Resolved
Machinery must be designed and maintained to provide safe operation of the towing vessel under normal and emergency conditions. Vessel lost port main engine and found water in the fuel. Remove water from fuel and ensure all machinery operating as designed to the satisfaction of USCG or TPO.
In accordance with CVC Policy Letter 17-10, the above deficiencies may be cleared by the operating company's TPO. TPO must submit a report to the cognizant OCMI within 72 hours of clearing the deficiencies.

Nuts utilized to secure housing unit with two oil filters for port engine backed off of bolts (due to vibration). Oil filters were punctured due to falling on grated deck. [name withheld] indicated that the bolts appeared not to be from original installation and were too short.
