A vessel must have one or more ring life buoys as follows: A vessel of more than 65 feet in length must carry a minimum of three ring life buoys. At least one ring life buoy must be fitted with a lifeline. If more than one ring life buoy is carried, at least one must not have a lifeline attached. Each ring life buoy with a floating waterlight must have a lanyard of at least 3 feet in length but not more than 6 feet, securing the waterlight around the body of the ring life buoy. The name of the vessel must be marked or painted in clearly legible letters and numbers: clearly legible block capital letters with the vessel's name. Each ring life buoy must be marked with Type II retroreflective material in accordance with 46 CFR 185.604. Vessel is fitted with three buoys; the port bow buoy has a waterlight incorrectly attached and lacks markings, including approval standard per 46 CFR 160.050. The starboard bow buoy requires retroreflective material and markings, and the aft buoy requires approval per 46 CFR 160.050. 46 CFR 180.70

Each disposable hydrostatic release unit (HRU) must be marked with an expiration date of two years after the date on which the unit is installed. The EPIRB was marked for three years, and was found to be expired. Vessel operator had a new HRU available. 46 CFR 185.740(b)

As appropriate for the intended voyage, a vessel must carry adequate and up-to-date: Charts of large enough scale to make safe navigation possible. Vessel's crew was not able to verify up-to-date electronic charts. 46 CFR 184.420(a)(1)

When operating a vessel on a body of water where the discharge of treated or untreated sewage is prohibited. Acceptable methods of securing the device include— Closing the seacock and removing the handle;(2) Padlocking the seacock in the closed position;(3) Using a non-releasable wire-tie to hold the seacock in the closed position; or(4) Locking the door to the space enclosing the toilets with a padlock or door handle key lock. Piping appears to be configured to bypass MSD tank and was missing required securing devices. Discharge piping was wasted and fractured. Rectify to the satisfaction of the MI. 46 CFR 184.704 33 CFR 159.7(c)

At each inspection for certification and reinspection the marine inspector shall examine the bilges and other spaces to see that there is no accumulation of oil or other matter which might create a fire hazard. Observed excessive oil in the bilges under the main engine. Remove and clean to the satisfaction of attending marine inspector. 46 CFR 176.25-50(b)(Old T)

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment other than small buoyancy compartments, under all practical conditions. Vessel's fixed bilge system was inoperable and unable to drain water from spaces. Correct to the satisfaction of attending marine inspector. 46 CFR 182.25-1(a)(Old T)

Penetrations and openings in watertight bulkheads must have means to make them watertight. In the crew accommodation space there are two penetrations aft port, one forward port, and two aft starboard that are not watertight. Correct to the satisfaction of USCG marine inspector. 46 CFR 177.13-1(Old T) 46 CFR 171.114(a)(2)(Old S)

The cockpit must be watertight. There are two leaks, port and starboard, in way of the companionway decks outboard of crew accommodations that are leaking into the voids below deck. Correct to the satisfaction of the USCG marine inspector. 46 CFR 177.13-1(Old T) 46 CFR 171.145(a)(Old S)

Examine all accessible parts of the exterior and interior of the hull, the watertight bulkheads, and weather deck. Where the internals of the vessel are completely concealed, sections of the lining or ceiling may be removed or the parts otherwise probed or exposed so that the marine inspector may satisfy himself as to the condition of the hull structure. Vessel had two leaks in deck bulk head of the Tool Room and one leak through the deck bulkhead of the forepeak void through the deteriated windlass. Correct to the satisfaction of the USCG marine inspector. 46 CFR 176.25-5(a)(1)(Old T)

Water for cooling the exhaust pipe must be obtained from the engine cooling water system or a separate engine driven pump. The center and starboard main engines are being primed via a garden hose attached to a water tank. Correct to the satisfaction of USCG marine inspector. 46 CFR 182.425 (b)(1)(Old T)

All vessels with crew members living on board shall be provided with crew accommodations of sufficient size, adequate construction, and with suitable equipment to provide for the protection and accommodation of the crew in a manner practicable for the size, facilities, and service of the vessel. Bolt was missing on the the plate for the strap down of the refrigerator. Correct to the satisfaction of USCG marine inspector. 46 CFR 177.25-1(a)(Old T)

No vessel subject to inspection under 46 U.S.C. 3301 will be navigated unless it is under the direction and control of an individual who holds an appropriate license or officer endorsement on his or her MMC. Master was unable to provide medical certificate and submittal documentation. Master to provide documentation prior to operating vessel to the satisfaction of USCG marine inspector.

The master shall conduct sufficient drills and give sufficient instructions to make sure that all crew mem]trers are familiar with their duties during emergencr es that necessitate abandoning ship or the recovery of persons who have fallen overboard. MOB drills need to be conducted to satisfaction of lnspector.

A vessel must be provi ded with a satisfactory arrangement for draining any watertight compartment, other than small buoyancy compartments, under all practicable conditions. Sluice valves are not permitted in watertight bulkheads. Conduct complete test of bilge suction system.

