All doors shall be capable of operation from either side by 1 person. The handicap buttons on both port/starboard salon doors are not functional. Provide operational evidence of handicap buttons prior to September 1, 2026.

The failsafe state must be evaluated for each subsystem, system, or vessel to determine the least critical consequence. There is no opolicy indicating if generators should be run in manual or auto and if the generators should be set to air or electric start first. Create policy for how generators should be run while underway prior to August 1, 2026.

All alarms must be provided with a means to test audible and visual annuciatiors. Engine Room Control alarm test is only shwoing visual and not sounding audible alarm. Provide means to test audible alarm prior to September 1, 2026.

The respective piping systems installed on ships and barges must have the necessary pumps, valves, regulation valves, safety valves, relief valves, flanges, fittings, pressure gages, liquid level indicators, thermometers, etc., for safe and efficient operation of the vessel. Liquid was actively leaking into void 5. Identify cause of leak and repair to the satisfaction of the MI. 46 CFR 56.01-1(b) 701c

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Port and starboard MESs are not currently onboard. Install MESs and provide servicing certificates to satisfaction of MI. 46 CFR 199.190 701c

At each inspection for certification and periodic inspection the marine inspector must conduct such tests and inspections of the main propulsion and auxiliary machinery and of its associated equipment, including the fluid control systems, as they feel necessary to check safe operation. Adapter to test generator L/O PSTP was not available. Procure adapter prior to 1AUG26. 46 CFR 61.20-3(a) 705c

Passenger vessels exceeding 100 gross tons constructed on or after July 1, 1935, and emergency systems for all ships, must use fuel that has a flashpoint exceeding 110 °F for internal combustion engine units. Such vessels must carry a sufficient quantity of fuel to supply the emergency electrical system. Fuel transfer pump was not operational. Repair to the satisfaction of the MI prior to 1AUG26. 46 CFR 58.50-1(b) 705c

On vessels required to have a final emergency power source by § 112.05-5(a) of this chapter, the following emergency lighting and power loads must be arranged so that they can be energized from the final emergency power source: Each load under § 112.15-1. Emergency light in engine room, prior to entering the boiler space was not working. Repair to the satisfaction of the MI prior to 1AUG26. 46 CFR 112.15-5(a) 705c

All piping, valves, and fittings shall meet the applicable requirements of subchapter F (Marine Engineering) of this chapter. Fire stations #1 and #7 were found with leaking valve packing. Repair to satisfaction of MI prior to 01AUG26. 46 CFR 76.10-15(a) 705c

Doors in bulkheads required to be Class A-60, A-30, or A-15 shall be of hollow steel or equivalent metal construction solidly filled with approved structural insulation capable of meeting the requirements for a Class A-15 bulkhead. Port side crow’s nest fire door and starboard side 02 deck amidships fire doors are not self-closing. Repair to satisfaction of MI prior to 01AUG26. 46 CFR 72.05-25(b) 705c

In the event of alterations involving the safety of the vessel, the applicable plans shall be submitted for approval covering the proposed work. Port side, aft food storage room has been converted to an emergency gear locker with associated equipment relocated into the space. Update fire control plan to satisfaction of MI prior to 01AUG26. 46 CFR 71.65-10 705c

Previously approved installations may be retained as long as they are maintained in good condition to the satisfaction of the Officer in Charge, Marine Inspection. Fixed fire fighting system in galley has been removed for years but pull station remains. Remove pull station to satisfaction of MI prior to 01AUG26. 46 CFR 76.05-20 705c

All electrical engineering details and installations shall be designed and installed in accordance with subchapter J (Electrical Engineering) of this chapter. Dead end wires were found in the emergency gear locker and port side car deck overhead. Remove wires or properly terminate in junction box/equipment to satisfaction of MI prior to 01AUG26. 46 CFR 70.25-1 705c

Electric installations and electric equipment must be inspected at the inspection for certification and periodic inspection to determine mechanical and electrical condition and performance. Emergency lighting covers on the 03 deck and starboard forward stairwell are broken. Repair/replace to satisfaction of MI prior to 01AUG26. 46 CFR 110.30-5 705c

Subdivision bulkheads shall be watertight. MI witness rusted through hole in bow thruster space starboard side forward overhead leading to outside. Provide repair proposal for approval from OCMI. 46 CFR 72.01-25(b)

An electric wiring must be installed to prevent any person from accidentally contacting energized parts. MI found dead end wire that was hot. de-energize wire, tag-out breaker. Remove wire ccompletely or cap off ends. 46 CFR 111.05-3

On vessels required to have a final emergency power source by § 112.05-5(a) of this chapter, the following emergency lighting and power loads must be arranged so that they can be energized from the final emergency power source: (b) The machinery, controls, and alarms for each passenger elevator.111.91-1 Power, control, and interlock circuits. Each electric power, control, and interlock circuit of an elevator or dumbwaiter must meet ASME A17.1. Missing required illuminated signal marked Elevator Emergency Power.

