Each boiler must be available for examination at the intervals provided in table 61.05-10. Vessel is overdue on boiler inspections. Vessel movement is restricted until the boiler inspections are completed. If the vessel needs to shift to another location, this office shall be notified and approval must be given prior to any movement.

For underwater hull repair completed on July 13, 2021 in Port Everglades, the area of repair must be inspected at next dry dock by a Coast Guard Marine Inspector.

When major alterations or major repairs of tank vessels become necessary the work shall be done under the direction of the Officer in Charge, Marine Inspection,and shall be in accordance with the regulations in effect for new construction insofar as possible. When minor alterations or minor repairs of tank vessels become necessary such work shall be under the direction of the Officer in Charge, Marine Inspection, and shall be in accordance with the regulations in effect at the time the vessel was contracted for or built, or in accordance with the regulations in effect for new construction insofar as possible. Vessel sustained an approx 2 inch wastage hole in way of the #3 port double bottom ballast tank. Although temporary repairs are made, additional repairs shall be made to the satisfaction of the cognizant OCMI and attending CG Marine Inspectors. Vessel is authorized to transit in accordance with submitted transit plan, to the Sector Miami area to make additional repairs in accordance with submitted repair proposal. Sector Miami shall be notified prior to any repairs being made and/or if any changes in condition occur. 705: Prior to offloading cargo.

The company should establish procedures to ensure that the ship is maintained in conformity with the provisions of the relevant codes and regulations and with any additional requirements which may be established by the company. Vessel SMS "HSE-section 20" pg. 8 Required that any marine casualty or accident or threat to thereof must be reported to USCG immediately. The USCG was not immediately notified of the ingress of water into the vessels #3 port double bottom ballast tank due to a hole in the hull. The incident was not logged in the ships official logbook until 4 days after the incident was discovered.

As deliniated in deficiencies 1-2, objective evidence attests to the vessels crew failing to effectivly impletment basic SMS procedures related to reporting damage to the vessel effecting its seaworthiness. The failiure of the implementation of the SMS poses a significant risk to the safety of the crew. An additional internal verification of the vessels SMS focusing on the above items is required within 90 days in acordance with CVC-WI-003(1)

Port boiler superheater safety valve failed to lift in accordance with it's stamped pressure.

Each survival craft, except for inflatable craft, must be stripped, cleaned, and thoroughly inspected and repaired, as needed, at least once each year. Starboard life boat to be stripped, cleaned and thoroughly inspected by a certified lifeboat company. The following has been noted engine cover is loose, side widows are etched fittings for dewatering are cracked.

The ship’s crew has installed new LED Lights on deck in a hazardous zone, submit installation plan info to USCG Marine Safety Center Plans must be submitted for Coast Guard review in accordance with § 110.25–3: Deck wiring or schematic plans of power systems and lighting systems.

Scuttles/Hatchway coamings at positions 1 and 2 the height above the deck coamings fitted with weather-tight hatch covers of steel or other equivalent material fitted with gaskets and clamping devices shall be as specified. Emergency escape from Auxiliary Machinery Space have scuttles which the locking pockets for the dogs are holed. Make pockets watertight.

Immerision suits shall be fitted with light and whistle. Immersion suit in Engine control room found without operational light.

Discharge of oil or oily mixtures into the sea from machinery space bilge is prohibited, unless the vessel is equipped with an approved, under 46 CFR 162.050, 15 ppm oil content meter. The vessel's OWS oil content meter was removed and sent ashore for maintenance/repair. Prove proper operation of oil content meter when reinstalled to the satisfaction of attending Coast Guard Marine Inspector.

essel's Chief Engineer identified numerous wasted generating tubes (12-14) in the starboard boiler and verified by a USCG [name withheld] to the amount of present plugged tubes in the same pass, vessel must provide documentation from the boiler manufacturer attesting that the amount of tubes to be plugged in that row (30+ out of 52) falls within allowable limits.

Vessel must submit in writing a repair proposal for the starboard generating tubes to be plugged.

In weather locations, on a tankship, electrical equipment must be instrinsically safe, explosion proof, or purged/pressure type. On the open deck, over the cargo area, there are two lighing fixtures(port and starboard side near manifolds) that have shattered covers. As a temp fix the lighting cable has been removed and terminated in an approved junction box. Make permanent repairs to the satisfaction of cognizant OCMI

Electrical installations on vessels must ensure protection of crew and other persons from electrical hazards. The upper deck light panel L-104 closure is being held shut with masking tape due to the closing device being inoperable.

Each survival craft launching and recovery appliance must be approved under 46 CFR 160 ... with a winch approved under 46 CFR 160 Subpart 160.115. The starboard side lifeboat winch data plate is missing an no approval specification could be verified. Provide approvel spec for winch.

Fire control plan does not reflect the purpose of which space is used for throughout vessel. Review fire control plan and make changes as needed prior to August 6, 2018.

Ventilation is starting to accumulate excessive lint/dust causing fire hazard. Clean all vents.

