The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board, due regard being paid to moving parts, hot surfaces and other hazards. PSCO observed loosed connections on lube oil sump line for number three main diesel engine. 74 SOLAS 04 II-1/26.1

The number of openings in watertight bulkheads shall be reduced to the minimum compatible with the design and proper working of the ship; satisfactory means shall be provided for closing theses openings. PSCO observed a missing insulation block from from a multi cable transit in water tight bulk head in engine room. 74 SOLAS 04 II-1.15.1

Muster and embarkation stations shall be adequately illuminated by lighting supplied from the emergency source of electrical power required by regulation II-1/42 or II-1/43. PSCO observed extinguished embarkation lights at lifeboats 3,5,9, and 11. 74 SOLAS 20 III/11.4

For determining the appropriate fire integrity standards to be applied to boundaries between adjacent spaces, such spaces are classified according to their fire risk as shown in categories (1) to (14) below. Where the contents and use of a space are such that there is a doubt as to its classification for the purpose of this regulation, it shall be treated as a space within the relevant category having the most stringent boundary requirements. The title of each category is intended to be typical rather than restrictive. The number in parentheses preceding each category refers to the applicable column or row in the tables. PSCO observed combustibles stowed in A/C machinery space on deck 4 midships in main vertical zone 2. 74 SOLAS 01 II-2/26.2.2

74 SOLAS 20 II-2/9.2.2.3.4.6 16ac ELECTRICALLY HEATED OVENS SHALL BE PROVIDED WITH A TIMER. PSCO FOUND MALE SAUNA, WHICH IS ELECTRICALLY HEATED, MISSING THE TIMER.

The stowage of combustibles is prohibited in a category 10 space. PSCO observed the stowage of combustible materials inside category 10 space. PSCO observed carboard boxes in F.S.D.A 3.6.08. 74 SOLAS 01 2-2/26.2.2

All life-saving appliances and arrangements shall comply with the applicable requirements of the Code. A life raft for use with an approved launching appliance shall: be provided with means for bringing the life raft alongside the embarkation deck and holding it securely during embarkation. During deployment of life raft, PSCOs observed the breaking system for life raft crane #3 to be inoperable and unable to hold the weight of the life raft, causing the life raft to lower uncontrollably. 74 SOLAS III/34 LSA CODE 4.1.4.1.2

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. PSCO observed the fuel shutoff valve for lifeboat #12 was seized open, preventing the cutoff of fuel in the event of an engine fire. 74 SOLAS 20 III/20.2

The stowage of combustibles is prohibited in a category 10 spaces. PSCO observed the stowage of combustible materials inside category 10 spaces, F.S.D.A.2.08 and F.S.D.3.6.08. PSCO observed stowage of combustible materials in both sides of the water making room and forward tank room. Additionally, PSCOs observed storage of combustible materials in the unfinished shower spaces in all Cloud 9 massage rooms. 74 SOLAS 01 II-2/26.2.2

PSCO observed fire doors TT.2.03 closing outside of the maximum time of 40s, and fire doors F.S.D.3.5.15, TT.2.02 and C.2.04 closing quicker than the minimum time of 10s. 74 SOLAS 01 II-2/30.4.2

Each survival craft shall be stowed fully equipped as required by this chapter and the Code... Except in the vicinity of the rudder and propeller, suitable handholds shall be provided or a buoyant lifeline shall be becketed around the outside of the lifeboat above the waterline and within reach of a person in the water. PSCOs observed missing buoyant lifeline on outside, aft, port side on lifeboat/tender #4. 74 SOLAS 20 III/13.1.4

Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures, the approximate time of closure for hinged fire doors shall be no more than 40 s and no less than 10s from the beginning of their movement. PSCO observed fire doors TT.2.03 closing outside of the maximum time of 40s, and fire doors F.S.D.3.5.15, TT.2.02 and C.2.04 closing quicker than the minimum time of 10s. 74 SOLAS 01 II-2/30.4.12

The arrangements for the storage, distribution and utilization of oil used shall be such as to ensure the safety of the ship and persons onboard. PSCOs observed oil accumulation around sludge transfer pump in engine room. 74 SOLAS 01 II-2/15.2

Fire-fighting systems and appliances shall be kept in good working order and readily available for immediate use. PSCOs observed sprinkler head located in the port side, deck 9 men's locker room shower stall drained of red ampule fluid. 74 SOLAS 20 II-2/14.2.1.2