171.111 Penetrations and openings in watertight bulkheads in vessels of 100 gross tons or more.(a) Except as provided in paragraph (f) of this section, each opening in a watertight bulkhead must have a means to close it watertight. The four valves in the engine room's aft bulkhead leading to the steering gear flat were all found open. One of the selfclosing valves was missing its valve body cap.

155.410 Pumping, piping and discharge requirements for non-oceangoing ships of 100 gross tons and above.(a) No person may operate a non-oceangoing ship of 100 gross tons and above that is fitted with main or auxiliary machinery spaces in the navigable waters of the United States unless: (1) The ship has at least one pump installed to discharge oily mixtures through a fixed piping system to a reception facility; (2) The piping system required by this section has at least one outlet that is accessible from the weather deck; (3) Each outlet required by this section has a shore connection that is compatible with reception facilities in the ship's area of operation; and (4) The ship has a stop valve for each outlet required by this section. (b) Paragraph (a) of this section does not apply to a ship that has approved oily-water separating equipment for the processing of oily mixtures from bilges or fuel oil tank ballast. The OWS had been removed. No Standard Discharge Connection was fitted.

All doors, except those that are kept normally closed, shall be of a type which are capable of release from the control station and from a position at the door. The release mechanism shall be so designed that the door will automatically close in the event of disruption to the control system. The aft galley fire door closes when operated, but fails to seal or latch securely. 46CFR72.05-25(b)(9)(ii)

The system shall be so designed and arranged that the overhead is effectively sprayed and all portions of the deck are covered. Sprinkler heads at the ends of zones 1 and 4 (forward of the car deck) failed to discharge water when activated. 46 CFR 76.23-10

Sufficient illumination must be provided by the emergency lighting source under emergency conditions to effect damage control procedures and to provide for safe egress from each space. Emergency lights were inoperable in the following locations: 1. Lido Bar (port side) 2. Port and starboard ladderwells on the 03 deck. 46 CFR 111.75-15(c)(2)

All electrical appliances, including, but not limited to, cooking equipment, dishwashers, refrigerators, and refrigerated drinking water coolers, must meet ANSI/UL safety and construction standards or equivalent standards under § 110.20-1 of this chapter. Also, this equipment must be suitably installed for the location and service intended. Three separate leaks were identified on the galley water heater's piping. Mineral buildup was observed at the leak points. 46 CFR 111.77-3

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Identified 03 issues with 3 life rings: 1. The life ring located on the port bridge wing had a dry-rotted grab line. 2. The light fixture associated with the aft starboard car deck life ring was non-operational. 3. The retroreflective tape on the forward starboard life ring was peeling off. 46 CFR 199.190 (a)

Each space used by passengers or crew must be fitted with lighting that provides for a safe habitable and working environment under normal conditions. Cracked overhead lights were identified in the cleaning gear locker and at the aft starboard ladderwell on the car deck. 46 CFR 111.75-15 (c)(1)

Electric installations on vessels must ensure protection of passengers, crew, other persons, and the vessel from electrical hazards. The electrical wiring in the car deck overhead (forward, portside) and under the port bridgewing lacked proper support and were hanging loose.. 46 CFR 111.01-1 (2)

Any insulation installed for heat and comfort, refrigeration (including air conditioning), or for any other purpose, and all material incidental to its installation, shall be approved Incombustible Materials. In the trash room, an overhead drainage pipe was found wrapped in tape and lacking insulation. Make permanent repairs prior to credit drydock.

Doors in “B” Class bulkheads shall meet the following requirements: Door frames shall be of rigid construction, and shall provide at least a 1/2 inch doorstop at the sides and top. The door sill of the aft storeroom was heavily deteriorated/corroded. CFR 72.05-25 (c)(6)

It is the intent of this subpart to provide such markings as are necessary for the guidance of the persons onboard in case of emergency…Posted CO2 Operating Instructions referenced Bow Thruster/Engine Room stop valve #14. There were three valves marked #14. One of these was marked #15 which was crossed out and handwritten #14. The C/E believed one of these was marked in error.

