Every tank barge shall have its name or number carved, punch-marked, or welded on the main beam, inside the cargo hatch, or other suitable permanent part of the vessel's structure for the purpose of identification. The vessel's name or number shall be so displayed at the highest part of the vessel's hull or permanent structure that the name or number can be seen from either side. Tank Barge SMI 4 did not have name of barge on the stern. Tank Barge SMI 4 shall have the name permanently affixed to the stern of the barge within 7 days. Also, Barge rep. shall provide satisfactory objective evidence of the Official Number permanently welded inside the structure of the barge for the purpose of identification.

Clean and gas free barge to make permanent repairs to fracture on the bottom corner of the #1 cargo tank located on the 8th floor frame forward of the aft bulkhead prior to any cargo operations. (800 gallons of diesel spilled into the #1 wings as a result of the fracture.)

COI not onboard during inspection. CG858 issued for COI annual inpsection.

Cargo Pump Relief Valve test record expired. Valve needs to be tested and records maintained onboard the vessel.

Cargo piping hydrostatic test record expired. Piping needs to be tested and records maintained onboard the vessel.

Provide satisfactory documentation to [name withheld]/Galveston that this vessel has operated in fresh water service at least 6 of the last 12 months. If the vessel has not been operated in fresh water service at least 6 of the last 12 months or satisfactory documentation cannot be provided, the vessel will be required to be inspected IAW the salt water intervals prescribed in 46 CFR 31.10-21(a)(1)

Conduct Vapor Tightness Test prior to loading. 33 CFR 156.170

Certificate of Inspection was expired.

Make repairs to the two holes on port side approximately between 10 to 15 feet back from bow, and between 3' and 6' draft. The holes are approximately 12" - 18" in size with some small cracks in areas around damage. To the satisfaction of the attend CG Marine Inspector.

Finish painting draft marks.

Replace or renew gaskets on #1 port side and #3 stbd side.

Replace or renew cargo stick level indicating gauge aft.

Provide updated copy of transfer procedures in accordance with 33 CFR 155.1045.

Discharge cargo and make vessel available for cargo tank internal examination by a Coast Guard Marine Inspector.

Make permanent repairs to leak in way of #1 cargo tank and air test tank in the presence of a Coast Guard Marine Inspector.

HULL REPAIR NEEDED. SUBMIT REPAIR PROPOSAL FOR HULL PLATE RENEWAL IAW GAUGERPT DTD 17JUN97 TO THE SAT.OF THE ATTENDING MARINE INSPECTOR. VSL NOT TO LOAD ANY CARGO UNTIL REPAIR PROPOSAL IS SUBMITTED, EVALUATED AND REPAIRS COMPLETED TO THE SATISFACTION OF THE ATTENDING MARINE INSPECTOR.

MULTIPLE FRACTURES IN WAY OF P/S BOW VOID BILGE KNUCKLE, ALSO ONE 1" FRACTURE IN WAY OF #3 WING VOID BOTTOM PLATE AND ONE 10" FRACTURE IN WAY OF#4 WING VOID BOTTOM PLATE.

Locate and permanently repair source of water leak in #4 starboard wing void in way of bilge knuckle. Gas free and make available for inspection by an attending marine inspector. This requirement shall be completed prior to carrying cargo onboard and no later than 03 Feb 97.

PROCEED TO REPAIR FACILITY AND MAKE PERMANEN REPAIRS IWO AFT PORT CORNER SIDE SHELL. NOT TO LOAD CARGO UNTIL REPAIRS ARE COMPLETE.

GAS FREE AND DRYDOCK BARGE TO MAKE PERMANENT REPAIRS TO HOLES IN THE BOW RAKE & STERN BOX, #2 & 4 STBD WING VOIDS.

INSULATE ALL EXPOSED STEAM PIPING IN WAY OF TANK TOPS AS PER 46 CFR 56.50-1(K).

REPLACE THE GASKET ON THE NO. 2 CARGO TANK ULLAGE OPENING. THIS ITEM IS TOBE COMPLETED PRIOR TO LOADING THE NEXT VESSEL CARGO OR THE DATE OF COMPLI- ANCE, WHICHEVER OCCURS FIRST.

AT LIGHT LOAD (MINIMUM DRAFT) PRESENT THE VESSEL FOR AN INSPECTION OF THE SIDE PLATE.

REMOVE ALL OIL AND WATER FROM THE PORTABLE CONTAINMENT BENEATH THE DIS- CHARGE VALVE FOR THE CARGO PUMP. THIS ITEM IS TO BE COMPLETED PRIOR TO OFFLOADING THE ONBOARD CARGO OR THE DATE OF COMPLIANCE, WHICHEVER OCCURS FIRST.

REMOVE ALL OIL AND WATER FROM BOTH CONTAINMENT DRIP PANS. THIS ITEM IS TO BE COMPLETED PRIOR TO OFFLOADING THE ONBOARD CARGO OR THE DATE OF COMPLI- ANCE, WHICHEVER OCCURS FIRST.

PROVIDE A FLAME SCREEN FOR THE GOOSE NECK VENT ON THE BOW RAKE.

PROPERLY MARK THE HAILING PORT ON THE VESSEL'S STERN.

CLEAN ALL OIL-SOAKED ABSORBENT MATERIAL FROM THE FORWARD DECK AREA. THIS ITEM IS TO BE COMPLETED PRIOR TO OFFLOADING THE ONBOARD CARGO OR THE DATE OF COMPLIANCE, WHICHEVER OCCURS FIRST.

REPAIR OR REPLACE THE RELIEF VALVE ON THE STEAM LINE. THIS ITEM IS TO BE COMPLETED PRIOR TO OPERATING THIS PARTICULAR SYSTEM OR THE DATE OF COMPLI- ANCE, WHICHEVER OCCURS FIRST.

REPLACE THE MISSING DOG ON NO. 3 PORT WING TANK ACCESS OPENING.

PROVIDE ON BOARD THE VESSEL A CERTIFICATE OF INSPECTION THAT REFLECTS THE CURRENT VESSEL OPERATOR.

PLAINLY AND LEGIBLY MARK DRAFT MARKS IAW 46 CFR 32.05-1.
