Two (2) ventilation closures on the weather decks were found extremely corroded, unable to close, and or missing, in addition to all other ventilation closures that were noted for repairs shall be repaired.

Cargo ships shall carry one or more totally enclosed lifeboats complying with the requirements of the code with aggregate capacity on each side as will accommodate the total number of persons on board. Vessel must repair, weight test, and preform operational demonstration of No. 4 Lifeboat Davit Winch. ABS may issue short term Safety Equipment Certificate for the purpose of completing this requirement at the vessels next port of call (Norfolk, VA) in which it must be completed to satisfaction of the attending ABS Surveyor. Vessel may not carry more than 99 total persons onboard until full-term Safety Equipment Certificate is issued.

FM 200 flex hoses past service life. Vessel required to replace all FM200 hoses that are past service life.

Rescue boats shall be stowed in a state of continuous readiness for launching in not more than 5 minutes. Port rescue boat sponson deteriorated beyond serviceability. Vessel has starboard side rescue boat to meet SOLAS requirements. Make repairs to port side rescue boat at completion of dry dock. 74 SOLAS (04) III/14

Medical certificates must be issued and remain current. 03 crew members had expired medical certificates.

Cables and wiring shall be installed and supported in such a manner as to avoid chaffing and other damage. Electrical cable in the following locations were found unsupported due to cable tray wasted or banding missing. 1) Helo deck fuel station overhead 2) Cargo hold #1 ladderwell at maindeck.

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: structural fire protection, including fire resisting divisions and protection of openings and penetrations in these divisions. 1) MCT at MJD-01-57-2 has missing fire stop, reducing fire rating of Class A bulkhead. 2) multiple fire screen doors were found in need of adjustment to ensure proper closure.

Each life raft or group of life rafts shall be stowed with a float free arrangement, complying with the LSA code, so that each floats free. Life raft stowage in 03 locations needs adjustment so that failure of 01 hydrostatic release does not hinder the float free arrangement of the other rafts.

Vessel Shall have tug escort from pilot station to pier at Naval Station Norfolk.

Replace number one lifeboat propeller.

Prove proper operation of vessels emergency fire pumps prior to leaving port.

Prove proper operation of bow thruster and related electrical components affected by SW line failure.

Identify & correct cause for failure of bow thruster SW cooling water supply pressure gage pipe connection.

Bilge level sensors do not provide audible alarm IAW SOLAS II-1/48.1; 51.1.1; & 51.1.3. 4.

Constant tension mooring line system in conjunction w/ installed roller fairleads cause catastrophic mooring line failure during standard mooring configuration.
