Each nameplate for a circuit breaker must show the electrical load served and the setting of the circuit breaker. The vessel’s switch board had multiple breakers that were mislabeled. Ensure switch board breakers are installed and labeled in accordance with the vessel’s approved plans.

Piping subject to internal head pressure from oil in the tank must be fitted with positive shutoff valves located at the tank. The valve on the day tank in the starboard machinery space was seized. Repair or replace to the satisfaction of the attending marine inspector.

Fire detection, manual alarm, and supervised patrol systems are not required except in special cases; but if installed, the systems must meet the applicable requirements of 46 CFR, part 76 of subchapter H (Passenger Vessels) of this chapter. The fire zones listed on the fire detection panel do not match the zones listed in the vessel’s PTSPs. Update PTSPs to match the fire detection panel.

Each battery-operated, relay-controlled lantern used in accordance with Table 112.05-5(a) must: Have rechargeable batteries, Have an automatic battery charger that maintains the battery in a fully charged condition; and Not be readily portable. The vessel’s emergency lights had the batteries removed. Replace batteries prior to getting vessel underway.

Each individual bilge suction must be fitted with a suitable strainer having an open area of not less than three times that of the suction pipe. In addition, a mud box or basket strainer must be fitted in an accessible position between the bilge suction manifold and the pump. The suction strainer on the bilge line in P4 void appeared to be wasted. O/O to clean out the strainer and test to the satisfaction of the attending marine inspector prior to getting underway

At each inspection for certification and periodic inspection, the inspector shall examine the tank tops and bilges in the machinery spaces to see that there is no accumulation of oil which might create a fire hazard. The catch basin for the fuel transfer pump in the port machinery space was full of diesel fuel.

Each cargo vessel must carry at least one rescue boat. Each rescue boat must be approved under approval series 160.156 and be equipped as specified in table 199.175 of this part. The distress signals and first aid kit in the vessel’s rescue boat were expired. Replace equipment prior to getting underway.

The alcohol testing of each individual [involved in an SMI] must be conducted within 2 hours of when the SMI occurred. Vessel’s BAC test swabs were expired. Obtain new test swabs

[Emergency procedures] training material must be on board each vessel and must consist of a manual of one or more volumes written in easily understood terms and illustrated wherever possible, or of audiovisual training aids, or of both. Vessel had no emergency procedures training aids. Provide training aids.

Sidelights mean a green light on the starboard side and a red light on the port side each showing an unbroken light over an arc of the horizon of 112.5 degrees and so fixed as to show the light from right ahead to 22.5 degrees abaft the beam on its respective side. Vessel’s sidelights were limited to an arc of 90 degrees. Move sidelights to a position where arc of 112.5 degrees is obtained.

Each lifejacket must have a lifejacket light approved under approval series 161.112 securely attached to the front shoulder area of the lifejacket. Lifejacket lights for lifejackets on navigation bridge were expired. Replace expired lights.

Each vessel must have a line-throwing appliance approved under approval series 160.040: Four rocket projectiles, each complete with bridle and leader of fire-resistant materials [and] not less than 4 primer-ejector cartridges which fit the chamber of the pistol, gun, or launcher. Two of the required cartridges were expired. Replace expired cartridges

[Operators must] maintain a ballast water management (BWM) plan that has been developed specifically for the vessel and that will allow those responsible for the plan's implementation to understand and follow the vessel's BWM strategy. Vessel had no BWM plan on board. Develop & retain BWM plan on board.

Design verification and Periodic Safety test procedure documents approved by the Coast Guard must be retained on the vessel. Vessel does not have approved Periodic Safety test procedures onboard. Vessel must provide proper procedures and conduct testing in the presence of Coast Guard Marine Inspector.

By March 15 of the year following the collection of the data in paragraph (a) of this section, marine employers must submit the data on the form titled U.S. Department of Transportation Drug and Alcohol Testing MIS Data Collection Form. Vessel Marine employer did not submit its 2023 MIS data as required.

All firehoses must be subjected to a test pressure equivalent to the maximum pressure to which they may be subjected in service, but not less than 100 p.s.i. One of the vessel's firehoses failed the pressure test. Hose was discarded and replaced.

