Each ship subject to the Bridge-to-Bridge Act must have on board a radio operator who holds a restricted radiotelephone operator permit or higher class license.

Licensees of radio stations are required to have current station documents as indicated in the table. Bridge to Bridge Radio Certificate not on board.

Each liferaft or group of liferafts must be arranged for float-free launching. The arrangement must ensure that the liferaft or liferafts, when released and inflated, are not dragged under by the sinking vessel. Improper float free arrangement identified on liferaft. Vessel crew rectified.

Carpet/matting in engine room presents possible fire hazard. 835 issued to remove/replace or provide documentation of fire-retardant properties.

Inflatable liferaft has not been serviced.

Station license required: Stations in the maritime service must be licensed by the FCC either individually or by fleet.

A towing vessel that is certificated for limited coastwise, coastwise, oceans, or beyond 3 nautical miles from shore on the Great Lakes whose contract for construction was executed on or after August 27, 2003, except for those specified in paragraph (a)(3) of this section, must be equipped with both: (1) An approved 160-B semi-portable fire-extinguishing system to protect the engine room; and (2) A fixed fire-extinguishing system installed to protect the engine room. Vessel does not have an approved 160-B extinguisher on board. Vessel is not permitted to operate in a commercial towing capacity until the 160-B semi-portable fire extinguisher is on board and installed to the satisfaction of the attending Marine Inspector. 46 CFR 142.315

A towing vessel may not be operated without having onboard a valid COI issued by the Coast Guard as required by 46 CFR 136.202. Owner has not reached out to the Coast Guard to scheduled, planned or submitted Application for Inspection. Owner must submit Application for Inspection and complete a Certificate of Inspection prior to movement.

Machinery and electrical systems must be designed and maintained to provide for safe operation of the towing vessel and safety of persons onboard under normal and emergency conditions. The vessel's crew reported a loss of propulsion on the Port Z-drive. Determine the cause of the loss of propulsion on the Port Z-drive and submit a repair proposal to be reviewed and approved by the cognizant [name withheld] repairs are completed conduct a sea trial to the satisfaction of the attending USCG Marine Inspector. 46CFR143.205

The vessel reported a loss of propulsion and subsequent shaft vibrations after potentially fouling the port azipod. Determine if the azipod is fouled/damaged. If fouled/damaged submit a repair proposal to the cognizant [name withheld] the port azipod is not fouled/damaged conduct a sea trial to the satisfaction of the attending USCG Marine Inspector.
