Vessel reported a loss of propulsion after the main engine lube oil exhaust valve experienced a high temp alarm. Vessel was slowing down to repair a scavenger air leak. Vessel must arrange for a tug assist of appropriate horsepower for their transit from the sea buoy to the dock. Troubleshoot necessary repairs and provide technician report of satisfactory repairs and operation.

The following fire protection systems shall be kept in good order so as to ensure their required performance if a fire occurs: structural fire protection, including fire-resisting divisions, and protections of openings and penetrations in these divisions. A-60 fire door between engine room and steering gear room has a hole where the lock mechanism used to be.

The guidelines developed by the Organization, MSC Circular 1432 section 7.11, call for testing (or replacing) portable foam concentrate containers at 5 to 10 year intervals (depending on type). Fire control plan shows a total of 20 3% AFFF 5 gallon pails. Of those spot-checked during the inspection, all were dated over 10 years ago with no record of foam analysis testing. Check all 20 containers and test or replace any that exceed the time allowances in MSC Circular 1432.

Vessel's port lifeboat's on-load release mechanism is inopperable. Repair to satisfaction of manufacturer and class. Cite: Solas 74 ChIII, Reg 16; LSA Code 4.4.7.6.2

The condition of the ship and its equipment shall be maintained to conform with the provisions of the present regulations to ensure that the ship in all respects will remain fit to proceed to sea without danger to ship or persons onboard. Vessel crew discovered damage to main hydraullic line to ram of aft loading ramp and made temporary repairs. Must make permanent repairs to main hydraullic line to the satisfaction of Class.

Emergency generator be capable both of supplying the services mentioned and automatically started and supplying the required load as quickly as is safe and practicable subject to a maximum of 45s. Emergency generator automatically started, emergency generator breaker would not close to supply ship with emergency power. Ships crew replaced faulty breaker rectifying the deficiency.

Measures shall be taken to the satisfaction of the Administration to ensure that the equipment is functioning in a reliable manner and that satisfactory arrangements are made for regular inspections and routine tests to ensure continuous reliable operation. Machinery Low Lube oil test equipment not functioning as intended. Replace or repair to comply with maintenance procedures. #1 SSDG over-speed shutdown was unsuccessfully tested from main switchboard, then manually manipulated governor with a wrench to activate shutdown. Repair or replace to comply with procedures.

Where a fixed fire detection & alarm system is required for the protection of spaces other than those specified in paragraph 5.1, at least one detector complying with the FSS Code shall be installed in each space. The function shall be tested under varying conditions of ventilation after installation. The 01 smoke detector installed inside the E/R workshop found hanging by a zip tie and would not function when tested in presence of MI.

During outbound transit to HI, VSL lost ability to operate main engine throttles remotely from bridge. VSL posted an engineer for local engine control. VSL is to repair failure pier side and have DNV surveyor provide report to duty MI prior to VSL sailing.

The machinery, boilers and other pressure vessels, associated piping systems and fittings shall be of a design and construction adequate for the service for which they are intended and shall be so installed and protected as to reduce to a minimum any danger to persons on board. When lifting the stern ramp the foot hydraulic line failed and discharged approximately one gallon of hydraulic fuel into the waterway. The vessel is required to repair the failed hydraulic line upon arrival at next port of call.

Each oil-fired boiler which is intended to be operated without manual supervision shall have safety arrangements which shut off the fuel supply and give alarm in case of low water level, air supply failure or flame failure. The auxiliary boiler "firing sequence controller" failed in NOV 2020. Boiler combustion failure alarm no longer alarms to engine control station. Crew has operated boiler in emergency operation mode under engineering watch since failure. No notification was made to DNV-GL or USCG until MAR 2021. Chief Engineer shall provide attestation that all boiler safety shutdowns have been tested since failure and boiler shutdowns to safe mode. Chief Engineer shall create boiler watchstanding standing orders to be approved by USCG and DNV-GL. Firing sequence controller shall be replaced within 30 days.

All machinery shall be designed, constructed, installed and protected to reduce danger to a minimum to persons on board with due regard to hot surfaces. Incinerator #1 found with severly damaged refractory causing paint peeling and hot surfaces on back corner. Properly replace refractory.

Shipboard incineration of garbage containing more than tracers of heavy metals is prohibited. USCG Certificate of Approval does not allow any amount of unburned components in ashes (0%). Unburned metal bands and metal strainer found within combustion chamber in ashes. MARPOL Annex VI/16.2.3

All steering gear components shall be of sound and reliable construction. Port side steering gear hydraulic suction piping found excessively leaking during testing. Repair leaking piping.

