Each inflatable lifesaving appliance and marine evacuation system must be serviced—Within 12 months of each subsequent servicing. Starboard side MES was last serviced in May 2025. Conduct required servicing within 14 days. 46 CFR 199.190(g) 16c

During steering gear testing with control transferred to the engine room, the local rudder angle indicator displayed 10 degrees port while the bridge tiller positioned midship. Evaluate, repair to ensure it accurately and reliably reflects the true position of the rudder at all steering control stations. 46 CFR 58.25-10(c)

MI observed a broken electrical conduit and broken windows in “Hot Dog Room” which is an A boundary per approved Fire Control Plan. Repair to satisfaction of MI. 46 CFR 72.05-10(c)(1)

During inspection, the remote reach rod for the port fuel tank shutoff valve was found inoperative. Make repairs and demonstrate proper operation to the satisfaction of attending MI. 46 CFR 58.01-25

Vessel reported during testing, the starboard shore power circuit breaker failed to trip when transferring electrical load from shore power to ships service generator. Make proper repairs to shore power breaker and test to verify that the shore power breaker automatically trips to satisfaction of MI. 46 CFR 111.30-25

Electric installations on vessels must ensure: Maintenance of services necessary for safety under normal and emergency conditions. Battery connections to the emergency generator were damaged and had no insulation in spots. Repair/replace to the satisfaction of MI. 46 CFR 111.01-1(a)(1)

Indication of the rudder angle must be provided both at the main steering station in the pilothouse and in the steering-gear compartment. The rudder-angle indicator must be independent of control systems for steering gear. Indicator marking in the steering gear space (starboard side) was damaged and not accurate. Repair to the satisfaction of MI. 46 CFR 58.25-25(a)

Each self-propelled vessel, except as provided in paragraph (d) of this section, must have an electric or mechanical engine order telegraph system from the navigating bridge to the engine room. Port EOT was not working properly. Repair to the satisfaction of MI. 46 CFR 113.35-3(a) 46 CFR 113.35-3(a)

All electrical equipment must be designed and installed to operate for the particular location and environment in which it is to be used. Vessel has heater in the engine room that is not designed for marine environment. Instructed operator to not use while underway. Operator is notifying all crew that it must be secured while underway. 46 CFR 111.01-19(a)

Containers, brackets, racks, and similar stowage locations for lifesaving equipment must be marked with symbols in accordance with IMO Res. A.760(18) indicating the device stowed in that location. Multiple MES lifesaving placards on deck #3 were missing or completely wasted. Replace placards to satisfaction of MI. 46 CFR 199.178(a) 46 CFR 199.178(a)

Doors shall be of the self-closing type capable of closing against a 3 ½ degree list. Starboard side aft salon fire door and both port and starboard amidships fire doors would not close and latch. Repair fire doors/frames to satisfaction of MI. 46 CFR 72.05-25(b)(9)(i)

It shall be the duty of the owner or master to see that the vessel's firefighting equipment is at all times ready for use and all such equipment is provided, maintained, and replaced as indicated. Flexible hose coming off the #9 CO2 cylinder for the engine room fixed CO2 system was found cracked. Replace hose to the satisfaction of MI. 46 CFR 78-17-80(b)

Each passenger vessel must have a public address system capable of broadcasting separately or collectively to the following stations: accommodation spaces and service spaces. Public address speaker in Hot Dog Room was inoperable during testing. Repair or replace speaker to satisfaction of attending MI. 46 CFR 113.50-10(g)

Each davit, winch, fall, and other launching appliance must be thoroughly inspected and repaired, as needed, each year. Wastage was identified on the port side rescue boat davit at the forward deck connection. Submit repair proposal for approval and complete repairs to satisfaction of MI prior to credit drydock. 46 CFR 199.190(f)(2)

Each prime mover must shutdown automatically upon lost of lubrication pressure to the generator. Vessel #1 SSDG did not shut down when conducting low lube oil pressure test. Make corrections to satisifaction of ABS/MI.

All marine engineering details such as piping shall be designed, constructed, and installed in accordance with subchapter F. Raw water hose to starboard shaft was leaking. Make repairs to satisfaction of MI.

