A vessel must comply with the applicable requirements for any radio installation, including the requirements for a station license and installation certificates. Vessel's VHF radio was programmed with MMSI number not prescribed to the vessel's FCC documentation. 46 CFR 184.502

Each life jacket must be marked with Type 1 retroreflective material approved in 46 CFR 164.018. Five life jackets require new retroreflective material located on the top section. 46 CFR 185.604(h)

At least one ring life bouy must be fitted with a lifeline. Life buoy did not have 60 ft. lifeling attached. 46 CFR 180.70(c)(2)

At each initial and subsequent inspection for certification, reinspection and at every other vessel inspection the marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. The vessel was discovered to have two cracked windows, one on the port side main deck and one in the pilothouse, as well as a window popped out on the starboard side in the forward part of the main deck. The vessel was awarded a credit drydock and authorized to operate pending the repairs with the following restriction: the vessel is limited in their route to operating only in Lakes, bays, and sounds until the repair is completed to the satisfaction of the attending marine inspector. 46 CFR 176.25-50

The marine inspector shall require that all observed unsafe practices and hazardous situations be corrected. Vessel deck water pump and motor assembly utilized as the fire pump required by 46 CFR 181.10-1 is not mounted properly. 46 CFR 176.25-50

All wiring and cables shall be suitable for marine service and the particular insulation used. Conductors, including generator leads and shore power cables shall be protected in accordance with current carrying capacities. Vessel is providing shore power through extension cord to vessel lighting system via GFCI outlet. 46 CFR 183.10-20 and 46 CFR 183.10-35

The 5 gallon hand fire pump shall be equipped with a suction and discharge hose suitable for use in firefighting. Vessel hand pump was not equipped for suction hose as required by regulation. 46 CFR 181.10-5

Remote fuel shut off stations shall be marked indicating the purpose of the valve and the direction of operation. Vessel fuel shut off did not indicate direction of operation. 46 CFR 185.30-20

The master and crew of a vessel will be familiar the content of the emergency instruction and have them mounted at the operation station. Vessel did not have the emergency instructions mounted at the operating station. 46 CFR 185.510 a

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel is missing NAVTEX receiver and needs updated FCC certificates to reflect the new installation. NAVTEX is currently on backorder. Restricted to <100nm from shore until NAVTEX is installed and is verified with FCC Communications Act Safety Radiotelephony Certificate along with newly installed SSB-DSC radio.

Vessels operated solely within one hundred nautical miles of shore must be equipped with a medium frequency transmitter capable of transmitting J3E emission and a receiver capable of reception of J3E emission within the band 1710 to 2850 kHz, in addition to the VHF radiotelephone installation required by paragraph (a) of this section, and must maintain a continuous watch on 2182 kHz. Additionally, such vessels must be equipped with either: (1) A single sideband radiotelephone capable of operating on all distress and safety frequencies in the medium frequency and high frequency bands listed in § 80.369(a) and (b), on all the ship-to-shore calling frequencies in the high frequency bands listed in § 80.369(d), and on at least four of the automated mutual-assistance vessel rescue (AMVER) system HF duplex channels (this requirement may be met by the addition of such frequencies to the radiotelephone installation required by paragraph (b) of this section). Vessel only had a VHF radio with an oceans route. Install proper radio requirements in accordance with CFR. 47CFR80.880(b)

On vessels of less than 20 meters in length, the sidelights, if necessary to meet the requirements of § 84.15, shall be fitted with matt black inboard screens. Vessel had high gloss inboard screens. Paint screens to meet the matt black requirements to the satisfaction of the attending marine inspector.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the Federal Communications Commission, as set forth in 47 CFR part 80. Vessel had an expired Safety Radiotelephony Certificate. Present a current Safety Radiotelephony Certificate to the USCG Marine Inspector for rectification. Rectify this item by 07 March 2022. 46 CFR 184.502

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. Found cracked weld on Stbd engine's rear mount on the outboard side. Repair to the satisfaction of the USCG Marine Inspector. 46 CFR 182.200

A vessel with installed toilet facilities must have a marine sanitation device that complies with 33 CFR part 159. Connections from toilet to drainage lines show deterioration and signs of leakage. Large baseball size bulge found in ventilation pipe of MSD. Repair MSD system to the satisfaction of the USCG Marine Inspector. 46 CFR 184.704

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level. Vessel has no audible and visual alarm for the Fuel Tank Void space. Vessel has no visual alarm in the FWD Void and Rudder spaces. Only the Engine Room bilge alarm is operational as designed. An additional deckhand is required to solely perform bilge watch until all bilge alarms are restored as designed. This requirement must be met while vessel is underway. 46 CFR 182.530

All vessels in lieu thereof, a screw-down valve without a reach rod may be fitted to the bilge line on the after side of the collision bulkhead if it is readily accessible under service conditions. Valve located aft of collision bulkhead has an inoperable reach rod. Valve must be repaired as per attending Marine Inspector. 46 CFR 182.25-5(d)

Each life float shall be fitted with a lifeline and shall be equipped with two paddles, a water light and paitner. Vessel's six person life float is missing a light. Vessel will reduce passenger count to forty-four passengers during night-time operations. 46 CFR 180.15-1(a)

