Vessel does not meet the requirements of subchapter M. Vessel has significant structural issues, firefighting and lifesaving equipment that hasn't be serviced, extension cords used as permanent cables, and hasn't implemented nor maintained required documentation, logs, servicing records for towing gear, health and safety, or any other required item. 46 CFR 137.120 (a)

The OCMI may require additional examinations of the vessel whenever they discover or suspect damage or deterioration to the hull plating, including: removal of the vessel from service to assess the extent of the damage and to affect permanent repairs.
Vessel's bulwarks were completed wasted through, in the forward most void on the starboard side a 1.5 inch opening was observed in the hull allowing sunlight to enter into the space. The aft void space appeared to be actively flooding and the bilge pump was continuously running to dewater it. Vessel must go to dry dock so that the hull can be assessed and repairs made. 46 CFR 137.300 (b) (2)

Each towing vessel must be maintained and operated so as to minimize fire hazards and to ensure that all bilges and void spaces are kept free from the accumulation of combustible and flammable materials and liquids.
Vessel's engines and generators were leaking an excessive amount of oil. The entirety of the vessel's bilge was filled with an excessive amount of oil. 46 CFR 142.220 ( a )
