A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs or a defect is discovered which affects the safety of the vessel or the efficacy or completeness of its lifesaving appliances, fire-fighting or other equipment, or whenever any important repairs or renewals are made. The survey shall be such as to insure that the necessary repairs or renewals have been effectively made, that the material and the workmanship of such repairs or renewals are in all respects satisfactory, and that the vessel complies in all respects with the regulations. The vessel's crew self-reported that the bow thruster is inoperable due to an active fluid leak, which resulted in a visible sheen on the water. The vessel is authorized to transit to the CSX Dock No. 2 in Toledo, OH, to conduct repairs. Repairs shall be completed to the satisfaction of the local Officer in Charge, Marine Inspection (OCMI). The vessel is authorized to transit under its own main propulsion but shall utilize commercial tug assistance to facilitate safe maneuvering and mooring near the repair berth. 46 CFR 91.30-1

At each inspection, the inspector shall examine the tank tops and bilges in machinery spaces to see there is no accumulation of of oil which might create a fire hazard. There was significant pooling of oil under the generators. Must be pumped out of the space prior to departure.

Nothing in this subpart shall be construed as limiting the inspector from making such tests or inspections as he deems necessary to be assured of the safety and seaworthiness of the vessel. When testing the #1 generator/prime mover, the emergency stop function on the EMC and the switchboard did not stop the prime mover. Emergency stop function must be repaired prior to use of the #1 generator.

An inspection either general or partial depending on the circumstances shall be made whenever any important repairs are made. Multiple steel repairs throughout the vessel must be inspected by Coast Guard or ABS prior to departure.

Illustrations and instructions must be conspicuously displayed at each muster station including the method of donning lifejackets and immersion suits.

No repairs or alterations affecting the safety of the vessel with regard to the hull, machinery, or equipment, shall be made without the knowledge of the Officer in Charge, Marine Inspection. The #5 cargo tank has a 2" fracture approximately 32ft from the cargo tank bottom allowing water from the #7 ballast tank to leak into the tank at a rate of 1-2 gallons per hour. Make repairs to the same. **The vessel is authorized transit from mile marker 330 on the Calumet River to Alpena, MI to offload. The vessel may then continue under ballast to Cedarville, MI to affect repairs. Repairs to be made prior to departing Cedarville, MI.

On 5 April 2025 Vessel captain notified MSU Cleveland that during pre-departure ballast tank checks that 2 leaks were discovered in the side shell in the #5 starboard ballast tank. They also found leaks between #5 and #6 ballast tanks, and a leak in cargo hold #4. Cargo hold and ballast tank interior bulkhead leaks were drill-stopped, gouged and rewelded with a full penetration weld. The 2 side shell leaks were cropped and renewed. ABS conducted testing on leaks and have accepted the repairs.

Vessel is authorized to will proceed to Escanaba, MI for offload then to Calsite, MI for permanent repair.

Master notified Sector Northern Great Lakes that while loading the vessel several pin hole leaks were discovered and in need of repair. The port no. 05 ballast tank has a pinhole leak below the water line where as it is allowing approximately 1 gallon of water ingress a minute (fixed system able to monitor and control ingress). Additionally the Port 07 and 08 also have pinhole leaks and are in need of repair. Vessel is to proceed to Cedarville, MI and make repairs to the satisfaction of ABS prior to departure.

Forward life ring light battery expired.

Life jacket and survival suit lights were operating but over half had expired batteries. Expired batteries were replaced on the spot.

Float free inflatable life raft weak link was improperly secured (painter was not tied securely to weak link). Corrected on the spot.

Vessel has a cracked weld and an inset caused by mechanical means causing water to leak from the 7SB into the #5 cargo hold. Vessel is currently loading at Port Dolomite and is authorized to transit from Cedarville, Michigan to Buffington Harbor in Gary, Indiana to effect temporary repairs to the satisfaction of the cognizant OCMI, with permanent repairs to be conducted at the vessel's 2024 drydock availability.

