There appeared to be 01 crack on the transverse frame in the lazarette. Effect repairs to satisfaction of the attending Marine Inspector. 46 CFR 177.100

Each item of survival craft equipment must be of good quality, and efficient for the purpose its is intended to serve. The life floats installed on the vessel had one permanently attached name plate that was not legible to read as required by 46 CFR 160.010-8 as referenced by 46 CFR 160.027-2. 46 CFR 180.175(a)

Must maintain updated charts for areas of operation. Charts last updated 2019.

Piping, valves and fittings in a firemain system must comply subpart G of 182. Firemain valves and piping leaking in engineroom.

Piping conveying gas or vapor exceeding temperatures of 150 deg must be suitably insulated. Exhaust pipes on the #1 and #4 MDE's missing lagging.

Individual bilge lines shall be provided. Strainer from seachest missing 2 nuts and being held in place with a ratchet strap.

Quarters and similar spaces shall be serviceable and in a sanitary condition. Sewage vent line behind false bulkhead is broken venting sewage gases into accommodation spaces.

Flexible hoses used in fuel/ oil service shall meet SAE J1942. Non standard hose used to replace manufacturer installed part. Make repairs as originally designed.

Batteries shall be protected from falling objects. Starting batteries for 04 MDE's are missing battery covers.

Life floats shall be connected with float free arrangements of proper strength for size if floats. Line to the floats we deteriorated and the links were for 11-21 floats, with the set-up being a 30 person float arrangement.

A vessel must carry one floating water light approved IAW 161.010. Housing for the water light cracked, not serviceable. Restricted to day time operations.

Pressure relief devices shall be tested twice in 5 not to exceed 3 years. Relief devices on the pressure vessels were last tested in 2019.

Garbage management plan required for vessels over 79 ft. No plan available for inspection.

There appeared to be 3 cracks in the lazarette rudder room in addition to over a foot of standing water. Determine extent of damage/source of water ingress in rudder room. Provide repair proposal. Effect repairs to satisfaction of Marine Inspector. - 46 CFR 177.100

On a vessel of at least 7.9 meters (26 feet) in length, a visual and audible alarm must be provided at the operating station to indicate a high water level in each of the following normally unmanned spaces. The audible alarms for bilge high level were not operating correctly due to loose connections. Properly terminate connections for alarms. - 46 CFR 182.530

The number of penetrations in a watertight bulkhead must be minimized. A penetration in a watertight bulkhead must be as high and as far inboard in the bulkhead as practicable, and made watertight. There were 2 blue wires penetrating the engine room bulkhead that were not water tight. -46 CFR 179.320

The EPIRB's battery is replaced after it is used, or before the date required by FCC regulations in 47 CFR part 80, whichever comes sooner. Battery in EPIRB is expired. - 46 CFR 185.728(c)

Exhaust gas must not leak from the piping or any connections. The piping must be properly supported by noncombustible hangers or blocks. Number 4 mde exhaust is cracked and leaking into the engine room. - 46 CFR 182.430(b)

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. There were vice grips holding the starter connections together on the number 2 mde. - 46 CFR 182.200(a)

A power driven vessel underway shall exhibit a sternlight. Sternlight inoperable at time of inspection. Repair prior to nighttime/limited visibility operations. - 33 CFR 83.23(a)(IV)

vessel must be provided with a satisfactory arrangement for draining any watertight compartment. Bilge piping in steering room wasted. Repair/replace at next drydock.- 46 CFR 182.500(a)

A hinged watertight bulkhead door must be fitted with indicator lights at the operating station showing whether the door is open or closed. Watertight door indicator label needs to be replaced in pilothouse – 46 CFR 179.330(b)

A vessel must comply with the applicable requirements as set forth in 47 CFR part 80. The vessel’s Navtex required by its certificated routes is inoperable. – 46 CFR 184.502

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or a kit with equivalent contents and instructions. First aid kit needs to be replaced (expired). – 46 CFR 184.710