Master reported a 7 degree difference in rudder angle in the follow-up steering mode. Provide evidence of fully functioning steering gear system in follow-up mode prior to next voyage.

The emergency power source must meet Table 1 to 112.05-5 and have the capacity to supply all loads at a unity (1.0) service factor, that are simultaneously connected to it... Emergency generator was unable to meet full operational tests. Repair emergency generator to the satisfaction of the marine inspector. 46 CFR 112.05-5

Starboard rescue boat block aft stanchion had a 4” crack comprising the ability to launch the rescue boat due to damaged stowage arrangement. Vessel has temporary repairs to the satisfaction of the attending [name withheld] a permanent repair plan to the satisfaction of the MI. 46 CFR 199.220

MI found oil accumulation in the bilge of the engine room along 26’ of the centerline at 1” pooling. 46 CFR 71.25-45

3 holes were found in the 01 deck beneath the elevator. 2’ x 3’ deck plating was cropped and renewed by ships crew without CG oversight. Vessel must perform satisfactory NDT for conducted repairs the attending MI prior to reinspection. 46 CFR 71.55-5 (a)

Port amidships door on 02 deck leading to the open deck was missing latch hardware and was unable to remain shut as a fire boundary. Repair prior to reinspection. 46 CFR 72.05-25 (b)

MI witnessed on the vehicle deck a bent welding rod acting as a support bracket for the drain line from the 01 deck as an unauthorized repair. Repair prior to reinspection. 46 CFR 71.55-1 (a)

Retro-reflective tape on 02 life rings on the stern and the port rescue boat had nonapproved tape. Replace with Type II retro-reflective material meeting subpart 164.018. Replace tape prior to reinspection. 46 CFR 78.47-45

#15 CO2 extinguisher had severe corrosion prompting doubt for cylinder integrity and efficacy in an emergency. Replace extinguisher prior to reinspection. 46 CFR 76.50-10

The general requirement for doors, other than watertight doors, are as follows:(1) All doors shall be capable of operation from either side by 1 person. The forward galley door is inoperable due to a faulty closing mechanism, which prevents it from sealing/closing properly in the event of an emergency. Repair the door to the satisfaction of MI.

Ground indicators must: Be at the vessel's ship's service generator distribution switchboard for the normal power, normal lighting, and emergency lighting systems. Ground detectors on the emergency switchboard were stuck in the pressed position. Repair or replace ground detectors on the emergency switchboard.

Rescue boats must be stowed—( (1) To be ready for launching in not more than 5 minutes. During the inspection, the rescue boat was unable to start due to a dead battery. Replace the battery with a functional one to the satisfaction of the attending MI.

UT thickness measurements show over 50% wastage of the structural components of the starboard rescue boat davit near the deck connection. Vessel is required to submit a repair proposal for approval prior to commencing work. Once satisfactory repairs have been made, davit must be subjected to weight test to the satisfaction of attending MI.

Excessive oil was found in the bilge along the centerline. Dispose of oil to the satisfaction of MI.

Ventilation dampers for the engine room are missing bushing around damper rods. Vent duct for engine room is holed below damper. Galley damper cannot close fully and EDG damper handle is broken. Repair fire dampers and vent ducts to satisfaction of MI.

Structural components of the rescue boat davits show signs of wastage near the deck connection. A third party servicing company is required to evaluate the serviceability of the rescue boat davits. Provide findings to MI. 46 CFR 199.190 (f)(2)

#1 SSDG failed to shutdown during loss of lube oil pressure PSTP. Repair and retest to satisfaction of MI. 46 CFR 111.12-5

During testing of emergency steering at the operating station, it was found that the instructions do not accurately reflect the actions of the operator during an emergency steering situation. Update and correct instructions to satisfaction of MI. 33 CFR 164.35 (k)

Open penetrations were found through A Class bulkheads at the starboard side operating station, galley/galley store room bulkhead, and galley store room/cleaning store room bulkhead. Repair penetrations to original structural fire protection standards to the satisfaction of MI. 46 CFR 72.05-10(c)(1)

