Where cables which are installed in hazardous areas introduce the risk of fire or explosion in the event of an electrical fault in such areas, special precautions against such risks shall be taken to the satisfaction of the Administration. Marine inspector observed intrinsically safe electrical cable connectors for bulkhead fixtures adjacent to oxygen/acetylene bottle storage, and paint locker heavily deteriorated. 74SOLAS(04)II-1/45.5.4

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended. Marine Inspector observed multiple deteriorated valve stems and handles for auxiliary boilers in machinery space and cargo steam piping on deck. 74SOLAS(04)II-1/26.1

Maintenance, testing and inspections shall be carried out ...in a manner having due regard to ensuring the reliability of fire-fighting systems and appliances. Control valve handles on the ships foam system found deteriorated at the system piping at the aft port corner of the house and at various foam monitors.74SOLAS(20)II-2/14.2.2.1

Provision shall be made to prevent overpressure in any oil tank or in any part of the oil fuel system, including the filling pipes. Any relief valves and air or overflow pipes shall discharge to a position which, in the opinion of the Administration, is safe. Marine inspector observed wasted tank vent piping for starboard HFO aft storage on weather deck. 74SOLAS(01) II-2/15.2.7

Maintenance, testing and inspections of life-saving appliances shall be carried out in a manner having due regard to ensuring reliability of such appliances. Marine Inspector observed that the starboard lifeboat remote lowering cable was rusted and twisted around the spool drum. 74SOLAS(20)III/20.3.1

Release clips for the bulwark opening for the STBD Jacobs ladder were seized closed and unable to be removed in an emergency.74SOLAS(20) III/20.3.1

Fire doors in main vertical zone bulkheads, galley boundaries and stairway closures other than power-operated watertight doors and those which are normally locked shall satisfy the follow requirements: the doors shall be self-closing and be capable of closing with an angle of inclination of up to 3.5° opposing closure. Marine Inspector observed that the C-Deck stairway door was not capable of closing and latching properly without manual intervention.74SOLAS(20) II-2/9.4.1.1.5.1

A ship of 300 gross tons or more constructed after June 30, 1974 must have a fixed container or enclosed deck area under or around each fuel oil or bulk lubricating oil tank vent, overflow, and fill pipe. Marine Inspector observed items being stowed in the containment of the port side scavenge air vent, inhibiting the containment from being able to meet its capacity requirement. 33CFR155.320

Sanitary accommodation should be provided for the exclusive use of the occupants of the hospital accommodation, either as part of the accommodation or in close proximity thereto. Such sanitary accommodation should comprise of a minimum on one toilet, one washbasin and one tub or shower. Marine inspector found hospital space without working water supply for the tub. MLC(06)B.3.1.8.4

In each washing space in a visible location there shall be information regarding procedures and resources to report crimes upon the vessel, including sexual assault and sexual harassment, and vessel owner or company policies prohibiting sexual assault and sexual harassment, retaliation, and drug and alcohol usage. Onboard a seagoing vessel of 500 GT or more, driven by main propulsion machinery of 1,000 HP/750 kW propulsion power or more or on an international voyage beyond the boundary line as described in part 7 of this chapter, no person may employ or engage any person to serve, and no person may serve, in a position requiring a person to hold an STCW endorsement, including master, chief mate, chief engineer officer, second engineer officer, officer of the navigational or engineering watch, or GMDSS radio operator, unless the person serving holds an appropriate, valid STCW endorsement issued in accordance with part 11 of this subchapter. Third assistant engineer onboard found without STCW endorsement for Office in Charge of an Engineering Watch (OCIEW) as required. Records provided by the company indicate he had been employed as an OCIEW for nearly 2 years.

Each vessel required to be fitted with an Automatic Radar Plotting Aid (ARPA) under § 164.38 of this part must be fitted with a device to indicate speed and distance of the vessel either through the water or over the ground. During inbound transit vessel found their speed log to be inoperable. Make proper repairs to satisfaction of RO. 33CFR164.40(a)

Manually operated valves in connection with the bilge pumping arrangements shall be in positions which are accessible under ordinary circumstances. T-handle for operating bilge suction valves in cargo pump room missing and valve was frozen. 74 SOLAS (97 CONSL) II-1/21.1.5

Replace Starboard lifeboat electric starting motor to the satisfaction of attending marine inspector no later than 01/10/2016. Vessel is allowed to proceed while carrying inflatable life-rafts of sufficient capacity in an arrangement approved by ABS until lifeboat is fully operational and tested by ABS and or Coast Guard.
