Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Upon first visit in June, 2023, the fuel return line on the ship service generator was observed to be severely damaged as a result of making contact with the positive battery post on the generator starting battery. Generator is currently isolated. Prior to using the generator, install new, properly rated fuel hoses and complete operational test of generator. 46 CFR 182.100 & 46 CFR 182.720.

The engine head, block, and exhaust manifold must be water-jacketed and cooled by water from a pump that operates whenever the engine is operating. Port main Engine leaking coolant while operating, in vicinity of alternator. Repair to satisfaciton of attending USCG marine inspector. 46CFR182.420.

The engine head, block, and exhaust manifold must be water-jacketed and cooled by water from a pump that operates whenever the engine is operating. Starboard main engine raw water intake pipe in civinity of heat exchanger leaking excessively. Repair to satisfaction of attending USCG marine inspector. 46CFR182.420.

A vessel must be provided with a satisfactory arrangement for draining any watertight compartment. Center void electric bilge discharge hose is leaking at the mating surface between the two flexible hoses. Repair hose to satisfaction of attending USCG marine inspector.

A submersible electric bilge pump may be used as a power bilge pump provided that... A positive shutoff valve is installed at the hull penetration. Center void electric bilge discharge thru-hull shutoff valve not installed at the hull penetration. Relocate shutoff valve to hull penetration to satisfaction of attending USCG marine inspector.

A submersible electric bilge pump may be used as a power bilge pump provided that... A positive shutoff valve is installed at the hull penetration. Engine room, engine driven bilge starboard discharge thru-hull fitting leaking around the thru hull. Water from outside the vessel seeping in around the body of thru-hull. Remove thru-hull fitting for USCG inspection of surrounding fiberglass material prior to reinstalling with appropriate gasket/sealant.

A vessel must be provided with a main steering gear that is: Of adequate strength and capable of steering the vessel at all service speed. In steering gear room, cotter pins are missing from rudder tie rod bolts. Install cotter pins in attaching tie rod bolts.

Each watertight bulkhead must be of sufficient strength to be capable of remaining watertight with a head of water to the top of the bulkhead. In steering gear space, starboard forward bulkhead in vicinity of water cooled exhaust pipe has water intrusion leaking in from the topside weather deck and the wood is rotting and decayed. Provide repair proposal for renewing rotten/damaged bulkhead to satisfaction of attending USCG marine inspector.

Water for cooling the exhaust pipe, other than a vertical exhaust, must be injected into the exhaust system as near to the engine manifold as practicable. The water must pass through the entire length of the exhaust pipe. In steering gear room, starboard wet exhaust pipe at aft flexible coupling leaking water around hose clamp. Resecure and prove proper operation with out leak to satisfaction of attending USCG marine inspector.

A vessel with installed toilet facilities must have a marine sanitation device that complies with 33 CFR part 159. In the center void, the sewage pipes leading from the toilet towards the holding tank have a y-fitting leading straight to overboard discharge. Overboard discharge valves not locked in the closed position while inside of 3nm. Provide locking mechanism for both discharge valves, and prove proper certification and functionality of holding tank.

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspections of the hull structure including the following: Inspection of all interior spaces to ensure that they are adequately ventilated and drained. Clean engine room bilge of oil and debris prior to completing functional test of fixed bilge discharge system.

Remote fuel shutoff stations must be marked in clearly legible letters at least 1" high indicating purpose of the valve and direction of operation. Remote fuel shutoff stations for the MDEs and generator on the back deck were either illegible or did not clearly specify the purpose of the valve and direction of operation. Re-label fuel shutoff stations.

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and intended use. Fuel shutoff valve handle for the port main diesel engine is corroded and worn. Replace fuel shutoff valve handle.

Each connection to a conductor or terminal part of a conductor must be made within an enclosure and have either: (1) A pressure type connector on each conductor; (2) A solder lug on each conductor; (3) A splice made with a pressure type connector to a flexible lead or conductor; or (4) A splice that is soldered, brazed, or welded to a flexible lead or conductor. The wire splice connection for the bilge switch in the aft steering gear room was compromised. Renew connection.

