The materials and construction of fuel lines, including pipe, tube, and hose, must comply with the requirements of this paragraph. When used, flexible hose must meet the requirements of § 182.720(e) of this part. 2ft section of fuel tank fill hose worn and wasted. Requires renewal. New hose replaced and verified during inspection. No further requirement. 46 CFR 182.455(a)(1)(iii)

On a vessel of not more than 19.8 meters (65 feet) in length carrying more than 49 passengers, the minimum capacity of the fire pump must be 189 liters (50 gallons) per minute at a pressure of not less than 414 kPa (60 psi) at the pump outlet. The pump outlet must be fitted with a pressure gauge. A fire pump may be driven by a propulsion engine. A fire pump must be permanently connected to the fire main and may be connected to the bilge system to meet the requirements of § 182.520 of this chapter. Port side fire main and pump inoperable. Fire pump is connected to fixed bilge system so port side fixed bilge system is also inoperable. Repair or renewal required. 46 CFR 181.300(b)(d)

The construction and arrangement of a vessel must allow the safe operation of the vessel in accordance with the terms of its Certificate of Inspection giving consideration to provisions for a seaworthy hull, protection against fire, means of escape in case of a sudden unexpected casualty, guards and rails in hazardous places, ventilation of enclosed spaces, and necessary facilities for passengers and crew. Both engine rooms have deteriorated insulation throughout each space. Replace all deteriorated insulation. 46 CFR 177.100

A vessel must carry a first-aid kit approved under 46 CFR 160.041. First aid kit had expired items. Provide first-aid kit with valid items. 46 CFR 184.710

Spanner wrench must be provided where a 1.5 inch fire hose is required. Vessel did not have spanner wrench. Provide spanner wrench. 46 CF 181.310(d)

A propulsion engine control system must be designed so that a loss of power to the control system does not result in an increase in shaft speed. Electronic propulsion controller installed on vessel. QFA and DVTP provided but test not completed. Conduct testing in accordance with approved QFA and DVTP. 46 CFR 184.620

The design of all exhaust systems must ensure minimum risk of injury to personnel. Port side engine room had engine exhaust leak. Make engine exhaust leak free. 46 CFR 182.430

Electrical equipment on a vessel must be installed and maintained to protect crew and vessel from electrical hazards. Starboard engine room had dead end wire near AC outlet. Remove dead end wire. 46 CFR 183.200

A pre-engineered fixed gas fire extinguishing system must be installed in accordance to manufacturers instructions. Vapor tight barrier/bulkhead penetration breached at wire run in starboard side engine room making coverage area larger than installed bottle. Make bulkhead penetration vapor tight. 46 CFR 181.420

Spanner wrench must be provided where a 1.5 inch fire hose is required. Spanner wrench not on board. Provide spanner wrench. 46 CFR 181.310(d)

Fire nozzle/hose must be corrosion resistant material. Fire hose nozzle connection corroded and has a leak. Provide hose and nozzle that are not corroded and do not leak. 46 CFR 181.320

Fire extinguishing equipment installed on a vessel must be maintained. CO2 system was partially removed and replaced by pre-engineered system. CO2 system components still located in port laz. Remove all associated CO2 system components. 46 CFR 181.120

Each vessel must have permanent draft marks at each end of the vessel or have permanent loading marks placed on each side of the vessel forward and aft to indicate the maximum trim and amidships to indicate the maximum allowable draft. No hull permanent markings on vessel. Provide permanent hull markings IAW stability letter. 46 CFR 185.602

All vessels must have navigation lights that are in compliance with Navigation Rules. Stern light obstructed. Provide unobstructed stern nav light with and arc of 135 degrees. 46 CFR 183.420

Machinery installed shall be suitable for its operation. 01 blower in port engine room not operational. Provide operational 01 blower. 46 CFR 182.100

An EAP training program must be conducted for the employer's crewmembers and supervisory personnel. The training program must include at least the following elements: the effects and consequences of drug and alcohol use on personal health, safety, and work environment; the manifestations and behavioral cues that may indicate drug and alcohol use and abuse; and documentation of training given to crewmembers and Supervisory personnel must receive at least 60 minutes of training. Provide proof of completed EAP training for all supervisory personnel.

On a vessel of at least 26 feet in length, an audible alarm must be provided at the operating station to indicate a high water level. Audible bilge alarm was not working during testing of bilge alarm. Replace audible bilge alarm at the operating station and ensure proper operation.

Materials used in piping systems must meet the requirements of this subpart and be otherwise acceptable to the cognizant [name withheld] fire main piping inboard of engine found ruptured and leaking into engine room. Replace fire main piping and secure leaking fire main.

An adult life jacket must be provided for each person carried on board a vessel. 19 Adult Life Jackets were condemned due to material failure. 136 Adult Life Jackets currently onboard, vessel allowed to carry 155 total persons. Provide 19 new or acceptable Adult Life Jackets to meet COI requirements.

Gauges to indicate lubricating oil pressure must be provided for all propulsion engines installed in the vessel. The gauges must be readily visible at the operating station. Starboard oil pressure gauge was found not working at the operating station. Provide working oil pressure gauge for starboard engine at the operating station.

15 adult PFDs failed inspection. Provide PFDs in accordance with 46CFR 180.72. Vessel limited to 140 total persons.

Radar picture was unreadable. Provide functional radar in accordance with 46CFR 184.404. Vessel limited to 49 passengers during night time operations.

Port and Stbd emergency fuel valve did not close completely. Repair in accordance with 46CFR 182.455.

Port engine exhaust was leaking into engine space. Repair in accordance with 46CFR 182.100.

Prove proper operation of all fire dampers actuated by CO2 system, and have the system certified by an approved fire system servicer.

Replace corroded oil supply line for the starboard main turbo.

Crop and renew sideshell plate pitting located in the starboard engine room at outboard frame 2 between #3 and #4 longitudinal stiffners.

Crop and renew sidehell plate pitting located inboard side in the starboard side compartment #2. Pitting found under and above all longitudinal stiffners from keel to waterline.

Crop and renew wasted hull plate under the port and staboard fuel tanks. Hull plate found wasted throughout the entire fuel tank compartment.

Crop and renew fractured longitudinal stiffners and gussets in crossdeck space as marked by inspector. Multiple stiffners found cracked aft of the water tank and aft of the stairwell. Transverse bulkhead above fractured stiffners found deformed inward 5/8".

Renew or replace wasted fuel shutoff valve on the starboard fuel tank.

Crop and renew wasted hull plate under the p/s fuel tanks; submit repair proposal to include weld profile, materials, etc.

Prove operation of auto bilge pump indication light for stbd tank space.

Repair fractured weld IWO centerline longitudinal stiffener on underdeck wing at blk #16 and fr # 15.

Repair fractured weld located in port fwd void, inboard side, IWO side long #7 from the CVK at blk #16.

PROVIDE UPDATED SERVICING CERTIFICATE.

Provide suitable crew access to cabin top.

Prove EPIRB registration and battery expiration date.

Provide a means to dewater the space forwrd of the collision bulkhead.

Post COI and stibility letter.

Prove proper operation of emergency fuel shut off valve for stbd fuel tank.

Provide FCC station license & Operation Permit to the satisfaction of the marine inspector. LEGACY COMPONENT DESC: VHF Radiotelephone

Perform Fire & Abandon Ship Drills to the satisfaction of the attending Marine Inspector.

Complete a new to zone inspection prior to operation of vessel.
