Doors in A-Class fire control boundaries must meet the following additional requirements: (3) A door must have a latch with a minimum throw of 20 millimeters (.75in). The aft main deck doors to refuge area were not latching and missing striker plate. Install striker plate and adjust the door to close to the satisfaction of the attending marine inspector.

The public address system must be a fixed installation and be audible during normal operating conditions throughout the accommodation spaces and all other spaces normally manned by crew members. Public address system was not audible on various decks and spaces at the pier. Repair public address system to the satisfaction of the attending marine inspector within seven days. While operating with passengers on board, ensure each deck is adequetly manned to relay communications from the operating station.

At each inspection for certification, the bilges may be examined to see that there is no excessive accumulation of oil, trash, debris, or other matter that might clog bilge pumping systems. Presence of mud observed in lazarette bilge. Remove accumulation of mud to the satisfaction of the attending marine inspector.

Each door, hatch, or scuttle, used as a means of escape, must be capable of being opened by one person, from either side, in both light and dark conditions. Door in escape route on 2nd deck was locked.

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 50 millimeters (2 inches) high: "EMERGENCY EXIT, KEEP CLEAR". Aft head and galley exit missing photo luminescent marking for exit markings. Install photo luminescent emergency exit marking to the satisfaction of the attending marine inspector.

Electrical equipment on a vessel must be installed and maintained to: (b) Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical equipment, and electrical shock. The attending marine inspector discovered a battery charger installation in lazarette that was temporarily installed into an electrical outlet. Submit plans for permanent electrical installation to the OCMI for approval.

On vessels with no overnight accommodation; public spaces that may be assumed to be occupied by a large number of persons when passengers are on board need only be served by a manual alarm system that meets the requirements in part 76 in subchapter H of this chapter. The manual call point outside the heads had no visual and audible alarm, and not operating as designed by the manufacturer compared to the other call points that had audible and visual alarms. Repair per manufacturer specifications and to the satisfaction of the attending marine inspector.

A vessel must comply with the applicable requirements for any radio and Electronic Position Indicating Radiobeacon (EPIRB) installations, including the requirements for a station license and installation certificates to be issued by the FCC, as set forth in 47 CFR part 80. The vessels crew were unable to provide evidence of holding a marine operator permit. Provide objective evidence that someone listed one the COI has a valid operator’s permit to the satisfaction of the attending marine inspector.

A vessel must carry either a first-aid kit that meets the requirements in 46 CFR 199.175(b)(10) or equivalent. 48 doses of analgesics were expired. Replace doses.

Each vessel must be provided with a visual and audible alarm at the operating station to indicate a high water level in each of the following normally unmanned spaces: a space with a non-watertight closure. Bilge high level alarm in women’s head below main deck inoperable. Repair to the satisfaction of the attending marine inspector.

Fixed fire extinguishing system design and install must be in accordance with the Marine Design, Installation, Operation, and Maintenance Manual approved for the system by the Commandant. The fixed fire extinguishing system is inoperable and does not meet the operational requirements of the listed manual. 46 CFR 118.410(a)(3)

46 CFR Part 119 contains requirements for operation of auxiliary machinery, which must be suitable for the vessel and for the purpose intended. The starboard generator does not have a started installed, will not start, and does not meet the purpose intended. 46 CFR 119.100

At each subsequent inspection for certification, the marine inspector may require that a man overboard drill be held under simulated emergency conditions. 46 CFR 115.808(g)

At each subsequent inspection for certification, the marine inspector may require that a fire drill be held under simulated emergency conditions. 46 CFR 115.810(d)

Rugs and carpet may be used in addition to deck coverings. Rugs and carpets must be constructed of 100 percent wool or equivalent as determined by a flame spread rating not exceeding 75 and a smoke developed rating not exceeding 100 when tested according to ASTM E 84. Provide approval and flame spread rating for new carpet. 46 CFR 116.423(a)(4)

A device must be provided to automatically shut down power ventilation serving the protected space and engines that draw intake air from the protected space prior to release of the extinguishing agent into the space. The automatic shutdowns for the power ventilationand generators did not actuate. Vessel to provide interim written procedures for manual shutdowns of the power ventilation and generators.