Electrical equipment on a vessel must be installed and maintained to: (a) Provide services necessary for safety under normal and emergency conditions; (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; (c) Minimize accidental personnel contact with energized parts; and (d) Prevent electrical ignition of flammable vapors. There are exposed dead end wires on the starboard main diesel engine, inboard side and above the starboard main diesel engine. Properly terminate or remove all exposed dead end wiring prior to renewing COI.

All cable and wire must be installed with metal supports spaced not more than 24" apart and in such a manner as to avoid chafing and other damage. Various wires were observed in the bilge, under the starboard main engine. In addition, the wires for the belt-driven bilge pump were not properly secured. Properly secure wires.

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Pooled oil was observed on the port and starboard main engines at the front-end seals. Conduct underway sea-trial to determine the severity of the compromised seal.

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Insulation above the starboard main engine was falling down and resting on the engine. Restore insulation.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Starboard engine raw water intake hose chafing gear is degraded. Restore chafing gear.

Electrical installations must be suitable for the roll, pitch and vibration of the vessel underway. Alarm for the starboard engine is not properly mounted and is able to move freely. Properly mount alarm.

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Bolt for the starboard engine IVO the heat exchanger was missing. Replace bolt.

An inflatable liferaft or inflatable buoyant apparatus must be serviced at a facility specifically approved by the Commandant for the particular brand, and in accordance with servicing procedures meeting the requirements of part 160, subpart 160.151. Servicing paperwork could not be provided for the raft onboard. Valid sticker was present on the raft. Provide servicing paperwork.

Machinery and equipment installed on each vessel must be suitable for the vessel and its operation and for the purpose intended. Alternator for the port main engine was found to have a cracked lobe. Complete satisfactory repairs or replace the alternator prior to carrying passengers.

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Steering gear room overhead hydraulic line wooden support blocks above rudder stocks are broken. Renew block to provide proper support for hydraulic lines.

A vessel must be provided with a main steering gear that is: Of adequate strength and capable of steering the vessel at all service speeds. The port rudder stock was found loose and resting on the emergency stop bolt. Port/starboard packing glands found loose, not tightened. Retorque port and starboard rudder packing glands, and port/starboard rudder retainer to manufacturer specifications.

A vessel must be provided with a main steering gear that is: Of adequate strength and capable of steering the vessel at all service speeds. The rudder tie rod between port and starboard rudders appears severely corroded. Make available for inspection by removing tie rod and fitting attachments at each end for hardware replacement and to determine suitability of continued service of tie rod.

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Port side fuel tank 2x4 wood framing is broken in vicinity of fuel shutoff valve handle. Repair/replace broken 2x4.

A vessel is inspected for compliance with the standards required by this subchapter. Machinery, equipment, materials, and arrangements not covered by standards in this subchapter may be inspected in accordance with standards acceptable to the cognizant OCMI as good marine practice. Port side engine room outboard hydraulic line overhead wooden support block is broken. Renew block to provide proper support for hydraulic lines.

Electrical equipment on a vessel must be installed and maintained to: Minimize accidental personnel contact with energized parts. Remove dead end wires on port side hydraulic solenoid valve outboard of battery bank in engine room.

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible; and Current tables. A cellphone and iPad were being used as electronic charting means but were not installed onboard the vessel. No paper chart was provided. Current tables were not onboard for review. Provide acceptable means of charting and provide up-to-date current tables for the vessel's area of operation.

Each ship transporting more than six passengers for hire equipped with a radiotelephone station IAW Part III of Title III of the Communications Act must carry a radio operator...Master did not have a marine radio operator permit or other appropriate license. Provide proof of license. 47 CFR 80.159(e)

The master and crew of a vessel will be familiar with the content of and have mounted at the operating station, emergency instructions containing the actions to be taken in the event of fire, heavy weather, or man overboard conditions. Emergency instructions were missing. Post e-instructions at the operating station.

The stability letter must be posted under glass or other suitable transparent material, such that all pages are visible, at the operating station. Stability letter was posted in the passenger accomodation space and not at the operating station. Post stability letter.

