The vessel must meet the following conditions: -One (01) escort tug is required while transiting within the boundary lines of San Francisco Bay. -Two (02) escort tugs are required while transiting within the boundary lines of San Diego Bay. -Vessel shall not transit in the waters of San Francisco Bay in less than 1/4 NM of visibility. -The vessel shall not carry any passengers or cargo during transit. - A 24 hr watch is required to monitor temporary repairs. -Vessel must maintain radio check-in with nearest USCG Sector on 2-hour schedule -The vessel shall be manned and operated in accordance with the Certificate of Inspection expiring 18 January 2027.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations in this subchapter. Vessel has inoperable stbd propulsion system due to previous marine casualty involving mooring line getting wrapped around shaft and shearing reduction gear output face. Vessel is restricted from operating in any commercial capacity until permanent repairs are witnessed by a Marine Inspector.

Vessel reported a line in the water that got wrapped around the stbd shaft causing damage to the main propulsion gear box. Vessel is restricted from conducting commercial operations until vessel is fully repaired. Means shall be provided whereby normal operation of propulsion machinery can be sustained or restored even though one of the essential auxiliaries becomes inoperative. Special consideration shall be given to the malfunctioning of the hydraulic, pneumatic or electrical means for control in main propulsion machinery including controllable pitch propellers. However, the Administration, having regard to overall safety considerations, may accept a partial reduction in propulsion capability from normal operation. Vessel had a mooring line wrap around the starboard shaft, which caused the reduction gear box housing to crack. Vessel removed starboard shaft and blanked both sides of the shaft tube with a steel flange,bolts, gasket and 5200. Conducted permanent repairs in LA and notify local OCMI for a follow-on inspection. See page 2 for flag state dispensation. Flag state dispensation is granted for vessel to make transit to Bollinger Fourchon or Bollinger Morgan City, LA and proceed directly to shipyard to conduct permanent repairs. Vessel is not allowed to carry passengers or work in any commercial capacity. Vessel is required to have two tugs escort them out of San Diego harbor and once in prior to arrival in new zone contact local OCMI. Vessel must ensure accepted risk assessment and mitigation measures remain in place for the duration of the voyage. Vessel must have on board a copy of Letter of Dispensation issued by Sector San Diego.