Operating station front window is cracked. Replace window to satisfaction of MI prior to reinspection. 46 CFR 71.30-5(a)

During testing crew identified cracks/holes in tubes and water intrusion into multiple fire tubes and burner tube. Hydrotest was unsat due to damage. Replace boiler tubes or submit plans for repair prior to boiler operation. 46 CFR 61.05-10 (c)

During emergency steering test in engine room control on steering pumps #1 and #2, the pumps hunted from tiller commands. Retest steering to the satisfaction of the MI prior to reinspection. 46 CFR 61.20-1

Safety relief valve for air receiver #2 did not reseat after testing. Replace safety relief valve to satisfaction of MI prior to reinspection. 46 CFR 54.15-10

Starboard battle lantern forward of the main engine is inoperable. Repair/replace battle lantern prior to reinspection. 46 CFR 112.39-3 (b)

The starboard main engine tachometer sensor cable had worn insulation and exposed wiring. Repair to satisfaction of MI prior to reinspection. 46 CFR 111.05-3

CO2 portable extinguisher #6 outside EDG space had a worn hose and broken handle. Replace hose and handle for CO2 extinguisher or replace the extinguisher prior to reinspection. 46 CFR 71.25-20 (a)(1)

Stuffing tube in EDG space must be fitted and secured to the kick pipe prior to reinspection. 46 CFR 111.81-3

Port main engine’s audible signal did not stop when engine calls matched from PHC signal. Repair/ replace EOT and conduct retest of port main engine EOT to the satisfaction of MI prior to reinspection. 46 CFR 113.35-3 (a)

Power converter in EDG space had flexible cord installation. Replace installation with hard-wired configuration prior to reinspection. 46 CFR 111.60-13 (b)

Sprinkler piping in zone 5 was found clogged rendering two nozzles inoperable. Additionally, two nozzles in zone 1 were found clogged. Repair/replace to normal ops prior to reinspection. 46 CFR 76.05-20

The inspector shall examine all accessible parts of the vessel's hull, machinery, and equipment to be assured that it is in a satisfactory condition. Structural I-beams on the car (main) deck were found separated from the outer bulkhead (curtain) with broken welds and signs of corrosion behind the beams. Vessel is required to determine the condition of the structural beams (UT shots) and submit a repair proposal to the satisfaction of the MI based on the findings.

A vessel must be operated in accordance with applicable laws and regulations and in such a manner as to afford adequate precaution against hazards that might endanger the vessel and the persons being transported. While underway, vessel had a soft grounding near South Crow Shoal near Cape May Point, NJ. Vessel to have damage survey completed by MI, prior to carrying passengers.

Each boiler, including superheater, reheater, economizer, auxiliary boiler, low-pressure heating boiler, and unfired steam boiler, must be available for examination by the marine inspector at intervals specified by Table 61.05-10, and more often if necessary, to determine that the complete unit is in a safe and satisfactory condition. Vessels refractory on boiler degraded. Repair to the satisfaction of attending MI.

Tests and inspections of the lifesaving equipment shall be carried out during each inspection for renewal of certification and periodic inspection. MI observed numerous expired items in the first aid kit/bag at the operating station. Remove and replace expired items from medical kits onboard.

The MI may inspect the steering gear more often if necessary. Master reported main steering pump blowing fuses. MI verified auxilliary pump operational remotely, and #1 main steering pump operational locally. Vessel to prove proper operation of steering prioer to carrying passengers. 46CFR61.20-1 701C

The main steering gear must be operable from the pilothouse by mechanical, hydraulic, electrical, or other means. This gear and its components must give full followup control of the rudder. Vessel experienced a steering reduction with unknown cause. Identify cause of reduction, and repair to the satisfaction of the attending marine inspector prior to movement.

Fuel piping hydrostatic test is due (last completed 27APR2023). Conduct test or provide proof of test to satisfaction of the MI.

Heating boiler out of service and not available for inspection. Make repairs and conduct tests to the satisfaction of the MI.

Insulation missing from steam piping in engine room directly above potable water pump motor controller. Install insulation on steam piping to the satisfaction for the MI.