Cylinders used for containing hazardous ship stores that are compressed gasses must be… (4) …maintained and retested in accordance with 49 CFR 173.34. Two Emergency Escape Breathing devices were stamped with a hydrostatic test date of 2012. 46 CFR 147.60 (a)

Carbon dioxide cylinders forming part of a fixed fire extinguishing system must be retested at least every 12 years… The Emergency Generator CO2 bottle was stamped with a hydro date of 3/12. 46 CFR 147.65

Motor Controllers(h)(2) Contain a durable heat-resistant elementary wiring diagram of the controller fixed to the inside of its door. The diagram indicated a Heater Circuit, but the heater circuit was not noted. 46 CFR 111.70-3

Each opening in a watertight bulkhead must have a means to close it watertight. The stbd side of the companionway leading to the bow thruster space had a ½” hole just above the main deck but below the watertight doors sill. 46 CFR 171.111

Vessel self reported that during mooring operations in Cape May, New Jersey the secondary electronic bow thruster tripped breaker and became non operational. Provide proof of rectification and causal analysis to Coast Guard satisfaction. Prior to Credit Dry Dock.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules. While mooring in Lewes, DE the vessel was unable to bring up RPMs on the port engine while going astern.

At each annual inspection, the inspector must ensure that the following tests and inspections of fire detection and extinguishing equipment have been conducted: Fixed fire extinguishing systems must be checked as noted in table 71.25-20(a)(2)... Carbon Dioxide: Test time delays, alarms, and ventilation shutdowns with carbon dioxide, nitrogen, or other nonflammable gas as stated in the system manufacturer's instruction manual. The installed System for this vessel is Kidde Marine CO2 which specified that "the tested delay period must fall in the range minus zero percent and plus 20 percent of the factory rated value (-0%, +20%). This range is based on the guidance provided in NFPA 12:20000 Standards on Carbon Dioxide Extinguishing Systems. The bow thruster is designed with a 60-second CO2 time delay. During the inspection, the time delay malfunctioned twice, with the first attempt lasting 91 seconds and the second 81 seconds (+20%: 72 seconds). Replace the CO2 time delay to meet the manufacturer's specified requirements.

Tests and inspections of the lifesaving equipment shall be carried out during each inspection for renewal of certification and periodic inspection, and shall include, as applicable, a demonstration of- (3) the proper condition of flotation equipment such as lifebuoy The life ring on the O2 deck (starboard side) was found to have dry-rotted grab lines.

The control system must be designed for automatic shutdown of the engine with actuation of audible and visible alarms at shutdown. Automatic shutdown must occur under the following conditions: Low lubricating oil pressure. #2 SSDG satisfactorily alarmed but failed to shutdown during loss of L/O pressure test.

All safety and relief valves for use on pressure vessels or piping systems must be designed to meet the protection and service requirements for which they are intended and must be set to relieve at a pressure which does not exceed the “maximum allowable working pressure” of the pressure vessel. #4 P/V relief valve leaked air during operational testing and lifted at 252 psi, exceeding the MAWP (250 psi) of the pressure vessel.

All portable fire extinguishers and semi-portable fire extinguishing systems must be maintained in accordance with NFPA 10. #28 CO2 portable fire extinguisher hose was worn and degraded.

The annual inspection shall include an inspection of the structure, boilers, and other pressure vessels, machinery and equipment. The inspection shall be such as to insure that the vessel, as regards the structure, boilers and other pressure vessels, and their appurtenances, piping, main and auxiliary machinery, electrical installations… is in satisfactory condition and fit for the service for which it is intended. Port and starboard shaft alleys underneath shaft seals had water accumulation in the bilge. Correct shaft seal leakage prior to credit drydock.

A main transverse watertight bulkhead may not have non-watertight penetrations below the bulkhead deck. ½” pinhole in bulkhead found on starboard side of ladder well entrance into forward bow thruster space. Correct prior to credit drydock.

The annual inspection shall include an inspection of the structure, boilers, and other pressure vessels, machinery and equipment. The inspection shall be such as to insure that the vessel, as regards the structure, boilers and other pressure vessels, and their appurtenances, piping, main and auxiliary machinery, electrical installations… is in satisfactory condition and fit for the service for which it is intended. Minor hydraulic leak found on #1 steering system pump around seal, creating a small pooling. Correct prior to credit drydock.