Each connection to a conductor or terminal part of a conductor must be made within an enclosure and have either: 1) A pressure type connector on each conductor; 2) A solder lug on each conductor; 3) A splice made with a pressure tyoe connector to a flexible lead or conductor; or 4) A splice that is soldered, brazed, or welded to a flexible lead or conductor. Vessel has unapproved twist-on connection :or wires near fuel tank.

A-Class bulkheads must be composed of steel or equivalent material, suitably stiffened and made intact with the main structure of the vessel, such as the shell, structural bulkheads, and decks. They must be so constructed that, if the vessel is sibjected to the standard fire test, they are capable of preventing the passage of smoke and flame for 1 hour. In addition, they must be so insulated with approved structural insulation, bulkhead panels, or deck covering so that, if subjected to the standard fire test for the applicable time period listed below, the average tempreature on the unexposed side does not rise more than 250 degrees F above the original tempreature, nor does the temperature at anu one point, including any joint, rise more than 325 degrees F above the original temperature. Structural fire protection insulation on port, forward bulkhead underneath water heater is missing.

Except as otherwise allowed by this subject, a vessel must comply with the structural design requirements of one of the standards listed below for the hull material of the vessel. Aluminum hull vessels: 1) For a vessel of more that 100 feet in length-Rules for building and classing aluminum vessels, ABS; or 2) ABS guide for high speed craft. Vessel has wasted frames along forward and aft bulkgeads in galley space. Vessel to repair at next drydock.

Door frames must be of rigid construction and provide at least a 0.5 inch overlap at the sides and top, except: Double doors capable of independent operation and latching may have a clearance between the doors of not more than 0.125 inches. However, if one door must always be closed first, means shall be provided to ensure that the doors close in the proper order. To the satisfaction of the attending MI, adjust the first second, and third deck double doors so the one door that must always be closed first does close first. 46 CFR 116.435(B)(5)(i)

When a deck scuttle serves as a means of escape, it must not be less than 455 millimeters (18 inches) in diameter and must be fitted with a quick acting release and a holdback device to hold the scuttle in an open position. The stairway from the galley to the second deck escape scuttle does not have a hold back device to hold it in the open position. Provide ans prove proper operation of the hold back device to the satisfaction of the attending MI. 46 CFR 116.500(m)

Fuel filter and strainer bowls must be highly resistant to shattering due to mechanical impact and resistant to failure due to thermal shock. Fuel filters fitted with bowls of other than steel construction must be approved by the Commandant and be protected from mechanical damage. Approval of bowls of other than steel construction will specify if a flame shield is required. Both generators Recor fuel filter bowls are missing a flame shield. Provide documentation that a flame shield is not required or if required, install the manufacturers approved flame shield to the satisfaction of the attending MI.

Each sewage retention device must have a means of indicating when the device is more than 3/4 full by volume. Type III sewage holding tank level indicator light on bridge is not working. Resolve to the satisfaction of the attending MI at the vessel's next dry dock in April 30, 2025. 46 CFR 121.704 33 CFR 159.83

A fire pump must be capable of both remote operation from the operating station and local operation at the pump. Fire pump only has resets in the engine room but not local operation at the pump. Provide local operation for the fire pump in the engine room to the satisfaction of the attending MI. 46 CFR 118.300(e)

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Bow thruster alteration deviate from approved plans. Proposed alterations must be approved by the Marine Safety Center (MSC) before work is started. Submit bow thruster alterations plans to MSC for approval. 46 CFR 115.700

Repairs or alterations to the hull, machinery, or equipment that affect the safety of the vessel must not be made without the approval of the cognizant OCMI, except during an emergency. Generator to be replaced in-kind this dry dock. Provide written specifications describing the details of proposed in-kind replacement of the generator for approval to the cognizant OCMI before work is started. 46 CFR 115.700