Entries shall be made in the Oil record Book on each occasion, on a tank to tank basis if appropriate, whenever any of the following machinery space operations take place on any ship to which this section applies…..Bunkering of fuel or bulk lubricating oil. November 2023 bunkering entries were not made in the Oil Record Book. Instructed Chief Engineer and Captain on Oil Record Book entries and instructions, deficiency cleared.

The diesel fuel system shall comply with 56.50-60. 46 CFR 56.50-60(d) Piping subject to internal head pressure from oil in the tank must be fitted with positive shutoff valves located at the tank. Valve for the #1 fuel tank on the starboard side was inoperable.

Every crewmember must participate in at least one abandon-ship drill and one fire drill every month. There was insufficient onboard to conduct a drill in the presence of the Marine Inspector. Vessel must conduct drills with full crew compliment to the satisfaction of the attending Marine inspector

Each of the following in or around the bilge area must be arranged or constructed so that it cannot be damaged by bilge water: (d) Electric cable. All electrical wiring needs to be properly secured from bilge area. Found several areas in the bilge where wiring was laying in fluid.

Each generator must meet the applicable requirements for constitution and testing in sections 4-8-3 of the ABS steel rules. Generator data plate showed ambient temperature showed generator being at 40 degrees C. Due to area of operation, generator load should be D-rated to 80 percent. Vessel is required to prove D rating or generators are to be upgraded to 50 degrees C.

Each ring life buoy must be of an approved type, and may be used if in good and serviceable condition. Life ring line deteriorating on starboard aft outside main engine space.

Whenever doubt exists as to the safety of an existing boiler, pressure vessel, or piping system, the marine inspector may require that it be gaged or checked to determine the extent of deterioration, and if necessary for safety may require the recalculation and reduction of the maximum allowable working pressure. All hydraulic pipping in bow pump room needs to be descaled and inspected by USCG or replaced. All hydraulic piping was wasted and had large amounts of scaling.

Suitable hand covers, guards, or rails shall be installed in way of all exposed and dangerous places such as gears, machinery, etc. Rails leading to lower level of port generator machinery space where detached from main structure.

Nothing in this subchapter shall be construed as limiting the inspector from making such test or inspections as deems necessary to be assured of the safety and sea worthiness of the vessel. Jacket water pipping leading form keel coolers had non metallic flexible hose that needs to be double hose clamped.

Except as allowed in paragraph (g) of this section, each alternating-current ship's service switchboard must have an ammeter with a selector switch that connects the ammeter to show the current in each phase. Amperage meter for #2 ship service generator was inoperable.

Except as noted in paragraph (c) of this section, all enclosed spaces within the vessel shall be properly vented or ventilated. Means shall be provided to close off all vents and ventilators. Ventilation closures need to be repaired for easy closure during emergency.

If the communications system uses a sound-powered telephone they must operate throughout the vessel. Vessel had several sound powered phone stations inoperable.

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. All bilges need to be free of debris and fluid.

(b) Each self-contained breathing apparatus must be of the pressure-demand, open-circuit type, approved by the Mine Safety and Health Administration (MSHA) and by the National Institute for Occupational Safety and Health (NIOSH), and have at a minimum a 30-minute air supply and full facepiece. (e) All equipment shall be maintained in an operative condition. DC Team 2 Locker SCBA's regulator was broken allowing air to escape from the damaged regulator. Additionally, the low air signaling device did not ring when tested. DC Team 1 Locker SCBA low air signaling device did not ring when tested. 46 CFR 96.35-5(b), 46 CFR 96.35-5(e)

Nonmetallic flexible hose must be in accordance with SAE J1942 (incorporated by reference; see 46 CFR 56.01-2) and may be installed only in vital and nonvital fresh and salt water systems, nonvital pneumatic systems, lube oil and fuel systems, and fluid power systems. Identified supply fuel hose to #2 generator that was severely deteriorated with dry rot and cracking. 46 CFR 56.60-25(b)(1)

(a) Each panelboard switching unit must be numbered. (b) Each panelboard must have: (1) A circuit directory cardholder; and (2) A circuit directory that has: (i) The circuit designation of each circuit; (ii) A description of the load of each circuit; and (iii) The rating or setting of the overcurrent protective device for each circuit. Identified multiple distribution panels in the starboard machinery space and starboard aft machinery space that were not labeled as per the above regulation. 46 CFR 111.40-11(a)&(b)