The annual inspection shall include an inspection of the structure, boilers, and other pressure vessels, machinery and equipment. The inspection shall be such as to insure that the vessel, as regards the structure, boilers and other pressure vessels, and their appurtenances, piping, main and auxiliary machinery, electrical installations… is in satisfactory condition and fit for the service for which it is intended. Minor hydraulic leak found on forward hydraulic bow thruster DCV, creating a small pooling. Correct prior to credit drydock.

Bulkheads or decks of the “A” Class shall be composed of steel or equivalent metal construction, suitably stiffened and made intact with the main structure of the vessel, such as shell, structural bulkheads, and decks. 03 welded over pad eyes on main vehicle deck were deteriorated and had holes deep between ½” – 1” on the aft third of along the centerline. Correct prior to credit drydock.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Marine Vessel Rules. The port side bridge wing controls were reported inoperable. 46 CFR 58.05-1(a)

At each inspection for certification and periodic inspection the marine inspector must conduct such tests and inspections of the main propulsion and auxiliary machinery and of its associated equipment, including the fluid control systems, as they feel necessary to check safe operation. Crew reported overspeed on #2 Generator does not shutdown generator or alarm. Repair overspeed protection to the satisfaction of the marine inspector.46 CFR 61.20-3(a)

The sidelights of vessels of 20 meters or more in length shall be fitted with matt black inboard screens. Screens for port/starboard nav lights are no longer matte black. Repaint screens to the satisfaction of MI. 33 CFR 84.05

Structural I-beams on the car (main) deck were found separated from the outer bulkhead (curtain) with broken welds and signs of corrosion behind the beams. Vessel is required to determine the condition of the structural beams (UT shots) and submit a repair proposal to the satisfaction of the MI based on the findings.46 CFR 71.30-5

Bulkheads of “A” Class shall be so constructed that they would be capable of preventing the passage of smoke and flame for 1 hour. Open cable penetrations in "A" Class boundaries and multiple holes were identified on the starboard side of the bridge bulkhead. Repair “A” Class boundaries to original fire protection standards prior to COI renewal inspection. 46 CFR 72.05-10 (c)(1)

Electric installations on vessels must ensure protection of passengers, crew, other persons, and the vessel from electrical hazards. The electrical wiring in the car deck overhead (forward, amidships) lacked proper support and was hanging loose. Additionally, the drainage piping along the centerline amidships was missing insulation. Make permanent repairs prior to COI renewal inspection. 46 CFR 111.01-1 (2)

Electric installations on vessels must ensure: Maintenance of services necessary for safety under normal and emergency conditions. All emergency stops near the entrance to engine room are extremely corroded. Repair or replace to the satisfaction of the marine inspector by next inspection. 46 CFR 61.20-3(a)

The main steering gear and rudder stock must be—Of adequate strength for and capable of steering the vessel at maximum ahead service speed, which must be demonstrated to the satisfaction of the cognizant Officer in Charge, Marine Inspection. Both rotary vane rudder post seals were leaking hydraulic oil. Repair to the satisfaction of the marine inspector prior to credit drydock. 46 CFR 58.25-10(c)

An inspection of passenger and crew quarters, toilet and washing spaces, serving pantries, galleys, etc., shall be made, in general, at least once in every month. The decking in the handicapped accessible cubicle of the male toilet (main deck) was found to be uneven and deteriorated, posing safety hazards for both crew and passengers. Repair or replace to the satisfaction of the marine inspector by next inspection. 46 CFR 71.45-1 (a)

Plans and specifications required... Fire control diagram showing location and type of all required fire-screen insulation, including main fire zone and subdivisions and enclosures. The onboard Fire Control Plan was a preliminary version and had not been approved. Additionally, there were several handwritten amendments that had not been verified by the Class or the Coast Guard. Replace with approved version of FCP. 46 CFR 71.65-5 (d)

Doors shall have a latch with a minimum throw of 3/4 inch which can be operated from either side of the door. A-60 Boundary doors to Engineering Operating Station do not remain shut. Make permanent repairs to the satisfaction of the attending Marine Inspector.

Materials used in the manufacture of tubing, pipes, valves, flanges, and fittings shall be selected from those specifications that appear in 46 CFR 56.60-1, Table 56.60-1(a) or 46 CFR 56.60-2, Table 56.60-2(a); or they may be selected from the material specifications of section I or section VIII of the ASME Boiler and Pressure Vessel Code. Observed Port main engine leaking air at the starter. Vessel is required to repair/replace starter or associated parts to the satisfaction of the attending marine inspector/surveyor.

