During the vessel's construction and when any modification to the lifesaving arrangement is done after construction, a vessel owner must obtain acceptance of lifesaving arrangements from the Commandant (Marine Safety Center(MSC)). The current lifesaving arrangement aboard the vessel must be reviewed by MSC before being accepted as a permanent arrangement for compliance with NVIC 2-81 Ch-2. You have submitted your request for MSC to review this modification. Notify local OCMI when approval is granted or if any further modifications are recommended by MSC. You are being given 30 days to receive final approval from MSC.

Each liferaft must be arranged to permit it to drop into the water from the deck on which it is stowed. A liferaft stowage arrangements meets this requirement if it-- (i) Is outboard of the rail or bulwark; (ii) Is on stanchions or on a platform adjacent to the rail or bulwark; or (iii) has a gate or other suitable opening large enough to allow the liferaft to be pushed directly overboard and, if the liferaft is intended to be available for use on either side of the vessel, such gate or opening is provided on each side of the vessel. Other than the stowage position for the liferaft required under § 199.261(e), no stowage position or muster and embarkation station for a survival craft on a tank vessel may be located on or above a cargo tank, slop tank, or other tank containing explosives or hazardous liquids. Until the vessel installs the appropriate liferafts for conditional occupancy, the vessel shall remain at its current berth. 46 CFR 199.40(d) 46 CFR 199.130(c)(3) 46 CFR 199.290(c) NVIC 02-81 Ch-2

No flammable or combustible liquids may be stowed in any accommodation, control, or service space (other than a paint locker). (d) No more than 208 liters (55 gallons) of combustible liquids may be stowed in any machinery space. Vessel had various oil drums out on deck.

The high-level alarm required by paragraph (a) of this section must - (2) Be identified with the legend “High-level Alarm” in black letters at least 50.8 millimeters (2 inches) high on a white background. The tank overfill alarm required by paragraph (a) of this section must - ((2) Be identified with the legend “TANK OVERFILL ALARM” in black letters at least 50.8 millimeters (2 inches) high on a white background. Vessel did not have adequate labeling of the high level and overfill alarm,

There must be ground detection for each power or lighting distribution system that is isolated from the ship's service power and lighting system by transformers, motor generator sets, or other devices. Ground detection system on 120/125V system located in port side machinery space had a inoperable lamp; due to the fault system was not accurately monitoring system for potential grounds. 46CFR111.05-21

Each vessel vapor connection flange must have a permanently attached 0.5" diameter stud at least 1.0 inch long projecting outward from the flange face. The stud must be located on top, midway between bolt holes. Vsl VCS flanges on port/stbd manifold lack properly mounted studs. 46CFR39.20-3(e).

Vessel has a cargo leak in the #5 tank. Vessel is allowed to continue transit to Boston to discharge but shall make themselves available for inspection and permanent repairs before loading cargo.

Starboard side vapor connection flange does not have a permanently attached 0.5 inch diameter stud at least 1.0 inch long projecting outward from flange face & reducer. 46 CFR 39.20-1(e)

Identification of the barge connector must be labeled "Connector for Barge Overflow Control System" and with the total inductance and capacitance of the connected switches and cabling. 46 CFR 39.20(b)(8)(ii)

Vessel does not have an installed eyewash station/shower located on the weather deck as per the IBC Code. IBC Code 14.3.4 Prior To Loading Cargo on the ICOF. Extended due date from 30APR to 14MAY to allow time for installation per company request.

COI states that vessel can not travel on international voyages. Evidence shows that on October 19, 2010, vessel transitted to Ontario.

V/W pin holed area on centerline aft bulkhead/trunk IWO #5 port cargo tank on seam prior to conducting cargo operations.

Mark vapor hoses as required in 46 CFR 39.20-1(f)(7).

Repair high level alarm to test at 1S panel on deck.
