Spaces must be equipped with a fire detection and alarm system of an approved type installed in accordance with 46 CFR part 76. MI observed that the fire detection system on the bridge was not serviced. 46CFR181.405(a)

All vessels shall be provided with a satisfactory arrangement for draining any watertight compartment. The bilge pump for the lazerette was inoperable. 46 CFR 182.25-1 (Old T)

If your vessel's Certificate of Inspection is renewed less often than annually, the marine inspector must check the settings of the safety or relief valves on all pressure vessels, except cargo tanks, twice every 5 years. No more than 3 years may elapse between any check and its immediate predecessor. MI was unable to verify latest inspection of pressure vessel safety relief valves. 46 CFR 61.10-5(i)(2)

A closed fresh water system may be used to cool the engine. MI observed a jacket water leak on the jacket water head tank for the port side generator. 46 CFR 182.20-10(2) (Old T)

Vessels described by 46 CFR 175.110(c) must have an interconnected fire detection system in compliance with § 181.450 installed in all enclosed areas where passengers and crew have routine access, including accommodation spaces. MI observed the fire detectors in crew accommodation space were not installed and were missing batteries. 46 CFR 181.405(c)

Installation shall be in accordance with 182.15-15. (182.15-15) That part of the exhaust system between the point of cooling water injection and the engine manifold shall be water jacketed or or effectively insulated. MI witnessed exhaust lagging before the water cooling jet on the FWD Port engine missing.

It is further intended that all machinery be installed and maintained in such a manner as to afford adequate protection against fire, explosion, machinery failure and personnel injury. Marine inspector (MI) found STBD generator main seal leaking. 46CFR182.01-5

All electrical installations shall be to the satisfaction of the Officer in Charge, Marine Inspection. Emergency lighting bulbs were found burnt out. 46CFR183.01-15(a)60

All vessels except non-self-propelled vessels shall be provided with suitable steering apparatus. MI witnessed leaks in hydraulic steering system. 46CFR182.30-1(a)

Except as further modified in this section, where fire extinguishing systems or equipment are not required, but are installed, the system or equipment and its installation shall meet the requirements of this part. MI witnessed trouble alarm on fire detection panel flashing as well as unlabeled detection zones. Label fire detection zones and provide a report from a certified technician attesting to trouble alarm servicing and proper operation of fire protection panel. 46CFR181.05-5(a)

The EPIRB required in § 180.40-1 of this subchapter is tested monthly, using the integrated test circuit and output indicator, to determine that it is operative. EPIRB monthly testing was not recorded in log books. 46CFR185.25-20(a)

The master shall conduct drills and give instructions as necessary to ensure that all crew members are familiar with their duties. Routine drills had not been logged in accordance with emergency instructions to the satisfaction of the OCMI. 46CFR185.25-10

The inspection and maintenance of the equipment listed in paragraph (a) of this section shall be logged or otherwise documented for review by the Coast Guard upon request. Survival craft maintenance was not documented. 46CFR185.702(d)

Each rescue boat and ring life buoy must be marked with Type II retroreflective material approved in accordance with 46 CFR 164.018 or other standard specified by the Commandant. MI witnessed worn retoreflective material on ring buoys. Replace to the satisfaction of CG MI. 46CFR185.604(h)

The Commandant may, after suitable investigations and tests, accept fire fighting equipment, which is of a type not contemplated by the regulations in this part, if the safety of the vessel is materially improved thereby. Engine room did not contain any integrated smoke detection system. Ensure vessel comes into compliance with MSIB 05-23 to the satisfaction of CG MI. 46CFR181.05-5

A vessel must be provided with a main steering gear that is of adequate strength and capable of steering the vessel at all service speeds. Identified hydraulic lines weeping at operating station. Repair leaks to the satisfaction of the attending marine inspector.

On or before March 11, 1999; an existing vessel must comply with the bilge high level alarm requirements in 182.530. Bilge high level alarm in lazerette was inoperable.

Life floats. Each life float must be fitted with a lifeline, pendants, two paddles, a painter, and a light. The middle life float needs new lifeline; lifeline was found to be frayed/worn.

Except when operating on lakes, bays, and sounds or rivers routes in clam weather, all hatches and openings in the hull, except loading doors, of a vessel must be kept tightly closed except when being used. Dogs on weathertight hatch on rear bulkhead of accomodation space need to be tightened to ensure closure.