All safety and relief valves for use on pressure vessels or piping systems shall be designed to meet the protection and service requirements for which they are intended and shall be set to relieve at a pressure which does not exceed the “maximum allowable working pressure” of the pressure vessel. Two (02) relief valves for air receivers (#1 & #4) failed to relieve at setpoint. Two (02) safety relief valves for air receivers (#2 & #3) did not reseat after testing. Vessel is required to replace all four (04) relief valves on each air receiver in 30 days to the satisfaction of the attending marine inspector/surveyor.

Each generator prime mover must have an overspeed device that is independent of the normal operating governor and adjusted so that the speed cannot exceed the maximum rated speed by more than 15 percent. Observed 2190 RPM during overspeed test of #2 generator. Generator is rated for 1800 RPM and should not exceed 2070 RPM for overspeed. Vessel is required to make repairs/adjust the governor

Except as otherwise provided for in this section, these requirements are applicable to the following fluid power and control systems: (1) Steering apparatus, main and auxiliary, including bow thruster systems. Port and starboard electric bow thrusters are wired in reverse order; To service to ensure operational readiness.

The items contained in first-aid kit shall be as listed in Table 160.041-4(b). Expired medicine identified in the first-aid kit located on the bridge; To replace expired contents.

In addition to the emergency power sources required under part 112 of this chapter, each self-propelled vessel and each mobile offshore drilling unit must have at least two electric generating sources. The no. 1 generator failed the operational test. Repair and demonstrate proper operation.

Fluid power motors and pumps installed on vessels subject to inspection shall be certified by the manufacturer as suitable for the intended use. Such suitability shall be demonstrated by operational tests conducted aboard the vessel which shall be witnessed by a marine inspector. Aft air compressor failed operational test. Repair and demonstrate proper operation.

Within 12 months of each subsequent servicing, except when servicing is delayed until the next scheduled inspection of the vessel, provided the delay does not exceed 5 months. Port side MES last serviced 17MAR2023. Conduct required servicing/place serviced equipment onboard.

The proper condition and operation of the survival craft and rescue boat launching appliances at loads ranging from light load to full load, except that any portion of the load test conducted in connection with replacement or end-for-ending of a fall since the vessel's last inspection or reinspection, need not be repeated. STBD side aft davit sheave seized several times throughout the rescue boat recovery. Repair davit to satisfaction of MI.

The frequency of the tests and inspections required by this section must be: for vessels, annually or as part of the biennial and mid-period inspections. Fuel piping hydrostatic test is due (last completed 27MAR2023). Conduct test to satisfaction of MI.

Repair fire doors to satisfaction of MI.

All safety and relief valves for use on pressure vessels or piping systems shall be designed to meet the protection and service requirements for which they are intended and shall be set to relieve at a pressure which does not exceed the “maximum allowable working pressure” of the pressure vessel or piping system. The No. 1 & No. 3 relief valve setting on the air receivers exceeded the MAWP of 250 psi.

Tests of the lifesaving equipment shall be carried out during each re-inspection and shall include a demonstration of the proper condition and operation of rescue boats. The aft fall wire sheave of the port rescue boat did not rotate during launch witnessed during inspection. Make operational prior to carrying passengers. 46 CFR 1999.45(b)(2)

Each vessel must have the following: Gyrocompass. Vessel reported gyrocompass power failure. Vessel must comply with the following restrictions until a technician's report has been reviewed and accepted by a CG marine inspector: (1)Transit must be made with visibility of two nautical miles or greater. (2)Daylight hour transit only.

Each vessel must have the following: Gyrocompass Vessel reported a gyrocompass that has a 15 degree error/offset. Vessel must comply to the following restrictions until the tech report has been review by a CG marine inspector. ? The inoperable equipment shall be blanked out to prevent inadvertent use. ? Vessel must have an operable magnetic compass & current deviation table. Vessel shall not steer using auto pilot with gyrocompass inputs. ? Vessel transit must be made with visibility of two nautical miles (NM) or greater, in addition to favorable visibility reports over the intended route. If visibility decreases below two nautical miles, vessel shall anchor at first available safe anchorage and contact this office. ? Daylight hour transit only. ? Repair of inoperable equipment and a copy of the Technician or Certificating Authority report attesting to proper operation must be accepted by CG 33 CFR 164.35(d) 705 C

