Where cargo pump shafts pierce gastight bulkheads, stuffing boxes with readily accessible gastight glands shall be provided. #1 and #3 cargo pump outer gastight glands had holes compromising gastight boundary. Renew torn gastight glands for #1 and #3 cargo pump shaft penetrations.

The applicable standards established by the ABS Marine Vessel Rules (incorporated by reference, see § 58.03-1), may be used as the standard for the design, construction, and testing of main and auxiliary machinery except as modified in this subchapter. Generator unable to shutdown due to loss of lube oil pressure as detected from sensor. Demonstrate proper alarm and shutdown of generator to the satisfaction of the marine inspector.

Each cargo tank of a tank barge must have an intrinsically safe overfill control system that has at least one tank overfill sensor switch per cargo tank that is designed to activate an alarm when its normally closed contacts are open. Cargo tank overfill alarms inoperable. Demonstrate operation to marine inspector.

If your vessel's certificate of inspection is renewed less often than annually, the marine inspector must check the settings of the safety or relief valves on all pressure vessels, except cargo tanks, twice every 5 year No more than 3 years may elapse. Vessel to provide relief valve setting report. 46 CFR 61.10-5(i)(2)

Repair or replace to the satisfaction of Marine Inspector. 46 CFR 58.01-5

A circuit directory that has:(i) The circuit designation of each circuit;(ii) A description of the load of each circuit; and(iii) The rating or setting of the overcurrent protective device for each circuit. Provide circuit directories for all panel boards onboard vessel. 46 CFR 111.40-11(b)

structural, and cargo tank internal examinations at scheduled intervals: Barge did not complete and missed the due dates for an ISE and Drydock examination. Barge can not conduct cargo ops or carry cargo until an ISE and Dry dock exam is completed.

CFR 46 31.10-21(c) - If, during an internal structural examination, cargo tank internal examination, or underwater survey, damage or deterioration to the hull plating, structural members, or cargo tanks is discovered, the Officer in Charge, Marine Inspection, may require the vessel to be drydocked or otherwise taken out of service to further assess the extent of the damage and to effect permanent repairs. The starboard side shell just below the head log on the starboard side was breached during a collision with another barge which also caused damage to internal structural members. All affected areas are to be repaired to the satisfaction of the attending Marine Inspector.

Cargo pump relief valves shall be tested at least once each year to determine that they function satisfactorily at the pressure at which they are set to open. Relief valves are overdue for testing. 46 CFR 35.35-70(b)

Cargo pump pressure gauge shall be tested at least once a year for accuracy. Gauges are overdue for testing. 46 CFR 35.35-70(c)

Each pressure-vacuum relief valve must - Be of a type approved under 46 CFR 162.017, for the pressure and vacuum relief setting desired. Pressure side on High velocity P/V valve stuck in the open position. 46 CFR 39.2011(b)(1)

Each vessel vapor connection flange face must have a permanent stud projecting outward that has a 12.7 millimeter (0.5 inch) diameter and is at least 25.4 millimeters (1 inch) long. It must be located at the top of the flange face, midway between boltholes, and in line with the bolthole pattern. Port side flange stud located on the side of the flange face.

Each pressure vessel, other than one exempted by this section, must be subjected to a hydrostatic test at a pressure of 1 1/4 times the maximum allowable working pressure twice within any five-year period, except that no more than three years may elapse between any test and its immediate predecessor. Unable to completely examine internals via manhole. Pressure vessel not exempted from hydrostatic testing. Conduct hydrostatic testing in the presence of a Marine Inspector. 46 CFR 61.10-5

Each vapor hose, vapor collection arm, pressure or vacuum relief valve, and pressure sensor is tested and inspected annually. VCS pressure gauges have not been calibrated. 33 CFR 156.170(g)(1)

The last 1.0 meter (3.3 feet) of vapor piping upstream of the vessel vapor connection and each end of a vapor hose must be painted in the sequence of red/yellow/red. The width of the red bands must be 0.1 meter (0.33 foot) and the width of the middle yellow band must be 0.8 meter (2.64 feet). Provide proof of proper color sequence for both port and starboard vapor collection piping systems.

Each cable entering a box or fitting must be protected from abrasion and must meet the following: (b) Cable armor must be secured to the box or fitting. Loose fitting found on bottom of 110V distribution box. Provide evidence of secured cable penetrations.

All electrical installations in hazardous locations must comply with the general requirements of section 33 of IEEE 45-1998 and NEC 2002. Inspector found armored protection corroded and fractured for electrical cables leading to fixed equipment located on deck in the hazardous location zone. Armored protection is intended to seal cables supplying power to the high level alarms and tank radar in cargo tanks 1P, 2P, 2S, 4P. Also found the armored protection corroded and fractured for the cable/conductors providing power to the electrical control valve located centerline of the vessel in the hazardous location zone. Vessel experienced a marine casualty on 10/26/2018. The power supply conductor for the mixing valve overheated due to resistance and the crew initiated the shut down for the power systems on the vessel to prevent a fire on deck in the hazardous location. Provide electrical drawings/calculations to prove the conductor and overcurrent protection are serviceable to protect the conductor for this valve. The use of associated conductors for this valve are prohibited until calculations are reviewed/approved. Manual use of the valve is permitted in the interim. Renew armored protection for cables in hazardous zone.