Each vessel must have the following: (a) A marine radar system for surface navigation. Vessels radar display screen was found inoperative when energized for surface navigation. 33 CFR 164.35(a)

Nonmetallic flexible hose may be used in lube oil, fuel oil and fluid power systems only where flexibility is required and in lengths not exceeding 30 inches. Nonmetallic flexible hose is installed on port day tank level indicator where flexibility is not required. 46 CFR 56.60-25(b)(4)

The transfer procedures required by § 155.720 must contain, either in the order listed or by use of a cross-reference index page, the contents listed in 155.750. Vessel transfer procedures do not meet this requirement. 33 CFR 155.750

Each machinery space must be ventilated to ensure that, when machinery or boilers are operating at full power in all weather including heavy weather, an adequate supply of air is maintained for the operation of the machinery. Port MDE supply exhaust fan inop. 46 CFR 58.01-45

Test procedure documents must be in a step-by-step or checkoff list format. Each test instruction must specify equipment status, apparatus necessary to perform the tests, safety precautions, safety control and alarm setpoints, the procedure to be followed, and the expected test result. Revision 4 of vessel PSTPs does not have procedures for testing Z-Drive low oil pressure alarm and generator prime mover alarms. 46 CFR 61.40-10(a)

The following plans must be submitted to the Coast Guard for approval in accordance with § 50.20-5 and § 50.20-10 of this chapter: (1) A general arrangement plan of control and monitoring equipment, control locations, and the systems served; (2) Control and monitoring console, panel, and enclosure layouts; (3) Schematic or logic diagrams including functional relationships, a written description of operation, and sequences of events for all modes of operation; (4) A description of control or monitoring system connections to non-vital systems; (5) A description of programmable features; (6) A description of built-in test features and diagnostics; (7) Design Verification and Periodic Safety test procedures described in subpart 61.40 of this chapter; (8) Control system normal and emergency operating instructions. Changes and upgrades to Vital System Automation completed after original installation have not been approved by Coast Guard. 46 CFR 62.20-1(a)

No marine employer shall engage or employ any individual to serve as a crewmember unless the individual passes a chemical test for dangerous drugs for that employer. Vessel did not have complete list of crewmembers. 46 CFR 16.210(a)

Security systems and equipment must be in good working order and inspected, tested, calibrated and maintained according to the manufacturer's recommendation. Crew unfamiliar Ship Security Alert System and its operation. 33 CFR 104.260(a)

As appropriate for the intended voyage, all vessels except barges, vessels operating exclusively on rivers, and motorboats other than those certificated for ocean or coastwise route, must carry adequate and up-to-date Charts and Notice to mariners. Vessels charts were not up to date and vessel did not have current Notice to Mariners. 46 CFR 97.05-5(a)(e)

Every person assigned duty as officer in charge of a navigational or engineering watch, or duty as ratings forming part of a navigational or engineering watch, or designated safety, prevention of pollution, and security duties onboard any vessel that operates beyond the boundary line, as described in part 7 of this chapter, must receive - (1) A minimum of 10 hours of rest in any 24-hour period; and (2) 77 hours of rest in any 7-day period. (f)The master must post watch schedules where they are easily accessible. They must cover each affected person under paragraph (a) of this section, and must take into (g)Records of daily hours of rest must be maintained onboard the vessel. Vessel currently does not track hours of work/rest of the crew. 46 CFR 15.1111(a)(f)(g)

A device that can continuously record the depth readings of the vessel's echo depth sounding device. The echo depth sounder was not operational when tested. 33 CFR 164.35(i)

Each navigation light must meet the following: (1) Meet the technical details of the applicable navigation rules. The following navigation lights were found with faults when the panel was energized: Fwd Anchor, Aft Anchor, and Middle Ram. 46 CFR 111.75(d)(1)

On vessels required by § 112.05-5(a) to have a temporary emergency power source, the following emergency lighting and power loads must be arranged so that they can be energized from the temporary emergency power source: (a) Navigation Lights. The navigation light panel was not operational under emergency power. 46 CFR 112.15(a)

