Nothing in this subpart shall be construed as limiting the inspector from making such tests or inspections as he deems necessary to be assured of the safety and seaworthiness of the vessel. The liftboat's forward port leg and starboard aft leg are due for 10-year leg inspections as per Enclosure 2 of CG-543 Policy Letter 07-02 on December 12, 2023. Vessel shall commence leg inspections by January 12, 2024.

As a result of hitting the Cameron jetty rocks, vessel sustained damage in way of insets in the #2 and #3 starboard ballast tanks as well as possible holes in the starboard #2 void that is currently temp repaired with an epoxy patch. No other breaches of the side or bottom plating has been observed. Damage to leg pads is suspected but unknown at this time. Vessel may proceed to yard in Galveston, Tx to make permanent repairs to hull and to further inspect leg pads. All repairs and inspections shall be made to the satisfaction of the cognizant OCMI / attending Marine Inspector.(705: prior to commercial operations)

All lift boats should be fitted with a level indicator to indicate level height and distinct alarm to warn of uneven leg sinkage when elevated. Marine Inspector observed that the level alarm was inoperable due to excessive corrosion between the metallic components required to make contact and cause the alarm to activate. Per NVIC 8-91 Enclosure (3).

The minimum number of approved 30-inch ring life buoys and the minimum number of which shall have water lights attached, shall be in accordance with Table 94.43-10(a). A minimum of two out of the three required ring life buoys on board shall have water lights attached. Marine Inspector observed only one ring life buoy with a water light attached. (OLD-I) 46CFR94.43-10(a)

Inflatable liferafts shall be stowed in such a manner that they will float free in the event of the vessel sinking. Marine Inspector observed the liferaft painter lines improperly secured to the tie-down strap connected to the pelican hook instead of being properly secured to the weak link which would prevent the liferafts from floating free if the vessel was to sink. (OLD I) 46CFR94.15-10(c)(4)

It shall be the duty of the master or person in charge to see that the lifeboats, rescue boats, liferafts, lifefloats, and buoyant apparatus are properly maintained at all times, and that all equipment for his vessel required by the regulations in this subchapter is provided, maintained and replaced as indicated. Marine Inspector observed retro-reflective tape strips on the rescue boat were damaged and/or worn and need to be replaced. (OLD I) 46 CFR 97.15-45

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. Marine Inspectors found the vessel rescue boat inoperable, crew was unable to start the rescue boat due to a failure with the internal computer chip. Effect permanent repairs to the affected equipment to the satisfaction of the attending US Coast Guard Inspector. 46 CFR 199.190

At least one-half the total number of lifebuoys on the vessel must each be fitted with a self-igniting light approved under approval series 161.010. Marine Inspectors found the starboard lifebuoy with an inoperable light. Vessel is to repair or replace the light to the satisfaction of the attending US Coast Guard Inspector. 46 CFR 199.70(a)(3)(ii)

Containers shall be embossed or bear a brass or equivalent corrosion-resistant name-plate, or otherwise be suitably and permanently marked, to plainly show in letters not less than ½ inch high the following wording: “HAND RED FLARE DISTRESS SIGNALS”. Marine Inspectors found the distress signals stowed in an unmarked watertight container. Vessel is to properly mark the stowage container to the satisfaction of US Coast Guard Marine Inspector. 46 CFR 160.021-6(c)

Electric installations on vessels must ensure: (2) protection of passengers, crew, other persons, and the vessel from electrical hazards. Marine Inspectors found lighting fixture within the galley hood to be missing its protective cover. Vessels to replace or repair fixture to the satisfaction of the attending US Coast Guard Marine Inspector. 46 CFR 111.01-1(a)

Monthly inspections required by NFPA 10 may be conducted by the owner, operator, person-in-charge, or designated member of the crew. Marine Inspectors determined that the vessel was not conducting monthly fire extinguishing equipment checks. Vessel is to begin conducting and logging monthly inspections to the satisfaction of the attending US Coast Guard Marine Inspector at the next COI inspection. 46 CFR 91.25-20(a)(1)(ii)

Each pump must be capable of delivering water simultaneously from the highest outlets at a Pitot tube pressure of approximately 50 PSI. The vessel was lowered into the water to run the installed pump (IAW 46 CFR 95.10-5(a)). The pump was air bound and the crew had to get a gallon of water to prime the pump, but were not able to open the plug with the pipe wrench. Furthermore, once the system was running, a crew member stated that the circuit breaker protecting the installed fire pump system would trip and secure power to the installed fire pump system. Finally, the fire hose attached to the starboard side fire hydrant on the main deck had two metal bands and a cam-lock connection to the fire hydrant. Water was observed spraying out of the connection during the operation of the two educator style fire pumps. Effect permanent repairs to the impacted systems to demonstrate an operable installed fire pump and a suitable fire hose to the satisfaction of the attending Marine Inspectors. 46 CFR 95.10-5(c)