Waste receptacles must be constructed of noncombustible materials with no openings in the sides or bottom. Combustible waste receptacle was being used in the passenger accommodation space. A second combustible waste receptacle was being used on the fantail. Replace trash can with one that is noncombustible or provide documentation showing the waste bin is noncombustible.

The manufacturer's instructions for onboard maintenance of survival craft must be readily available for a vessel of not more than 19.8 meters (65 feet). The inspection and maintenance of the equipment shall be logged or otherwise documented for review by the Coast Guard upon request. Each EPIRB must be tested monthly and shall be logged or otherwise documented. Lifesaving equipment maintenance log was not available for review. Monthly EPIRB tests were not logged. Provide logbook. 46 CFR 185.702 & 46 CFR 185.728

An identification code, recognized by NOAA must be programmed in each EPIRB unit to establish a unique identification for each EPIRB station. NOAA registration sticker was missing. Place NOAA sticker on EPIRB. 47 CFR 80.1061(e)

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. Aspirin, wire splint and ammonia inhalants were missing. Provide new aspirin, wire splint & ammonia inhalants.

Guards for exposed hazards. An exposed hazard, such as gears or rotating machinery, must be properly protected by a cover, guard, or rail. Both main diesel engines and the ships service generator are missing the belt guards. The generator is also missing the generator winding guard. Properly install all missing machinery guards. 46 CFR 177.960

Electrical equipment on a vessel must be installed and maintained to: (a) Provide services necessary for safety under normal and emergency conditions; (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; (c) Minimize accidental personnel contact with energized parts; and (d) Prevent electrical ignition of flammable vapors. All batteries in the engine room are missing covers and or terminal covers. Terminal connections on the port outboard batteries terminal connectors are secured with deck screws. Properly provide covers and or terminal covers for all exposed battery terminals and ungrounded connections throughout the engine room prior to carrying passengers. 46 CFR 183.200

The USCG will issue a medical certificate to a mariner meeting the medical and physical standards for merchant mariners, will be issued for a maximum period of 5 years. Master [name withheld] could not provide a medical certificate during the inspection. Master of vessel is to provide proof of medical certificate to attending marine inspector.

The port main diesel engine forward, outboard, engine mount is loose and not properly torqued to manufacturers specs. Properly repair prior to renewing COI. 46 CFR 182.100

Electrical equipment on a vessel must be installed and maintained to: (a) Provide services necessary for safety under normal and emergency conditions; (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; (c) Minimize accidental personnel contact with energized parts; and (d) Prevent electrical ignition of flammable vapors. There are exposed dead end wires on the port main diesel engine, outboard side and on the aft engine room bulkhead port side battery switch. Properly terminate all exposed dead end wiring prior to renewing COI. 46 CFR 183.200

In systems and applications where flexible hoses are permitted to be clamped: Two clamps must be used on each end of the hose, or one hose clamp can be used if the pipe ends are expanded or beaded to provide a positive stop against hose slippage. The port main diesel engine raw water cooling hoses that are slipped onto pipe from the sea strainer are not double clamped. Properly double clamp all hoses that are not connected to hose barbed fitting prior to renewing COI.46 CFR 182.410

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. The fuel return line on the ship service generator is severely damaged as a result of making contact with the positive battery post on the generator starting battery. Properly renew the damaged fuel hose with a proper fuel hose. 46 CFR 182.100 & 46 CFR 182.720.

A submersible electric bilge pump may be used as a power bilge pump... and a positive shutoff valve is installed at the hull penetration. The positive shutoff valve has flexible hose between the thru hull fitting and the valve. Install shutoff valve at the thru hull fitting.

All cable and wire must be installed with metal supports spaced not more than 610 millimeters (24 inches) apart, and in such a manner as to avoid chafing and other damage. Battery cable and wire runs in engine room need metal supports, currently all zip ties.

Starting motors, generators, and any spark producing device must be mounted as high above the bilges as practicable. The port engine alternator mounting bolt is stripped and will not torque, currently using wood block wedge to hold tension on alternator installation. Repair alternator mount to secure alternator in place using originally designed installation, without use of wood block wedge. 46CFR182.410.