Fire doors in main vertical zone bulkheads, galley boundaries and stairway enclosures, the approximate time of closure for hinged fire doors shall be no more than 40 s and no less than 10s from the beginning of their movement. PSCO observed a wooden pallet blocking FD F.S.D.A.2.08, preventing it from closing properly. 74 SOLAS 01 II-2/30.4.2

One portable fire extinguisher intended for use in any space shall be stowed near the entrance to that space. The fire extinguisher located in the incinerator room was not mounted in an approved bracket. 74SOLAS(01)II-2/6.6 10c

Where A class divisions are penetrated for the passage of electrical cables, pipes, trunks, ducts, etc., or for girders, beams or other structural members, arrangements shall be made ensure that the fire resistance is not impaired. Deck 2 elevator lobby FZ 2 Fr. 96 cabling passing through the overhead not properly insulated. 74SOLAS(01)II-2/18.1.1 10c

All lifesaving appliances and arrangements shall comply with the applicable requirements of the Code. All life-saving appliances shall be fitted with retro-reflective material where it will assist in detection. Lifeboat #1 missing retro-reflective tape on top of cabin in way of operator hatch port and starboard. 74SOLAS(20)III/34 LSA code 1.2.2.7 10c

Doors and door frames in B class divisions and means of securing them shall provide a method of closure which shall have resistance to fire equivalent to that of the divisions, this being determined in accordance with the Fire Test Procedures Code except that ventilation openings may be permitted in lower portion of such doors. B-15 door behind stage leading to dressing room was found with wedges preventing door from closing. 74SOLAS(01)II-2/9.4.1.1.2 10c

Within the perimeter of such stairway enclosures, only public toilets, lockers of non-combustible material providing storage for non-hazardous safety equipment and open information counters are permitted. Luggage cart with cleaning supplies was found in cat 2 passenger aft stairway deck 2. 74SOLAS(01)II-2/29.2 10c

Not less than on half of the total number of lifebuoys shall be provided with lifebuoy self-Igniting lights, not less than two of these shall also be provided with lifebuoy self-activating smoke signals and be capable of quick release from the navigation bridge complying with the requirements of paragraph 2.1.2 of the LSA Code. Lifebuoy waterlight near lifecraft crane 6 has a Duracell MN908 battery which is not on the manufacturer's battery list on the ACR SM-2 label. It is to be verified that all lifebuoy lights onboard have the proper battery type to the satisfaction of the RO. 74 SOLAS 20 III/7.1.3 11117 - 16a

During preparation and launching, the survival craft, its launching appliance, and the area of water into which it is to be launched shall be adequately illuminated by lighting supplied from the emergency source of electrical power required by regulation 111/42. The emergency lighting illuminating the water between lifeboats #9 and #11 (stbd side), between lifeboats #1 and #3, and 2 of 3 bulbs on emergency lighting between lifeboats #10 and #12 were found inoperable at time of boarding to commence the examination. 74 SOLAS 20 III/16.7 11112 - 10c

A minimum of 2 two-way radiotelephone apparatus for each fire party for fire-fighter's communication shall be carried on board. Those two-way portable radiotelephones shall be of an explosion-proof type I or intrinsically safe. Ships constructed before 1 July 2014 shall comply with the requirements of this paragraph not later than the first (SOLAS Safety Equipment) survey after 1 July 2018. Spot-checking of radios used by the fire party found some Motorola DGP 5050 radios with Motorola PMNN4544A batteries. The label on the radio states that the radio is intrinsically safe only when used with Motorola batteries NNTN8129A. Also we found a Motorola XPR 7550 with an unlabeled battery. RO to verify that all radios (And associated batteries) used by the fire party are properly rated as explosion-proof or intrinsically safe. 74 SOLAS 20 11-2/10.10.4 07111 - 50a

The means of escape, including stairways and exits, shall be marked by lighting or photoluminescent strip indicators placed not more than 300 mm above the deck at all points of the escape route, including angles and intersections. The marking must enable passengers to identify the routes of escape and readily identify the escape exits. Additionally, escape route signs and fire equipment location markings shall be of photoluminescent material or marked by lighting. PSCO observed 03 flights of stairs on deck A, stairs C260 to WTC 13 with inoperable low location lighting. The means of escape, including stairways and exits, shall be marked 74 SOLAS 01 11-2/28.1.5.10 by lighting or photoluminescent strip indicators placed not more than 300 mm above the deck at all points of the escape route, including angles and intersections. The marking must enable passengers to identify the routes of escape and readily identify the escape exits. Additionally, escape route signs and fire equipment location markings shall be of photoluminescent material or marked by lighting. PSCO observed 03 flights of stairs on deck A, stairs C260 to WTC 13 with inoperable low location lighting. 74 SOLAS 01 11-2/28.1.5.10 07120-10c

