RCT 101

Barge rep reported while on loading cargo it was discovered that the #1 and #3 wing void had water ingress. Location of water entry is currently unknown. Barge is restricted from conducting any cargo operations. Vessel is authorized to transit from Kinder Morgan Chicago to IMT shipyard. 03 pumps are to be staged on deck of barge and voids are to be routinely monitored during transit. Conduct permanent repairs to the satisfaction of the attending Marine Inspector. **UPDATE 09MAY22** Barge has been authorized to be moved from Lemont, IL to Geismar, LA for permanent repair. Barge is still not authorized to conduct any cargo operation until permanent repair has been completed. Barge is to be hauled out for dry dock NLT 20JUN22. Repairs are to be conducted to the satisfaction of the attending Marine Inspector. Any deviation of the submitted tow plan shall be reported to the nearest OCMI zone.

The transfer procedures required by 33 CFR 155.720 must contain all contents of 33 CFR 155.750 in the order listed or by use of a cross reference-index page. Vessel transfer procedures did not include all contents and procedures were not in the order/did not contain a cross-reference index page.

The #1 Port void at the deck line, on the hull, has a sharp inset that has caused a crack in the plating. Also the #3 port void has a 6" crack, on the deck, at the deck roll. Provide a marine chemist certificate and a repair proposal to the Coast Guard. Once approved, by the Coast Guard, effect repairs to the satisfaction of the attending Coast Guard Inspector.

Reach rods must have a means of indicating if the valve is open or closed. The valve indicator on the #3 cargo tank is missing. Repair or replace the valve indicator.

Vessel must have approved Vessel Security Plan. Vessel must provide an approved VSP prior to cargo operations

Noted numerous sideshell and deck gunnel insets that are identified on attached work list (company Surveyor) and additional call out by marine inspector (port corner, sharp inset and fracture). The fracture was temporally welded for transit and is to be cropped and re-new to original.

Found High Level / Overfill Alarms inoperable during COI Inspection. Issued requirement to repair and present for test on 20OCT10.

3' X 3' temp lap repair in bow slope sheet, stbd side, approximately 2' above water line. Issued PTP for transit to CNC Main Yard, New Orleans. Vessel requires significant repairs, exceeding capabilities of Baton Rouge shipyards.

BR01 Complete required inspections (COI expired vessel is loaded)

REVISE TRANSFER PROCEEDURES TO COMPLY WITH 33 CFR 155.750, SPECIFICALLY, REVISE LIST OF PRODUCTS TRANSFERED TO MATCH COI AUTHORIZATION, DESCRIBE THETRANSFER SYSTEM ON THE VESSEL AND INCORPORATE OR REFER TO PROCEEDURES AND CHECKS FOR VAPOR RECOVERY SYSTEM.

INSTALL VALVE POSITION INDICATORS ON CARGO VALVES AT NEXT CTI (17JUL00).

Make permanent repairs to puncture in deck plate in way of aft stbd timberhead, outboard.

Make permanent repairs to aft stbd timberhead now found missing with base pad slightly separated from deck plate, outboard side.

THE VAPOR CONTROL SYSTEM IS NOT TO BE USED UNTIL THE HIGH LEVEL ALARMS ARE OPERATIONAL.

Provide documentation of existing pressure vacuum valve or replace requiredone with the settings outlined in 46 CFR part 39. Vapor control system is not to be used until item is corrected.

Provide oil tranfer procedures for the vapor control system IAW 33 CFR 155.750 (d)-(7). Vapor control system is not to be used until item is corrected.

OFFLOAD CARGO AND PRESSURE TEST CARGO PIPING TO 1.5 X MAWP PRIOR TO_x001C_ LOADING CARGO AND NO LATER THAN 28AUG92.

#2 CENTERLINE CARGO TANKTOP IS BUCKLED THROUGH IT'S ENTIRE LENGTH FROM AN IMPLOSION. TANKTOP EXTERNAL FRAMING IS WARPED. PRIOR TO LOADING CARGO MAKEVSL AVAILABLE FOR INTERNAL STRUCTURAL EXAM, CARGO TANK INTERNAL EXAM & MAKEPERMANENT REPAIRS TO THE SATISFACTION OF THE ATTENDING MARINE INSPECTOR