Bulkheads and decks—Bulkheads and decks must be classed as A–60, A–30, A–15, A–0, B–15, B–0, C, or C' based on the following: A-Class bulkheads or decks must be composed of steel or equivalent material, suitably stiffened and made intact with the main structure of the vessel, such as the shell, structural bulkheads, and decks. They must be so constructed that, if subjected to the standard fire test, they are capable of preventing the passage of smoke and flame for 1 hour. In addition, they must be so insulated with approved structural insulation, bulkhead panels, or deck covering so that, if subjected to the standard fire test for the applicable time period listed below, the average temperature on the unexposed side does not rise more than 139 °C (250 °F) above the original temperature, nor does the temperature at any one point, including any joint, rise more than 181 °C (325 °F) above the original temperature:Penetrations in A-Class fire control boundaries for electrical cables, pipes, trunks, ducts, etc. must be constructed to prevent the passage of flame and smoke for one hour. In addition, the penetration must be designed or insulated so that it will withstand the same temperature rise limits as the boundary penetrated. Until it can be determined that the installed structural fire protection insulation for the generators cable penetrations and main cable trunk connecting the main deck accommodation to the second deck accommodation and the main cable trunk connecting the second deck accommodation to the control station (pilot house) has a CG certificate of approval and has been installed in accordance with the manufactures specifications to the satisfaction of the attending MI the vessel will be restricted from carrying passengers. 46 CFR 116.415(2)(ii)

Bulkheads and decks—Bulkheads and decks must be classed as A–60, A–30, A–15, A–0, B–15, B–0, C, or C' based on the following: A-Class bulkheads or decks must be composed of steel or equivalent material, suitably stiffened and made intact with the main structure of the vessel, such as the shell, structural bulkheads, and decks. They must be so constructed that, if subjected to the standard fire test, they are capable of preventing the passage of smoke and flame for 1 hour. In addition, they must be so insulated with approved structural insulation, bulkhead panels, or deck covering so that, if subjected to the standard fire test for the applicable time period listed below, the average temperature on the unexposed side does not rise more than 139 °C (250 °F) above the original temperature, nor does the temperature at any one point, including any joint, rise more than 181 °C (325 °F) above the original temperature:Penetrations in A-Class fire control boundaries for electrical cables, pipes, trunks, ducts, etc. must be constructed to prevent the passage of flame and smoke for one hour. In addition, the penetration must be designed or insulated so that it will withstand the same temperature rise limits as the boundary penetrated. Vessel passenger carriage is reduced to 150 or fewer passengers with non-approved Coast Guard structural fire protection materials for the generators cable penetrations and main cable trunk connecting the main deck accommodation to the second deck accommodation and the main cable trunk connecting the second deck accommodation to the control station (pilot house). Owner/operator provided thirty days to procure and install Coast Guard approved structural fire protection insulation in the above locations. Installation to be in accordance with the Coast Guard manufacturers type approval certificate to the satisfaction of the attending [name withheld], owner/operator to provide manufacturers Coast Guard type approval certificate and installation instructions to the attending MI. 46 CFR 116.415(2)(ii)

There must be at least two sources of power for the electrical equipment of each fire detecting and alarm system. The normal source must be the main power source. The other source must be the emergency power source or an automatically charged battery. If the other source is an automatically charged battery, the charger must be supplied from the final emergency power source. Upon loss of power to the system from the normal source, the system must be automatically supplied from the other source. The fire detecting system back-up battery power supply is disconnected. Provide two sources of power for the fire detecting system IAW with approval granted by MSC Project No. P009118/Serial: E2-1701528 dated May 02, 2017. 46 CFR 113.10-9

Decks between various spaces must meet the requirements of Table 116.415(c). Vessel missing insulation above starboard engine and starboard bulkhead near fire pump. Renew insulation to original thickness and rating. 46 CFR 116.415

