Alarms must be visible and audible at each operating station. MI observed that the starboard alarm panel was not giving any visual indication. O/o shall repair or replace alarm panel so that it produces visual and audible alarms at the operating station.

Emergency lighting must be provided for all internal crew working and living areas. Emergency lighting sources must provide for sufficient illumination under emergency conditions to facilitate egress from each space and must be either:(1) Automatic, battery-operated with a duration of no less than 2 hours; or(2) Non-electric, phosphorescent adhesive lighting strips that are installed along escape routes and sufficiently visible to enable egress with no power. MI observed that there was no emergency lighting provided in the forepeak, which is a designated escape route, and no emergency lighting was installed in the main deck crew berthing area. O/o shall install an approved emergency lighting system in the forepeak and in the main deck crew berthing areas, consisting of either automatic battery-operated fixtures or approved phosphorescent adhesive strips.

Each towing vessel must carry on board and maintain…Charts or maps. Marine charts or maps of the areas to be transited, published by the National Ocean Service (NOS), the ACOE, or a river authority that satisfy the following requirement: The charts or maps must be of a large enough scale and have enough detail to make safe navigation of the areas possible. Vessel was found operating with electronic charting system (Rosepoint Coastal Explorer) that does not have a type approval or manufacturer’s declaration of conformity and therefore does not meet the chart equivalency requirement. O/o is required to have up-to-date paper charts or utilize an electronic charting system that has a type approval or manufacturer's declaration of conformity.

Paints, coatings, or other flammable or combustible products onboard a towing vessel must be stored in a designated storage room or cabinet when not in use. A 40-B portable fire extinguisher must be located near the storage room or cabinet. MI observed flammable materials improperly stored, and no 40-B portable fire extinguisher installed nearby. O/o shall properly store all flammable and combustible material products and install a 40-B portable fire extinguisher in the immediate vicinity, or remove the flammable and combustible material products from the vessel.

Each towing vessel operating on oceans, coastwise, or Great Lakes routes must have a means to take blood pressure readings, splint broken bones, and apply large bandages for serious wounds. MI noted vessel did not have means to take blood pressure readings. O/o shall demonstrate ability to take blood pressure readings.

Sufficient lighting suitable for the marine environment must be provided within crew working and living areas. Electrical systems and equipment must function properly and minimize system failures and fire and shock hazards. MI noted midship starboard and port light fixtures improperly installed. O/o shall properly install light fixtures to ensure safety and sufficient lighting. 46 CFR 143.410(a) 46 CFR 143.400(a)

Electrical connections must be suitably installed to prevent them from coming loose through vibration or accidental contact. MI noted extension cord utilized as permanent electrical connection for mechanical ventilation of lazarette. O/o shall remove extension cord and hardwire the electrical connection for the mechanical ventilation.

Piping and machinery components that exceed 220 °C (428 °F), including exhaust manifolds, and turbochargers, must be insulated. #1 MDE Found with section of lagging missing from exhaust header.

Electrical components that pose an electrical hazard must be in an enclosure. Spotlight on top of the wheelhouse had a splice in the wiring that was not in a junction box.

The master must ensure that all life-saving equipment is properly maintained and ready for use at all times. MI's noted that 02 life-rings on bridge were degraded and not effective for lifesaving. Replace and add appropriate markings and retro tape.

Provide means to test the following alarms; Main engine low lubricating oil pressure; Main engine high cooling water temperature; Auxiliary generator engine low lubricating oil pressure; Auxiliary generator engine high cooling water temperature; High bilge levels; and Low fuel level, if fitted with a day tank.

Update Certificate of Documentation (COD) with international tonnage from the vessel's International Tonnage Certificate.

For towing vessels with berthing aboard, a sufficient number of additional lifejackets must be carried so that a lifejacket is immediately available for persons at each normally manned watch station. Provide 01 additional lifejacket as indicated.

In addition to the immersion suits required under paragraph (a)(1) of this section, each watch station, work station, and industrial work site must have enough immersion suits to equal the number of persons normally on watch in, or assigned to, the station or site at one time. However, an immersion suit is not required at a station or site for a person whose cabin or berthing area (and the immersion suits stowed in that location) is readily accessible to the station or site. Provide 04 additional immersion suits (02 on bridge).

Each towing vessel must be equipped with a means to detect smoke in the berthing spaces and lounges that alerts individuals in those spaces.

Replace missing exhaust lagging on STBD main engine.

Replace missing flywheel covers on main prop engines.

Each watertight door and watertight hatch must be marked on both sides in clearly legible letters at least 25 millimeters (1 inch) high: “WATERTIGHT DOOR—KEEP CLOSED” or “WATERTIGHT HATCH—KEEP CLOSED”.

Individual circuit breakers on switchboards and distribution panels must be labeled with a description of the loads they serve. Relabel breaker panels indicated.

Each space where crew may be quartered or normally employed must have at least two means of escape. Adjust escape hatch so that it may be readily opened from the inside.

Sause Brothers DER provided documentation that all Sause Brothers supervisors have received the annual 1-hour drug and alcohol (EAP) training required by 46 CFR § 16.401.

ON 14SEP94, THE M/V MIKI HANA, OPERATED BY SAUSE BROTHERS INC, WAS OPERATING AT PIER 20, HONOLULU, HI. A USCG BOARDING OFFICER FOUND THE M/V MIKI HANA WAS EQUIPPED WITH INSTALLED TOILET FACILITIES AND WAS NOT EQUIPPED WITH AN OPERABLE TYPE III MARINE SANITATION DEVICE.
