The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards.
The fuel oil purifier room had excessive standing fuel oil in the purifier space which was caused by excessive fuel oil leaks by the generators located directly above the purifier room as well as minor leaks in the fuel oil purifier room. Repair or replace to the satisfaction of the attending USCG Marine Inspector and Class Surveyor.

When isolation of exhaust pull hand was exercised it broke. Also, there is no way to secure all closure devices on the supply side dampener side.

The company and the ship shall comply with the requirements of the International Safety Management Code. For the purpose of this regulation, the requirements of the Code shall be treated as mandatory.
The company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. The various tasks should be defined and assigned to qualified personnel.
Vessel has drain lines at both port and starboard manifolds that connect to the slop tank, however cargo residuals in manifolds are being drained directly to discharge containment and then pumped to the slop tank which is not in accordance with any written procedures.

Main cargo pump shutdown switch located at the port and stbd manifold has exposed wires due to missing packing where the wires enter the box. In weather locations electrical equipment in a class 1, div 1 must be intrinsically safe, explosion proof or purged and pressurized if within 10ft of a cargo valve. The same applies to the framo shut down on the stbd manifold.
