On U.S. inspected ships, oily water separating equipment and bilge alarms must be approved under 46 CFR 162.050. Content meter for the Oily Water Separator digital display lights are not functioning as designed. Replace or repair content meter unit. 33 CFR 155.380

Vessel is required tug assist of sufficient horsepower and bollard pull rating when transiting in and out of the harbor. 74 SOLAS I/11

Where A class divisions are pierced for the passage of electrical cables... shall be made to ensure the fre resistance is not impaired. Multi cable transit under the bridge console found missing sections of fre stop. 74 SOLAS II-2/23(a)

The current carrying capacity of each circuit shall be permanently indicated, together with the rating or setting of the appropriate overload protective device. Multiple distribution panels throughout vessel found with inaccurate circuit directories. 74 SOLAS II-1/27(a)(vii)

Wiring shall be supported in such a manner as to avoid chafng or other injury. Shore power cable glands found incorrectly installed on the starboard shore power box. 74 SOLAS II-1/27(a)(vi)

Every company should develop, implement, and maintain a safety management system which includes the following functional requirements: instructions and procedures to ensure safe operation of ships and protection of the environment in compliance with relevant international and fag state legislation. Various Lock Out/Tag Out tags were found affxed to various electrical installations in the ship’s service spaces. LOTO tag numbers were not utilized, and were not logged in the Lock Out/Tag Out Log Book as required per the Ship’s SMS (C-01-170, Rev. 7, Lockout-Tagout Safety Procedures). Company shall conduct an internal audit focused on the lockout-tagout procedures.

Steering failure on primary steering control resulting in rudder being stuck Port 10 degree. Vessel has back up steering which is operating as designed.

(a) No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. Vessel's #2 Ships Service Generator shut down due to high frequency resulting in a loss of power and loss of propulsion.

(a) Vital systems that are automatically or remotely controlled must be provided with— (1) An effective primary control system; (2) A manual alternate control system; (3) A safety control system, if required by § 62.25-15; (b) Automation systems or sub systems that control or monitor more than one safety control, interlock, or operating sequence must perform all assigned tasks continuously, i.e., the detection of unsafe conditions must not prevent control or monitoring of other conditions. Operational readiness: Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. The bowsing slip-hook attachment on both port and starboard lifeboats are seized and do not operate as required.

Training manual: a training manual shall be provided and shall explain; operation and use of fire-fighting systems and appliances. Training manual does not explain operation of the two SCBA fire-fighting appliances used onboard.

General arrangement plans (fire control plan) shall be permanently exhibited...clearly showing fire sections; fire alarm systems; fire extinguishing appliances. Fire control plan (FCP) is missing the proper labeling for the installed CO2 piping, CO2 discharge alarm, and CO2 protected space. FCP does not properly identify CO2 protected spaces. Bosun stores noted as not having CO2 system installed, but has CO2 protecting space.

In every inspection/survey of the ship, the Administration shall fully guarantee the completeness and efficiency of the inspection and survey, and shall undertake to ensure the necessary arrangements to satisfy this obligation. Approved PSTPs were not available during inspection, provide approved Periodic Safety Test Procedures as well as records indicating when lasts tests occurred. Additionally, provide historic documentation of any automation equipment changes/updates completed to the vessel and associated plan approval for such changes/updates to include updating to Konsberg alarm system. 74 SOLAS (20) I/6

Vessel shall conduct internal audit focusing on procedures for Oil Record Book entries, proper procedures for documenting/repairing inoperable equipment, and lock out/tag out procedures. Audit and corrective action shall be completed within three months. 74 SOLAS (20) IX/3 ISM Code Part A/1.4

Every ship of 400 gross tonnage and above shall be provided with an Oil Record Book Part I. The Oil Record Book shall be in the form specified in appendix III to this Annex. MI found that Oil Record Book consisted of inaccurate entries and inconsistent measurements. MARPOL I/17.1

Personal equipment shall consist of the following: electric safety lamp (hand lantern) of an approved type. During fire drill, MI found that some of the firefighters did not have flashlights. 74 SOLAS(20) II-2/10.1.1 FSS Code 3.2.1.1.5

Personal equipment shall consist of the following: axe with handle provided with high voltage insulation. During fire drill, none of the firefighters were equipped with axes. 74 SOLAS(20) II-2/ 10.1.1 FSS Code 3.2.1.1.5

In all classes of watertight doors indicators shall be fitted which show, at all operating stations from which the doors are not visible, whether the doors are open or closed. WTD position indicator switch in shaft alley was found disconnected and no signage to keep closed while underway was present. 74 SOLAS (74) II-I/13(f)(V)

Lighting fittings shall be arranged to prevent temperature rises that would be injurious to the wiring, and to prevent surrounding material from becoming excessively hot. Exterior lighting fixture on main deck at CT 4P was found to have a fractured wiring housing. 74 SOLAS (74) II-1/27(a)(v)

Fuel oil piping system on main engine fractured and leaking. Properly repair system to the satisfaction of the attending marine inspector.

