The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. The fuel oil purifier room had excessive standing fuel oil in the purifier space which was caused by excessive fuel oil leaks by the generators located directly above the purifier room as well as minor leaks in the fuel oil purifier room. Repair or replace to the satisfaction of the attending USCG Marine Inspector and Class Surveyor. 74 SOLAS (97) II-1, 26.1

The purpose of this regulation is to contain a fire in the space of origin. For this purpose the following functional requirements shall be met, the integrity of the divisions shall be maintained at openings and penetrations. When isolation of exhaust pull hand was exercised it broke. Also, there is no way to secure all closure devices on the supply side dampener side. SOLAS 14 II-2/9.1.3

The company and the ship shall comply with the requirements of the International Safety Management Code. For the purpose of this regulation, the requirements of the Code shall be treated as mandatory. The company should establish procedures, plans and instructions, including checklists as appropriate, for key shipboard operations concerning the safety of the personnel, ship and protection of the environment. The various tasks should be defined and assigned to qualified personnel. Vessel has drain lines at both port and starboard manifolds that connect to the slop tank, however cargo residuals in manifolds are being drained directly to discharge containment and then pumped to the slop tank which is not in accordance with any written procedures. SOLAS 14 IX/3.1 ISM 2014 Reg 7

Provide evidence to show the last set point check of the steam piping relief valve located just outside the starboard enterance to the pumproom. relief valve settings must be checked twice within any 5 year period and no more than 3 years may elapse between any check and its predecessor. No evidence of this valve being tested could be provided.

The escape piping for the safety relief valve located just outside the stbd entereance to the pumproom allows the steam to be released horizontally. It appears a piece of piping was removed from the flange. Escape piping must lead as near vertically as practitable as possible.

Main cargo pump shutdown switch located at the port and stbd manifold has exposed wires due to missing packing where the wires enter the box. In weather locations electrical equipment in a class 1, div 1 must be intrinsically safe, explosion proof or purged and pressurized if within 10ft of a cargo valve. The same applies to the framo shut down on the stbd manifold.

Seven overhead lights in the pumproom are missing screws that secure the globe to the fixture. A cargo handling room must not have any electric cable or other electric equipment except intrinsically safe and explosion proof lighting fixtures.

Efficient guard rails must be fitted on all exposed parts of the super structure decks. The top handrail located near the stbd lifeboat station has broken welds.

Steam piping in the purifier room has significant leak that is covered with chem suites and rope to minimize the leak. Condition of the ship and its equipment shall be maintained to conform with the provisions of the current regulations.

The #2 SSG has significant diesel leak at the duplex strainers and drain line line leading below deck to the purifier room. Condition of the ship and its equipment shall be maintained to conform with the provisions of the current regulations.

The stbd steering pump has a significant hydraulic leak at the control valve. Condition of the ship and its equipment shall be maintained to conform with the provisions of the current regulations.

Movement of machinery oil from tank to tank shall be logged in the ORB Part I. Provide objective evidence that oil transfred sludge (lube and fuel) to the common sludge tank are being recorded.

Fire Control Plan does not accurately reflect current arrangements. Provide updated Fire Control Plan to reflect current arrangements in accordance with the Safety Equipment Certificate.

#2 STBD hydraulic supply piping failed due to corrosion. Vessel required to replace piping in kind.

Prove proper operation of installed (required equipment) Oily Water Separator prior to sailing to the next U.S. Port (Valdez, AK) to the satisfaction of attending CG Marine Inspector or attending ABS Class Society as required by 33 CFR 155.370 (a); 33 CFR 155.380; and, in accordance with the CG Aternate Compliance Program.

Prove proper operation of inoperative OWS discharge monitoring device to the satisfaction of the attending Coast Guard Marine Inspector or ABS Class Society Surveyor in accordance with Coast Guard Alternate Compliance Program (ACP).

Port life boat engine inoperative, Vessel to make repairs in Puget Sound, WA.

Complete repair to the inoperative Emergvency Fire Pump to the satisfaction of Class.

Complete repair to the inoperative No. 3 Generator to the satisfaction of Class.

Repair all fractures ivo #1P and #1PW.

Issued Letter of Deviation 02Oct01 prior to entry for inoperative radar. Tobe repaired prior to departure.

Affect permanent repairs to GMDSS-SITOR-NBEP equipment prior to next U.S. port, to the satisfaction of the cognizant OCMI and Class.

Replace starboard anchor. If anchor is not replaced prior to the next portcall in Nikiski, AK, the use of a suitable tug while mooring to the KPL dock will be required.

FOAM ANALYSIS TEST DATED 06SEP00 REFLECTED FOAM TEST FAILING DRAIN TIME. NEW SAMPLE TAKEN FOR RETEST.

Replace lost anchor with new upon arrival port of New York.