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces and machinery spaces. - 46 CFR 181.405(c)

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR.” Emergency exit labeling needs to be relabeled (lettering worn). - 46 CFR 185.606

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. Dead end wiring was found throughout various locations of vessel. Remove all unused dead-end wires. – 46 CFR 183.200

When flammable or combustible liquids are carried, additional fire protection shall be provided in accordance with 49 CFR 176.315. Required AFFF cans for carriage of portable tanks are expired. - 49 CFR 176.315

Floating water lights must be attached around the body of the life float or buoyant apparatus by a 10 mm (3/8 inch) lanyard, resistant to deterioration from ultraviolet light, and at least 5.5 meters (18 feet) in length. Floating light lines need to be around the body of the floats. – 46 CFR 180.175(f)(4)

Each EAP education program must include the display and distribution of a community service hot-line telephone number for crewmember assistance. Put up an EAP phone number poster per 46 CFR 16.401. - 46 CFR 16.401.

Each battery must be secured to protect against shifting, and must be mounted in trays lined with a material that is resistant to damage by electrolyte. Battery on top of pilothouse is not secured in its tray and the tray is not lined.

A vessel's hull must not carry current. Main switchboard ground indicator shows a 1 amp ground.

When Class 1 doors are installed below the main deck, an indicator light which warns when the door is open is installed at the operating station. The engine room door indicator is not working.

At each inspection, the vessel shall be ready for inspections of machinery essential to the operation of the vessel including cooling systems. Starboard main engine exhaust has a crack approx. 8 inches from the overboard discharge, causing exhaust cooling water to leak inside the hull.

Bilge piping from manifold aft to rudder room along the port side shell had 14' section where piping material was severly wasted beyond material condition of piping.

Firemain piping must comply with subpart G, part 182, of this chapter. Firemain piping was found to be leaking upon inspection. Ensure that firemain piping is free of leaks as per 46 CFR 181.320; To the satisfaction of the Coast Guard by 19NOV19.

Each pressure type wire connector and lug must can utilize twist-on type wire nuts under the following conditions: The connections must be made within an enclosure and the insulated cap of the connector must be secured to prevent loosening due to vibration. Electrical equipment on a vessel must be installed and maintained to protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; Junction box did not cover the connections of the wiring; Adjust junction box as per 46 CFR 183.340(i)(1) and 46 CFR 183.200(b); To the satisfaction of the Coast Guard by 26NOV19.

At each initial and subsequent inspection for certification the owner or managing operator shall be prepared to test the steering systems of the vessel and make them available for inspection to the extent necessary to determine that they are in suitable condition and fit for the service intended; Cracks were discovered on the stanchions supporting the rudder; Complete repairs on stanchions supporting the rudder as per 46 CFR 176.814; To the satisfaction of the Coast Guard.

Each survival craft must be: Secured to the vessel by a painter with a float-free link permanently attached to the vessel except that a float-free link is not required if the vessel operates only on waters not as deep as the length of the painter; Painter was not secured to the essel; Secure painter to the vessel as per 46 CFR 180.130(a)(1); To the satisfaction of the Coast Guard.

All alcohol testing devices used to meet the requirements of this part must be currently listed as per 46 CFR 4.06-15(a)(2); Alcohol test strips onboard were expired; Replace all expired alcohol test strips as per 46 CFR 4.06-15(a)(2); To the satisfaction of the Coast Guard.

The master or person in charge of a ship to which this section applies shall ensure that the ship is not operated unless a garbage management plan meeting paragraph (b) of this section is on the ship and that each person handling garbage follows the plan. Provide for the discharge of garbage by means that meet Annex V of MARPOL. Describe procedures for minimizing, collecting, processing, storing, and discharging garbage. Designate the person who is in charge of carrying out the plan; Vessel did not have a Waste Management Plan; Create a Waste Management Plan as per 33 CFR 151.57; To the satisfaction of the Coast Guard by 26NOV19.