All unfired pressure vessels must be installed to the satisfaction of the OCMI. The design, construction and original testing of such unfired pressure vessels must meet the applicable requirements of subchapter F (Marine Engineering) of this Chapter. Gauge on port side air compressor only goes to 200 PSI; MAWP on tank is also 200 PSI; replace gauge to the satisfaction of the attending Marine Inspector.

When a vessel is equipped with two or more generators to supply ship's service power, the following requirements must be met: The generator circuit breakers must be interlocked to prevent the generators from being simultaneously connected to the switchboard. Generator interlock is missing.

Whenever there is a hazardous condition, as defined by 175.400 of this chapter, on board the vessel the owner, master, agent or person in charge shall immediately notify the COTP of the port or place in which the vessel is located of the hazardous condition. Hot water heater relief shall be piped into bilge.

Vsl must be marked IAW 46 CFR part 67. Vessel name and hailing port must be marked on the stern of the vessel. Missing stern area marking of vsl name and hailing port.

Fire extinguishing equipment installed on a vessel in excess of the requirements of 46CFR181.400 and 181.500 must be designed, constructed, installed, and maintained in accordance with a recognized industry standard acceptable to the Commandant (CG-ENG). Note 46 CFR 181.400 talks to Detection systems. Also New installations of fire protection equipment on an existing vessel, which are completed to the satisfaction of the OCMI on or after March 11, 1986, must comply with the regulation of this part.(New T-boat regs). Provide documentation/inspection tag showing the fire detection system has been inspected and functioning in accordance with manufacture recommendations.

Life Ring located on the aft pilothouse deck was found with water intrusion, effecting its buoyancy, due to damaged shell plug. Replace in kind.

Shore power connection box, located in the aft aux room, was found with housing broken and exposing personnel to the potential electrical hazards for the 220 electrical system. Replace connection box with suitable approved type.

Exhaust gas must not leak from the piping or any connections. The piping must be properly supported by non-combustible hangers or blocks. #3 engine exhaust flexible piping had a 2" x 5" hole in it, leaking exhaust in the engine room. #1 and #2 generators exhaust lines are not properly supported, they were missing bolts in hangers. SUN DAY shall replace or repair the faulty flexible exhaust pipe, #1 and #2 generator hanger bolts to the satisfaction of the Coast Guard Marine Inspector.

Sewage pump cable had a connection that is not of the pressure type that meets UL486A. Replace connections at the splicing with the appropriate connectors.

At each inspection for certification and at every other vessel inspection the bilges and other spaces may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might create a fire hazard, clog bilge pumping systems, or block emergency escapes. Engine and generator bilges were found with excessive fuel, oil and water in bilges. Pump all bilges dry, test machinery for leaks and make repairs accordingly prior to vessel's departure.

Fuel supply valve coming out of fuel tank has a leak on the valve stem. Complete repairs in order to stop fuel from leaking from valve.

Fuel water separator "Racor" was found to have a leak on the bowl on the Starboard center engine. Complete repairs in order to stop the fuel leak.

Rescue platform did not have a means of being secured to vessel.

Primary radio was not equipped with digital select calling capability.

Forward engine room watertight door hand wheel was loose.

Fire monitor suction pipe seachest valve hand wheel was not attached.

Man hole covers were not stenciled to keep clear.

Crewmember's rigger harness did not have a long enough tag line for climbing down onto the rescue platform.

Port and Starboard quarters each had cracked welds where the main deck joins the bulwarks near the scuppers.

VHF radio's Digital Selective Calling feature was not functional/operational.

Did not have a NAVTEX receiver; must have one installed or reduce route to <100NM.

Did not have the correct sized bell (11.8" or 300mm).

Several lights were missing a metal pin which does not allow the light to function as manufactured.

2 ring lifebouys had worn out retro-reflective tape which needed to be replaced.

Light was not attached around the body of the buoy.

3 floating lights were not attached around the body of the lifefloats, and were not marked with the name of the vessel.

Rescue platform had steel shackles with excessive corrosion, solid deck plate instead of a grated deck plate, and demensions were less than 36" X 36".

PFD stowage compartments were not clearly marked to reflect the correct number of PFDs stored inside each respective compartment.

Provide service report for Fire ext.

Missing 1 life jacket. Orginally 17 life jackets failed (Kapok) Port Captain was able to replace all except one on site.

Crop and renew port side shell IWO engineroom. Approximate area is six feet by ten feet. Excessive pitting taking place at longitudinal framing and side shell marriage.