Each battery tray must be chocked with wood strips or their equivalent to prevent movement, and each tray must have non-absorbent insulating supports on the bottom and similar spacer blocks at the sides, or equivalent provisions for air circulation space all around each tray. The batteries for the GMDSS were not properly secured for shifting or movement. 46 CFR 111.15-5(d)

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. Electrical conductor penetrations in the wheelhouse deck located under the steering console are not sealed to prevent the spread of fire from spaces below. 46 CFR 92.05-1(a)

All washbasins, showers, and bathtubs shall be equipped with adequate plumbing, including hot and cold running water. No hot water in AB x4 cabin and no hot or cold water in dragtender x2 cabin. 46 CFR 92.20-25(c)

The purpose of this part is to ensure a dependable, independent, and dedicated emergency power source with sufficient capacity to supply those services that are necessary for the safety of the passengers, crew, and other persons in an emergency. 01 emergency light are not operational in the port side engine compartment. Additional emergency lighting is needed for egress routes in the fwd stbd pump room space. 46 CFR 46.112.05-1(a)

The general construction of the vessel shall be such as to minimize fire hazards insofar as is reasonable and practicable. The steel overhead in the mate 2 cabin was found holed and improperly sealed. Seal the deck to prevent the spread of fire between decks. 46 CFR 92.05-1(a)

Sufficient facilities where the crew may wash and dry their own clothes. Dryer vent found disconnected from the dryer. Dryer lint was found covering the back of the dryer and hot water tank behind the dryer. Reconnect and clean space. 46 CFR 92.20(a)

Where air pipes to ballast and other tanks extend above the freeboard or superstructure decks, the exposed parts of the pipes shall be of substantial construction. Forepeak/Bosn locker portside ballast vent cover located on the fwd port side is missing a gasket. 46 CFR 42.15-50(a)

The sidelights of vessels of 20 meters or more in length shall be fitted with inboard screens painted matte black. The panel behind the port/stbd running lights shall be painted matte black. International Navigation Rules

The marine employer must have a sufficient number of alcohol testing devices readily accessible on board the vessel to determine the presence of alcohol in the system of each individual who was directly involved in the SMI. Vessels alcohol testing swabs are expired. 46 CFR 4.06-15(a)(1)

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Galley port hole has been augmented with port cover and light removed and a electrical vent installed. Provide documentation with approval for the change. 46 CFR 91.45-1(a)

If twist-on type of connectors are used, the connections must be made within an enclosure and the insulated cap of the connector must be secured to prevent loosening due to vibration. Identified spliced bare wiring and one wire with twist on connector for controller located in the portside bosn locker above 240 breaker panel. 46 CFR 111.60-17(b)

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection(OCMI). In the event of alterations involving the safety of the vessel, the applicable plans shall be submitted for approval covering the proposed work. The general scope of the plans shall be as noted in §91.55-5. For plans required for electrical engineering, equipment and systems, see subchapter J of this chapter. The OCMI must be notified before alterations or modifications that deviate from approved plans; or repairs, alterations, or modifications that affect the safety of the vessel. A new rescue boat was installed with electricity as primary source of power. Provide plans for review to Marine Safety Center in accordance with 46CFR110.25-3, 46CFR110.25-1, and NVIC 8-84 to be verified by attending Marine Inspector. 46 CFR 91.45-1(a) 46CFR91.55-10(a) 46CFR91.55-5(f) 46CFR110.30-7(a)

Each vessel must meet the applicable requirements in subchapter S of this chapter. This subpart applies to each vessel for which the lightweight displacement and centers of gravity must be determined in order to do the calculations required in this subchapter. American Bureau of Shipping has issued a condition of Class for a dead-weight survey to be conducted prior to next annual survey and must be completed not later than 18 May 2022. 46 CFR 93.01-1, 46 CFR 170.174

Before the vessel leaves port and at all times during the voyage, each lifesaving appliance must be in working order and ready for immediate use. The rescue boat’s gunnel is cracked in three separate areas near the lift points of the boat. Two areas located near the port stern and one area on the starboard side, amidships. Provide a repair proposal or replace. 46 CFR 90.27-1, 46 CFR 199.190(a)

