USNS CHARLES DREW

Cargo ships shall carry one or more totally enclosed lifeboats complying with the requirements of section 4.6 of the Code of such aggregate capacity on each side of the ship as will accommodate the total number of persons on board. USNS CHARLES DREW currently does not have 100% capacity on each side of the vessel. SOLAS III-31/1.1.1

Each vessel vapor connection flange face must have a permanent stud projecting outward that has a 12.7 millimeter (0.5 inch) diameter and is at least 25.4 millimeters (1 inch) long. It must be located at the top of the flange face, midway between boltholes, and in line with the bolthole pattern. Vapor connections on both port and starboard sides are painted over. 46 CFR 39.2001(j)

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to the ship or person on board. Each crew berthing area shall be equipped with information regarding- (A) vessel owner or company policies prohibiting sexual assault and sexual harassment, retaliation, and drug and alcohol usage; and (d)... In each washing space in a visible location there shall be information regarding procedures and resources to report crimes upon the vessel, including sexual assault and sexual harassment, and vessel owner or company policies prohibiting sexual assault and sexual harassment, retaliation, and drug and alcohol usage. Vessel is to display information and procedures for reporting of SASH incidences in accordance with 46 USC references. 46 USC 11101(a)(5) 46 USC 11101(d)

Main and emergency switchboards shall be so arranged...nonconducting mats or gratings shall be provided at the front and rear of the switchboard. Matting was not present at switchboards in the steering gear room. SOLAS II-1/45.2

The general alarm system shall be capable of sounds the general emergency alarm signal. Attending marine inspector observed a inconsistency within the GA/PA system. SOLAS III/3.10 LSA Code 7.2.2.1

Fire detection and fire alarm system shall be kept in good order so as to ensure their required performance if a fire occurs. There is an active fault in the fire detection panel for Zone 7. 74 SOLAS (09) II-2/Reg 14.2.1.1.2

Launching and recovery arrangements shall be such that the appliance operator on the ship is able to observe the survival craft at all times during launching and for lifeboats during recovery. During launching of the #3 lifeboat, the davit's lowering handle disconnected from the cable. 74 SOLAS (14) III/Reg 16.3

The emergency generator switchboard hertz meter is inoperable and the 12V ground detection indicating lights are blinking. 74 SOLAS (09) I/Reg 11

Subject to exceptions, 46 U.S.C. 8104 requires that when a master of a seagoing vessel of more than 100 GRT establishes watches for the officers, sailors, and oilers, “the personnel shall be divided, when at sea, into at least three watches and shall be kept on duty successively to perform ordinary work incidental to the operation and management of the vessel.” Only 2 QMED-oilers/Ratings Forming Part of an Engineering Watch )RFPEW - STCW III/4) positions are filled. As noted in the crew list, one position listed as the UJE (W) position is vacant, leaving the two QMED oilers are standing two watches and not in a three watch system. Fire detection and fire alarm systems shall be kept in good order so as to ensure their required performance if a fire occurs. Five faults were identified in the fire detection panel. SOLAS (2009) II-2/Reg 14.2.1.1.2

Logs of ship stations which are compulsorily equipped for radiotelephony must contain applicable log entries and the time of their occurrence. General entries for testing GMDSS equipment was made in the ship’s GMDSS log book. It did not identify required entries as required in 47 CFR 80.409. Cite: 47 CFR 80.409(e)

Nautical charts and nautical publications, such as…tides tables and all other nautical publications necessary for the intended voyage, shall be adequate and up to date. The program supplying tides and currents was no longer functional on board the vessel. Cite: SOLAS (2009) V/Reg 27

The rations [lifeboat] shall be packed in permanently sealed metal containers or vacuum packed in a flexible packaging material with a negligible vapour transmission rate. Flexible packaging materials shall be further protected by outer packaging if needed to prevent physical damage to the food ration and other items as result of sharp edges. Identified 3 rations with broken vacuum seal and some rations were stored without outer packaging. Cite: SOLAS (2009) III/Reg 14.4, LSA Code 4.1.5.1.18

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Both the port and starboard bridge wing lifebuoys had wasted SOLAS tape, the lifebuoys color is faded, the lifebuoy grablines are deteriorating, and could not get the starboard self-igniting light to illuminate. Test procedure documents must be in a step-by-step or checkoff list format. The crew and technical representatives utilized the "Pre and Post Commissioning Safety Check procedures" to carry out annual testing of equipment. In conversations with the crew, it was determined that the isolation valves within the #1, #2, and #3 Salt Water Cooling systems leak by substantially. Inspect all isolation valves within the Salt Water Cooling system to verify their operational integrity. 74 SOLAS (09) I/Reg 11(a)