07120 - The escape markings must enable PAX to identify the routes of escape and readily identify the escape exits. Additionally, escape route signs and fire equipment location markings shall be photo-luminescent material or marked by lighting. The emergency escape route to exit the OWS Compartment is not appropriately marked; therefore the escape ladder is not readily visible or easy to find within the compartment. Additional marking is needed to guide a person to the escape ladder. 74 SOLAS (01) II-2/28.1.5.10 16ac

07122 - General arrangement plans shall be permanently exhibited for the guidance of the ship's officers, showing clearly for each deck the control stations, the various fire sections enclosed by "A" class divisions, the sections enclosed by "B" class division together with particulars of the fire detection and fire alarm systems the sprinkler installation particulars of the fan control positions, the position of dampers and identification numbers of the ventilating fans serving each section. Plans and booklets shall be kept up to date; any alterations thereto shall be recorded as soon as practicable. In 2018, there was a modification in Deck 3, MVZ4 were the existing "fun Shops" was reconfigured to include a "promo" spaces in the center category (8) Rodeo Drive. The fire control approved on 21SEP2022 does not reflect these modifications. 74 SOLAS (20) II-2/15.2.4.1 50ac

11131 - Every crew member with assigned emergency duties shall be familiar with these duties before the voyage begins. Lifeboat operator of outboard lifeboat #6 and #8 was not familiar with the proper operation of #8 lifeboat and could not locate the fuel shutoff and first aid kit in #6. 74 SOLAS (20) III/19.2.1 50ac

07109 - Passenger ships carrying more than 36 passengers shall be equipped with an automatic sprinkler, fire detection and fire alarm system of an approved type and complying with the requirements of regulation 12. Sprinklers shall be placed in an overhead position and spaced in a suitable pattern to maintain an average application rate of not less than 5L/m^2/min over the nominal area covered by the sprinkler. The compartment at FSDA6.05 had artwork with frames stored above the line necessary to allow effective coverage by the sprinklers installed in the compartment. 74 SOLAS (01) II-2/36.1.2 and 12.3 10c

Category two stairways on multiple decks had free standing hand sanitizer dispensers and different advertisement free standing signs. 74 SOLAS 01 II-2/29.2

A door designed to reopen upon contacting an object in its path shall reopen not more than 1m from the point of contact. Class "A" door in front of Sauna (F.S.D. 9.5.0.4) reopened more than 1m from point of contact. 74 SOLAS 01 II-2/30.4.11

Doors and door frames in "B" class divisions and means of securing them shall provide a method of closure which shall have resistance to fire equivalent to that of the divisions except that the ventilation openings may be permitted in the lower portion of such doors. B-15 door from top deck of the gym (Deck 10) leading to corridor did not fully close and latch. 74 SOLAS 01 II-2/31.1.1

An International Load Line Certificate shall not be delivered to the ship until the surveyor has certified the marks are correctly and permanently indicated. Load Line on the hull of the ship did not match International Load Line Certificate. ILLC Reg 9

Escape routes shall be maintained in safe condition. Approximately 15 chairs were stored in a corridor outside the art gallery conference room impeding the width of the corridor and blocking visibiity of the low location lighting. Additonally, stairwell 60 on decks 3, 4, & 5 meas of escape were significantly impeded by unattended luggage. SOLAS 20 II-2/13.1.2

All Life-saving appliances shall be fitted with retro-reflective material where it will assist in detection. Lifeboys throughout deck 3 have deteriorated retro-reflective tape. SOLAS 20 III/34 LSA 1.2.7

Where davit arms are recovered by power, safety devices shall be fitted shich will automatically cut off power to the davit arms before the davit arms reach the stops. The limit switches on liferaft stations No.4 and No.8 were inoperable. SOLAS 20 III/16

Crew members shall be trained to be familiar with the arragements of the ship. Th emain laundry attendant, medical administrative assitant, and assitant waitress in Truffles Restaurant were unfamiliar with the spaces and how to make notification of or address a fire in those spaces. 74 SOLAS II-2/15.2.2.1

Doors in "B" class divisions shall be provided with a method of closure which shall have fire resistance equivalent to the division. PSCO Observed a B15 doo rby elevator 10 on deck 3 was unable to close. SOLAS 20 II-/9.4.1.2.1