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. Starboard engine oil sump drain requires metallic threaded cap on drain line to prevent engine sump from draining oil.

Scantlings, material, fastenings, workmanship and arrangements of each vessel are such as to provide for safe operation in accordance with the terms of its certificate of inspection. Attending marine inspector observed substantial damage to the hull and railings as well as numerous unapproved temporary repairs made to the hull. Provide repair proposals for all damaged portions of the hull and for all improperly made repairs for approval prior to starting each repair. 46CFR177.01-5 (Old T)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. When repairs are made during an emergency, the owner, managing operator, or master shall notify the OCMI as soon as practicable after such repairs or alternations are made. Owner/Operator has shown continued noncompliance with submitting repair proposals to CG and failing to notify after repairs had been made during an emergency. Make appropriate callouts during repair process as required by 46 CFR 176.700 to OCMI via phone and/or email.

A vessel on which a pre-engineered fixed gas fire extinguishing system is installed must have the following equipment at the operating station: 1. A light to indicate discharge 2. An audible alarm that sounds upon discharge and 3. a means to reset devices used to automatically shut down ventilation systems and engines as required by paragraph (a)(3) of this section. Pre-engineered system for generator was tested. Generator shut down but no light or audible alarm was given at the operating station. The override feature did not operate as intended; generator tried to turn on but would shutt off. Rig pre-engineered system to meet 46 CFR 181.420.

Provision must be made for closing all supply duct cowls or scoops...for a space protected by a fixed gas extinguishing system. All closure devices must be mounted in the vicinity of the vent. Starboard side engine room air supply vent is being secured with carpets that do not fully cover the vent. The carpets being used are not secured at the bottom and are only being held by eye bolts. Two additional holes have no means of securing. Provide suitable fire damper for stbd vent.

A hatch exposed to the weather must be watertight. A hatch cover must have securing devices. Hatch cover for steering space had a gasket that was gouged. Securing devices were not in place at the time of inspection. Renew gasket and complete hose test to prove watertightness.

Fuel supply and return hoses to both port and starboard main engines are not fire rated. Provide proper fire sleeves with metallic end fastenings for each fuel hose that is not fire rated (bright blue cotton jacketed, FC-300).

A vessel that is exposed to salt water more than three months in any 12 month period since the last examination must undergo a drydock examination and internal structural at least once every two years. Vessel has two outstanding deficiencies from Sector Puget Sound (Dtd. 12APR2022) to provide plan to local OCMI prior to commencing work on the port quarter side shell delamination and port side amidships temp repair. Vessel repairs have not been completed preventing a credit drydock from being issued.

VESSEL HAS 13 OUTSTANDING DEFICIENCIES, 8 OF WHICH ARE PRIOR TO CARRIAGE OF PASSENGERS. VESSEL OPERATOR HAS NOT MADE VESSEL AVAILABLE TO CLEAR OUTSTANDING DEFICIENCIES AND CONTINUES TO OPERATE. VESSEL IS DETAINED UNTIL "PRIOR TO CARRIAGE OF PASSENGERS" DEFICIENCIES HAS BEEN CLEARED BY THE ATTENDING MARINE INSPECTOR.

STBD quarter IWO the transom to side shell joint there is impact damage effecting all layers of glass. A probe can be passed completely through the hull. Complete proper permanent repairs in accordance with NVIC 8-87 or other accepted standard. Prior to carrying passengers/prior to receiving credit for drydocking. Provide repair plan to local OCMI prior to commencing work.

Port quarter side shell delaminating in way of the transom to side shell joint. Complete proper permanent repairs in accordance with NVIC 8-87 or other accepted standard. Prior to carrying passengers/prior to receiving credit for drydocking. Provide repair plan to local OCMI prior to commencing work.

Port side, amidships temp repair requires proper permanent correction. Complete proper permanent repairs in accordance with NVIC 8-87 or other accepted standard. Prior to carrying passengers/prior to receiving credit for drydocking. Provide repair plan to local OCMI prior to commencing work.

Aft hatch to steering space gasket gouged and will not seal. Renew gasket.

