors or passageways used solely by crew members must have a clear opening not less than 710 millimeters (28 inches). Passenger and crew cabin doors measure less than the minimum requirement for means of escape. When more than six passengers are carried, passengers are prohibited access to overnight accommodation spaces.

Vessels described by 46 CFR 114.110(e) must have an interconnected fire detection system in compliance with 46 CFR 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. Vessel's existing fire detection system does not produce audible alarms within passenger accommodation spaces. When more than six passengers are carried, the vessel may not operate on a coastwise or oceans route and passengers are prohibited access to overnight accommodation spaces.

The fire detection and alarm system must be capable of immediate operation at all times that the vessel is in service. Newly installed fire detection system indicated a panel fault with incomplete wiring. Vessel to provide technician servicing report.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. The starboard main engine experienced knocking and heavy black exhaust smoke while under load. Vessel rep to submit repair plan proposal to [name withheld] repairs are made, attending marine inspector must witness operational tests to ensure engine operates as designed. Vessel must secure the starboard engine while underway enroute to disembark passengers at Kailua Pier. After passengers are disembarked, vessel is not authorized to operate underway with passengers until deficiency is rectified and cleared by attending marine inspector.

Passageways used solely by crewmembers are required to have clear opening not less than 28 inches. Passageway at entrance of galley space to access hatch to outer deck is less than 28 inches.

Each ladder used as a means of escape must be mounted at least 7 inches from the nearest permanent object. The escape ladder from the crew accommodation space is mounted less than 7 inches from the bulkhead.

A generator designed for 40 C (104 F) may be used provide the generator is derated to 80% of full load rating and overcurrent devices reduced accordingly. Overcurrent devices were not reduced accordingly.

Each bulkhead penetration by nonmetallic piping must be accomplished using metallic bulkhead fitting with one or more metallic shutoff valves. Hose penetration on port side of engineroom forward bulkhead did not have acceptable fittings or shutoff valves.

All escape hatches and other emergency exits used as means of escape must be marked on both sides. Escape hatch exit in salon not properly marked. Ensure proper marking for escape hatch.

Portable and semi portable extinguishers must be inspected and maintained IAW NFPA 10 (Incorporated by reference). Found 03 portable fire extinguishers onboard with excessive corrosion. Replace extinguishers 3, 3A and 5.

If more than one ring life buoy is carried, at least one must not have a lifeline attached. All ring life buoys onboard had lifeline attached. Ensure that at leaset one ring buoy does not have a lifeline attached.

Vessel with a load line is required to have a valid load line certificate. Vessel does not have valid load line certificate. Ensure load line is painted over prior to getting underway and removed at next drydock.

The number of penetrations in a watertight bulkhead must be minimized and made watertight. Cable penetrations were modified. Seal all cable penetrations to ensure watertightness.

Prove proper operation of CO2 shutdown. When demonstrated failed to operate as designed.

Repair all fuel oil and lube oil leaks on #3 SSDG. Oil saturated engines turbo and fuel oil leaking from filter bank. Rectify prior to operation of #3 SSDG.

Replace flexible hose in discharge piping on #2 fire pump.

Provide CO2 placard next to audible alarm.

Replace flexible hose fitted in HVAC cooling line, #2 ME cooling water discharge line, and add a hose clamp to #1 ME cooling line.

Repair salt water flushing line for forward blackwater tank. Line had a hole in it.

Provide general alarm placard in engine room.

Make all electrical enclosures in engine room and steering compartment splash proof. Enclosures were found missing stuffing tubes, open, and missing knockouts.

Repair leaking type 2 MSD located in engine room.

Provide insulation for EDG turbocharger.

Provide ventilation for tank void space at next drydock.

Vessel did not have crew onboard at time of inspection. Marine Inspector must witness drills at a later date.

Gasoline canisters are stored in an open rack on the upper passenger deck creating a hazardous area accessible to passengers. Remove all gasoline canisters from passenger deck until suitable storage is provided.

The number and identification of the items stowed inside, and their sizes must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits. Life jacket stowage not properly labeled. Ensure proper labelling for life jacket stowage.

Placards containing instructions for donning and use of life jackets aboard the vessel must be posted in conspicuous places regularly accessible to crew and passengers. Placards not posted in conspicuous places. Ensure placards are posted in conspicuous places and readily accessible to crew and passengers.

A vessel of more than 65ft in length must carry at least one rescue boat; Vessel is required by COI to have (02) rescue boats onboard. Only one boat is operational due to other boat not having pontoon. Repair other rescue boat.

Vessel completed alterations to grey water and sewage piping systems, to include modifications of tanks and electrical systems, without receiving OCMI approval. Title 46 CFR 176.700(a) requires that alterations to the machinery or equipment that affect the safety of the vessel to be made w/ OCMI approval. You must submit plans to the USCG for approval & make vessel available for inspection.

Remove or properly secure loose items in CO2 space, which include crutches, e-board, vacuum, etc. and train crew on new location.

Remove and properly store oil spill response equipment in location other than emergency generator room and train crew on new location.

Floating light was barely able to maintain floatation while thrown overboard during drill. Discovered unapproved battery spec number. Approved replacement battery required with float test.