AIS Class A device. The following vessels must have on board a properly installed, operational Coast Guard type-approved AIS Class A device: A self-propelled vessel that is certificated to carry more than 150 passengers. Vessel reported AIS not providing required information for course/speed to radar.

The installation of all systems of a marine engineering nature, together with the details of design, construction, and installation, shall be in accordance with sub chapter F. Inspector found residual water in steering room, due to minor leakage of rudder post packing. Remove residual water and tighten packing to the satisfaction of the attending MI. 46 CFR 77.03-1(a) 02105

Each navigation light indicator panel must visually and audibly signal the failure of each light. Several navigation light failure alarms were inoperative. 46 CFR 111.75-17(b) 10109

Each vessel must have a general emergency alarm system. Two newly installed general alarm sounding devices on the vehicle deck and on the passenger deck are not sounding. 46 CFR 113.25 08101

On vehicular ferries, suitable chains, cables, or other barriers shall be installed at the ends of the vehicle runways. One wire rope on the aft cargo net has only 50% of the strands remaining next to the sewage. 46 CFR 78.40-1 06101

Each lifebuoy must be marked in block capital letters with the name of the vessel and the name of the port marked on the stern of the vessel. Markings on vehicle deck lifebuoys are faded and need renewing. 46 CFR 199.70(a)(2) 11117

Inspection of lifesaving appliances for certification shall ensure the proper condition of lifebuoys. The plug on the starboard bridge wing is damage. Repair or renew this life buoy. 46 CFR 199.45(a)(3) 11117

The fire control plan shall be kept up to date. Add the crew lifejackets in the 02 Deck Galley concession area and the 03 Deck snack bar to the fire control plan. 46 CFR 78.45-1(a)(4) 07122

Periodic tests and inspection shall be made of the main and auxiliary machinery, boilers, and other equipment as prescribed in this part. Master reported loss of port side thruster while transiting from Cape May, NJ to Lewes, DE. Vessel to prove proper operation of thruster prior to departure/carriage of passengers. 46 CFR 61.01-1 (a)

All marine engineering such as piping, valves, fittings, etc.. shall be designed and installed IAW the provisions of Sub-chapter F. Vessel reported a fuel leak coming from the supply line to the #2 generator located at the threaded connection where it meets a steel threshold. Prior to movement, proper repairs must be completed to the satisfaction of ABS and Coast Guard.

Lifeboat and rescue boat release gear must be operationally tested under a load of 1.1 times the total mass of the survival craft. The port side rescue boat releasing gear must be tested prior to commercial operations.

Portside forward salon (4-5 speakers) PA system speakers are not operational.

Vessel reported that bow thruster had become inoperative. Provide causative analysis of failure and make repairs. Vessel is restricted from further movement upon arrival at the Cape May Ferry terminal following a single voyage from the Lewes Ferry terminal.

Each credentialed crew member shall become familiar with the relevant characteristics of the vessel appropriate to his or her duties and responsibilities prior to assuming those duties and responsibilities. After an extensive dry dock, re-power, and installation of a new automation system the vessel masters on board were not confident they could safely operate the vessel due to lack of training. Also, there was no training plan provided that outlined the subsequent training of all crew members that did not already participate in the vessel sea trials after completion of the dry dock period. Provide OCMI with a training plan to familiarize all crew with vessel operations affected by changes made during the dry dock period. Every crew member that has not received training is restricted from operating the vessel with passengers until evidence of completion of training has been provided to the OCMI. 46 CFR 15.405

Each boiler must be available for examination by the marine inspector at intervals specified by Table 61.05-10. Auxiliary boiler is overdue for inspection and is restricted from use until required inspections are completed. 46 CFR 61.05-10 (a)

All piping, valves, and fitting shall meet applicable requirements in sub chapter F. Sprinkler system valve in E/R leaking significantly. 46 CFR 76.23-20

Furniture shall be constructed of entirely approved incombustible materials. Vessel is not in full compliance of structural fire protection standards outlined in 46 CFR Subchapter H. Submit detailed repair/compliance plan to address the installation of non-compliant furniture to the Marine Safety Center within 30 days of issuance of the 2021 COI. 46 CFR 72.05-55(a)(1)

An inspection shall be made whenever any important repairs or alterations are undertaken. Vessel is required to be in full regulatory compliance of structural fire protection standards prior to issuance of a valid 2022 COI. 46 CFR 71.55-5

No fire stop within the marine cable transit under the port bridge equipment console. Make repairs and prove structural fire protection in accordance with 46 CFR 72.05-10.