All vessels shall be provided with an approved adult type life preserver for each person carried. Each PFD must have at least 200 sq. cm. of retrorefelctive material on the front and back side IAW 46 CFR 160. Limit passenger count from forty-nine to forty-seven until 02 PFD's are replaced or repaired. 46 CFR 180.25-5(a)

The number and identification of the items stowed inside, and their sizes, must be clearly marked and legible with letters and numbers for lifejackets. Vessel is required to mark and label all PFD stowage containers IAW the attending Marine Inspector. 46 CFR 185.604(f)

Each EAP education program must include a display and distribution of a community service hot-line telephone number for crewmember assistance. Correct to the satisfaction of the attending Marine Inspector. 46 CFR 16.401(a)

46 CFR 181.15-10: Fire hose may be commercial fire hose or equivalent of not over 1/2 inch diameter or garden hose of not less than 5/8 inch nominal inside diameter. Hose shall be in one piece not less than 25 feet and not more than 50 feet in length. Vessel has two garden hose leaking; both require replacement.

46 CFR 180.25-10: (a) Life preservers shall be distributed through the upper part of the vessel in protected places convenient to the persons on board. (c) Life preservers for children, when provided, shall be stowed separately. All adult and child lifejackets onboard were not convenient to persons onboard/readily accessible; lifejackets were stowed under aluminum enclosed stairs forward of accommodation space.

The part of the exhaust system between the point of cooling water injection and engine manifold shall be water jacketed of effectively insulated. Port main propulsion engine exhaust was found uninsulated prior to water injection.

At each inspection for certification and re-inspection the marine inspector shall examine the bilges and other spaces to see that there is no accumulation of oil or other matter which might create a fire hazard. Approximately 4-6" of oily waste was found the the engine room bilge.

Vessels operating 20 NM to 100 NM require (1) MF radio. Vessel does not have a MF/HF marine radio. Restricted to no more than 20 NM offshore.

Port side exhaust pipe has corroded, causing hole. Crop and renew piping prior to operation.

Three areas of corrosion/wastage of bottom plating in stern void. Crop and renew during next haul-out scheduled for 14 May 2018 if wastage limits are exceeded.

At each inspection for certification and at every vessel inspection all observed unsafe practives, fire hazards, and other hazardous situations must be corrected. Flammable oils stored in the engine room.

A vessel must have two independent means on controlling each propulsion engine. Controls must be provided for engine speed, direction of shaft rotation, and engine shutdown. One means may be the abilty to disconnect the remote engine linkage to permit local operation. Bolt on engine throttle must be made permanent.

SPVs must comply with applicable FCC requirments for radio and EPIRB installations. Radio installations must be installed in accordance with FCC regulations. A report from a authorized FCC technitions is required to ensure proper repairs to damaged equipment.

Electrical equipment on a vessel must be installed and maintained to protect vessel from electrical hazards including fire, cause by or orginating in electrical equipment, and electrical shock. Vessel has exposed wires in engine room.

Electrical equipment on a vessel must be installed to protect passengers, crew, other persons and the vessel from electrical hazards. Including fire, caused by or originating in electrical shock. Light on deck damaged and has exposed wires.

On sport fishing vessels where it would can be shown that higher rails would interfere with the normal operation of the vessel, rails of at least 30 inches in height may be permitted. Rails need to be replaced to original height. 46 CFR 177.35-1

All vessels must have navigation lights that are in compliance with the applicable sections in the international/ inland navigation rules. Port, Starboard, Stern and anchor lights are not in accordance with 33 CFR 83.21 (a)(b)(c). Restricted to daylight operations.

Each window, port hole, and its means of attachment to the hull or deckhouse must be capable of withstanding the measured load from wave and wind conditions expected due to its location on the vessel and the authorized route of the vessel. Windows are severly cracked. Restricted to lakes, bays and sounds route.

Penatrations and openings in water bulkheads must have means to make them watertight. Forward void gooseneck vent damaged and will not prevent downflooding.

Penetration in forward superstructure must be repaired in accordance with 177.300 (d)

Life rings must be readily assessable and stowed in a way that can be readily cast loose. Life rings bracket is damaged and unable to be properly stowed.

No major repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge and approval of the Officer in Charge, Marine Inspection. Found holes in hull plating. The holes are located in the deck plate insert forward most part of bow next to the rub rail port side.

Port Rub rail pipe at foremost point of the rub rail at the bow peak of the vessel. A blow through hole on the Port upper most part of hull plating inside of old insert at the bow of the ship. A blow through hole on the Stbd upper most part of hull plating inside of old insert at the bow of the ship.

Chart on board is outdated, uncorrected and of inadequate scale for safe navigation.

1. Vessel has 16"(length) x 6"(height) x 3" (depth) inset damage to hull plating and reflecting longitudinal framing damage on interior. Crop and renew starboard damage on hull plating, longitudinal and transverse frames to satisfaction of attending marine inspector.

Bilge screw-down reach rod closing device for collision bulkhead is fractured and inoperable. Repiar/renew closure device.

Vessel has 10" of damage to rub rail on starboard side. Repair to the satisfaction of attending marine inspector.

Vessel had excessive hydrolic fluid in steering gear compartment. Remove and clean compartment to the satisfaction of the attending marine inspector.
