The liftboat's forward port leg and starboard aft leg are due for 10-year leg inspections as per Enclosure 2 of CG-543 Policy Letter 07-02 on December 12, 2023. Vessel shall commence leg inspections by January 12, 2024.

As a result of hitting the Cameron jetty rocks, vessel sustained damage in way of insets in the #2 and #3 starboard ballast tanks as well as possible holes in the starboard #2 void that is currently temp repaired with an epoxy patch. No other breaches of the side or bottom plating has been observed. Damage to leg pads is suspected but unknown at this time. Vessel may proceed to yard in Galveston, Tx to make permanent repairs to hull and to further inspect leg pads. All repairs and inspections shall be made to the satisfaction of the cognizant OCMI / attending Marine Inspector.(705: prior to commercial operations)

All lift boats should be fitted with a level indicator to indicate level height and distinct alarm to warn of uneven leg sinkage when elevated. Marine Inspector observed that the level alarm was inoperable due to excessive corrosion between the metallic components required to make contact and cause the alarm to activate. Per NVIC 8-91 Enclosure (3).

Each pump must be capable of delivering water simultaneously from the highest outlets at a Pitot tube pressure of approximately 50 PSI. The vessel was lowered into the water to run the installed pump (IAW 46 CFR 95.10-5(a)). The pump was air bound and the crew had to get a gallon of water to prime the pump, but were not able to open the plug with the pipe wrench. Furthermore, once the system was running, a crew member stated that the circuit breaker protecting the installed fire pump system would trip and secure power to the installed fire pump system. Finally, the fire hose attached to the starboard side fire hydrant on the main deck had two metal bands and a cam-lock connection to the fire hydrant. Water was observed spraying out of the connection during the operation of the two educator style fire pumps. Effect permanent repairs to the impacted systems to demonstrate an operable installed fire pump and a suitable fire hose to the satisfaction of the attending Marine Inspectors. 46 CFR 95.10-5(c)

Fire drills must, as far as practicable, be planned with due consideration given to the various emergencies that may occur for that type of vessel its cargo. Marine Inspectors requested that a fire drill be conducted in the laundry room. Marine Inspectors observed that the natural ventilation hatch was not secured on the bulkhead, the mariner with the portable fire extinguisher was inside the space impacted by fire during the entire drill, and the Marine Inspectors did not observe effective communications between the wheel house and the crew on deck. During a test of the vessel’s rudder system at the operating station, Marine Inspectors observed that the rudder angle indicator depicted a rudder swing from 50 degrees starboard to only 20 degrees port. The EPIRB must be tested using the integrated test circuit and output indicator to determine that it is operative. Marine Inspectors did not observe a record of the monthly check of the EPIRB, and the Master stated that a record was not kept to verify that a monthly check of the EPIRB was completed.