No deckhands were available at time of inspection and no drills were conducted to determine operational readiness of vessel or crew. Conduct satisfactory man overboard, abandon ship, and firefighting drills in presence of marine inspector prior to carriage of passengers. 46 CFR 176.808(a), 701c

A vessel must carry adequate and up-to-date…Charts of large enough scale to make safe navigation possible. Vessel had no electronic charting system or corrected paper charts on board. Obtain approved electronic charting system suitable for oceans route or appropriate paper charts for route and areas of operation. 46 CFR 184.420(a)(1), 701c

A pre-engineered fixed gas fire extinguishing system must…Automatically shut down all engines that draw intake air from within the protected space. Installed Fireboy system does not shut down vessel’s generator installed in protected space, which draws air from within the space. Rig pre-engineered fixed gas system to shut down generator on activation, or relocate generator or generator air supply. 46 CFR 181.420(a)(3), 701c

Provision must be made for closing all supply duct cowls or scoops…for a space protected by a fixed gas extinguishing system. Starboard side engine room air supply vent has no provision for securing air supply to engine room. Provide suitable fire damper for starboard side vent. 46 CFR 182.465(h), 701c

Suitable covers, guards, or rails shall be installed in way of all exposed and hazardous places such as gears, machinery, etc. Port engine had only orange plastic temporary fencing installed to prevent human contact with rotating machinery. Install permanent suitable guard or cover capable of preventing human contact with rotating machinery. 46 CFR 177.35-15(a) (“Old T”), 50c

A submersible electric bilge pump may be used…provided that…a positive shutoff valve is installed at the hull penetration. Ball valve of forward bilge line was installed several feet inboard of hull penetration. Install positive shutoff valve at the through-hull penetration. 46 CFR 182.520(e)(7), 50c

Fwd peak overhead sheathing and deck winch framing has been degraded due to water intrusion. Prepare and submit a repair proposal to include time frame for repair to the [name withheld] winch shall not be utilized until repairs have been completed.

Ring Buoy floating light was not in place as required. Provide floating light prior to conducting nighttime operations.

The anchor & fwd masthead lights were not operational. Renew lights prior to conducting night time operations.

Port main engine guard not in place as required. Install guard as required.

Re-label fuel oil shut off valves with name and direction of closure.

Provided updated charts and nav pubs to include light list and coast pilot.

Provide proof of EPIRB NOAA registration to attending Marine Inspector.

Provide evidence of satisfactory servicing of fixed firefighting system by qualified servicing individual/organization to the satisfaction of the attending marine inspector. - 46 CFR 176.810(b)(2)

Provide evidence of satisfactory servicing of portable fire extinguishers by qualified servicing individual/organization to the satisfaction of the attending marine inspector. - 46 CFR 176.810(b)(1)

Attach fire hose to hydrant and provide for storage in a way that does not cause a hazard to passengers/crew. Vessel temporarily reduced to 49 passengers until rectified. - 46 CFR 181.15-10(g)

Install fire protective sleeve on exposed section of port engine fuel supply line. - 46 CFR 182.720(e)

Reinstall guard on port engine alternator. - 46 CFR 176.830

Electrical cable for midship light in engine room requires busihing to prevent chafing. Install bushing. - 46 CFR 176.806

Replace Life ring buoy

Replace Life Ring Light battery with the correct one as listed on the case and correctly attach to ring bouy.

Repair steering cross bar arm on port side where it meets the port steering rudder arm

Remove electric extension cord that is being used to jump the port engine starter solenoid.

Remove extend cord running from the galley to bridge

Install guards on all rotating machinery in the engine room.

Install covers on all lights in the engine room

Remove dead end wiring in Pilot house

Conduct underway man over board, fire and abandon ship drills with the attending CG Inspector to include run of the fire pump by 15 June 2019

Remount two water pumps in center void next to water tank to prevent movement

Provide update FCC Radio Safety certificate with correct call sign.

Remove all bungee cords and line from both main engine shutdowns

Install valves on all electric Bilge pumps piping at the hull during next dry docking

Provide written repair plan for the fwd deck repair in the area around the fwd pilot house and anchor windless to the Local OCMI for approval during next dry dock.