Fire drills must, as far as practicable, be planned with due consideration given to the various emergencies that may occur for that type of vessel its cargo. Marine Inspectors requested that a fire drill be conducted in the laundry room. Marine Inspectors observed that the natural ventilation hatch was not secured on the bulkhead, the mariner with the portable fire extinguisher was inside the space impacted by fire during the entire drill, and the Marine Inspectors did not observe effective communications between the wheel house and the crew on deck. Conduct satisfactory fire drill in the presence of the Marine Inspectors. 46 CFR 97.15-35 46 CFR 199.180(f)(1)

The position of the rudder, if power-operated, must be shown at the principal steering station. During a test of the vessel’s rudder system at the operating station, Marine Inspectors observed that the rudder angle indicator depicted a rudder swing from 50 degrees starboard to only 20 degrees port. [name withheld] stated that there were rudder stops in place to prevent the rudder to swing beyond 20 degrees port. Demonstrate that the rudder system and its associated equipment are operational to the satisfaction of the attending Marine Inspectors. 46 CFR 96.05-1(a) 46 113.40-5

Each EPIRB and each SART, other than an EPIRB or SART in an inflatable life raft, must be tested monthly. The EPIRB must be tested using the integrated test circuit and output indicator to determine that it is operative. Marine Inspectors did not observe a record of the monthly check of the EPIRB, and the Master stated that a record was not kept to verify that a monthly check of the EPIRB was completed. Complete a test of the EPIRB per the monthly test requirements and log that test and future tests to the satisfaction of the attending Marine Inspectors. 46 CFR 96.06-1 46 CFR 199.190(e)(2)

Fire pumps must be fitted with a pressure gage on the discharge side of the pumps. Marine Inspectors observed that the fire hydrant located on the main deck at the base of the forward bulkhead of the super structure (starboard side) had a pressure gauge that was inoperative. The gauge indicated 00 PSIG even though water was being supplied to that hydrant and through its associated hose by the vessel’s two educator style fire pumps. Effect permanent repairs to the impacted equipment to the satisfaction of the attending Marine Inspectors. 46 CFR 95.10-5(e)

Marine Inspectors observed that galley refrigerators were not secured to the bulkhead or deck, thereby causing an unsafe condition to the vessel crew. Effect modifications to ensure the refrigerators are secured to prevent shifting to the satisfaction of the attending Marine Inspectors. 46 CFR 91.27-15

Alcohol test strips (Drug and Alcohol program required) have expired. Replace prior to departure.

Public Announcement system need in conjunction with general alarm is inoperable. General alarm is functioning. Repair PA system within 30 days from this exam.

Main fire pump motor not working. (Submersible fire pump is operational, but not suitable when vessel is underway). Repair fire pump motor and prove proper operation of the main fire pump.

Main switchboard does not have a functioning ground detection system or a "power available" indicator lamp. Repair both within 30 days from this exam and prove proper operation to the satisfaction of the attending marine inspector.

Multiple portable fire extinguishers found on board did not have attached test/inspection data, bringing into question the validity of the third party's(Island Fire & Safety Co.,Inc) inspection report. Provide proof that the required annual inspection of the portable fire extinguisher have been completed to all installed extinguishers.

Starboard leg tower forward brace uppoer weld was not complete and contained a 1/2" gap. Shipyard to repair and 3rd party to complete NTD.

Load Line marks were missing.

Engine room WTD gasket was installed improperly. Crew to replace

Vessel name and hailing port on stern needs renewed.

Quantity of life buoys was 3 IAW old subchapter I. Vessel requires 8 IAW chapter L all OSV site.

Portside strut propeller shaft water lubricated bearing was damaged by line. Replace shaft bearing prior to departing Amelia, LA to the satisfaction of the attending marine inspector.

Rudder angle indicator is not functioning or indicating rudder position.

CO2 bottle found empty. Inspect, recharge and replace.

All manual closure doors for engine room vents must be maintained or repaired to allow proper closure.

Update fire fighting plan to show current arrangment of Fire fighting and Lifesaving equipment.

Vsl to prove proper operation of all steering and propulsion systems prior to conducting operations.

Battery boxes were not secured or properly covered

Laundry dryer ventilation duct was crushed and exhaust was venting into laundry space

Three extinguisher were not serviced

Self closing fire doors did not fully close on 3 decks in the stairwell

E-lighting fixture in galley was inoperable

PML's expired or missing

Fuel line for Gen-set was leaking IWO port side engine room fwd bulkhead.

First aid kit contained expired medicines

Repair inoperable showers. The vessel is limited to 24 persons on board until repairs are complete.

Remove file cabinet from upper deck passage. Cabinet is not secured to prevent movement and obstructs an egress route.

Renew deteriorated and broken electrical junction box IWO 02deck aft house bulkhead light fixture above external walkway

Renew broken flourescent light fixture in port engine room above access ladder.

Permanently install wiring for the power to the load indicator and radio charger for the forward crane.

Repair leaking fuel return line in way of piping return to the fuel storage tank.

Provide corrected charts for vessel operating area.

Provide documentation for minimum and maximum load angle proof tests.

Replace seal on start air regulator for the starboard main engine.

Use proper pressure connectors and properly deadend wiring in the pilot house console.
