PSCO discovered temporary piping repairs on the IGS, connecting to the atmosphere vent piping. Vessel could not provide proof that this temporary repair was reported to the company, piping in need of permanent repairs. 40ac.

Resolution: The identified piping has now been permanently repaired by renewal of the holed section. Following the repair the piping was tested under pressure of 950 millimetre water gauge with no leakage noted.

Resolution: After removal of the fire plans it was confirmed the self-closing door symbols were as per the fire planlegend but difficult to see under the perspex cover. These have now been remarked in in order to show more clearly that the doors are self-closing.
