The lights prescribed in these Rules (Subpart C) shall have an intensity as specified in Annex I to these Rules (33 CFR part 84), so as to be visible at the following minimum ranges: (v) A white, red, green or yellow all-round light, 3 miles. The port side navigation light was not operational at the time of inspection. 33CFR83.22(a)&(v)

xcept as provided in paragraph (d) of this section, no person may use any equipment listed in paragraph (c) of this section for transfer operations unless the vessel or facility operator, as appropriate, tests and inspects the equipment in accordance with paragraphs (b), (c) and (f) of this section and the equipment is in the condition specified in paragraph (c) of this section..(f)(3) The frequency of the tests and inspections required by this section must be: For vessels, annually or as part of the biennial and mid-period inspections. The calibration of the pressure gauges at the transfer station did not have a record of tests or inspections readily accessible. Provide documentation of required testing of equipment.

Red warning signals: During transfer of bulk cargo while fast to a dock, a red signal (flag by day and electric lantern at night) shall be so placed that it will be visible on all sides. While transferring bulk cargo at anchor, a red flag only shall be displayed. The red electric lantern (light) was not operational during the time of inspection. 46 CFR 35.30-1

Non-conducting deck coverings, such as non-conducting mats or gratings, suitable for the specific switchboard voltage must be installed for personnel protection at the front and rear of the switchboard and must extend the entire length of, and be of sufficient width to suit, the operating space. The inert gas generator space was missing non-conductive matting installed in front of the blower switchboard. 46 CFR 111.30-11

All lifesaving appliances and arrangements on tank vessels must be in accordance with sub chapter W (Lifesaving Appliances and Arrangements) of this chapter... Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. The light provided on the lifebuoy, starboard side, aft of the transfer station was not operational. 46 CFR 31.36-1 46 CFR 199.190(a)

Electrical installations on vessels must ensure: maintenance of system integrity through compliance with the applicable system requirements (NEC) to which plan review has been approved. Vessels cargo deck area explosion proof/intrinsically safe fittings found painted/taped over obscuring the approval data and rendering their condition in question. Close up inspection by a qualified person to be conducted to ascertain condition of equipment and fittings within the hazardous area. 46 CFR 111.01-1(a)(3) 46 CFR 111.105-5

Electrical installations on vessels must ensure protection of passengers, crew, other persons, and the vessel from electrical hazards. Junction box on the port aft notch area of the barge has a non securing cover for the electrical supply cable from the tug. 46 CFR 111.01(a)(2)

No cargo tank hatches, ullage holes, or Butterworth plates shall be opened or shall remain open without flame screens.. the ullage opening at cargo hold #6 port was found missing its cover. 46 CFR 35-30.10

The terms, endorsements and conditions set forth on the certificate of inspection shall have the same force as the regulations contained in this sub chapter. Vessel found inconsistently complying with the provisions of conditional occupancy as outlined on the COI and in the operations manual. 46 CFR 31.05-15

Reachrod and vavle found to be seized for forepeak dewatering.

Provide completed copy of oil transfer procedures, posted and available for use by personnel involved in transfer procedures. 33CFR155.720-750

Provide detailed report outlining location, length (NDT), and position relative to hazardous locations for each of the seven identifed fractures on the deckhouses.

Make repairs to the deteriorated/holed conduit standpipe for the transfer drive antenna. 46CFR171.122(b)

Provide proper operation of the #4 prime mover. All repairs chall be made to the satisfaction of the attending CG Marine Inspector.

PV valves could not be opened for examination as tanks were laden and inerted. All PV valves will be opened and examined at the barge's upcoming drydock.

Barge has two pressure vessels which are not listed on the COI. Make available for internal inspection at upcoming drydock. (Safety valves already tested and found to be sat.)

Aft rake had not been opened and gas freed. Make available for internal inspection at next drydock.

Need to properly fix piping on the Inert Gas System (IGS), noticed that the vessels sea suction piping that draws water from the ballast system had a hole in he piping, discussed the issue with the vessel rep. Representative explained that prior to departure that would cut and re-weld new piping joint.

Make temporary watertight repairs to damaged hull plating in the port skeg void aft of the Intercon ladder. Repairs to be made prior to departure.

Repair damaged electrical "tug to barge" shore-tie connection.

Make permanent repairs to the damaged section of the port skeg void. Vessel restricted from all cargo operations until repairs are completed. Repairs to be completed NLT 15 MAY 2007 or next dry docking, whichever comes first.

Restricted from Cargo Ops until MSC completes plan review.