No extensive repairs or alterations, except in an emergency, may be made to a lifesaving appliance without advance notification to the OCMI. The rescue boat winch and falls were replaced by ship’s crew one month prior to inspection without prior approval and with equipment not verified to meeting standards applicable to the vessel. The new winch installed on the vessel took 5 minutes and 30 seconds to hoist from the water to the stowed position (approximately 20-30 feet) without any personnel on board. In accordance with 46CFR199.160(g), new installations must hoist at a rate of not less than 59 feet per minute. 46 CFR 90.27-1, 46 CFR 199.10(j), 46 CFR 199.160(g)

Lifesaving appliances and arrangements are subject to tests and inspections described in paragraph (a) of this section whenever a new lifesaving appliance is installed on the vessel. The test in paragraph (a)(1) of this section must be carried out whenever a wire fall for a launching appliance is replaced or turned end-for-end. The winch and wire fall for the rescue boat were replaced by the ship’s crew one month prior to inspection and no test or inspection was conducted. 46 CFR 90.27-1, 46 CFR 199.45(c)

All marine engineering details such as valves and their appurtenances installed on the vessel, shall be designed, constructed, and installed in accordance with the provisions of subchapter F (Marine Engineering) of this chapter. Valves installed on the outside of the oil tanks must be arranged with a means of manual control locally at the valve and remotely from a readily accessible and safe location outside of the compartment in which the valves are located. Stbd day fuel oil tank and #1 fuel oil storage tank remote control valves were found inoperable. 46 CFR 90.20-1, 46 CFR 56.50-60(d)(1)

Radio equipment required by this subpart must be maintained to provide the availability of the functional requirements specified in §80.1081 and to meet the performance standards specified in §80.1101. The vessel is equipped with radio installations to operate in Sea Areas A1, A2, and A3. However, no equipment required for sea areas A2 and A3, or the NAVTEX have been maintained by annual servicing or on board testing as required by 47 CFR 80.1067(a) and 47 CFR 80.1105(i)(2). The vessel regularly changes ports for contract dredging operations, taking it past sea area A1 which was confirmed using plotted charts on board from pervious voyages. The vessel’s INMARSAT equipment was not properly equipped, including a missing keyboard and an inoperable printer. 46 CFR 90.27-1, 46 CFR 199.60(a), 46 CFR 80.1105(b), 47 CFR 80.1067(a), 47 CFR 80.1105(i)(2)

Rescue boats must be stowed in a position suitable for launching and recovery. The rescue boat stowage platform is severely deteriorated due to corrosion and is no longer structurally sound. Provide repair proposal. 46 CFR 90.27-1, 46 CFR 199.140(a)(2)

In the inspection of hulls of vessels, the standards established by the American Bureau of Shipping, see part 90, subpart 90.35 of this chapter, respecting material and inspection of hulls, and the certificate of classification referring thereto, except where otherwise provided for by the rules and regulations in this subchapter, subchapter E (Load Lines) of this chapter, shall be accepted as standard by the inspectors. In addition to the requirements in §42.09-25, the surveyor of the assigning authority shall examine the items, etc., listed in this section, to determine if in satisfactory condition and meeting applicable requirements in this subchapter. Significant corrosion, pitting, and deterioration found on main deck and items subject to survey under Subchapter E. Provide white metal blast cleaning of main deck and all items as listed in Subchapter E to provide complete access for survey for the purpose of determining satisfactory condition or provide repair proposal. 46 CFR 91.15-1, 46 CFR 42.09-30(a)

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection(OCMI). In the event of alterations involving the safety of the vessel, the applicable plans shall be submitted for approval covering the proposed work. The general scope of the plans shall be as noted in §91.55-5. For plans required for electrical engineering, equipment and systems, see subchapter J of this chapter. The OCMI must be notified before alterations or modifications that deviate from approved plans; or repairs, alterations, or modifications that affect the safety of the vessel. Multiple new installations of electrical equipment identified. Provide plans for review to Marine Safety Center in accordance with 46 CFR 110.25-3, 46 CFR 110.25-1, and NVIC 8-84 to be verified by attending Marine Inspector. 46 CFR 91.45-1(a), 46 CFR 91.55-10(a), 46 CFR 91.55-5(f), 46 CFR 110.30-7(a) 26Jan2022: Electrical plans for entire vessel has been submitted to MSC for approval. All piping, valves, and fittings shall meet the applicable requirements of subchapter F (Marine Engineering) of this chapter. A patch on the fire main pipe located forward of the cargo generator in the port machinery space was found by the attending Marine Inspector. Provide repair proposal. 46 CFR 91.45-1(a), 46 CFR 95.10-15(a)

