Vessels starboard engine shutdown during operation. Repair/replace as needed to bring port engine back to full operational capabilities.

On 06 February 2021 at approx. 23:15 local time the crew of the ITV KELLE B was conducting checks prior to getting underway at Green River mile 42 when it was discovered that the starboard generator would not start. Vessel authorized to move deadship to EMS facility at Ohio River mile 786 to conduct repairs. Repairs and any temporary arrangements for a temporary generator shall be conducted to the satisfaction of the vessel's TPO and reported to Coast Guard Sector Ohio Valley before placing the vessel back into service.

All seals and gaskets must be properly maintained to prevent leaks of flammable liquid; STBD MDE Racor selector valve is dripping approx. 1 drop every 30 sec from the stem. STD Gen fuel supply piping has fuel leaching at hose to pipe coupling. Port MDE fuel return line on fwd upper portion of engine show signs of leaching.

At least two lifebuoys on a towing vessel greater than 26 feet must be fitted with a floating electric water light and attached by the lanyard with a corrosion-resistant clip to allow the water light to be quickly disconnected from the lifebuoy. The clip and line must have at least a breaking strength of 50 lbs.

When conducting a survey of a towing vessel, the surveyor must determine that the item or system functions as designed, is free of defects or modifications that reduce its effectiveness, is suitable for the service intended, and functions safely in a manner consistent for vessel type, service and route. A visual examination and tests of the vessel and its equipment and systems in order to confirm that their condition is properly maintained and that proper quantities are onboard. Vessel’s last [name withheld] stated that vessel wasn’t equipped with Pressure Vessels > 5 Cubic Feet; MI noted that vessel was equipped w/ 2 Pressure Vessels > 5 Cubic Feet. Survey also indicated the presence of distress signals, but they were not onboard during inspection. Additionally, the checklist indicated the presence of ring life buoys and the waterlight arrangement was "not applicable". Corrective action plan needs to be created to address the discrepancy.
