Onboard training, musters, and drills must be in accordance with 46 CFR subchapter W (Lifesaving Appliances and Arrangements). Conduct man overboard, fire, and security drills to the satisfaction of the attending USCG MI by 28May26.

The main steering gear and rudder stock must be capable of moving the rudder from 35° on either side to 35° on the other with the vessel at its deepest loadline draft and running at maximum ahead service speed, and from 35° on either side to 30° on the other in not more than 28 seconds. Vessel was only able to move the rudder stock 28 degrees in the follow up mode on the number 1 steering pump during vessel steering checks prior to departure.46CFR58.25-10(C)(2)

Instrumentation listed in Table 62.35-50 must be of the continuous display type or the demand display type. Displays must be in the ECC or in the machinery spaces if an ECC is not provided. L/O temperatures and RPM are listed in the tables indicated in both CFR and ABS Part 4-9-4. The vessel has a designated Engine Control Center, therefore, the displays/alarms indicated by tables above are to be attained at the station. Both MDE L/O temperatures and one RPM gauge are tapped over due to being inaccurate. Vessel to provide working accurate gauges in the ECC, as designed, to the satisfaction of the attending USCG MI. 46CFR 62.25-20.b.5.

Where pressure-indicating gages are used, they shall be chosen to be compatible with the pressure to be indicated. The size of the visual display, the fineness of graduations, and the orientation of the display will be considered. In no case shall the upper range of the gage be less than 1.2 times nor more than 2 times the pressure at which the relieving device is set to function. The vessels 130PSI (relief) start air receiver has 0-400 PSI gauge. Provide a gauge within range to regulation to the satisfaction of the attending USCG MI. 46CFR 54.15-5.f.

During preparation and launching, the survival craft, its launching appliance, and the area of water into which it is to be launched are illuminated by lighting supplied from the vessel's emergency source of electrical power. The vessels upper deck emergency lighting was seized in inward rotation and unable to illuminate the embarkation area. Housing was adjusted and corrected on the spot. 46CFR 199.150.f.6.

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Crop and renew wasted/holed side shell area on the port side midship area adjacent to drain plenum above the freight deck to good metal prior to next credit drydock. A temporary doubler plate has been approved until permanent repairs are made.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel. Vessel to inspect repair and test steering system to the satisfaction of the Coast Guard Prior to the passage of passengers or cargo.

The #1 SSDG muffler piping at the side shell is wasted. Crop and renew to good metal and test to the satisfaction of the attending USCG Marine Inspector.

Approx. 125 gallons of water was removed from the bilge under the CO2 bottles located in the CO2 void space. The source of the water intrusion could not be identified due to that section of bilge is filled with concrete ballast. Vessel is authorized a one time transit from the Steamship Authority Dock located in Hyannis, MA to the Steamship Authority property in Fairhaven, MA. After arriving in Fairhaven, the vessel shall be scheduled for dry dock to determine the cause of the water intrusion and to make permanent repairs. This transit is approved pending a review and approval by the local OCMI of a detailed transit plan outlining the manning and safety precautions to be taken from the voyage from Hyannis, MA to Fairhaven MA.

All piping, valves, and fittings shall meet the applicable requirements of subchapter F (Marine Engineering) of this chapter. The pipe connecting fire pump S213 to the fire main has a pin hole at the elbow on the starboard side of the vessel. Make permanent repairs to the affected area of the fire main piping.

Each generator must meet the applicable requirements for construction and testing in section 4-8-3 of the ABS Marine Vessel Rules. Port Generator #2 was hunting and needs to be tested for proper shutdowns.

E-light in deck locker #27 is out.

Where mechanical ventilation is provided for spaces other than cargo and similar spaces which are protected by a carbon dioxide extinguishing system, provisions shall be made so that the ventilation system is automatically shut down with the operation of the system to that space.Pressure trip on the middle fire damper in engine space did not work during fixed fire extinguishing tests. Replace pressure trip.

Piping found wasted, renew to orginal specs.

When alteration or repair of boilers, pressure vessels, machinery, safety valves or piping systems becomes necessary, the work shall be done under the cognizance of the Officer in Charge Marine Inspection. Vessel reported a weeping fuel leak on its Port Main Engine fuel return line. Vessel is authorized a one time transit without passengers or cargo from Nantucket to Hyannis MA. Once in Hyannis, effect repairs to the satisfaction of the attending/reviewing Marine Inspector.

