(tankers) - Each tank ship with VCS must be fitted with a pressire sensing device that activates a high pressure alarm when the pressure of not more than 90% of the lowest pressure relief valve setting. The breather valve was the lowest pressure relief valve setting. The breather valve was the lowest pressure relief valve and has a setting of 1200. Alarm set point was found at 1260mmWG. Alarm set point was adjusted. Relevant manuals and plans should be amended to the changes. 33 CFR 155.750(d) and 46 CFR 39.2013.

Resolution: Vessel provided revised VECS manual amending the new setting of 1080 mm wg (90%of the beather valve) Class approved and will endorse the manual during the next Class attendance.
No electrical equipment shall be installed in any space where flammable mixtures are liable to collect including those on board tankers unless the adminnistration is satisfied that such equipment is of a type which will not ignite the mixture concerned and appropriate to the space concerned.
PSCOs observed 24 of 29 explosion proof lighting in the cargo pump room and 2 of 2 explosion proof lighting in SOPEP locker with cracks in housing, compromising the integrity of the fixture.

PSCO observed #4S cargo tank was filled exeeding the overfill set point for the tank with no visual or audio alarms indicating within the cargo control room.

In tankers of 20,000 DWT and above, the protection of cargo tanks shall be achieved by a fixed inert gas system in accordance with the requirements of the FSS Code. For inert gas systems of both the flue gas and IGG type (vessel has IGG) audible and visual alarms shall be provided to indicate [.9] high gas pressure.
PSCOs requested to witness test of high gas pressure alarm as part of IGG checks. It took approxamately 2 hours for the ship to deminstrate the high gas pressure audible and visual alarm.

Resolution: PSC Miami received Lloyds report stating as per Res. MSC 81(70) the vessel is authorized to keep current life jackets onboard as long as they remain suitable for service. Deficiency is rectified.
