DURING ISE, THE FOLLOWING WAS FOUND IN THE BOW RAKE: SLAG AND MISSING WELDS WERE FOUND ON FRAME 2 WELD ATTACHMENT TO THE PORT TURN OF BILGE, AND FRAME 1 WEB IS BUCKLED AND FRACTURED NEAR THE STARBOARD SIDE SHELL. REPAIR TO THE SATISFACTION OF THE ATTENDING MARINE INSPECTOR.

Scantlings, material and workmanship...shall be at least equivalent to the requirements of the American Bureau of Shipping or other recognized class society. During required annual survey of bow and stern rake it was discovered that port side insert with 2 side shell longitudinal support beams had never been welded. Vessel is to grind and weld. Furthermore, lower side longitudinal support support beam on stbd side had fractured. Also found a crack in the headlog support frame. Vessel is to make proper repairs prior to departure to the satisfaction of the MI. 1. Bow rake port side frame 1-2/3, missing weld on side shell upper angle. Clean up and weld. 2. Bow rake port side frame 1-2/3, missing weld on side shell lower angle. Clean up and weld. 3. Bow rake stbd side frame 2-1/3, crack in side shell angle support. Crop at least 12” in all directions and renew in kind. 4. Bow rake stbd side head log inboard of push knee, crack in steel supports. Crop at least 12” in all directions and renew in kind.

Found the following broken/cracked welds throughout the bow and stern rakes. Company to repair to the satisfaction of the MI. 5. Bow rake port side frame 2, bottom angle attachment to transverse support cracked weld fwd and aft side of transverse. Scarf and re-weld. 6. Bow rake port side frame 1, bottom angle attachment to transverse support cracked weld outboard fwd and aft side of transverse. Scarf and re-weld. 7. Bow rake port side frame 1, bottom angle attachment to transverse support cracked weld inboard fwd and aft side of transverse. Scarf and re-weld. 8. Bow rake port side frame 1, overhead angle attachment to transverse support cracked weld #1. Scarf and re-weld. 9. Bow rake port side frame 1, overhead angle attachment to transverse support cracked weld #2 Scarf and re-weld. 10. Bow rake port side frame 1, overhead angle attachment to transverse support cracked weld #3 Scarf and re-weld. 11. Bow rake port side frame 1, overhead angle attachment to transverse support cracked weld #4. Scarf and re-weld. 12. Bow rake port side frame 2, where lower corner of machinery space meets transverse support, bottom angle needs rewelded. Scarf and re-weld. 13. Stern rake stbd side frame 36, overhead angle attachment to transverse support cracked weld. Scarf and re-weld. 14. Stern rake port side frame 35, overhead angle attachment to transverse support cracked weld. Scarf and re-weld.

Found a wire hanging loose and running without being supported. 15. Stern rake port side, wire run in need of clamps. Either install double wire clamps or weld extra clamps. 46 CFR 31.35-1 46 CFR 111.60-5 IEEE 45.8-2016 Section 6.8

All tank vessels, the construction or conversion of which is started on or after July 1, 1951, shall conform to the requirements in this subpart in construction of hulls. T/B NEW DAWN sustained damage to aft port quarter during allision. Vessel is not permitted to carry cargo until damage assessment and repairs are conducted. Vessel is permitted to transit back to Vancouver with no cargo onboard. Repairs must be completed to the satisfaction of the attending Marine Inspector prior to returning to service.

When Minor repairs of tank vessels become necessary such work shall be under the direction of the OCMI. Received verbal report that a fracture was discovered through the hull of the fore peak tank of the vessel 17FEB2022. Company made temporary repairs without notification to the OCMI. Vessel is to make proper permanent repairs to the satisfaction of OCMI prior to cargo operations.

Provide repair proposal for #5 Starboard Void: Broken welds in overhead deck stiffener FWD and AFT of frame 28.

Provide repair proposal for #1 starboard void: Noted seven fractured Bottom longs between frames four and five as well as four fractured bottom long welds at frame four and one broken weld in the overhead deck stiffener.

Provide a repair proposal for the number one port void: Noted three fractured bottom long welds at frame number four.

Provide a repair proposal for the stern rake: The corner joint weld on the starboard side shell and main deck was affected by undercut.

Provide a repair proposal for the number two cargo tank: one missed weld identified on the tank top outboard longitudinal stiffener connection to the aft bulkhead.

Crop and renew the 04 sections of bilge roll (03 sections along vessel port side and 01 section along the vessel starboard side) that were identified by the gauging report to show significant signs of degradation in excess of 25%. [Approx. size of inserts: 48" by 55", 48" by 36", 48" by 60" and 12"by 24".]

Crop and renew the steel along the port side of the forward rake (est. size of the insert is 4' by 4').

Crop and renew the steel along the starboard side of the aft rake along the bilge knuckle (est. size of insert is 4' by 6').

Crop and renew the steel along the bottom shell and bilge knuckle on the starboard side of the forward rake. (est. size of insert in 5' by 8').

Make permanent repairs to bow rake transverse collision bulkhead as marked and detailed in CG worklist dtd 05 Jan 96._x001C_

SET RELIEF VALVE FOR THE HYDRAULIC STARTING ACCUMULATOR SYSTEM BELOW THE MAXIMUM ALLOWABLE WORKING PRESSURE OF THE SYSTEM (3000 PSI)

TEST CARGO PIPING TO 1.5 TIMES MAXIMUM ALLOWABLE WORKING PRESSURE.

RESET RELIEF DEVICE FOR HYDRAULIC ACCUMULATORS TO WITHIN THEIR MAXIMUM ALLOWABLE WORKING PRESSURE.

PROVIDE NEW SWITCH FOR PORT RUNNING LIGHT

AMEND OIL TRANSFER PROCEDURES TO INCLUDE THAT FIRE EXTINGUISHER BOX SHOULD BE OPENED DURING OIL TRANSFERS AND OPERATION OF MACHINERY AND THAT TANKERMAN WILL PROVIDE PORTABLE RED LIGHT DURING CARGO OPERATIONS AT NIGHT.

PROVIDE COVER FOR SHORE CONNECTION

SUBJECT NO. 1 STARBOARD CARGO TANK TO A HEAD PRESSURE TO PROVE SOUNDNESS OFBULKHEAD UPON FILLING OF TANK NOTIFY MSO OFFICE FOR INSPECTION.

TEST RELIEF VALVE AND CARGO PIPING AND CALIBRATE PRESSURE GAUGE IN ACCORDANCE WITH 33 CFR 156.170(C)(2)(3)(4).

RENEW STARBOARD BRACKET AT TURN OF BILGE IN FORWARD RAKE, SECOND BRACKET FROM BULKHEAD.
