The cognizant OCMI may require additional examinations of the vessel whenever he or she discovers or suspects damage or deterioration to hull plating or structural members that may affect the seaworthiness or fitness for the route or service of a vessel. These examinations may include a drydock examination, including:
(1) An internal structural examination of any affected space of a vessel, including its fuel tanks;
(2) A removal of the vessel from service to assess the extent of the damage and to affect permanent repairs
Vessel has multiple failures in its hull plating, several of these failures have been temporarily repaired with what appears to be shingles & epoxy, some have not, possibly allowing water ingress into the hull. Additionally, vessel's bilges have accumulated at least 5" of water, while moored, over the past month. Vessel is required to be placed in drydock and attended by CG Marine Inspectors to conduct both Internal Structural and Dry-Dock Examinations. Plans for this haul-out shall be submitted to the CG within 14 days.

Pressure vessels over 5 cubic feet in volume and over 15 pounds per square inch maximum allowable working pressure (MAWP) must be equipped with an indicating pressure gauge (in a readily visible location) and with one or more spring-loaded relief valves. The total relieving capacity of such relief valves must prevent pressure from exceeding the MAWP, as established by the manufacturer, by more than 10 percent. MI found an installed ball valve (in closed position) between pressure vessel and relief valve, rendering the relief valve useless. Remove ball valve.

Structural standards on the existing towing vessel has been called into question per 46 CFR 144.300. Marine Inspectors found several areas throughout the vessel with wastage. One spot in particular, the starboard side quarter, at the main deck and the rub rail, had a indent with wastage that may be penetrating the lazarette. Clean, grind, and make area available for further inspection by the attending marine inspector. Grind all marked areas down to good metal for further inspection and contact Sector New Orleans for further inspection.

The cognizant OCMI may require review of a vessel's watertight and weathertight integrity. This review may be performed by an individual who meets the requirements of § 144.140. The review may include an examination of a plan that shows the original placement of decks and bulkheads. Vessel has two fwd voids which the crew stated are ballast tanks. However, no schematics were available for review to verify. Provide verification of ballast tank designations. Additionally, tanks are at river level, provide proof of watertight integrity.

Inspectors attend your vessel on 26FEB24, 28FEB24, and 29FEB24, during the visit on 29FEB24 Senior Marine Inspectors (MI) observed the following:
1. Leaking of both port and STBD shaft packings
2. Significant wastage found throughout the vessel to include: Stern void, rails, davit, watertight doors, Bulwarks
3. Actively leaking generators: Oily/dusty windings, Excessive coolant temperature
4. MDEs undergoing unscheduled overhaul maintenance, including the changing of six power packs between the two engines
5. Excessive accumulation of oil found in the bilge
6. Emergency fuel shutoffs inoperable
Based on the observations from the attending Senior MIs, we are requiring the vessel to go to emergency drydock, to complete successful ISE, hull, tail shaft examination. In addition, complete external survey of the vessel to identify substandard items on-board the vessel and report those findings to the Sector New Orleans [name withheld] receiving survey report, vessel must undergo a full Inspection for Certification. Prior to any movement vessel must submit a transit plan/deadship tow plan for review by Sector New Orleans.

On 24-25th of Feb2024, Sector New Orleans received a report of an oil spill on board the vessel and that the crew was in the process of cleaning the spill. During the investigation, it was found that a crew member had hired prostitutes, and the prostitutes attended the vessel. This is in direct violation of the company’s Safety Management System for unauthorized visitors to the vessel. To stop the flow of fuel in the event of a fire or break in the fuel line, a remote fuel shutoff valve must be fitted on any fuel line that supplies fuel directly to a propulsion engine or generator prime mover.
