Operational tests and checks of all safety and limit controls, combustion controls, programming controls, and safety relief valves must be conducted by the owner, chief engineer, or person in charge at the inspection for certification, periodic inspection, and when directed by the Officer in Charge, Marine Inspection, to determine that the control components and safety devices are functioning properly and are in satisfactory operating condition. Thermal fluid heater was INOP & tagged out. Prior to carrying grade "E" cargo, OCMI must witness all operational tests required of thermal fluid heater. 46 CFR 61.30-20

Electric installations must ensure protection of passengers, crew, and vessel from electrical hazards. Missing cap on the thermal fluid oil tank level sensor, causing exposed wiring.

Inland barges must have appropriate equipment and supplies ready for immediate use to control and remove on-deck oil cargo spills. The spill kit on board the barge was missing required equipment.

The owner, master, or person-in-charge must ensure that portable fire extinguishers are inspected and maintained in accordance with NFPA 10. Fire extinguishers on board had not been inspected by a certified or licensed fire extinguisher servicing agency in the last year.

Each lighting fixture in the weather or exposed to splashing water must be waterproof. Broken glass light enclosure, located by the entrance to the engine room.

Vessel must have a stability letter before it is placed in service. Vessel does not have stability letter/booklet information onboard or available and the information is not present on the COI. Vessel must make stability letter/booklet information made available.

Vessel with a capacity of 250 or more barrels (10500 gallons) of oil shall provide transfer procedures. Vessel has a capacity of approx.. 15000 gallons of transferable fuel oil and was not able to provide transfer procedures. Vessel must make transfer procedures available.

Vessel Response Plan must include a list of COTP zones in which it intends to handle, store, or transport oil. The current Vessel Response Plan does not list Houma as an authorized operating COTP zone.

Hydraulic system must be free of all leaks. There is a hydraulic leak on the ballast water pump supply line before the pump motor in the ballast pump room. Vessel must correct the leak.

Lights in auxiliary machinery space must be arranged so the failure of one branch circuit does not leave the area without light. Two overhead lights in the ballast pump room were extinguished.

Warning sign shall be displayed to warn persons. The starboard side warning sign was worn and weathered and could not be easily read. Vessel must provide legible warning sign on the starboard side.

Each tank vessel must undergo dry dock inspections in accordance with table 31.10-21(a). Vessel shall be taken out of service until the vessel can be placed in a gas free status. Vessel representative shall submit a letter to CG stating the vessel is taken out of service, and when and how the vessel will be transported to the clean facility.

Cargo residue was found throughout the #1 P&S void space. Vessel was issued an CG 835v requiring to gas free the #1 cargo tanks and conduct an air test. To be conducted prior to credit Internal Structural Exam.

If, during an internal structural examination, cargo tank internal examination, or underwater survey, damage or deterioration to the hull plating, structural members, or cargo tanks is discovered, the Officer in Charge, Marine Inspection, may require the vessel to be drydocked or otherwise taken out of service to further assess the extent of the damage and to effect permanent repairs. As a result from a previous incident, cargo migrated into the the #1 port & starboard void tanks; whereas, cargo residues congealed to the structural members and internal plating. The void tanks must be effectively cleaned to the satisfaction of the local OCMI. To be completed prior to credit ISE.

#1 and #2 cargo pump relief valves were not tested annually. 46 CFR 35.35-70(b)

Wire must be in an enclosure. Spliced wires on cargo pumps 1 and 2 prime movers were not in an enclosure. 46 CFR 111.60-11(a)

Provides a visual indication of the liquid level in the in the cargo tank when the cargo 46 when the cargo level is within 1.0 meter (3.28 feet)of the tank top. Rising stick near cargo tank 2S does not extend fully and must be fixed or replaced. CFR 39.20-3(b)(1)

Ground detection is required on 120v and 440v distribution panels. Eliminate grounding that causes flickering in detection panels. 46CFR111.05-21

Every portable extinguisher shall be supplied with a suitable bracket which will hold the extinguisher securely in its stowage location on vessels, and which is arranged to provide quick and positive release of the extinguisher for immediate use. Vessel must repair its broken extinguisher bracket between the cargo manifolds. 46CFR162.028-3(g)

Thermal fluid heater operational tests and checks of all safety relief valves must be conducted by the owner, chief engineer, or person in charge at the periodic inspection. Tests of safety relief valves on the thermal fluid heater have not been conducted. 46CFR61.30-20

Each transfer pipe system must not leak under static liquid pressure at least 1.5 times the MAWP. The equipment test of the vessel's cargo piping required under 33CFR156.170(c)(4) was conducted at 100 psi. Correct MAWP (125 psi) determined by the pressure at which the transfer piping relief valve is set. Test to 1.5 times the MAWP or approved alternative.

Annual testing of nonmetallic transfer hose (Ser. No. 137341-01) required under 33CFR156.170(c)(1)(iii), last conducted on 15May2013. Test or replace transfer hose prior to use.

Vessel reported flooding in #2 wing voids. Vessel is authorized a single transit to a reception facility to offload its entire cargo load. After complete discharge, the vessel is authorized a single transit to a cleaning facility. After cleaning, vessel is authorized to proceed to drydock and be made available for CG inspection of hull damage IVO #2 void tanks. Prior to loading cargo.

Vessel must demonstrate proper operation for 440 volt ground detector panel. Provide ABS plans for current 440 volt ground detection panel. 46 CFR111.05-21(b). Extended due date from 20OCT to 10NOV per vessel request and approval from [name withheld].

Vessel must lock out and tag out the thermal fluid heater until vessel demonstrates proper operation. 46 CFR 61.30-20 Rectify prior to cargo operations involving use of thermal fluid heater.

Cargo discharge pressure gauge shall be installed at a location point visible with respect to the pump controls. Vessel does not have cargo discharge pump gages installed. 46 CFR 32.50-5

Vessel must repair anchor to the satisfaction of USCG prior to cargo ops.

Do not use Thermal Fluid Heater for cargo transportation until attended by a CG Marine Inspector or ABS Inspector to verify safety shutdowns on system.

Vessel was outside of the allowed 90 day before/after window for inspection in accordance with the COI anniversary date. The COI date is 08 Aug 2011, your inspection was not completed until 13 Jan 2012.

Repair the STBD out board cargo pump emergency shutdown prior to cargo operations w/ that cargo pump. No later than 18Nov2010.

Conduct field test procedures IAW USCG-approved test procedures on the fired thermal fluid heater.

Complete routine maintenance on stbd cargo pump prior to carrying cargo.

Replace gasket on port cargo pump engine and clean or replace lagging on engine exhaust prior to operating cargo pump engine.

Re-seal viewing ports on stbd deck shack.

Renew the damaged hydraulic line on the starboard prime mover supply line for cooling fan motor.

Test the overspeed and low lube oil shutdown on the generator.

Stencil cargo piping with date of hydro testing in accordance with 33 CFR 155.800

Replace all non-metallic, fiber-reinforced hose lines (includes fuel, lube oil, hydraulic oil) with SAE-J1942 Coast Guard approved hoses.

Re-new elbow for electrical conduit on low level switch for Thermal Fluid Heater (TFH) expansion tank located port side. 46 CFR 111.105-17. Correct to the satisfaction of the attending marine inspector.

PROVIDE ON BOARD A VALID LOAD LINE CERTIFICATE.