Vessel is to effect permanent repairs to the #2 and #3 stbd cargo holds by 31MAR2024.

A survey, either general or partial, according to the circumstances, shall be made every time an accident occurs. Master made notification to [name withheld], of a grounding of the vessel. Master attests to no damages, pollution, or water ingress as a result of the grounding. Master has requested to sail to Buffalo, NY where ABS is scheduled to attend the vessel and tug for survey after incident. Vessel Chief engineer or other representative shall conduct hourly rounds to ensure vessel remains in safe condition while in transit.

A fracture was reported in the No. 06 ballast tank above the water line. Vessel is authorized to transit to Ashtabula, OH for winter lay-up. Vessel is to make permanent repairs prior to departure. Repairs must be made to the satisfaction of class. Master shall take hourly readings of tank levels and shall immediately notify the cognizant OCMI if any conditions of the vessel change.

Each generator must meet the applicable requirements for construction and testing in section 4-8-3 of the ABS Steel Vessel Rules. Vessel reported that 1 of 4 generators were unable to start prior to getting underway due to issue with starter. Repair generator to satisfaction of attending marine inspector prior to end of 2023 winter layup period.

A survey shall be made every time an accident occurs which affects the safety of the vessel. Vessel ran aground OOA 12-22-2022. Provide ABS report of survey, including any affected ballast tanks prior to departing Sector Buffalo.

The TD is set to release at 61 seconds but didn't release until 92 seconds, which is beyond the +20% standard set by NFPA. Prove proper operation of time delay prior to movement of the vessel. §95.16-45(a)(2)

Hull Fracture in the FWBT #2 port with water ingress. fracture is in way of deep web frame just above turn of the bilge. Temporary Cement box has been constructed to stop water ingress while vessel transits to unload cargo and then to a repair facility.

Vessel sustained a hull fracture during mooring evolution IVO the #1 ballast tank on the starboard side. Complete permanent repairs to the satisfaction of Class and Coast Guard prior to departure.

Temporary repairs were affected to an area of bottom plate where welder damage occurred causing pinholes in the bottom plate. The area is within the #6P ballast tank at fr 53 deep web floor connection. Box patch is installed. Remove box patch and make permanent repairs to the area by insert.

Provide a CG approved first aid kit to meet the requirements of the accepted conditional occupany of the barge.

Install a safety net to cover the area below the gangway access to the towing vessel in the notch to prevent falls between the connection. This item is within the vessels SMS although not installed.

Install exhaust lagging off of the turbo elbow on the #4 unloading generator.

Make modifications to the SMS to ensure compliance with personnel transfer requirements for logging while at sea. The SMS currently states that personnel will have access as needed. This should be modified for Captain or Bridge personnel to authorize transit and log persons transferring with a time on and off. See SMS.

Make permanent repairs to 2-3" fracture within the 7S ballast tank at fr 65 where the long bulkhead meets the slow of cargo hold.

An unloading generator was renewed without the knowledge of the Cognizant OCMI. Company to provide specifications of the generator removed, generator installed, and any alterations undertaken to the cognizant OCMI per 46 CFR 91.45-1 (a) & 46 CFR 110.30-7

Contact a CG MI to attend and verify the generator installation. Demonstrate proper operation of the new unloading generator and conduct any other tests deemed necessary to the satisfaction of the attending CG MI per 46 CFR 91.27-15

Repair 3" diameter hole in portside #3 ballast tank to satisfaction of attending marine inspector.

Make repairs to hydraulic unit pump seal for cargo gates system.

Provide non-conducting deck covering at front and rear of switchboard.

Make repairs to #4 generator to meet standards of ABS rules for steel vessels 4-8-3.

Label circuit #5 in panel 4P109.

