A pre-engineered fixed gas fire extinguishing system must: Automatically shut down all power ventilation systems and all engines that draw intake air from within the protected space. Port engine room power ventialltion did not properly secure when tested. 46 CFR 181.420

The following spaces must be equipped with a fire detection and alarm system of an approved type installed in accordance with 46 CFR part 76, except when a fixed-gas fire extinguishing system that is capable of automatic discharge upon heat detection is installed or when the space is manned: A space containing an internal combustion engine of more than 50 hp. Port generator space heat activated alarm also activated the port engine room when tested. 46 CFR 181.450(a)(2)

Electrical equipment on a vessel must be installed and maintained to:(a) Provide services necessary for safety under normal and emergency conditions;(b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock;(c) Minimize accidental personnel contact with energized parts; and (d) Prevent electrical ignition of flammable vapors. Light in port fidley was missing the required cover and the light above the starboard forward MDE had a cover that was melted and could not be properly secured. 46 CFR 183.200

The Bridge-to-Bridge Act and the regulations of this part apply to the following vessels in the navigable waters of the United States:(a) Every power-driven vessel of 20 meters or over in length while navigating. Bridge-to-bridge radio certificate was not present during inspection. 47 CFR 80.1001

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: “EMERGENCY EXIT, KEEP CLEAR”, unless such markings are deemed unnecessary by the cognizant [name withheld] forward engine room excape hatch was missing required placard at time of inspection. 46 CFR 185.606

Make corrections to the recently approved electrical one line diagram and resubmit to MSC for approval.

Each ring life buoy with a floating waterlight must have a lanyard of at least 910 millimeters (3 feet) in length, but not more than 1,830 millimeters (6 feet), securing the waterlight around the body of the ring life buoy. Line connecting life buoy to floating waterlight is showing signs of deterioration. Replace line. 46CFR180.70(d)(2)

Individual bilge suction lines must be led to a central control point or manifold and provided with a stop valve at the control point or manifold and a check valve at some accessible point in the bilge line. A stop-check valve located at a control point or manifold will meet the requirements for both a stop valve and a check valve. The check valve for the bilge suction pipe in the port engine room, the bolt that holds the flapper valve has worked loose which will make the check valve inoperable. Properly repair or replace the check vavle. 46 CFR 182.510(c)

Electrical equipment on a vessel must be installed and maintained to: (a) Provide services necessary for safety under normal and emergency conditions; (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock; (c) Minimize accidental personnel contact with energized parts; and (d) Prevent electrical ignition of flammable vapors. Properly terminate the electrical wiring for the coffee machine in the ships concession stand. 46 CFR 183.200

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew... As noted at the last dry-dock corrosion and pitting has continued to progress, several pits were discovered during the 2023 annual inspection that exceed 25% wastage limits in the following spaces/locations; 1) Stbd water tank void space – several isolated pits measuring approximately 1/32” aft along the keel. 2) Stbd water tank void space – single pit. That was located aft in the space near insert – measured 1/8” (pictures attached) 3) Port MSD tank void space –several isolated pits measuring approximately 1/32” aft along the keel. 4) Stbd fuel tank space – pitting all along the keel aft of the fuel tank. Several pits measured 1/16”, some over and some under. Observed some pitting under generator but unable to measure due to space restrictions. 5) Stbd engine room space – pitting all long the keel from the forward part of the room to the aft. Several pits measured over 1/16”, some measured under. Clean all effected areas and submit temp and permanent repair proposal to the OCMI for approval and make any and all repairs. Permanent repairs to be made at the vessels next scheduled dry-dock 30Nov2023. 46 CFR 177.100 After further review, temp repairs will not be allowed. Due to substanstial wastage, permanent repairs must be made prior to operating vessel.

A hatch exposed to the weather must be watertight. The port jet room deck hatch cover had one loose dog that is missing a bolt. Replace missing bolt and ensure hatch cover seals watertight when dogged closed. 46 CFR 179.360

Combustibles insulated from heated surfaces. Internal combustible engine exhaust components must be insulated. The starboard engine room aft engine is missing a section of lagging on the turbo charger. Properly install insulation on exposed section of exhaust on the turbo charger. 46 CFR 177.405 & 177.970.

A number of child size life jackets equal to at least 10% of persons permitted on board must be provided. Three of the required child size lifejackets were not in serviceable condition. Provide three child size lifejackets on board vessel.

The fire pump outlet must be fitted with a pressure gauge. The gauges from both the electric and mechanical fire pumps were not operational. Provide operational gauges on both fire pumps.

A visual and audible alarm must be provided at the operating station to indicate a high water level. The starboard e/r high level alarm is not alarming audibly. Repair high level alarm to alarm audibly.