Electrical equipment on a vessel must be installed and maintained to: Protect passengers, crew, other persons, and the vessel from electrical hazards, including fire, caused by or originating in electrical shock. LED lighting mounted to deck railing is supplied power by a flexible cord plugged-in to receptacle outlets mounted on the main deck structure to receptacle outlets on after ladder wells. 1) Remove or make permanent lighting fixtures; 2) Provide proof the exterior lighting fixtures electrical system is less than 50 volts or it must comply with the requirements of UL 595; and 3) power LED lighting fixture with a branch circuit from a local distribution panel. 46 CFR 120.200(b)

For vessels required to meet the damage stability standards of subpart H of this chapter, when Class 1 doors are installed below a deck the molded line of which at its lowest point at side is less than 7 feet (2.14 meters) above the deepest load line, an indicator light for each door which warns when the door is open must be installed on the bridge. Indicator light on QAWTD at bulkhead 45 for access to engine room is constantly illuminated. Adjust limit switch so indicator light only illuminates when the door is opened. 46 CFR 170.255(e)

Each vessel must have adequate emergency lighting fitted along the line of escape to the main deck from all passenger and crew accommodation spaces located below the main deck. Galley ladder leading to the deck scuttle to the main deck is not readily apparent to a person in dark conditions. Provide emergency lighting to the satisfaction of the attending MI. 46 CFR 120.432

Watertight doors and watertight hatches must be marked on both sides in clearly legible letters at least 25 millimeters (1 inch) high: “WATERTIGHT DOOR - KEEP CLOSED” or “WATERTIGHT HATCH - KEEP CLOSED”, unless such markings are deemed unnecessary by the cognizant OCMI. 46 CFR 122.610

A framed chart or diagram must be installed in the wheelhouse or control station adjacent to the detecting cabinet indicating the location of the various detecting zones and giving instructions for the operation, maintenance, and testing of the system. This chart, or a a separate card or booklet to be kept near the chart, must have tabulated spaces for the date and signature of the licensed officer of the vessel who must witness or conduct the periodic tests. Provide. 46 CFR 76.27-80(b)(6)

At each initial and subsequent inspection for certification, the owner or managing operator shall be prepared to conduct tests and have the vessel ready for inspection of its fire protection equipment, including the following:(2) Inspection of piping, controls, and valves, and the inspection and testing of alarms and ventilation shutdowns, for each fixed gas fire extinguishing system and detecting system to determine that the system is in operating condition; Conduct servicing of fixed fire extinguishing system in the presence of attending USCG inspector. 46 CFR 115.810(a)(2)

Inspection and testing of smoke and fire detecting systems (including sensors and alarms) and fire confining appliances (such as fire screen doors and fire dampers). During testing of smoke detectors crew had insufficient testing smoke and crew not able to demonstrate testing of heat detectors. Crew to provide testing smoke to complete sample testing of smoke detectors in the presence of the attending MI. In addition, provide Service Company to test sampling of heat detectors in the presence of the attending MI. 46 CFR 115.810(a)(7)

The fire detection and alarm system must notify crew and passengers of a fire when appropriate; Fire pull station for the lower aft port side head on main deck, local alarm did not sound when pulled. Repair alarm system for at this location. 46 CFR 76.27-5(e)

Rugs and carpets shall not extend up bulkheads or vertical surfaces more than 10 centimeters (4 inches) above the deck. Carpet extended more than 4in above the deck in the forward storage main deck space. 46 CFR 116.423(4)(I)

Each lifeline on a ring buoy must be of a dark color if synthetic, or of a type certified to be resistant to deterioration from ultraviolet light. Aft port ring buoy lifeline was faded and frayed. Renew lifeline. 46 CFR 117.70(c)(6)

The name of the vessel must be marked on some clearly visible exterior part of the port and starboard bow. Name of vessel missing from port bow. Mark port bow with name of vessel. 46 CFR 67.123(a)

For the purpose of this subpart, rooms containing “fire resistant furnishings” are considered to be those in which: Furniture such as chairs, sofas, and similar items are tested and meet the requirements in UL 1056 “Fire Test of Upholstered Furniture,” or meet the requirements in § 72.05-55 in Subchapter H of this chapter. Provide test reports, manufactures certificates, or other evidence of testing to the required standards for acceptability of the Hampton Bay lawn furniture in 02 deck accommodation. 46 CFR 116.423(a)(1)