REMOVE SCALE FROM STARBOARD TUNNEL OUTBOARD BULKHEAD PITTING IN WAY OF APPARENT COATING FAILURE, CRITICAL AREAS INCLUDE WELDMENT AND PLATING BETWEEN GATES 27-18. PROVIDE EVIDENCE TO LOCAL OCMI THAT CORROSION IS WITHIN TOLERANCES OR SUBMIT REPAIR PROPOSAL.

REMOVE BRASS PET COCKS FROM RACOR FUEL FILTERS.

PROVE OPERATION OF ALL THREE (3) GENERATOR OVERSPEED TRIPS, REVERSE POWER RELAYS, LOW OIL ALARM/SHUTDOWNS AND ENGINE HIGH TEMPARTURE ALARMS.

RENEW OLD/WASTED FLEXIBLE FUEL HOSES ON THE THREE (3) GENERATOR RACOR FUEL FILTERS.

Make barge ballast tanks, #1-#4 wings, gas free nad available for further evaluation by the attending CG inspector.

Effect drydock and repairs to ballast tanks #1-#4 wing, and nay other discrepencies not at that time by the attending CG inspector.

MAKE REPAIRS TO SECOND INBOARD FROM AFT VERTICAL WEB IN NO. 3 PORT BALLAST TANK. VSL ALLOWED 2 ROUND TRIPS OR 30 DAYS WHICH EVER COMES FIRST.

PROVE OPERATION OF #3 GENERATOR REVERSE POWER RELAY.

REPAIR PORT RUNNING LIGHT. VESSEL IS LIMITED TO DAYLIGHT OPERATION UNTIL LIGHT IS OPERATIONAL.

THE VESSEL IS TO BE REMOVED FROM SERVICE IN THE MONTH OF FEB 1992 AND THE BALLAST TANKS OPENED FOR INSPECTION BY A COAST GUARD INSPECTOR.

REPLACE MISSING FAN GUARDS #1, #2, & #3 GENERATORS.

PROVIDE COVERS FOR FLOURESCENT LIGHTS IN ENGINE ROOM.

PROVE OPERATION OF #3 GENERATOR OVERSPEED TRIP AND REVERSE POWER TRIP.

REPLACE MISSING DOG ON #4S BALLAST TANK VENT COVER.

FREE-UP ALL DOGS ON W/T HATCHES, DOORS AND VENTILATION OPENINGS.

SERVICE 2 DISCHARGED FIRE EXTINGUISHERS IN TUNNEL.

HOSE TEST DECK INSERT ABOVE GENERATORS.

REPLACE BROKEN BULBS AND GLOBES IN TUNNEL.

REPAIR BROKEN AIR LINE TO SLOPE SHEET VIBRATOR (STBD SIDE 3/4 AFT).

VEE AND WELD CRACKS (AS MARKED) ON ALL HATCH COAMING COVERS.

PROVIDE CG FORM 2692 (COMPLETED) AND NAVAL ARCHITECT REPORT ON CAUSE OF FAILURE OF STRUCTURAL MEMBERS IN THE BALLAST TANKS. SUBMIT W/IN 30 DAYS.

COMPLETE THE MODIFICATIONS WHICH ARE PROPOSED BY THE NAVAL ARCHITECT AND APPROVED BY ABS FOR THE BALLAST TANKS. COMPLETE W/IN 180 DAYS.

PROVIDE "CLAS APPROVED" PROPOSED MODIFCATIONS FOR BALLAST TANK STRUCTURAL MEMBERS. COMPLETE W/IN 90 DAYS.

LABEL FUEL SHUT OFF VALVES.

REPAIR LIGHTING GENERATOR OVER SPEED TRIP.

REPLACE MISSING DOOR AND PROVIDE CIRCUIT DIRECTORY FOR CARGO HULL LIGHTING PANEL.

PROVIDE METAL BOWLS FOR RACOR FUEL FILTER STBD GENERATOR.

SECURE DECK LIGHT STBD SIDE 2ND LIGHT FROM FORWARD.