The Oil Record Book Part II shall be completed on each occasion, on a tank-to-tank basis if appropriate, whenever cargo/ ballast operations take place in the ship. In Feb 2022, Code “L” transfers were listed in the Oil Record Book Part II. Several entries were made with Code “L” in 2021. The vessel is not equipped with dedicated clean ballast tanks. The Company should ensure that each ship is manned with qualified, certificated and medically fit seafarers in accordance with national and international requirements. The vessel was not provided a UJE (W) position, the crewmember assigned as the Rating Forming Part of an Engineering Watch (STCW III/4)/ QMED-Oiler position as required by the COI. A crewmember's medical certificate notated a medical waiver which required compliance, but did not have a copy of the waiver on board the vessel nor did the medical officer. The safety management system should provide for measures ensuring that the company's organization can respond at any time to hazards, accidents and emergency situations involving its ships. While lowering the #4 lifeboat, a crewmember used his hands to ensure the brake cable would properly pay out when the boat was lowering. Reviewed the SOLAS training manual and could not find any reference to the process. Also, the SOLAS training manuals were not found in all identified locations. The diagrams were difficult to discern the operation of the lifeboat onload release mechanism. The SOLAS training manual on board is not detailed and differs from theSOLAS training manual in the SMS listed as a 2019 "draft" and is more comprehensive. The company should ensure that the policy is implemented and maintained at all levels of the organization both ship based as well as shore based. Numerous food stores on board were found to be passed their labeled expiry date with no documentation provided either extending their lifecycle or deeming them unfit for human consumption per NAVSUP P-486, Vol I, Rev 9 Food Service Management: 1. The Medical Services Officer onboard stated a monthly Food Operation Inspection is conducted but noted no expired food items within stores (DD-2973 Inspection Reports from the last 2 months were furnished supporting this statement). Recommend an external SMC Audit of the food handling procedures onboard regarding the processing of expired food, the systemic mechanism of verifying food is fit for human consumption, as well as determining the periodic required availability of shore side support personnel to the fleet (namely an Army Veterinarian or “authorized medical representative” as outlined by NAVSUP P-486, Vol I, Rev 9), to be conducted by the RO (ABS). Doors required to be self-closing shall not be fitted with hold-back hooks. Hold back hooks on self-closing doors in fire boundaries were found through the vessel during the course of the inspection.

Means of control for flammable liquids in the space shall be provided. Numerous chemical and cleaning products were found stored within the passage way at Frame 83.

EVERY INFLATABLE LIFERAFT SYSTEM SHALL BE SERVICED AT INTERVALS NOT EXCEEDING 12 MONTHS. LAST LIFERAFT SERVICING WAS 20MAR2020. SOLAS 74 A (2020) III/20.8 16a

FIRE FIGHTING PRESSURE VESSELS TO: MMR, PURIFIER ROOM, CARGO PUMP ROOM, AFT-MID-FWD PAINT LOCKER, INCINERATOR, EDG, AVIATION LIQUID LOCKER) ARE PAST DUE AS OF NOV2020 FOR HYDROSTATIC TESTING. SOLAS 74 A (2020) II-2/14.2.2.2 , 16a

ALL LIFESAVING APPLIANCES SHALL BE IN WORKING ORDER AND READY FOR IMEDIATE USE. ALL FIRST AID KITS IN LIFEBOATS AND FRBS ARE EXPIRED. SOLAS 74 A (2020) III/20.2 16a

CREW MEMBERS WITH ENCLOSED SPACE ENTRY OR RESCUE RESPONSISBILITIES SHALL PARTICIPATE IN AN ENCLOSED SPACE ENTRY AND RESCUE DRILL AT LEAST EVERY TWO MONTHS. LAST ENCLOSED SPACE DRILL WAS CONDUCTED JAN2021. SOLAS 74 A (2020) III/19.3.6

Satellite EPIRB shall be capable of floating free if the ship sinks and of being automatically activated when afloat. It was reported that due to heavy weather the vessel's EPIRB fell and cracked its case. EPIRB cannot currently be stowed in float free arrangement. SOLAS 74 A. (2020) IV Reg 7.1.6.4