Containers and other similar stowage locations for lifesaving equipment must be marked correctly. 08 life jacket stowage boxes had incorrect donning of life jacket placards. Provide correct donning placards. 46CFR199.70

Each lifebuoy must be marked in block captial letters with name of the vessel and name of the port. Both forward port and stbd life buoys on boat deck need to be restenciled. 46 CFR 199.70(a)(2)

All watertight doors shall be operated locally by manual power and are in operable condition. Stbd side aft fire door on O2 deck does not close completely. Adjust door to close completely

Electrical Insulation on vessels must ensure protection from passengers and crew from electrical hazards. Galley air handler room cover was missing from electrical box allowing exposed wires. Provide cover for electrical box. 46 CFR 111.01-1

NO1 Air Receiver relief valve lifted at 260 PSI exceeding MAWP. Replace relief valve on No1 air receiver. 46 CFR 77.03-1 46 CFR 54.15-10(a)

Portable Extinguisher #24 on aft ladder well leaving engine room did not have annual service. Service or replace with serviced extinguisher of the same rating. 46 CFR 71.25-20(a)(1)

During inspection found hydraulic oil on steering pedestal. Clean and remove accumulated hydraulic oil. 46 CFR 71.25-45

Drain plug found missing from M/E Raycor Fuel filters. Install plug to filter drain. 46 CFR 77.03-1 45 CFR 56.50-60(f)

Heat tape electrical boxes found not properly secured on aft car deck. Properly mount electrical boxes. 46 CFR 77.05-1(a) 46 CFR 111.81-1(e)

Port and stbd shaft well bilge pumps found not properly mounted. Properly mount or remove pumps. 46 CFR 71.30-5(a)

Means shall be provided for closing all openings to the space protected from outside such space... The construction shall be such that openings in the upper portion of the space can be closed off... by permanently installed means. STBD stack of upper louver was inoperable. 46 CFR 76.15-35(c)

Each marine evacuation system must be capable of deploying as designed. Associated life-rafts to port and starboard MES were not rigged properly. 46 CFR 70.28-1 46 CFR 199.145(a)

All lifesaving appliances and arrangements must be in accordance with Sub-chapter W. Life-saving equipment placarding was absent or painted over throughout vessel. 46 CFR 70.28-1

All lifesaving appliances and arrangements on passenger vessels must be in accordance with Sub-chapter W. Each side of each rescue boat bow must be marked with - the name of the vessel. Starboard side rescue boat name was stenciled incorrectly. 46 CFR 70.28-1 46 CFR 199.176(a)(1)(i)

The installation of all electrical systems shall be in accordance with the requirements of Sub-chapter J. Port side salon door did not close when the bridge pressed the closure button. Repair closure device so door closes when that bridge button is activated. 46 CFR 72.05-25(b)(9)(ii)

Fan room #1 below the wheelhouse is missing structural fire protection insulation. Replace insulation that was removed. 46 CFR 72.05-1

Generator (1) had build up of grease and soot on the insulation. Ensure machinery is maintained to minimize fire hazards. 46 CFR 72.03-5

The fan room by the 03 deck lido bar is missing insulation near the vent. Replace insulation with approved material. 46 CFR 72.05-10

During the inspection, 03 fire doors were found to be not fully self-closing. Make adjustments and prove proper operation of the starboard crow's nest door, port forward passenger salon door, and starboard aft passenger salon door.

#2 generator loss of lube oil alarm and shut down were not operative.

All bulkheads or decks of the "A" class shall be composed of steel or equivalent metal construction. The fan room on deck 3, classified as an A class boundary, has an opening on the starboard side bulkhead which does not have a proper A class closure. This is currently an opening in bulkhead with no means to maintain A Class fire boundary. Make repairs to meet current fire control plan.

Crows nest lounge starboard door has cracked glass in the window. Make repairs and prove structural fire protection.

Piping systems shall be completed to conform to requirements of the regulations and engineering design. Fuel sensing line on the port main engine was observed to be experiencing excessive vibration. Install hanger or other means to secure piping and prevent damage from vibration.

Vessel is required to have lights in the machinery space with a temporary emergency power source to allow essential operations. Battle lantern in aft section of engine room is not operational and requires repair.

All portable fire extinguishers shall be maintained in accordance with NFPA 10. Two extinguishers in the bowthruster space have not been serviced since 2019. Provide proof of servicing, have these extinguishers serviced, or replaced in kind.
