All electrical installations in hazardous locations must comply with the general requirements of section 33 of IEEE 45-1998 and NEC 2002. Inspector found armored protection corroded and fractured for electrical cables leading to fixed equipment located on deck in the hazardous location zone. Armored protection is intended to seal cables supplying power to the high level alarms and tank radar in cargo tanks 1P, 2P, 2S, 4P. Also found the armored protection corroded and fractured for the cable/conductors providing power to the electrical control valve located centerline of the vessel in the hazardous location zone. Vessel experienced a marine casualty on 10/26/2018. The power supply conductor for the mixing valve overheated due to resistance and the crew initiated the shut down for the power systems on the vessel to prevent a fire on deck in the hazardous location. Provide electrical drawings/calculations to prove the conductor and overcurrent protection are serviceable to protect the conductor for this valve. The use of associated conductors for this valve are prohibited until calculations are reviewed/approved. Manual use of the valve is permitted in the interim. Renew armored protection for cables in hazardous zone.

Ground fault detection lights located at the 120 volt distribution panel and step down transformer indicate a fault condition. Inspector used an IDEAL Sure Test analyzer 61-165 at the appliance branch circuit in the control station to identify the true RMS ground to neutral voltage. True RMS ground to neutral voltage indicates 33 volts and reverse polarity at the appliance branch circuit, presenting a dangerous electrical shock hazard to personnel on board. Identify the fault condition and complete satisfactory repairs.