Semi portable extinguisher cylinders in engine room are more than 12 years past the required hydrostatic testing date. Prove hydrostatic testing of cylinders and flexible connections. Flexible connections to be tested to 1000 psig. Complete prior to the carriage of passengers.

Exhaust lagging in stack space of both main engines are contaminated with oil presenting a fire hazard. Provide cause of oil contamination and corrective action. Replace lagging in kind.

120v receptacle powering the E lighting on fwd bulkhead stbd side engine room showed reverse polarity when tested with circuit tester.

Aft embarkation light at stbd rescue boat has damaged outer insulation. Replace cable in kind; Electrical tape is not a suitable repair.

Port rescue boat davit has wasted cable hangers and Jbox with damaged stuffing tube. Renew/replace in kind.

#2 SSDG has oil dripping onto the electrical connections of a sensor (inboard side of engine). Make provisions to protect sensor from oil.

Vessel crewmembers reported exhaust in the port main engine crankcase. Identify cause of the exhaust and make repairs to the satisfaction of an attending marine inspector prior to movement.

During function testing of the vessel's CO2 system, a leak was detected in the pressure alarm line. The time delay was also over the allowable 66 seconds. Make repairs to the damaged CO2 line and conduct pressure testing of the repaired line in the presence of an attending marine inspector. Upon successful completion of the pressure testing, conduct a full operational test of the CO2 system prior to movement.

The "Lido Bar" door does not properly self close. Make proper repairs to door to ensure proper operation within a fire boundary.

Motor controllers for the starboard lifeboat and hot start in the engine room have loose box connectors/stuffing tubes. Repair or replace box connectors/ stuffing tubes to provide adequate enclosure ingress protection.

Multiple receptacles in the salon area do not have a grounding pole. Prove proper installation of a grounding pole in all receptacles that operate at 100 volts or more.

The vessel reported excessive vibration on the #1 SSDG while testing. An analysis of the cause of the vibration and any corrective action taken shall be submitted to the CG. Proper operation of the #1 SSDG shall be proven to the attending CG inspector.

Aft bridge window is cracked and sealed with duct tape. Replace window to required fire boundary standard. 46 CFR 72.05-30(a)

Forward port salon door does not close properly. Repair latch and prove proper operation. 46 CFR 72.05-25(b)(9)

Port rescue boat was improperly marked. Mark both sides of the rescue boat bow with the vessel's name in block capital letters. 46 CFR 199.176(a)(1)

re-condition and properly attach wire connections, fuses, and ensure the motor controller boxes are weather tight for the rescue boat davits.

ensure all electrical panel boards have a directory with rating or setting of overcurrent devices. the panels should be fitted per approved drawings.

Port side passenger salon door does not latch properly and has a hole in it. Repair latch and fill hole to ensure fire boundary is maintained.

Fire extinguisher brackets numbers 12 and 13 are rusted through. Renew brackets to ensure proper mounting.

Door handle on stbd fwd salon door is in need of repair. Repair and prove proper operation.

Stbd side life ring buoy near crew room needs new SOLAS tape.

The port MDE F/O cross over hose was found chaffed and needs to be replaced. 46 CFR 56.60-25(b)

There is a ground on the E-circuit. The vessel noted the ground was on the mast light. 46 CFR 111.05-21

The vessel noted the seal for the electric bilge pump needs replaced. proper maintenance must be preformed. 46 CFR 56.50-55

The ECR and Pilot house port MDE shaft Tach is not correct. the proper shaft RPM must be reported to control stations. 46 CFR 6.35-5(b)(3)

The #1 SSDG was found with excessive amounts of oil leaking from the frw end and unattached hose clamps. the vessel shall find the cause of oil leak and make repairs. hose clamps shall be reattached. 46 CFR 58.10-10

The vessel reported the #2 SSDG was ubale to supply power to the bus. The vessel shall prove proper operation of the #2 SSDG.