Vessel is equipped with a weather tight stern door. The stern door suffered a failure of a guide wheel that resulted in the partial separation of the wheel from the wheel assembly and lodged the remaining portion of that wheel assembly into the guide track. This failure resulted in inability of the door to close or open. Vessel is authorized to operate with bow door in its current state until permanent repairs can be made. Vessel captain is to determine the safe operation of the vessel with the bow door partial open prior to departure. 46CFR71.55-1

Valves for drawing fuel or draining water from fuel are not permitted in fuel oil systems except ... Such a valve shall be fitted with a cap or a plug to prevent leakage. The F/O filter/separator on the #1 boiler is missing the mechanical plug in the bottom of the housing. Provide IAW with regulation. 46 CFR 56.50-60 (f)

The respective piping systems installed on ships and barges shall have the necessary ...for safe and efficient operation of the vessel. The discharge piping of the upper A/C heat exchanger has active leaking and corrosion at or near the side shell. Renew portion of effected pipe at next available dry-dock. 46 CFR 56.01-1 (b)

Security systems and equipment must be in good working order and inspected, tested and calibrated and maintained according to the manufacturer's recommendations. Some of the cypher locks locked immediately upon closing while others took anywhere from 3-8 seconds allowing time for a unauthorized individual to gain access to restricted areas. Ensure all cypher locks lock immediately upon closure or as intended by the manufacturer.

In the event of an accident to a boiler, unfired pressure vessel, or machinery tending to render the further use of the item unsafe until repairs are made, or if by ordinary wear such items become unsafe, a report shall be made by the chief engineer immediately to the Officer in Charge, Marine Inspection, or if at sea, immediately upon arrival at port. Vessel reported that the stbd main engine shaft seal is leaking. Affect repairs to the satisfaction of the CGMI. Vessel is authorized a one time transit without passengers from Hyannis, MA to Fairhaven, MA. to complete repairs. Notification must be made during departure and arrival. UPDATE: 08JUN2023 - Inner seal repair completed to stop water ingress. After completion of sea trial vessel will transit from Farihaven MA. to Thames Shipyard in New London CT. to haul out and replace entire seal.

Each individual bilge suction shall be fitted with a suitable bilge strainer having an open area of not less than three times at of the suction pipe. Bilge strainer was wasted at suction in void #3. Renew bilge strainer.

All electrical engineering details and installations shall be designed and installed in accordance with subchapter J (Electrical Engineering) of this chapter. Removed dead ended electrical cable and bracket hanging from overhead on the freight deck or properly terminate in a junction box.

Vessels of 1,000 gross tons and over shall have permanently exhibited for the guidance of the officer in charge of the vessel, general arrangement plans showing for each deck the fire control stations, the various sections enclosed by fire-resisting bulkheads, together with particulars of the fire alarms, detecting systems, the sprinkler installation (if any), the fire extinguishing appliances, means of access to different compartments, decks, etc. Obtain an approved fire control plan and post.

The thickness of inlet and discharge connections outboard of the shutoff valves, and exclusive of seachests, must be not less than that of Schedule 80 for nominal pipe sizes through 8 inches. C/R wasted through spool piece at sideshell for sprinkler system in the auxiliary machinery room prior to next credit drydock.(doubler accepted as temp repair)

If during a voyage any equipment required by this part stops operating properly, the person directing the movement of the vessel may continue to the next port of call, subject to the directions of the District Commander or the Captain of the Port, as provided by part 160 of this chapter. Vessel reported a main steering gear leak. Make repairs to the satisfaction of the attending Coast Guard Marine Inspector prior to carrying passengers or cargo.

The respective piping systems installed on ships and barges shall have the necessary....flanges, fittings for safe and efficient operation of the vessel. Multiple unguarded friction points were noted between the entire hydraulic steering pipe and their associated support hanging brackets. At the next SSA yard period, vessel to un-secure all hanging brackets & make pipe underneath availible for inspection.

Ground fault indicator on the electrical panel located in the emergency generator room was displaying a ground fault. The fault has been identified in the EP-7 Lighting Panel circuit

While installing new wiring for the new fire detection system, sections of fire insulation were removed through out the vessel. New through hole penetration for wire runs were also installed. Fill all wire runs with fire retardant putty and replace all missing fire retardant insulation with appropriate insulation for the fire boundary as listed on the vessel's fire control plan.

All fire boundary doors through out the vessel either needed assistance to shut or shut and would not latch. Adjust all fire doors so they are self closing and properly latch.

Crash bar on port side 3rd deck door leading from snack bar area to the outside of the vessel was broken and the latch would not retract from the door. Replace crash bar on port side 3rd deck door leading from snack bar area to the outside of the vessel.

The installation of all systems of a marine engineering nature shall be in accordance with the requirements of 46 CFR subchapter F. Marine Inspector witnessed port engine unable to start as designed without manual manipulation of the governor. Company to troubleshoot engine failure, conduct necessary repairs, and conduct sea trials in presence of attending Marine Inspector prior to carriage of passengers.

In addition to the emergency power sources required, each self-propelled vessel must have at least two electric generating sources. Vessel reported the main generator went offline and the emergency generator came on. Determine cause and repair to the satisfaction of the Coast Guard Marine Inspector.

Each vessel must have a stability letter issued by the Coast Guard. Vessel's stability letter is not valid as lightship weight changes are above 2%. Provide documentation from the Marine Safety Center regarding the vessel's stability resulting from the latest deadweight survey conducted on 06May2022.

Bulkheads and decks must meet structrual fire protection as per 46 CFR 72.05-10. New cable installation for camera system is running through penetrations in structural fire protection bulkheads and decks with no fire stop. Install fire stop for the penetrations.

