74SOLAS(2014 CONS.)IX/3.1 ISM PART A, 10.2.2 The company should ensure that any non-conformity is reported, with its possible cause, if known. PSCO discovered temporary piping repairs on the IGS, connecting to the atmosphere vent piping. Vessel could not provide proof that this temporary repair was reported to the company, piping in need of permanent repairs. 40ac.

Resolved 17 November 2016
Resolution: The identified piping has now been permanently repaired by renewal of the holed section. Following the repair the piping was tested under pressure of 950 millimetre water gauge with no leakage noted.

Resolution: After removal of the fire plans it was confirmed the self-closing door symbols were as per the fire planlegend but difficult to see under the perspex cover. These have now been remarked in in order to show more clearly that the doors are self-closing.

PSCO found a penetration of a Class A fire division, the duct/pipe/cable space penetration into the linen locker, C-Deck. 50c.

Resolution: Vessel submitted tech report indicating a new GPS has been installed and is operating normally at this time. All connected equipment has good GPS data.