Vessel General Permit (VGP) applies to discharges incidental to the normal operation of all non-recreational vessels of 79 feet or greater in length; Vessel did not have a Permit Authorization and Record of Inspection (PARI) form onboard and did not submit annual report to EPA; Obtain PARI form and submit annual report to the EPA as per 40 CFR 122.2; To the satisfaction of the Coast Guard by 26NOV19.

Vessel not complying with Vessel General Permit (VGP). Vessel did not have a Permit Authorization and Record of Inspection (PARI) form on-board (VGP 1.5.3); Vessel was not conducting routine visual inspections weekly as required (VGP 4.1.1; 4.1.1.1); Vessel owner/operator was not conducting comprehensive vessel inspections annually as required (VGP 4.1.3); and vessel owner/operator was not submitting annual report to EPA as required (VGP 4.4.1). Deficiency will remain open until next annual inspection to verify weekly inspections were conducted and annual report was submitted.

EPIRB Battery was expired.

Non-Marine grade wire was used to wire switch for vacumm regulator for head.

Guard missing on STBD Main Engine around rotating machinery.

Engine Room, Port Blower, missing screen and access panel. Vessel shall provide proof of repairs to satisfaction of attending marine inspector.

Engine Room, Stbd. Blower, filthy with oil residue. Vessel shall provide proof of cleaning to satisfaction of attending marine inspector.

Engine Room, #4 outboard stbd. engine, blowback hose needs to be replaced. Vessel shall provide proof of repairs.

Provide emergency light for forward passenger area to cover emergency exit.

Renew gasket on emergency escape exit in forward passenger area.

Renew all shaft tachometers at operating station.

Renew #2 main engine shut down at operating station.

Gouge and reweld engine mount on stbd engine, outboard side.

Renew Nav lights, anchor and lower not under command lights.

Vessel was involved in allision on 24FEB07 in Port O'Connor, Texas. Vessel sustained a 8" X 1/4" gash in portside hull, amidship, approx 3' above the water line. Temp repairs completed sat. Master issued outstanding CG-835 to make permanent repairs at next scheduled drydock NLT 06OCT07.

Clean both generator armatures.

Repair oil leaks in stbd generator

Remark lifefloats with capacity.

Move sea painter for STBD lifefloat aft of A/C unit.

Repair leaks in fire pump (port side) and adjust packing gland to stop leak on port overboard discharge next to fire pump.

Vessel conducted unapproved repair to leaking fuel tank by spot welding two pinhole leaks in the top of tank. Top of tank is the deck of the fwd part of the engine room. Holes located on the port aft top of tank and stbd tank top just inside fwd watertight door. Pressed tank up to 5 psi with sat results. Issued open 835 for vessel to properly repair fuel tank at next hull exam in Oct 2007.

Vesssel was found to be operating for greater than 12 hours in a 24 hour period without the required licensed mate. Provide and maintain the required minimum crew for a voyage away from the dock of more than 12 hours.

Develop and maintain written operating proceedured for sounding fuel tanks and tank usage to prevent aft fuel tank from running dry and causing all propulsion engines to shut down.

Check the engine fuel oil suction lines in the fuel settling tank to verify they are of adequate length to prevent the fuel supply lines from becoming air bound and causing the vesels engines to shut down.

Provide emergency broadcast placard.

Provide dryer exhaust vent.

Provide Ball / Check valve with screen on fresh water vent.

Provide storm rails on stove.

Properly wrap exposed wiring at the forward, port side engine room bulkhead.

Properly wrap exposed engine exhaust piping.

Starboard outboard clutch oil pressure and engine oil pressure gauges leaking.

Renew outboard port MDE air supply line.

Renew chaffing gear on all 4 MDE supply and return fuel hoses.

Professionally clean oil saturated port and starboard generator armatures.

Professionally clean oil saturated exhaust fan motors in the engine room.

Replace gasket in the forward passenger compartment escape scuttle.
