Each length of fire hose must have a nozzle that has a 1/2 inch solid stream orifice or a combination nozzle. The nozzle must be bronze or another material that has strength and corrosion resistance properties equal to those of bronze. Starboard fire main nozzle not opening and closing as designed. Replacement or repair required. 46CFR181.15-10(d)(2)

Main propulsion machinery installed on each vessel is suitable for the vessel and its operation. Starboard aft MDE transmission oil cooler hose wasted. Requires renewal. 46CFR182.01-5(a)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. An unknown amount of damage may have been sustained to hull and running gear after running hard aground in Lahaina. A drydock is required to complete a full and satisfactory Coast Guard inspection of the vessel and to make the necessary repairs. No passengers are to be carried prior to issuance of credit drydock. 46 CFR 177.100

A person may not employ or engage an individual in a position required to hold and MMC unless that individual maintains a current medical certificate. Captain had expired medical certificate. 46 CFR 15.401(c)

Electrical equipment used in the following locations must be dripproof. Electrical equipment exposed to corrosive environments must be of suitable construction and corrosion resistant. Battery terminals and wires at terminal corroded. 46 CFR 183.210

Electrical equipment must be installed to protect from electrical hazards. Junction box not attached to bulkhead. 46 CFR 183.200

Vessel must undergo a drydock and internal structural examination (ISE) at least once every two years. Vessel operator notified [name withheld] that the vessel will not be meeting the required drydock ISE inspection due to Lahaina Wildfires. Notified operator that missing drydock inspection will invalidate COI and require new COI inspection. Carriage of passengers prohibited until credit drydock inspection, ISE, and COI inspection are completed. 46 CFR 176.600(a)(1)

At each initial and subsequent inspection for certification of a vessel, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of lifesaving equipment and systems, including the following: Inspection of each hydrostatic release unit to determine that it is in compliance with the servicing and usage requirements of 185.740 of this chapter. Hydrostatic release for EPIRB expired. Install new hydrostatic release. 46 CFR 176.808(a)(5)

All machinery and equipment must be installed and maintained in such a manner as to afford adequate protection from causing fire, explosion, machinery failure, and personnel injury. 02 pipes on port side aft engine room cooling exhaust water discharge are holed and leaking. Provide new piping for holed sections of piping. 46 CFR 182.100

Electrical equipment on a vessel must be installed and maintained to minimize accidental personnel contact with energized parts. Dead end wires found near upper deck pilothouse. Remove dead end wiring. 46 CFR 183.200 (c)

Electrical equipment on a vessel must be installed to protect the vessel from electrical hazards, including fire. Multiple plugs at helm plugged into multi plug outlet extender. Remove multi plug outlet extender and provide proper outlets for each plug. 46 CFR 183.200 (b)

Connections must be made to battery terminals with permanent type connectors. Existing battery connection uses temporary wing-nut connector. Properly install permanent type connectors on all battery installations. 46 CFR 183.350

Equipment must be installed and maintained in such a manner as to afford adequate protection from failure, environmental harm. Steering piping under helm leaking hydraulic oil. Secure leak and make changes to fitting to ensure leak does not reoccur. 46 CFR 182.100

Electrical equipment on a vessel must be installed to provide safety under normal and emergency conditions. More than 4 connections made to battery post under helm. ABYC dictates good marine practice and having no more than 4 connections to a battery post considered good marine practice. Make battery post have no more than 4 connections. 46 CFR 183.200

Electrical equipment on a vessel must be installed to provide safety under normal and emergency conditions. J-box on mast not secured. 02 conductors on mast frayed. Secure J-box and renew wiring. 46 CFR 183.200

All-round light (anchor light) means light showing an unbroken light over an arc of the horizon of 360 degrees. All-round light obstructed. provide unobstuceted all-round light. 33 CFR 83.21

Steering gear must be provided with an independent rudder angle indicator at the operating station. Port wing operating station had indicator with cloudy glass. Replace glass to ensure indicator can be viewed. 46 CFR 182.610

Each fire hydrant must have a valve installed to allow the firehose to be removed while the fire main is under pressure. Handle to the fire main valve on starboard side damaged. Provide new valve handle. 46 CFR 181.310

Equipment must be installed and maintained in such a manner as to afford adequate protection from failure. Bow thruster gauge was not operational. Provide operational bow thruster gauge. 46 CFR 182.100

A ammeter which can be used for measuring current of a generator that is in operation must be provided for a gen rated at 50 volts or more. Ammeter on #2 panel not operational. Provide operational ammeter. 46 CFR 183.320

All electrical equipment and circuits must be clearly marked. Breakers in engine room not marked. Provide marking showing each breaker. 46 CFR 183.220

Equipment must be installed and maintained in such a manner as to afford adequate protection from failure. Inboard engine raw water through hull valve handle broken in half. Provide new valve handle. 46 CFR 182.100

The employer shall provide an Employee Assistance Program for all crew members that includes both education and training on the effects and consequences of drug and alcohol use. No evidence of crew training or supervisory personnel 60 minute training. Provide evidence of required training completed. 46 CFR 16.401(b)

A self priming power driven fire pump must be installed on each vessel carrying more than 49 passengers. Combination fire/bilge electrical pump failed during testing. Provide operational fire/bilge pump. 46 CFR 181.300

Steering system must be installed and maintained in such a manner to prevent machinery failure. Hydraulic oil leak from steering system found under helm. Make repairs to ensure no leak. 46 CFR 182.100

Steering system must be installed and maintained in such a manner to prevent machinery failure. 03 pins/bolts holding steering ram to rudder arm corroded. Cannot tell how must of the pins have worn. Replace pins/bolts. 46 CFR 182.100

Inboard Port Engine system must be installed and maintained in such a manner to prevent machinery failure. Excessive oil leak possibly from air box creating unsafe condition and potentially environmental hazard. Find where oil leak is from and secure source. 46 CFR 182.100

Ring life buoy line on aft deck deteriorated. Provide new line for life ring buoy. 46 CFR 180.70

Watertight Electrical fitting above wheelhouse damaged. Fitting is no longer watertight. Make repair to fitting to provide watertight integrity. 46 CFR 183.100

Closure of supply ventilation to engines must be provided. Ventilation closures did not make vapor tight barrier. Provide vapor tight closer of ventilation in case of fire. 46 CFR 182.465

An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and Supervisory personnel must receive at least 60 minutes of training. Provide proof of completed EAP training for all required personnel. 46 CFR 16.400

Pressure vessels must be tested in accordance with 46 CFR 176.812. Port and stbd engine room air receivers were overdue for inspection.

Electrical systems must be ready for inspection in accordance with 46 CFR 176.806. Forward cabin breaker panel was corroded and in need of renewal. To be corrected in 90 days.

Oil soaked lagging found on stbd outboard and port inner main engine turbos. Renew lagging to minimize fire hazards in accordance 46 CFR 176.830.

Renew hot water heater due to deterioration at base.