Each vessel that relies on electricity to power the following loads must be arranged so that the loads can be energized from two sources of electricity. The port generator lost all cooling fluid and is overheating. 46 CFR 120.310(a)(1)

The installation requirements for machinery and boilers for steam and electrically propelled vessels are contained in applicable regulations in subchapter F (Marine Engineering) and subchapter J (Electrical Engineering) of this chapter. The raw water in take sensor on the stbd engine is not installed. 46 CFR 119.220(a)

The stbd generator battery has a wire connecting it to a decommissioned battery charger. Remove the decommissioned wire. 46 CFR 119.220(a)

A vessel must comply with the applicable requirements for any radio installations, including the requirements for a stations license and installation certificates to be issued by the FCC, as set forth in 47 CFR part 80. Vessel does not have a valid Bridge-to-Bridge certificate. 46 CFR 121.502

The person in command of a documented vessel must have on board that vessel the original Certificate of Documentation currently in effect for that vessel. Current COD has the incorrect owner address. 46 CFR 67.313(a)

Fire extinguishing equipment installed on a vessel in excess of the requirements of 118.400 and 118.500 must be designed, constructed, installed, and maintained in a manner acceptable to the Commandant. The fire control panel shows a system trouble message for the panel batteries having low voltage.

A fire control plan must be posted on the vessel in a location that is accessible and visible to all passengers. The plan must show escape routes, areas of refuge, embarkation stations, the location of fire protection/emergency equipment, compartment titles and hazard classifification of accommodation and service spaces, and structural fire protection boundaries. The placement of the fire extinguishers does not match the fire control plan. 46 CFR 116.530

Connections must be made to battery terminals with permanent type connectors. Spring clips or other temporary type clamps are prohibited. The battery terminals in the engine compartment require cleaning and removal of wing nuts. 46 CFR 120.350(d)

On the aft top portion of the port generator their is a short flexible connection that is cracked and requires replacement. 46 CFR 119.220(a)

Each launching appliance and each survival craft and rescue boat on a vessel must be in good working order and ready for immediate use before the vessel leaves port and at all times when the vessel is underway. The stbd IBA is out of service and is not required for the vessels route. Remove the IBA. 46 CFR 122.700(a)

Each vessel operating in warm water may be provided with life floats of an aggregate capacity that will accommodate at least 50% of the total number of persons permitted on board. The both of the vessel’s inflatable buoyant apprentices are either not installed or passed due for servicing.

Provisions must be made for closing all supply duct and exhaust duct openings for a space protected by a fixed gas extinguishing system. Vessels exhaust duct dampers for engine room protected by fixed CO2 do not close adequately. Supply side unable to verify. Ensure exhaust dampers close satisfactory and make supply side available for inspection.

At each inspection of a vessel, the owner of managing operator shall be prepared to conduct: operation test of electrical apparatus, which operates as part of or in conjunction with a fire detection or alarm system installed on board a vessel, by simulating, as closely as practicable, the actual operation in case of fire. Vessel was unable to satisfactory test fire detectors in engine space.

Unless otherwise specified in this section, the Vessel Security Officer must keep records of the activities as set out in paragraph (b) of this section for at least 2 years and make them available to the Coast Guard upon request. The VSO was unable to provide all of the security drill records for the previous 2 years.

Vessels operated solely within 20 nautical miles of land must be equipped with a VHF-DSC radiotelephone installation meeting the requirements of § 80.1101(c)(2). VHF radio LAT/LONG for DSC operation.

At each inspection for certification and at every other vessel inspection all observed unsafe practices, fire hazards, and other hazardous situations must be corrected and all required guards and protective devices must be in satisfactory condition. The main deck and O1 deck double door emergency exits have a lock that is not mechanically connected to door handle. This means that if the double doors are closed and locked a person would have to ensure that they unlock the lock before turning the door handle. From inside the space, turning the door handle should automatically unlock the lock for unobstructed emergency egress.

