Has USB auto-run been disabled on Matson Owned systems? 2. Are blocks or locks intact and applied per plan." The manning requirements for a particular vessel are determined by the OCMI after consideration of the applicable laws, the regulations in this part, and all other factors involved, such as: emergency situations, size and type of vessel, installed equipment, proposed routes of operation including frequency of port calls, cargo carried, type of service in which employed, degree of automation, use of labor saving devices, and the organizational structure of the vessel. Vessel has open ABS finding 2448.3 for a malfunctioning engine room alarm repeater on bridge and is awaiting repairs. In the interim, vessel to maintain full engine room watch until alarm panel is repaired to the satisfaction of ABS. 46 CFR 15.501

Where “A” Class divisions are pierced for the passage of...ducts..., arrangements shall be made to ensure that fire resistance is not impaired. Fire damper in OS Locker on bridge has a fire damper access that has been blanked off and cannot be operated. 74 SOLAS II-2/23(a)

The master, owner, operator, agent, or person in charge of any vessel equipped with ballast water tanks that operates in the waters of the United States must follow these practices: Maintain a ballast water management (BWM) plan that has been developed specifically for the vessel and that will allow those responsible for the plan's implementation to understand and follow the vessel's BWM strategy and comply with the requirements of this subpart. Vessel did not have correct BWM Plan onboard when called for by Marine Inspector. Vessel had old plan until approved one was found and presented while inspectors were onboard. Rectified on the spot. 33 CFR 151.2050(g)

The Administration of a Party may issue an exemption from specific provisions of this Annex for a ship to conduct trials for the development of ship emission reduction and control technologies. Vessel is currently operating under an exemption from MARPOL Annex VI Reg 14.4 issued by USCG and EPA. However, the vessel has submitted a modification to the exemption and is awaiting final approval. Vessel must obtain updated permit. Please note, vessel may be subject to fines under MARPOL Annex VI for alterations not covered under the existing exemption permit if operating within the waters, other than those under U. S. jurisdiction, of parties signatory to MARPOL Annex VI. MARPOL 73/78 (2017 ed.) Annex VI/Reg 3.2

Where A Class divisions are pierced for the passage of electric cables arrangements shall be made to ensure that the fire resistance is not impaired. MCT in overhead of garage was to be repaired to satisfaction of attending surveyor.

In all new and existing ships, fire-extinguishing appliances shall be kept in good order and made available for immediate use at all times during the voyage. CO2 dustcaps at end of distro lines have not been removed & blown through IAW manufacturer's, prove/blow through.

Except as provided in paragraph (3) of this regulation, ventilator openings shall be provided with weather-tight. Vent closure 2-72-2 fresh air supply is not weather-tight, hinges and hardware needs repair or replacement. Refer to ABS class report number HL3749663.

All access openings in bulkheads at ends of enclosed superstructures shall be fitted with doors of steel or other equivalent material, permanently and strongly attached to the bulkhead, and framed, stiffened and fitted so that the whole structure is of equivalent strength to the unpierced bulkhead and weather-tight when closed. Weather-tight door across from fire damper 2-64-1 needs dogs adjusted. Light is coming through upper corners of door. Refer to ABS class report number HL3749663.

Each liferafts shall be stowed as to permit manual release of one raft or container at a time from their securing arrangements. Liferaft hydrostatic release incorrectly installed on starboard liferaft forward and starboard liferaft aft. HL3749663

Means shall be provided for stopping ventilating fans serving machinery and cargo spaces and for closing all doorways, ventilators, annular spaces around funnels and other openings to such spaces. Ventilation louvers on supply vents #2 and #3 did not fully close. Refer to ABS class report number HL3749663.

The hull, superstructure, structural bulkheads, decks and deckhouses shall be constructed of steel, except where the administration may sanction the use of other suitable material in special cases, having in mind the risk of fire. Cable penetrations in bulkheads leading to the aft auxiliary electrical equipment room and the penetration near ladder well leading to the 51'-10 1/2" flat were impaired. Refer to ABS class report number HL3749663.

