Cargo tanks in which Grade A, B, or C liquids are to be transported shall be fitted with either individual pressure-vacuum (P/V) relief valves or shall be fitted with a venting system consisting of branch vent lines connected to a vent header and be fitted with a pressure-vacuum relief valve. The port side slop tank was fitted with an additional goose neck vent underneath it's P/V valve, not consistent with the vessel's branch diagram. Make appropriate modifications to return venting arrangement for the slop tank back to the configuration of the original drawings. Rectify prior to using the port side slop tank.

On all tank vessels, the construction or conversion of which is started on or after November 10, 1936, the cargo pumps shall be designed and installed to minimize the danger of sparking. Special care shall be exercised in the design of packing spaces in order to secure ample depth and accessibility of glands. Where cargo pump shafts pierce gas tight bulkheads, stuffing boxes with readily accessible gas tight glands shall be provided. Observed wasted gaskets on both Port and STBD shaft seals of cargo pumps that allowed air to pass from the hazardous zone into the non-hazardous rated space. Cargo restricted to D/E cargo.

Grounded neutral and high-impedance grounded neutral alternating current systems must have a suitably sensitive ground detection system which indicates current in the ground connection, is able to withstand the maximum available fault current without damage, and provides continuous indication of circuit status to ground. A provision must be included to compare indications under fault conditions with those under normal conditions. Ground detection lighting was found extinguished.
