The cargo tank safety relief valves shall be inspected at least once in every 2 years.(b) The safety relief valve discs must be lifted from their seats in the presence of a marine inspector by either liquid, gas, or vapor pressure at least once every 5 years to determine the accuracy of adjustment and, if necessary, must be reset. PV valves observed with a test date of JULY 2019. Replace valve or conduct a test as required and to the satisfaction of the Marine Inspector. 46CFR38.25-10(a)(b)

The COFR Operator must apply for and ensure the vessel is covered at all times by a current COFR, by complying with the requirements and procedures set forth in this subpart, including the reporting requirements in § 138.150. In preparation for the COI Annual, it was noticed that the vessel did not have a valid COFR as verified by the National Pollution Fund Center's (NFPC) database. Sector Houston-Galveston verified with NFPC of the lack of a renewal application within the past 30 days. Vessel is not to move or conduct cargo operations as operational restrictions have been placed on the vessel until satisfactory evidence of a COFR has been verified by NFPC to the satisfaction of the Coast Guard. 33 CFR 138.40 (b) 705c

Port side light is obstructed by the prime mover. Verify both side lights have an arch of visibility of 112.5 degrees prior to departure of COTP Zone Port Arthur. May transit during daylight hours only with no restrictions in visibility or not as lead barge in tow.

Prime mover turbo missing lagging. Verify proper insulation is installed on both turbo and exhaust piping prior to cargo operations involving prime mover.

COI documentation not available during. Vessel did not have current COD with new name, security plan approval letter. Provide all required documentation for COI. Vessel unable to conduct operations prior to carrying valid COI.

Vessel may transit from Southwest Brady Island Shipyard Houston, TX to a facility in the Port of Houston and discharge the cargo of bunker fuel to a ship. The barge may return empty to a repair facility in the Port of Houston. Prior to reloading cargo make permanent repairs to the 6" hole and 6' split seam and damaged knuckle in the Stbd Bow rake.

PRIOR TO LOADING CARGO, DETERMINE SOURCE OF CARGO LEAK INTO VESSEL'S STEAM HEAT PIPING AND MAKE PERMANENT REPAIRS TO SAME.