Two window heater units on the bridge are not connected properly. Remove wiring or properly install window heaters.

Remove dead end wiring in the aft control station box.

Prove proper operation of the over-speed trip on the #1 SSDG. 46 CFR 62.35

Vessel reported starboard main engine failure while departing Cape May terminal. Provide statement attesting to cause of shutdown and repair proposal. Vessel may transit back to the Cape May terminal with a tug assist.

Vessel's generators are arranged for parallel operations. the arrangement is required to have either reverse power or reverse current trips. Vessel must prove proper operation of this safety. Vessel must not operate in a parallel condition until proven.

Vessel must demonstrate CO2 shut down and alarm of the EDG engine and space.

The AMR valve on the bilge manifold is leaking. Vessel must repair or replace this valve.

The hot water boiler in the engine space could not be operated. Vessel must demonstrate proper operation of all safety controls prior to operating.

The following areas must have CO2 signage: CO2 soda storage, CO2 cylinder firefighting storage, and the maine engine and EDG spaces.

Vsl is broadcasting: NAME with Vsl type precursor (NAME must not include Vsl type precursors, e.g. F/V, M/V) and incomplete dimension information (AIS Antenna Position “X” equals 0).

Throughout the vessel's weather deck, metallic armor or sheathing is deteriorated and non-continuous. Replace or repair identified cables.

KB01 Port bridge wing rudder angle indicator is obscured and cannot be read. Replace port bridge wing rudder angle indicator as original.

Remove Hydraulic/water mixture in the steering void and clean as necessary. Further evaluate steering system & rudder post seals to ensure any leakage is identified and if necessary, repairs made.

Clean & inspect battery boxes for Emergency Generator & General Alarm Batteries.

Either replace the #15 CO2 fire extinguisher on the car deck (midway stbd side) or have the extinguisher serviced by an authorized servicing facility.

DC01: Aux steering breaker trip is inoperable. Prove breaker trip operation to attending MI.

DC02: Prove proper operation of Gen paraleling and reverse power relays.

Prove proper operation of #6 general alarm bell.

Prove proper operation of self closing fire doors on O2 deck.

Make enginer room bilges available for isnpection. Bilges contained approx 4 inches of oil.

Service rescue boat falls in accordance with manufacturer's recommendations.

Repair fire stop penetration in forward port bulkhead leading to accomodation space/salon.

Remove daisy-chained surge protectors at WIFI station outside galley.

Replace water light line on forward port side ring buoy.

Vessel must make engine room bilges clean and available for quarterly visual inspections to monitor hull repairs and ensure further corrosion of hull plating does not occur. Condition of bilges should be documented at via photograph at inspections. Vessel is to provide results of corrosion survey to determine the cause of hull plate wastage to USCG Marine Inspectors for review.

Prep, prime and paint areas discussed in : bow thruster bilges and bilges in voids 3,5,6, and 7.

Repair crack in upper longitudinal in void#1 and fwd and after ladders in void#4.

Repair wasted area around starboard access hatch to void #7.

During vessel's ISE, MIs witnessed excessive pitting and deteriorated welds in way of keel. An area approximately 12'x30' was marked by USCG and ABS to under go ultrasonic testing. Results show significant wastage of seam welds and bottom shell plating in the sample area as much as 75%. Vessel is restricted from carrying passengers until a plan is approved by a USCG Marine Inspector.

Repair car deck sprinkler system leaks IVO Zone 3 FR 82 and FR 85. Prover proper operation of sprinkler heads IVO Zone 2 FR 50 and Zone 5 FR 45.

Provide proof of hydrostatic testing for galley dry chemical system.

Prove proper operation of main and auxiliary steering gear overload alarms to satisfaction of CGMI.

Replace T-fittings on fixed CO2 system w/ appropriate fittings.