Each survival craft must be (1) Secured to the vessel by a paln ter with a ffoat-free link permanently attached to the vessel except that a float-free link is not required if the vessel- operates only on water deep as the length of the painter HRU was improperly installed and needs to be installed properly to satisfaction of inspector.

Vessel must carry one floating waterlight, unless it is limited to daytime operation, in which case no floating waterlight is required. Each floating waterlight installed after March 11, 1997, on a wessel carrying only one ring buoy, must be attached to the lanyard with a corrosion-resistant clip. The clip must hawe a strength of at least 22.1 ktLograms (50 pounds), and affow the waterlight to be quickly disconnected from the ring life buoy. Floating fight was not properly installed to buoy. Needs to be installed properly to the satisfaction of inspector.

No major repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment shall be made without the knowledge and approval of the Officer in Charge, Marine Inspection. Noted a buckled frame on the port side while conducting internal structural exam in the forepeak space. In addition, noted inset on the starboard bow approximately 2 feet wide and 2 inches deep. prepare repair proposal and complete repair to the satisfaction of attending marine inspector.

The marine inspector shall examine the bilges to see that there is no accumulation of oil. Noted oily water accumulation in engine room and steering room in the bilge. Clean bilges to satisfaction of marine inspector. 46 CFR 176.25-50(b)

The marine inspector shall examine and test the following items to the extent necessary to determine that they are in proper operating condition, safe electrical condition, and fit for the service intended. Noted multiple electrical outlets that were in an open ground and hot/neutral rev condition, and daisy chaining multi-outlet connectors. Repair to correct status. 46 CFR 176.25-15(a)

On a vessel of more than 65 ft in length, the public address system must be a fixed installation and be audible during normal operating conditions throughout the accomodation spaces and all other spaces normally manned by crew members. No overnight passengers until rectified.

Fuel tank vent pipes must be fitted with removable flame screens. Noted the flame screens for fuel tank vent pipes were clogged. Repair or replace flame screens. Vessel rep informed Marine inspector that the portside fuel ventilation goose-neck piping had sheered off when attempting to replace the flame screen. Provide a repair proposal and repair or replace to the satisfaction of the attending Marine Inspector.

Each life float and buoyant apparatus must be secured to the vessel by a painter and a float-free link. Inflatable liferaft installed as a substitute for life float or buoyant apparatus as allowed by 180.10-30(c) was only secured by a hydrostatic release unit. Crew was educated on proper inflatable life raft stowage and painter was attached to vessel on the spot.

All escape hatches and other emergency exits shall be marked on both sides using at least 1-inch letters: "Emergency Exit, Keep Clear." Doors on either side of the accompdation space and overhead hatches in the generator room and engine room require proper labeling. 46 CFR 185.30-15

A vesel must carry either a first-aid kit approved under 160.041 or a kit with equivalent contents and instructions. First aid kits onboard are missing multiple required items including ammonia inhalants, iodine applicators, tourniquet, and eye dressing. Replace all missing items to make a complete kit as required by 160.041.

All vessels over 26 feet in length shall be provided with individual bilge lines and bilge suctions for each watertight compartment. Engine workshop has 2 suction points due to centerline fuel tanks preventing communication from one side of the bilge to the other. Starboard side pickup could not be proven functional. Repair and demonstrate functionality. Lazarette pickup also needs to be tested in presence of USCG. 46 CFR 182.25-5

Vessels operated more than 100 NM but not more than 200NM from the nearest land, be equipped with a NAVTEX receiver conforming to the following performing standards. Vessel is restricted to 100NM until radio installation meets marine inspector satisfaction. Vessel is not equipped with NAVTEX printing capability or approved alternative.

Each disposable hydrostatic release unit must be marked with an expiration date of two years after the date on which the unit is installed. Hydrostatic release is missing expiration date. Vessel is limited to carry 25 passengers until def is rectified.

Fire fighting equipment and appliances- all vessels shall be provided with a hand operated portable fire pump having a capacity of at least 5 gallons per minute. Vessel could not provide a portable hand fire pump. 46 CFR 181.10-5

Other structural condition- Except for engine exhausts each inlet or discharge pipe that penetrates the hull below a line drawn parallel to and at least 6 inches above the deepest subdivision load line must have means to prevent water from entering the vessel if pipe fractures or otherwise fails. Overboard discharge piping for forward shower has no means to secure or prevent the flow of water in. At vessels deepest draft discharge is within 6 inches of waterline. 46 CFR 171.119 (b)

Route restricted to less than 3NM from shore. The EPIRB hydrostatic release unit has expired. Each expired life saving item shall be replaced during the annual inspection. Replace the EPIRB hydrostatic release.

Fuel shutoff valves were labelled improperly. Remote fuel shutoff valves must be labelled with direction of operation. Correctly label shutoff valves.