Complete but simple instructions for operation of a fixed gas fire extinguishing system must be located in a conspicuous place at or near each pull box and stop valve control. Vessels remote instructions do not have a labels for the pull stations, provide adequate labels.

All escape hatches and other emergency exits used as means of escape must be marked on both sides in clearly legible letters at least 2 inches high. Vessel has door to 1st passenger deck up forward missing emergency exit sign.

Materials used in piping systems must meet the requirements of this subpart and be otherwise acceptable to the cognizant [name withheld]/replace leaking water pipe on port side overhead in gray water holding tank compartment.

Propulsion machinery must be suitable in type and design for propulsion requirements of the hull in which it is installed and capable of operating at constant marine load under such requirements without exceeding its designed limitations. The vessel reported a failure of the reduction gear on the starboard engine and inability to repair due to lack of available parts. Demonstrate repair and operation of starboard engine's reduction gear to the satisfaction of the attending marine inspector. Vessel must utilize towing vessel (s) of adequate size and horsepower when shifting between piers within Honolulu harbor until deficiency is cleared.

he owner, managing operator, or a qualified servicing facility as applicable shall conduct the following inspections and tests:(1)For portable fire extinguishers, the inspections, maintenance procedures and hydrostatic pressure tests required by chapter 4 of NFPA 10 with the frequency specified by NFPA 10; Monthly inspections were not conducted on any of the portable fire extinguishers. Have qualified servicing facility conduct servicing of all portable fire extinguishers onboard. 46 CFR 115.810(b)(1)

Fuel filters fitted with bowls of other than steel construction must be approved by the Commandant and be protected from mechanical damage. Approval of bowls of other tan steel construction will specify if a flame shield is required; starboard main diesel engine fuel filter did not have a flame shield installed. Install flame shield in accordance with manufacturer instructions. 46 CFR 119.455

The number and identification of the items stowed inside and their sizes, must be marked in clearly legible letters and numbers on each container for life jackets and immersion suits; Aft locker did not have markings. Add markings IAW 46 CFR 122.604 (f) 46 CFR 122.604(f)

The minimum number of portable fire extinguishers required on a vessel must be acceptable to the cognizant OCMI, but must be not less than the minimum number required by Table 118.500(a) of this section; Add one 40-B:C extinguishers outside of fuel tank space and two 2-A on 01 and 02 decks. 46 CFR 118.500 (a)

The fire detection and alarm system must control and monitor input signals for all connected detectors and manual pull stations or call points; detection and alarm system was showing the wrong location when the pull station on the 01 deck was pulled. Repair system so that the proper space is displayed. 46 CFR 76.27-5(c)

Every vessel equipped with an inflatable survival craft must have approved placards or other cards containing instructions for launching and inflating inflatable survival craft for the information of persons on board posted in conspicuous places by each inflatable survival craft; vessel did not have placards posted. Add placards. 46 CFR 122.518(a)

A fire control plan must be posted on the vessel in a location that is accessible and visible to all passengers. The plan must show escape routes, areas of refuge, embarkation stations, the location of fire protection/emergency equipment, compartment titles and hazard classification of accommodation and service spaces, and structural fire protection boundaries; Fire control plan was not up to date due to removal of galley fire fighting equipment. Update fire control plan to current configuration on vessel. 46 CFR 116.530

Several starboard side dry dock plugs found backed out with two to three threads visible. Remove all dry dock plugs and clean for inspection by USCG.

Carpet found removed on deck 1 and deck 2. Operator stated that new carpet is to be installed on both decks. Provide manufacturer specificaitions for carpet USCG prior to installation.

All sea chests found with marine growth. Clean marine growth and threads for sea valves for inspection by USCG.

Lighting fixture foudn fractured on deck 2 aft stairwell. Renew as original.

Exhaust leak found on port and starboard generators exhaust piping. Temporary repair putty found on exhaust piping is unsatisfactory. Renew exhaust as original.

