Doors in A-Class fire control boundaries must meet the following additional requirements: (3) A door must have a latch with a minimum throw of 20 millimeters (.75in). The aft main deck doors to refuge area were not latching and missing striker plate. Install striker plate and adjust the door to close to the satisfaction of the attending marine inspector.

The manual call point outside the heads had no visual and audible alarm, and not operating as designed by the manufacturer compared to the other call points that had audible and visual alarms. A-Class bulkheads must be composed of steel or equivalent material, suitably stiffened and made intact with the main structure of the vessel, such as the shell, structural bulkheads, and decks. They must be so constructed that, if the vessel is sibjected to the standard fire test, they are capable of preventing the passage of smoke and flame for 1 hour. In addition, they must be so insulated with approved structural insulation, bulkhead panels, or deck covering so that, if subjected to the standard fire test for the applicable time period listed below, the average tempreature on the unexposed side does not rise more than 250 degrees F above the original tempreature, nor does the temperature at anu one point, including any joint, rise more than 325 degrees F above the original temperature. Structural fire protection insulation on port, forward bulkhead underneath water heater is missing.

Approval of bowls of other than steel construction will specify if a flame shield is required. Both generators Recor fuel filter bowls are missing a flame shield. Provide documentation that a flame shield is not required or if required, install the manufacturers approved flame shield to the satisfaction of the attending MI.
