Vessel's anchor chain hawse pipe holed through aprox 3"x6". Vessel to make permanent repairs and provide ABS report attesting to repairs and successful testing of said repairs prior to departure.

13199: Electrical installations on vessels must ensure maintenance of services necessary for safety under normal and emergency condicitons. 40K hotel services generator gauge panel is loosely bolted to generator and is chaffing through gauge wiring. Correct prior to operation of generaor 46CFR111.01-1(a)(1)

06199: Pressure-vacuum relief valves shall comply with the requirements of subpart 162.017. All cargo tank PV valves are missing CG approval tags 14 cargo tanks, 2 vapor header, and aft residual tank PVs are missing tags as per 46CFR16.017. 46CFR151.15-5(b)

Each vessel vapor connection flange face must have a permanent stud. Port forward vapor connection missing stud. 46CFR39.2001(j) 46CFR39.2001(j)

46CFR32.60-1:Scantlings, material, and workmanship, subdivision of cargo spaces, fitting of cofferdams, and testing of tanks shall be at least equivalent to the requirements of the American Bureau of Shipping or other recognized classification society. CG Marine Inspectors attended vessel to conduct a damage survey on the OSG 243. Marine Inspectors observed a 10'X17'X3' hull penetration at the waterline on the port side in vicinity of the #5 wing ballast tank. There was also a 2'X3' penetration on the cargo trunk that extended over the #5 & #6 Port Side Cargo Tanks. There was also damage noted to the Port Side cantilever, which was completely crushed, and approximately 30' of crushed railing. Prior to movement, you shall submit a completed damage survey from a certified marine engineer, or from a Recognized Class Society Surveyor. You shall also submit a repair proposal to our office for review prior to conducting repairs. If you determine the vessel needs to shift to another berth to adequately complete the damage survey, you shall submit a transit plan to our office for review. You shall also contact Sector Houston VTS for any additional instructions.

Emergency towline was cut during storm following it becoming fouled around starboard prop of the vessel OSG Independence.

Prior to departure from New Orleans, lock out/tag out redundant electrical emergency shutdown system.

Notify class of the malfunction with the electrical emergency shutdown system for the pumps. Have class attend the vessel within 14 days to verify adequacy of installation for the emergency shutdown system.

Provide plan approval for redundant emergency cargo pump shutdown to the satisfaction of the attending marine inspector IAW 46 CFR 110.01-3

Pin hole leak was found in # 7 port cargo tank top. Proceed to Tampa, FL to offload. Prior to loading cargo, effect repairs to the satisfaction of attending Marine Inspector.

Vapor hoses did not have their annual hydrostatic test.

2" slop transfer hose did not have annual hydrostatic test.

Replace and prove proper operation of the #3 generator reverse power relay.

02 Sensor for Inert gas system failed while discharging crude oil. Due to failure IG is venting to atmosphere. Replace sensor and complete satisfactory test of IG system to attending Coast Guard Marine Inspector prior to offloading oils requiring Inert Gas.

Found 8 foot section of cargo piping with severe pitting. Hydrostatic testing to 1.5 MAWP required prior to transferring cargo through this line.

The #2 cargo pump emergency shutdown was inoperable.

Cargo hoses were stenciled with the incorrect MAWP.

MAWP of the air receiver is 200 psi. The relief valve setting is 125 psi. Could not reach 125 psi with either compressor. Required to remove the relief valve and bench test.

Properly clean and repaint, contrast in color, vessel's load line both port and starboard side. Deficiency must be completed on/before 08Jul09.

Provide seals for the cargo pump drive shafts at the pump house bulkhead on all pump houses.

Provide adequate number of washers for the emergency lightering equipment.

Provide (2) 8"X6" reducers for emergency lightering equipment.

Install flange stud on vapor mannifold connections as need. (Two forward connections)

Remove fuel drum from generator room prior to sailing.

Replace petcock in fuel/water separator for compressor with metal plug; or fit threaded plug into existing petcock valve.

Replace fractured exhaust bracket/hangar in #1 cargo pumproom in way of overhead.

Calibrate all cargo, VCS, and air receiver pressure gauges.

Hydrostatically test all fuel oil transfer hoses to 150 percent MAWP. Document MAWP for each hose. This item to be completed prior to using hoses in any petroleum transfer.

Make repairs to air system piping as necessary in two sections: 3-inch pipe and 6-inch pipe. To be completed prior to sailing.

remove fuel oil from dck of generator room prior to sailing.

Mark all fuel hoses with test dates of last Hydrostaic test prior to next transfer.

REINSTALL MISSING INSULATION AT EXHAUST MANIFOLD AND EXHAUST PIPING AT #1 CARGO PUMP ENGINE.

CROP OUT AND RENEW #4, #5, SIDE SHELL LONGS., AND #4, #5, #6 TRANSVERSE SHELF BRACKETS AND RELATED SIDE SHELL WHERE CRACKED OR BUCKLED IN THE PORT BOW RAKE. DAMGE IS TO BE INSPECTED AND LOGGED PRIOR TO EACH COASTWISE VOYAGE.

CROP AND RENEW #4,5 SIDE SHELL LONGS,AND #4,5,AND 6 TRANSVERSE SHELF BRAC- KETS AND RELATED SIDE SHELL WHERE CRACKED OR BUCKLED IN THE PORT BOW RAKE. VSL'S ROUTE RESTRICTED TO LAKES,BAYS AND SOUNDS,UNTIL PERMANENT REPAIRS AREAFFECTED.

DEMONSTRATE OPERATION OF THE #2 CARGO PUMP ENGINE EMERGENCY SHUTDOWN.

MAKE THE VESSEL'S TWO FIRED THERMAL FLUID HEATERS AVAILABLE FOR INSPECTION AND OPERATIONAL TESTING TO THE SATISFACTION OF THE COGNIZANT OCMI.

PREPARE THE TWO AIR COMPRESSOR OIL BATH PRESSURE VESSELS FOR INTERNAL EXAMINATION OR HYDROSTATIC TEST IN ACCORDANCE WITH 46 CFR 61.10-5.

PROVIDE EVIDENCE THAT ALL CARGO PUMP DISCHARGE PIPING RELIEF VALVES HAVE BEEN SATISFACTORILY TESTED IN ACCORDANCE WITH 46 CFR 35.35-70(B).

PROVE OPERATION OF THE FOLLOWING PRESSURE VESSEL RELIEF VALVES: GEN. STARTING AIR RECEIVER, AIR COMPRESSOR STARTING AIR RECEIVER, OIL BATH PRESSURE VESSEL IN ACCORDANCE WITH 46 CFR 61.10-5(J).