During preparation and launching, the survival craft, it launching appliance, and the area of the water which it is to be launched shall be adequately illuminated. Emergency lights by lifeboats No. 1, the starboard side rescue boat, and the second to last light starboard side aft were inoperable. SOLAS 20 III/16.7

07105 - Fire screen doors in "A" class divisions that are designed to reopen upon contacting an object in their path shall reopen not more than 1 m from the point of contact [30.4.11]. Multiple sliding fire screen doors on decks 3 through 7 found to reopen >1m after contacting an object in their path. SOLAS (01) II-2/30.4.11

11129 - Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Limit switch for liferaft davit #4 inoperable. SOLAS (20) III/20.2

04109 - Fire drills shall be conducted in accordance with provisions of regulations III/19.3 [15.2.2.5]. Parties responsible for fire extinguishing shall be organized and these parties shall have the capability to complete their duties at all times [15.2.1.3]. During initial fire drill (scenario: fire in main galley), fire squad assigned to carry out search and rescue of any personnel casualties did not locate the simulated personnel casualty placed in the space by the ship's staff, although the casualty was reported to the bridge as found and removed from the space. Also, fire screen doors enclosing the escalators that pass from the main galley up into the dining room were not closed, which could permit spread of fire into the large dining room area (spans 2 decks). SOLAS (20) II-2/15.2.2.5 and 15.2.1.3

Before the hip leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Lifeboat tender #3 steering found inoperable on starboard side. Emergency steering remains operational.

All fire doors in stairway enclosures main vertical zone bulkheads and galley boundaries which are normally kept open shall be capable of release from a cerntral control station and from a position at the door. Fire doors A.4.22 and A.4.12 are sliding fire doors between the crew galley and crew mess. They are not responding to centralized control. 97 SOLAS Ch. II-2 Regulation 41-1 6.5

In spaces where a limited supply of flammable liquids is necessary, it is permissible to install an approved flammable liquid cabinet to stow small quantities for daily use. The cabinet must be at least at the level required by SOLAS II-2. Identified an NFPA 30 cabinet in starboard side storeroom, Deck 2, MVZ 6. Storeroom is listed as a category 13 space on the vessel's approved space categorization plan. Cabinets meeting NFPA 30 only provide protection for 10 minutes and should only be used in Category 11, 12 or 14 spaces. 74 SOLAS (01) II-2/26.2.2

Above the bulkhead deck there shall be at least two means of escape from each main vertical zone or similarly restricted space or group of spaces, at least one of which shall give access to a stairway forming a vertical escape. Deck 10, MVZ 6 gymnasium port side emergency exit door is capable of being locked. 74 SOLAS (01) II-2/28.1.2

Before the ship leaves port and at all times during the voyage, all lifesaving appliances shall be in working order and ready for immediate use. Lifeboat #4 stbd engine did not start. 74 SOLAS (14) III/20.2

0630 - Before the ship leaves port and at all times during the voyage, all lifesaving appliances shall be in working order and ready for immediate use; the forward release hook on Lifeboat #16 and Lifeboat #12 did not release properly. Crew attempted to rectify without success. Due to (02) lifeboats not being deployable, total capacity of lifeboats available onboard has been reduced to 2400 people, and total number of persons allowed on board has been reduced to 3200 persons to maintain compliance with 74 SOLAS (14) III/21.1.1.1 74 SOLAS (14) III/20.2

9800 - Electrical installations shall be such that the safety of passengers, crew, and ship from electrical hazards will be ensured; circuits for the ADG fan and DG room aft supply fan #5651/042 were ground faulted causing the insulation resistance on the emergency switchboard to be lower than the program alarm threshold. Vessel is to tag both circuits out and repair to the satisfaction of class.

1510-Navigation Equipment-Ship is to be fitted with a functioning voyage data recorder. Ships's VDR triggered the off-line alarm and is not showing any ship's data on the bridge monitor-74SOLAS14 cons. V/20.1.1

According to PSSC, the #2 tenderboat is currently out of service due to hull damage and the vessel's remaining non-substandard lifeboats do not meet the 75% carriage capacity. Total persons reduced by 150.*TENDERBOAT HULL DAMAGE REPAIRED, HOWEVER, FULL LOAD TESTING MUST BE COMPLETED PRIOR TO RETURNING TO SERVICE AND APPROVED BY RO. 74 SOLAS (09) REG III 21.1.1
