The OCMI will ensure that every tank vessel is of a structure suitable for the carriage of flammable and/or combustible liquids in bulk and for the proper grade or grades of cargo the vessel carries while in service. Make permanent repairs (crop/renew) to damaged starboard stern corner and associated internal structures within 30 days. Repairs are to be to the satisfaction of the attending USCG Marine Inspector.

Tank Barges must undergo a periodic inspection within 3 months before or after the second or third anniversary of the date of the vessel's certification for the COI to remain valid. Vessel missed their periodic inspection and must complete a COI Inspection prior to conducting any cargo operations.

Following collision incident, vessel suffered a single 3" x 1" fracture on weather deck port side in way of #1 wing void accompanied with gunwale inset and bent structural framing. Vessel also suffered a single 3" x 1" fracture on weather deck starboard side in way of #1 wing void accompanied with gunwale inset and bent structural framing. Prior to loading cargo, permanent repairs to be made to the satisfaction of attending marine inspector. Vessel authorized to proceed to Mosaic fleeting area while permanent repair plans are developed. While at fleeting area, serviceable dewatering pump to be ready to operate onboard and rounds of damaged areas to be made every 04 hours. USCG Sector New Orleans Domestic Inspections to be notified before vessel departs Mosaic fleeting area.

Discovered 14" crack in way of port side of headlog, penetrating into bow void.

Make permanent repairs to the side shell/knuckle inset IWO the #2 port wing void.

Make arrangements and complete the required Credit Dry Dock Examination.

Replace port and starboard stern rub rails. Towing cables are cutting into stern box due to rails missing.

Install proper amount of bolts in flange and blank on cargo vent pipe. Bolts must equal holes.