Identified bottom plate corrosion in the bilge under the galley. Provide NDT report for affected area to be further evaluated for repair. Area to be cleaned and coated if repairs are not deemed necessary.

Identified one damaged and warped bulkhead, located forward fo the ladder, in the gray water tank space and two bulkheads in the bow thruster tunnel space, located forward of the bow thruster tunnel. Provide measurements to the USCG for observed damage to be further evaluated for repair.

Port and starboard main engine sea water valves found corroded. Renew as original.

Aft sea keeper gate valve found corroded. Operator intends to resurface and blue fit. Replace if valve no longer meets specifications.

Ball valve missing for starboard engine room air breather sea valve. Renew as original.

A class bulkheads or decks must be composed of steel...in addition, they must be so insulated with approved structural insulation. Port side engine room overhead and steering space forward bulkhead insulation is missing. 46 CFR 116.415(a)(2)(i); 50c

All cable and wire must be installed with metal support spaced not more than 24 inches apart...the use of plastic tie wraps must be so limited to bundling...and not used as a means of support. Multiple electrical cables in engine room are missing metal supports. 46 CFR 120.340(b)(4); 50c

Nonmetallic piping materials, including nonmetallic flexible hose assemblies, must meet the requirement of 56.60-25 in Subchapter F of this chapter. Flexible nonmetallic piping found in use as sea chest vent piping does not meet Subchapter F.

Submit repair proposal for port side loss of propulsion prior to conmmencing any repairs. Proposal shall be to the satisfaction of USCG. Repairs shall be made to the satisfaction of the attanding marine inspector prior to carrying passengers.

Properly install structural fire insulation on bulkhead separating the fixed CO2 storage location from the machinery space. Ensure insulation has required vapor barrier and provides full coverage of the required area.

Ensure electrical cables are properly secured w/ metallic clamps every 24 inches. Clamps should be fitted w/ chaffe protection to avoid damaging the cable.

Provide an emergency disconnect switch on each positive battery cable, as close to the battery as practicable.

Conduct insulation resistance tests w/ a megohmeter on both ship service generators, and provide report of readings to [name withheld] must be taken to show insulation resistance between each phase, and from each phase to ground (vessel's metallic hull).

Provide means for IBAs to be manually deployed.

Provide manual pull station for fire alarm in bridal room on upper deck.

Re-install missing cotter pin for STBD rudder connecting bolt for hydraulic ram & cross bar connection.

Properly seal penetrations in A-class bulkhead between engine room and fire fighting CO2 storage location.

Provide repair proposal for damage sustained to port bow area prior to April 2017 dry dock.

Install required fire detection devices in spaces with equipment listed under 46CFR 118.400(c) and in spaces listed under 46CF 118.400(e)

Provide adequate ventilation for small battery installations located under bridge console, and for battery banks installed for vessel's gyro stabilizers.

Submit Non-Tank Vessel Response Plan to MSC for review and approval

Provide approved structural fire protection plan which designates top open deck area as a type 5, 6 or 7 accommodation space. Or submit fire load calcs for carpet and furnishings installed and submit new structural fire protection plan to MSC for review which designates top deck area as an accomodation space.

Ensure installed time delay is fitted with the required by-pass valve.

Ensure all control devices for fixed CO2 system are located outside of the protected space. Currently the time-delay, which is required to have a by-pass valve (making it a control device) is located within the protected space.

Ensure all circuit breakers are properly rated for their installed service. Currently circuit breakers used for the vessel's DC electrical system are rated for AC voltage, not DC voltage. An AC over-current device may not be able to adequately/safely open a DC current fault.

Repair oil and coolant leaks on Port generator. Rear oil seal on generator is leaking and oil is collecting on the underside of the turbo charger. There is also coolant leak at the back end of the generator that needs to be identified and repaired.

Provided approved Fire Control Plan

Install approved rescue boat or request determination from OCMI to be able to recover MOB without use of a rescue boat.

Address all high level float wiring problems and install IAW CFRs

Re-install missing insulation on FWD Port bulkhead in engine room in way of cable run.