Correctly end wire in fan room cabin deck.

Vessel emitted turbine oil, which was used as stern tube lubrication, through the shaft seal into the Penobscott Bay. Vessel is to submit repair plan proposal to Coast guard and Classification Society; once approved, vessel is to affect permanent repairs to affected system prior to departure from port.

Vessel has altered the original wire rope anchor point on the foundation to the Port lifeboat launching appliance fwd and aft arms from the original design, must recieve authorization from OEM and submitauthorization from the OEM and submit drawings, material list, through the [name withheld] for approval by MSC/CVC.

Vessel made repairs and alterations to the port and stbd life boat launching appliances. Repair and alterations were made without advance notification to the OCMI. Vessel is required to submit plans for approval to ABS and the Marine Safety Center. Repairs must be made with ABS and Coast Guard oversight. Upon completion of repairs proper testing must be conducted. A weight test must be conducted and witnessed by a Coast Guard Marine Inspector. Stbd life/rescue boat will be taken out of service until all above requirements are fulfilled.

Vessel had identified a stern tube leak when departing last port. Provide a dive survey with root cause and any corrective action or required repairs.

Port anchor windlass sliding clutch failed.

Discovered a small pin hole leak on the emergency fire main piping, replace the piping to good metal.

The paint locker vent piping is badly corroded and needs to be replaced.

The butterworth piping arrangement brackets appear to have undue strain (i.e. springs on brackets are fully compressed). Verify that the brackets for this system are installed in acoordance with ANSI B31.1.

The relief valve did not work properly on the air reciever for the deck lower air compressor, replace the relief valve.

While conducting the PSTP's for the throttle control system, item 2.2, the L/O pressure trip did not work in accordance with the approved PSTP's. Prove proper operation of the test or consider changing the PSTP to correct the test procedure properly.

Repair or replace supply vent closure to aft storeroom.

Repair inoperable stern tow light.

Relocate steam line termination above foam supply line valve IWO forward Cargo Control Room bulkhead.

Replace damaged armored cable leading to explosion proof light forward of Cargo Control Room on the port side.

Repair brackets and replace metal cable hangers on electrical junction boxes under cargo manifold.

Replace leaking fire main hydrant valve at forward most port side hydrant.

Replace missing front panel on starboard ECDIS.

Properly repair/replace malfunctioning forward hatch on port side lifeboat.

Vessel shall provide technician service report attensting to fully operational radar required by 33CFR164.37(a).

Make permanent repairs to the s band marine radar system before departing Searsport, ME.

Vessel has installed a new ODME, prior to use USCG MI needs to witness the following: 1 Hydro test of all newly installed piping associated with changing over of slop tanks, 2 Provide welders certificates, 3 Demonstrate that crew who operate the new ODM can properly operate it, 4 Demonstrate that the information can be safely retrieved. See attached CG835, activity narrative and uploaded document

#2 block valve on fire main was inoperable in the open position. Blocks #2, #3 and #4 on the foam main were inoperable in the open position. repair or renew block valves.

Aft 2nd deck weathertight accomodations door. Lower panel on door frame had severe wastage. Crew caulkled with fire resistant material and weleded a steel plate over. Door frame will be replaced when back in the USA. Extended 30 days. See narrative.

Some fire and foam main valve block indicators are unreadable. Valve blocks should clearly inidicate open or close position. Reapir or renew indicators.

Prove proper operation of port lifeboat winch brake. Brake lever does not automatically lower in the stop position when released.

Wasted top elbow on main deck rail next to stores crane. repair or renew.

Upon testing of newly installed hydrostaic release hooks on port lifeboat, hydrostatic release not functioning as designed. Prove proper operationo of hydrostatic sfety release on hooks.

Cargo tanks 4C, 5C, 7P and 7S were changed from an existing zinc coating to a Carboline 9570 epoxy coating, therefore changing the cargoes the tank can carry. Current CAA does not reflect new coatings on those tanks. Vessel needs to apply to MSC to recevie an updated CAA and P & A manual prior to utilizing tanks for new cargo not specifically listed for those tanks.

Update lifesaving training manual to reflect new procedures for new lifeboat hook release system and ensure crew is provided training on new hook release procedures.

Lifeboats were removed from vessel for refurb. All required lifesaving equipment was removed and stored. Ensure all required lifesaving equipment is provided back to lifeboat upon reinstall of port and stbd lifeboat.

Provide Radiography Report for the newly welded saturated steam line on the starboard boiler. Include technicians credentials.

Provide thickness gauging report for the port steam drum IWO relieved pit.

Provide statement from boiler manufacturer that the boiler is still safe to operate with the existing abnormality and that the design MAWP has not been affected.

Request that a Magnetic Particle/Radiography test be completed in way of the abnormality in the starboard steam drum with results forwarded to the Cognizant OCMI.

Request calculations are preformed to ascertain that the design MAWP of the starboard boiler is not affected with the abnormality in the steam drum.

Complete a 1 1/2 MAWP hydro of the starboard boiler to prove the strength of the boiler.