Liferafts shall be stowed as to float free in the event of the ship sinking. All 04 inflatable liferafts were found improperly installed, with the painter line not attached to the weak link, thus preventing the liferafts from floating free. 74 SOLAS III/16(m)

The international shore connexion required in this Chapter shall be kept aboard the ship together with a gasket of any suitable material for 10.5 kilogrammes per square centimeter (150 pounds per square inch) service, together with four 15 millimetre (5/8 inch) bolts, 50 millimetres (2 inches) in length and eight washers. The port side international shore connection is missing the gasket and the starboard side connection is missing a bolt and two washers. 74 SOLAS II-2/5(h)

Per ship's SMS, danger tags shall list the installer's name and be signed by all newly reporting crew members. Ship's crew was not following tag out procedures as noted by a lack of information on currently installed danger tags. 74 SOLAS (14) IX/3.1 ISM Code A/1.4.2

The three engine room international shore connections were found not slotted.

EDG fuel shut off valve is functional but need to be tightened and maintained. Hand wheel was loose.

Emergency Escape hatch located starboard side from the 2nd deck to the 3rd deck was found with not all 4 dogs making contact.

Aft Aux Electric Equipment RM located port side Aft House Upper Deck was found with a hold back. The door to the space was marked with as CO2 protected space and siren and deliver nozzle were noted.

Carpenters Shop starboard side 81'-8" Flat. The door was marked as CO2 protected space and a siren and delivery nozzle were visible. The door was missing the latch and was tied open.

Vent shut off for damper no. 100 located in the Aft Aux Electric Equipment RM located port side Aft House Upper Deck was found with remote reach rod disconnected.

Aft house door starboard side at 81'-8" level was found with the upper latch loose and needing adjustment.

120v REFRIG, UPPER DECK & FOSCLE DK POWER panel no. FDR 1FP-105 was found missing lower latch key tumbler and with breaker no. 16 tagged out due to possible ground.

#1 chiller expansion joint found cracking. Ships crew created a job in the PM system to renew the expansion joint. The fresh waster cooling to the chiller is non critical system.

Crane hydraulic oil tank found leaking. Located port side deck machinery room.

Florescent lights found missing covers. Located on the old pool deck.

Fire Control plan were blurry. The fire control booklet on the bridge was accessible and easy to read.

Fire-fighting systems shall be kept in good working order. CO2 dispersant head found caked in HFO behind the main engine fuel oil booster pumps. Vessel shall clear the head of all oils or renew as original.74SOLAS(14)Ch.II-2 14.2.1.2

Structural fire protection shall be kept in good order to ensure their required performance. Doors leading from the engine room to the purifier room were found ill fitting to their frames and allowing the passage of light/vapors between spaces. Required A-0 boundary shall be reinstated. 74SOLAS(14)Ch.II-2 14.2.1.2 and 74SOLASCh.II-2 Reg. 20 Table 1

Vessel shall not maintain excess oils and debris in the bilges. Excess turbine oils found under the SSTG in and around heated oil piping. Vessel shall ensure all excess oils are removed.

Watertight escape hatch from the ECR found out of adjustment and unable to seal. Adjust hatch to ensure it is watertight.

Emergency steering instructions include the labeling of valves to be used, identified valve "F" missing label on the stbd steering gear installation. Provide label.

Engineering flat rudder angle indicator linkage found unbolted and near failure. Renew missing bolts in linkage.

Fire control panel in the port tunnel was found with 5 inoperable indicator lights. Repair inoperable lights.

The #6 cylinder jacket water temperature probe was found lose from its housing and was identified as too large to be properly secured. Provide correct size temperature probe and secure as instructed by manufacturer.

Cylinder 1 and 2 were identified as missing exhaust temperature gauges. Gauges were subsequently found on the engine behind the cylinders having broken free from its mountings. Re-install gagues in a visible area for watch standers.

Auxiliary boiler found with inboard leaking water level glass gauge. Repair gauge.

During round of the engine room, fire extinguisher #49 was found off station. Replace extinguisher to it's assigned location.

Two steam valves in the steam heating inlet line to the fuel heater were found leaking. Repair/repack valves to ensure they will not cause personnel injury.

Jugs of boiler chemicals and kerosene were found improperly stored behind the MCCC motor controller panel in the engine room. Kerosene shall be stored in a paint locker and the chemicals are to be properly secured in their shelves.

SSDG number 1 found with section of exhaust lagging soaked with oil. Renew lagging to minimize fire hazard.

Ladder from the engine room to shaft alley found lose and unbolted from its upper foundations. Secure ladder.

Door to aft steering flat found with CO2 Warning sign painted over and illegible. Remove paint from sign or provide legible warning.

A survey, either general or partial, shall be made every time a defect is discovered which affects the safety of the vessel or whenever any important repairs are made. Vessel personnel noticed a slight leak on the main engine fuel line at an expansion joint.

When repair of machinery or piping systems becomes necessary, the work shall be done in accordance with the regulations in effect at the time of build or modification.