Tag out TLIs for the fuel day tank and storage tank. Manually sound fuel tanks. Calibrate TLIs and prove proper operation to the satisfaction of the attending USCG Marine Inspector.

Crop and renew both stbd and port side aft air plenums on the freight deck to good metal where wasted through prior to completion of Steamship Authority's next scheduled credit drydock Feb 2022. Doubler plates have been accepted as a temporary repair.

Update master PSTPs to reflect correct test procedures. Provide copy to local OCMI for review.

#3 fire station is wasted and not able to provide adequate coverage of its intended zone.

A sanitary issue in the crew quarters exists, whereby raw sewage came up through the drains. The crew quarters now smell strongly of raw sewage. Air quality testing required, as well as an inspection of the MSD piping.

all retro-reflective tape on the rescue boat is missing.

Prove proper operation of port fire pump and install pressure gauge on the discharge outlet.

The vessel owner/operator must obtain an approved NTVRP Approval Letter by 14 Dec 2019.

All 3 of the vessel's required orange handheld flares are expired.

Life ring buoy near the rescue boat was found to be unserviceable.

Retroreflective tape on rescue boat is faded, and in some areas, completely missing.

Excessive oily water in the engine room bilge, specifically under the fuel oil purifier.

Onboard garbage cans must be constructed of non-flammable material. Numerous garbage cans are plastic.

Escape hatch cover for emergency egress from the engine room is missing. It is set to be repaired during the vessel's upcoming Drydock.

Parallel circuit will not parallel, make repair to circuit breaker to satisfaction of CG Marine Inspector.

Gages on boilers do not read accurately as installed. Address to the satisfaction of the attending USCG Marine Inspector.

Repair WTD in E/R. Lower ram is leaking excessively.

Repair PA speaker on vehicle deck stbd side aprox 20 ft aft of fire station #15.

Replace the water logged life preservers.

Provide updated charts of the operating area or update charts with the locan notice to mariners.

Repair the masthead and stern lights

Repair the 2 General Alarm bells, mezzanine starboard side

Adjust Fire Screen Door 02 level starboard side forward

Repair the broken bracket on the #1 sprinkler zone, starboard side

Replace the broken window, starboard side aft mezzanine (Fire window).

Vessel's rescue boat kept stalling out when the boat was underway during a man overboard drill. Vessel shall provide another working rescue boat prior to leaving Vineyard Haven.

EDG could not be started due to the engine being air bound after fuel filter change. Prove proper operation of EDG prior to carrying passengers.

Panel boards located in the EDG space in the back part of the space has missing blanks where there would be breakers.

The junction box for the "FD-3" fire damper located in the EDG space needs a cover.

The power cord in the EDG space on the left side of the EDG switchboard needs to be permanently mounted.

The sound powered telephone on the car deck portside forward is missing numbers to dial a location.

Remove old wiring found near the hot water boilers in the ER.

Properly secure/mount overhead cables found throughout the ER, Aux machinery space, steering gear oil line, and CO2 hose reel.

Conduct fire, man overboard, and security drills to the satisfaction of the attending USCG Marine Inspector.

Provide approved electrical plans for the wiring, transformer, load anaylisis for the rescue boat davit.

Noted several fire screen doors with hold backs throughout the vessel. Required vessel crew to remove all hold backs throughout the vessel.

Noted panelboards in passenger spaces were not locked. Required vessel to lock panelboards that are accessible by passengers to be locked.

Sound powered phone located in the crew mess not operational. Required vessel to make permanent repairs to sound powered phone.

Fire axes were not properly labeled with vessel's name. Label fire axes throughout the vessel.

Crew mess area fire station was not properly labeled. Label fire station with number.

Multiple crew sleeping racks did not have a light. Repair or replace rack light with a working one.

#2 crew berthing space A/C vent louvers were broken. Repair or replace.

#2 crew berthing has an extension cord. Remove cord.

AVH01: Electrical wiring exposed IWO car deck, sprinkler zone 1 (port)

AVH02: Rescue boat winch & fire/ambulance have an extension cord supplying power. Provide a plan to install a more permanent power supply for both.

AVH03: Side shell door IWO sprinkler Zone 3 will not close all the way. Repair dogs & seal.

AVH04: Door does not close properly. Repair door

AVH05: P/V for air horn relief valve is broken, replace valve

AVH06: Extension cord is feeding power to P/V supplying air to ships whistle. Install a more permanent electrical supply.

07AVH: Located in the concession area there multiple cords and wires strips. Remove power strip and install a more permanent electrical power supply.

08AVH: there are multiple trash cans that are combustible throughout the ship. Trash cans should be non-combustible type.

Provide a copy of EMI Doc. No. Y15022C, Rev 2 (FEMA/DVTP) and EMI Doc. No. Y15022SC, Rev 1 (Steering Control System Manual) to this office.

Conduct testing of Rev 2 of DVTP and Rev 1 of PSTP to the satisfaction of the attending USCG Marine Inspector as per Marine Safety Center letter E2-1600266 dtd 26Jan16.