Equivalent protection may include lifelines, wire rope, chains, and bulwarks, which provide strength and support equivalent to fixed rails. Deck rails must be designed and constructed to withstand a point load of 91 kilograms (200 pounds) applied at any point in any direction, and a uniform load of 74 kilograms per meter (50 pounds per foot) applied to the top rail in any direction. The life lines at the aft of the vessel were attached using carabiners of substandard quality. Ensure lifelines can with stand point and uniform loads applied to lifelines.

Prove proper operation of MDE #4. Engine overheated when getting underway for drills.

Provide service report for STBD engine room fire extinguisher. Red activation light illuminated in bridge.

Repair broken switch box at base of stbd E/R stairs.

Replace missing screw in junction box cover for battery charger aft bulkhead stbd E/R

Replace missing lagging #4MDE and secure loose lagging.

Replace broken bilge manifold valve port E/R.

Replace missing closure device on STBD aft ventilation duct.

Remove fuel level indicators or meet requirements of 182.445.

Repair or replace upper deck fire hose nozzle.

Properly secure coffee maker to counter top.

Replace missing guard bolts on #4 MDE.

Properly attach bonding strap on #4 MDE.

Properly attach aural warning horn for high bilge alarm in wheelhouse.

Make repairs to raw water leak on fwd secion of #4 MDE.

Identify cause of #3 MDE "F1 S" Emergency Monitor" alarm and make appropriate repairs.

Identify cause of #4 "ECU" alarm and make appropriate repairs.

Conduct Man Overboard and Fire Drills to the satisfaction of an attending USCG Marine Inspector.

Prove proper operation of fire pump and bilge pumping arrangement. Unable to complete during annual inspection due to freezing conditions.

Determine source of water ingress and make permanent repairs in port side jet room/ lazarette.

Prove proper operation of port side jet room / lazarette bilge high water alarm.

Properly mount unmounted water pump in starboard engine room just below fwd ladder.

Adjust hose clamp to remove snag hazard of bolts.

Label both sides of engine room hatch, "Hatch to remain closed at all times."

Vessel's fixed fire fighting system bottle in starboard engine room wsa installed with bottle facing bulkhead instead of the engine room. Orientate the bottle so it faces the engine in accordance with manufacturers instructions.

Starboard fwd ventilation closure for starboard engine room is not closing when fixed fire system is activated. Repair ventilation closure to close upon activation of fixed fire fighting system.

Port fwd engine's raw water pipe from the sea chest was leaking and had a temporary repair. Repair/renew pipe to original condition prior to operating engine.

Vessel's fire extinguishing system was discharged making fire extinguishing system unusable. OCMI was not notified in accordance with cite. Ensure immediate notification is made to OCMI via Sector Command Center.

Vessel shall provide plan review documents and drawings for newly installed fire pump and associated piping for approval by December 1st 20219.

Vessel shall provide Coast Guard Marine Safety Center with information on all added weight with location for tracking of weight changes that could affect the vessel’s Stability Letter by December 1st 2019.

Vessel needs to provide means to engage/operate fire pump locally

Provide Final approval for engine control DVTP for CG Inspector.

Repair exhaust cooling water leak on both port and starboard aft engines

Repair all exhaust leaks on all main engines

The vessel must be provided with a stability letter; a vessel repower has voided the existing stability letter.

Following a repower, vessels shall submit "Loss of Power" QFA's to MSC and submit to testing IAW approved procedures; testing has been conducted IAW submitted plans; however, plans have not yet been approved.

Exposed hazards must be properly protected by a cover; both port and starboard aft engine flywheels were exposed.

Each vessel that relies on electricity to power vital systems those systems must be capable of being energized from two sources; vessel is equipped with an electric fire pump and only one generator.

02113 - Repair the 4"x.5" crack in the #1 Port Void to the satisfaction of the attending USCG Marine Inspector.

Perform fire and abandon ship drills to the satisfaction of the attending CG Marine Inspector.

Install required IBAs and make available to CG Marine Inspector.

Replace unservicable lifejackets or repair and make available for esting IAW NVIC 2-63.

Rermove dead-end wire in starboard engine room (outboard forward engine).

Replace all worn and cracked flex hoses on all main engines and generator.

Prove annual servicing of all installed pre-engineered fix gas FF systems. Noted: Been two years since serviced.

Repair leak in salt water supply line for sewage system in starboard engine room.

Replace crushed bilge discharge hose in starboard generator space.

Fix salt water leak on forward aft side generator.

Find and repair source of fuel on starboard side fill tubing.

properly adjust and repair exhaust lagging in both engine rooms.

Fix exhaust leak in port engine room aft engine.

Prove proper operation of installed bilge system.

Prove proper operaion of upper fire station.

Repaint all permanent loading marks on the hull.

Renew the retroflective tape on the life ring buoy on the port side bridge wing. 46 CFR 180.70