Steering gear control shall be provided on the navigation bridge and in the steering gear compartment. Emergency steering gear controls at starboard pump found inoperable to port side due to improperly sized plunger pin button to solenoid.

All inlets and discharges in the shell plating shall be fitted with efficient and accessible arrangements for preventing the accidental admission of water into the ship. OWS discharge pipe found with pinholes just inboard of the skin valve. Permanently repair piping.

A weld in a section of pipe that is part of the main fuel system conditioning unit developed a crack and subsequent fuel leak.

Before loading vehicles for stowage each vehicle must be inspected and stowed in accordance with the applicable sections of 49 CFR 176.905 unless the cargo hold has been designated "specially designed and approved for vehicles". The ships crew as well as general arrangement plans denote that the vessel's holds are designated "specially designed and approved for vehicles". This designation was predicated on a Statement-of-Fact issued by DNV-GL on 19FEB2016 in which exclaimed no electrical equipment was installed less than 460mm and all other electrical equipment was installed anti-explosion type. With the exception of the watertight decks each vehicle deck is perforated which may allow the passage of vapors shifting the less than 460mm criteria and the installed electrical equipment could not be verified to be intrinsically safe, flame proof, pressurized/purged, or an increased safety type (e.g. open outlets, marine grade lighting, ramp controllers). Nor were shipboard procedures to electrically isolate the spaces provided. Provide a corrective action plan to ensure vehicles are stowed in accordance with 49 CFR 176.905. 49 CFR 176.905 46 CFR 90.10-38 46 CFR 96.05-1 46 CFR 111.105

An A-0 fire boundary shall be provided between the deck separating the steering gear space (7) and cargo space (8). The cable penetration between deck 5 and the steering gear space was found holed and modified. Permanently repair the cable penetration (block type) in accordance with the manufactures instructions. Temporary repair in way of fire stop caulking authorized. The crew stated that other penetrations have been modified in the same manner. Verify the integrity of all fire/watertight boundary penetrations and provide a corrective action plan to remedy. SOLAS(97) II-2/44

Lighting fittings shall be so arranged as to prevent temperature rises which could damage the cables and wiring, and to prevent surrounding material from becoming excessively hot. Identified lighting without a protective light cover and damaged bulbs located on cargo deck 6 at frame 175 and frame 180. Replace lighting installation in kind. SOLAS 74 (2001 cons) Ch. II-1 Reg 45.7

A fixed carbon dioxide system must comply with the manufacturer's approved Design, Installation, Operation and Maintenance Manual that meets Chapter II-2, Regulation 10.4 of SOLAS 2000 Amendments and the below supplemental requirements (ref. 46 CFR 34.15, 76.15, 95.15). Upon completion of the piping installation, and before the cylinders are connected, a pressure test in accordance with the manufacturer's Design, Installation, Operation, and Maintenance Manual shall be conducted. Per the manufacturer, flexible connections between cylinders and distribution piping of fixed carbon dioxide fire extinguishing systems must be renewed or tested at the appropriate test pressure. The flexible hose on the paint locker fixed fire extinguishing system connecting the extinguishing agent bottle to the manifold is not marked with the MAWP or test pressure date. A review of the maintenance records for the fire extinguishing system revealed that the flexible hose has not been replaced or pressure tested within the last 5 years or more. Replace the flexible hose in kind or provide proof of an appropriate pressure test in accordance with the system manufacturer. SOLAS 74 (2000 Amendment) Ch. Every oil fuel pipe, which, if damaged, would allow oil to escape from a storage, settling or daily service tank situated above the double bottom, shall be fitted with a cock or valve directly on the tank capable of being closed from a safe position outside the space concerned in the event of a fire occurring in the space in which such tanks are situated. Remote operated fuel valve for diesel tank in Group #4 did not shut when actuated. Valve was exercised and operated successfully when re-tested. SOLAS74 (2001 cons)II-2/15.2.5

Provide check valve for CO2 low pressure system in accordance with Global Fire and Safety Inc. report dated 6 Mar 2019. Class has issued a condition regarding this 835.

Provide retro reflective tape for all lifesaving appliances. Current retro tape is not SOLAS approved tape. DNV has issued as a condition of class.

Demontrate proper operation of the MDE in the way of the exhaust leaks and burnt lagging.

Demonstrate proper operation of the MDE in the way of lube oil leaks from the exhaust valve actuator.

Provide servicing and proper documentation of the portable fire extinguishers throughout the vessel.

Port life boat Davit: the aft outboard sheave is showing signs of metal delamination. Provide class reort of condition.