Fire pumps may be used for other purposes provided at least one of the required pumps is kept available for use on the fire system at all times. Branch lines connected to the fire main for purposes other than fire and deck wash shall be so arranged that adequate water can be made continuously available for firefighting purposes. Attending Marine Inspector found anchor wash supply valve handle missing resulting in crew unable to isolate fire main for adequate water supply for firefighting purposes. Provide repair proposal. 46 CFR 95.10-5(f)

Fire detection systems must meet the applicable requirements of 46 CFR, part 76 of subchapter H (Passenger Vessels) of this chapter. Attending Marine Inspector discovered fire detection panel trouble alarm in zone 2. 46 CFR 95.01-1(a)

Fire pumps must be fitted with a pressure gage on the discharge side of the pumps. Fire pump pressure gage is inoperable. 46 CFR 95.10-5(e)

For inspection procedures of Lifesaving appliances and arrangements, see subchapter W (Lifesaving Appliances and Arrangements) of this chapter. Each lifejacket must have a lifejacket light approved under approval series 161.112 securely attached to the front shoulder area of the lifejacket. Two lifejacket lights were found to be expired by attending Marine Inspector. 46 CFR 91.25-15, 46 CFR 199.70(b)(4)(i)

A vessel owner or operator must comply with the VRP exercise requirements of 33 CFR 155.1060. Quarterly, annual, and triennial exercise requirements have not been completed. 33 CFR 155.5060(a)(2), 33 CFR 155.1060(a)

A vessel response plan (VRP) submitted to meet the requirements of §155.5035 must identify the training to be provided to persons having responsibilities under the VRP, including members of the vessel crew, the qualified individual, and the spill management team. Vessel VRP identifies training requirements that have not been completed. 33 CFR 155.5055(a)(1)

Hose used to transfer oil or hazardous material must meet the requirements of §154.500 of this chapter. Each hose also must be marked with the date of the latest test required by 33 CFR 156.170. No person may use any equipment listed in paragraph (c) of this section for transfer operations unless the vessel or facility operator, as appropriate, tests and inspects the equipment in accordance with paragraphs (b), (c) and (f) of this section and the equipment is in the condition specified in paragraph (c) of this section. Vessel transferred waste oil to a barge at sea using vessel provided transfer hoses not meeting the requirements of 33CFR154.500 on 06APR2021, 05APR2021, and 23MAR2021 per vessel Oil Record Book. 33 CFR 156.150(a)

No person may transfer oil or hazardous material to or from a vessel unless each person in charge, designated under §§154.710 and 155.700 of this chapter, has filled out and signed the declaration of inspection form described in paragraph (c) of this section. Vessel received fuel oil at sea from another vessel without completing a Declaration of Inspection on 24APR2021, 14APR2021, and 10APR2021 per vessel Oil Record Book. 33 CFR 156.150(a)

Each ship of 400 gross tons and above other than an oil tanker shall maintain an Oil Record Book Part I (Machinery Space Operations). Entries made in Oil Record book not in accordance with Oil Record Book instructions on 23APR2021, 14APR2021, and 06APR2021. 33 CFR 151.25(a)

A vessel owner or operator must comply with the vessel response plan training requirements of 33 CFR 155.1055. A vessel owner or operator shall ensure the maintenance of records sufficient to document this training and make them available for inspection upon request by the Coast Guard. No training records were available for attending Marine Inspector review. 33 CFR 155.5055(a)(2), 33 CFR 155.1055(b)

When a marine employer determines that a casualty or incident is, or is likely to become, an SMI, the marine employer must ensure that the following alcohol and drug testing is conducted: The alcohol testing of each individual must be conducted within 2 hours of when the SMI occurred, unless precluded by safety concerns directly related to the incident. Vessel does not have an onboard method of alcohol testing when over 2 hours from a testing facility.

Vent outlets from all tanks which may emit flammable or combustible vapors must be fitted with a single screen of corrosion-resistant wire of at least 30 by 30 mesh, or two screens of at least 20 by 20 mesh spaced not less than one-half inch (13mm) nor more than 1 1/2 inches (38mm) apart. The clear area through the mesh must not be less than the internal unobstructed area of the required pipe. The starboard fuel vent is improperly installed with a single 20 by 20 mesh flame screen.