The following basic principles underlie the regulations in this chapter and are embodied in the regulations as appropriate, having regard to the type of ships and the potential fire hazard involved: restricted use of combustible materials; containment and extinction of any fire in the space of origin. Lifeboat batteries were not marked with an expiration. Vessel must prove acceptable life of the battery and mark accordingly, or renew annually.

Electrical installations shall be such that the safety of passengers, crew and ship from electrical hazards will be ensured. Electrical panels 2-89-2 & 2-90-2 in STBD tunnel have open breaker slots that need to be covered by blanks.

Doors and frames in "B" Class divisions shall have resistance to fire. Doors shall be non-combustible, and openings in doors shall be fitted with non-combustible materials. The BOSN's state room kick-out panel was replaced with plywood.

The following basic principles are embodied having regard to the type of ship and potential fire hazards involved: containment and extinction of any fire in the space of origin. The following multiple cable transits need resealed as per type as design approvals. Emergency gear locker overhead port side. Emergency gear locker STBD bulkhead. Port side stores crane wedge block was intproperly installed.

A/C dampers within accomadations were INOP.

Port side dampers on top of garage had 02 louvers that failed to seat.

Flashlight INOP in port LB.

Submit permanent repair proposal for longitudinal fracture located at approximately frame no. 67 on starboard side for approval of next U.S. port currently determined to be Sector Los Angeles/Long Beach.

Until operation and control of the vessels propulsion system are verified by an attending CG marine inspector or class surveyor, the vessel is required to utilize two assist tugs of adequate size and horsepower when entering or departing any port within sector Honolulu's marine inspection zone.

Renew missing guards for engine room cylinder lube oil tank sight glass.

Renew broken pipe hanger for EDG hydraulic oil start system.

Make aux boiler upper drum water level gauge glasses readable.

Amend or remove #2 SSDG differential trip Automation Test Procedures to account for inability to perform test as written.

Make repairs to broken sound powered phone contact maker at the port side bunker station.

Fracture identified at the bulwark outboard of the port bunker station just below the connection to the garage. Make repairs to fracture and inspect for same on the stbd side.

Provide mechanical plug for the vent containment on the portside adjacent to container bay 29.

Provide non conductive matting in front of all switchboards in the 02 deck fwd machinery space.

Vessel is permitted to carry cargo enroute to Los Angeles, CA to perform modifications to the installed Yokogawa steering power control system, in order to correct known design flaw regarding the power control relays for the steering solenoid operated directional control valves. Vessel must notify the Los Angeles OCMI prior to begging the modifications.

Vessel is required to be escorted by two assist tugs of adequate horsepower when entering or departing any U.S port until the above mentioned corrective modifications are made to the Yokogawa steering power control system and the corrections are verified and tests are witnessed by an attending CG marine inspector or attending class surveyor at the discretion of the cognizant OCMI.

While enroute to Los Angeles, CA and operating at max normal cruising speed, record amperage readings at both solenoids for each of he steering system 4-way directional control valves. Readings should be taken while rudder is moving PORT to STBD.

Record voltage at both soleniods for each of the 4-way directional control valves. These voltage readings should be taken simultaneously at both solenoids while the rudder is moved to PORT, moved to STBD and moved to admidships.

Provide results of the amperage and voltage readings for the 4-way directional control valve soleniods to the Sector Honolulu office at [email withheld]

Upon arrival in Los Angeles, CA check & record the oil levels for each steering system 4-way directional control valve soleniod which was operated during the transit from Honolulu. Provide the results to the Sector Honolulu inspections office.

At completion of steering control system modifications, conduct the following tests prior to departure: - Demonstrate proper operation of steering control system. - Verify that voltage at each soleniod for the steering system 4-way directional control valves is at 115 volts AC, no more than +\- 10% as specified by the soleniod manufacturer.

After departing Los Angeles, CA, repeat items #3 and #4 and provide the results to the Sector Honolulu inspections office at [email withheld]