Each propulsion, power, lighting, or distribution system having a neutral bus or conductor must have the neutral grounded. Ground fault detection lights located at the 120 volt distribution panel and step down transformer indicate a fault condition. Inspector used an IDEAL Sure Test analyzer 61-165 at the appliance branch circuit in the control station to identify the true RMS ground to neutral voltage. True RMS ground to neutral voltage indicates 33 volts and reverse polarity at the appliance branch circuit, presenting a dangerous electrical shock hazard to personnel on board. Identify the fault condition and complete satisfactory repairs.

Exposed, noncurrent-carrying metal parts of fixed equipment that may become energized because of any condition must be grounded. In general, connections and terminations to all conductors must retain the original, electrical, mechanical, flame-retarding, and, where necessary, fire-resisting properties of the cable IAW 46 CFR 111.60-17. Inspector found the grounding wire damaged or disconnected on the tank radars for cargo tanks 2S, 3S, 4S, and 4P. Renew grounding conductors as original.

Provide repair plan for inset at frame 17 in #3 port side void, buckled in way of side shell long

Provide repair plan for 1st tripping bracket from aft in aft peak. Currently buckled in way of bilge knuckle.

Renew VCS piping IAW 46CFR39

Provide hazardous location equipment list to accompany hazardous location plan

Replace all drain valve handles for cargo lines

Repair or replace wasted/broken grounding straps for cargo radar gauging devices.

Replace deteriorated enclosured for cargo pump remote emergency shutdown located amidships.

Replace or re-stencil wasted valve-position indicators.

Replace wasted valve handles on pump discharge drain valves

Provide test documentation for cargo tank P/V valves.

Conduct repairs & demonstrate proper operation of installed cargo tank high level alarm system.

Vessel allowed to take on cargo IAW special permission request submitted by Ryland Brown on 15 July 2015 with following additional reqs: vessel only allowed to take on cargoes allowed for open gauging. vessel must post a live continuous watch for each tank being loaded and a roam watch for wall tanks not beeing actively loaded

Vessel restricted from using the installed cargo rado tank level gauging system until installation has been verified to be intrinsically safe IAW IECEx Certificate of Conformity LC1 11.003x & LC1 11.004x, EMS Cargo Radar IECEx test procedure, & Marine Safety Center letter E2-1300825 dated 18 Mar 2013. Circuit breaker system must be locked & tagged out.

Vessel restricted from using VCS system and carrying subchapter D cargoes listed on the cargo authority attachment until cargo authority attachment has been validated & placed onboard.

Provide overspeed shutdown device that is independent of the normal operating governor for the generator or have manufacturer submit ECU/ECM plans to the Marine Safety Center for determination of an equivalent level of safety.

Replace unapproved mats under electrical panels with approved insulated mats IAW 46 CFR 111.30-11.

Replace safety relief valve on pressure vessel in lieu of pressure test IAW 46 CFR 32.35-15.

Ground Fault. Replace 2 30 amp and 1 15 amp breaker. Perform megger test of switchboard IAW 46 CFR 111.05-25.

Hole/crack in explosion proof cover on fwd flood light mast. Replace IAW 46 CFR 11.105-3.

#6 Port overfill alarm did not sound. Repair and do not fill tank until verified by [name withheld] 46 CFR 39.20-9.

Valve indicators not well maintained. Renew on/off indicators and repair broken indicators IAW 46 CFR 56.20-9.

#1 port glass ullage view port leaking and squeegee not working. Repair IAW 46 CFR 39.20-3.

Missing signs for carrying subchapter O cargo. Install proper signs in proper locations IAW 46 CFR 35.30-1(b) and 46 CFR 151.45-2(e).

Prove proper operation of cargo pump prime mover alarms and trips.

Permanently affix loadline markings on both Port and Starboard side.

Properly label high level and overfill alarms at office.

Configure wiring for DC pwoer to the emergency shutdowns to prohibit movement of cabling when removing battery box cover.

Prove proper operation of the newly installed tug bunkering cargo piping prior to conducting tug bunkering operations.

Provide corrected oil transfer procedures reflecting the addition of the tug bunkering cargo piping and any resulting changes to the transfer procedures for the tug bunkering cargo piping.

Crop and renew inset damage forward rake starboard side in way of longitudinal bulkhead just below the main deck.

In # 1 starboard void - crop and renew # 1 double bottom frame 8 1/2 in way of side longs # 4 and # 7; renew tripped area of swash bulkhead, starboard side.

In # 1 starboard void - reweld under deck stiffener # 11 between frames # 1 and # 3 aft of forward bulkhead.

In # 2 starboard void - crop and renew frame # 10 and # 10.5 in way of side longs # 4 and # 7 and crop and renew frames # 11 and # 11.5 in way of side longs # 4 and # 7.

In # 3 starboard void - crop and renew transverse bulkhead # 4 between bottom longs # 8 and # 9 to include vertical stiffener in way of bottom long # 8.

Provide copy of hose hydrostatic test report.

Provide classification report determining the status of loadline mark for portside of the barge.

Replace fittings and hose lines with approved hose lines and fittings for generator's fuel lines.

Provide valid Certificate of Documentation.

Install and demonstrate operation of overspeed trip on generator.