Design Verification and Periodic Safety test procedure documents approved by the Coast Guard must be retained aboard the vessel. Procedures on vessel are not updated to reflect current installation. Make corrections and submit to MSC for approval.

Nonmetallic flexible hose must have factory-assembled end fittings requiring no further adjustment or field attachable fittings. Hose end fittings must comply with SAE J1475 (incorporated by reference, see §56.01-2). Field attachable fittings must be installed following the manufacturer's recommended practice. Flexible hoses for steering hydraulics on port and starboard hydraulic power units dry-rotted, no longer in compliance with standard.

The cooler structure is fabricated from material of the same thickness and quality as the hull plating to which it is attached except that in the case of half round pipe lesser thickness may be used if specifically approved by the Commandant. In any case the structure, with the exception of the hull proper, need not exceed three-eighths inch in thickness. Piping on keel cooler for port and starboard MDE wasted.

Each individual bilge suction shall be fitted with a suitable bilge strainer having an open area of not less than three times at of the suction pipe. In addition a mud box or basket strainer shall be fitted in an accessible position between the bilge suction manifold and the pump. All bilge suction lines are missing strainers.

All pressure vessels other than unfired steam boilers shall be protected by pressure-relieving devices that will prevent the pressure from rising more than 10 percent above the maximum allowable working pressure, except when the excess pressure is caused by exposure to fire or other unexpected source of heat. Unable to demonstrate accuracy of relief valve. Demonstrate test, or provide documentation of test.

The material, design, construction, workmanship, and arrangement of main propulsion machinery and of each auxiliary, directly connected to the engine and supplied as such, must be at least equivalent to the standards established by the ABS Steel Vessel Rules (incorporated by reference, see 46 CFR 58.03-1), except as otherwise provided by this subchapter. Starboard MDE crankcase exhauster pipe has unauthorized repair.

Enough lights throughout machinery spaces to allow essential operations and observations under emergency conditions and to allow restoration of service. Multiple inoperable emergency lights found in port and starboard machinery spaces.

Class II pressure vessels must be stamped with the ASME "U" symbol. 1. The starboard machinery space air receiver data plate is affixed but not legible. Obtain a data plate from the manufacture. If data plate is unobtainable this pressure vessel will be condemned, and removed/replaced with new. 2. The forward pump room air receiver does not have a data plate affixed. This pressure vessel is condemned and will be removed.

The inspection shall ensure that the structure is in satisfactory condition and fit for the service for which it is intended. The platform that provides access to the rescue boat is deteriorated and unsafe for crew access. Provide repair proposal and make permanent repair.

Each cargo vessel must carry, on each side of the vessel, life rafts with an aggregate capacity on each side sufficient to accommodate the total number of persons on board.

The inspection shall ensure that vessel equipment and structure are in satisfactory condition and fit for the service for which it is intended. The fire main cross over, steel braided, flexible hose was found leaking when testing the fire pump. Renew flexible hose.

The inspection shall ensure that vessel equipment and structure are in satisfactory condition and fit for the service for which it is intended. The stbd MDE keel cooler piping, on main deck, was found with unauthorized temp repair. Make permanent repair or replace piping.

Each life jacket must have a life jacket light. The bridge life jackets were missing lights.

Each immersion suit must have a life jacket light. Immersion suits on bridge missing lights & retroreflective material was degraded. Provide appropriate lights and renew retro.

Self-igniting lights may not be attached to the lifebuoys required to be fitted with lifelines. All lifebuoys had both a light and lifeline attached. Ensure life rings only have either light or line attached, not both.

At least two lifebuoys, each with attached self-activating smoke signals, must be stowed where they can be quickly released from the navigating bridge. Lifebuoys fitted with smoke signals must also be fitted with lights. The lifebuoys near the bridge were not fitted with smoke signals.

No repairs or alterations affecting the safety of the vessel hull, machinery, or equipment shall be made without the knowledge of the [name withheld] water cooling pipe in stbd HPU Room fwd overhead for deck hydraulics has unapproved soft patch. Properly repair/replace the pipe.

No repairs or alterations affecting the safety of the vessel hull, machinery, or equipment shall be made without the knowledge of the [name withheld] line fwd valve bypass piping on main deck, between stbd engine & z drive, has unauthorized temporary repair. Properly repair/replace the pipe.

Ventilator openings shall be provided with efficient weathertight closing appliances. The dryer room ventilation closure on the aft side of the house was missing its gasket. Properly install new gasket.

The inspection shall be such as to ensure that the vessel equipment is fit for the condition for which it is intended. Multiple eye wash solutions were expired. Replace all the expired eye wash solutions.

Non-metallic flexible hose may be used in vital salt water systems. The keel cooler hoses, for the #2 generator, need to be double hose clamped. Add another hose clamp to each hose.

The installation of portable fire extinguishers shall be to the satisfaction of the Coast Guard. The bracket for extinguisher #14 was missing. Properly mount the extinguisher.

The installation of portable fire extinguishers shall be to the satisfaction of the Coast Guard. The extinguisher outside of the stbd engine compartment impeded the watertight door closure device. Relocate extinguisher to permit proper closure.

All installations shall be of marine type engines suitable for intended services. Temperature gauge on #2 generator was found inoperable. Port main engine manifold air temp gauge glass was broken. Stbd main engine temperature gauges were found inoperable. Repair all gauges and demonstrate proper operation.

Each space used by crew must be fitted with lighting that provides for a safe working environment. The aft hold lower lighting was found to be inoperable. Repair lighting.

Each battery box must have a watertight lining. Multiple battery boxes in port engine room were found to be unserviceable.

Each deck box must be weathertight. The battery boxes on stbd bridge wing were found not to be weathertight. Repair/replace battery boxes.

If twist on type connectors are used, the connections must be made within an enclosure and the insulated cap must be secured to prevent loosening due to vibrations. Wire nut connectors were found near the 480V generator tachometer, tank level indicator fwd of 480V generator, and gauge panel aft of 480V generator. Batteries in the boxes on bridge wings were found to have wing nuts on the terminal posts. Replace all with proper connections.

Electrical installations must ensure protection of crew and the vessel from electrical hazards. The control panel in way of the generators was found to have a hole. The propulsion summary panel aft of stbd main engine was found to be loose. Properly repair or replace panels.

Use of National Standard fire hose coupling threads for 1-1/2" and 2-1/2" hose sizes. The portable firefighting fog applicators do not fit the existing fire hose nozzles. Provide proper connections or remove applicators from the vessel.

The fire control and lifesaving equipment plans must be approved. The firefighting and lifesaving plan onboard does not accurately reflect the equipment installed. Revise plan and submit to OCMI for review and approval.

All washbasins shall be equipped with adequate plumbing, including hot and cold running water. Water in common head on galley deck was not hot. Hot water supply line in common head on berthing deck was broken off. Ensure hot water is available in all heads.

Sufficient illumination must be provided by the emergency lighting source. The emergency light by the stbd side inflatable life raft is not working. Make repairs.

Each vessel must have means of communication at steering gear room, if outside the engine room. The sound powered phone #7 was not working when contacted from the bridge. Make repairs.

Clear instructions must be provided on the vessel that detail the actions each person on board should follow in the event of an emergency. There were two conflicting sets of Station Bills posted. Re-address Station Bill contents and post accurate Bill.

Vsl is broadcasting invalid MMSI, invalid Call Sign, incorrect Name and greater than expected vessel length

Crop and renew damage to the follwing areas. 1) STBD access ladder in fwd ram space. 2) Aft ram space replace wasted frames, port side shell stiffner. 3) FWD port void hatch cover. 4) Potable water void space ladder. 5) STBD aft peak side long.

FWD fire hose reel wasted and holed with jagged and sharp edges. Repair.

Install a means of securing the fire damper in way of starboard side of wheelhouse/electrical space.

Renew/repair battery box beneath ladder to stbd entrance to wheelhouse.

Install temporary repair to bulkhead btwn electrical space and Chief's cabin where holed. Complete permanent repairs at next drydock.

Demonstrate operation/replace light fixture aft switchboard flat.

Vessel has a newly installed industrial generator aft of the accommodation space / centerline. Submit to USCG Marine Safety Center updated weight calculations to include new cover and wiring plan.
