A ship when in a port or an offshore terminal of another party is subject to inspection by officers duly authorized by such party concerning operational requirements under this annex, where there are clear grounds for believing that the master or crew are not familiar essential shipboard procedures relating to the prevention of pollution by garbage. The entry for each discharge into the sea under Regulations 4, 5, and 6 or Section 5.2 of Chapter 5 of Part II-A of the Polar Code shall include date and time, position of the ship (latitude and longitude), category of the garbage, and the estimated amount (in cubic meters) discharged. For discharge of cargo residues, the discharge start and stop positions shall be recorded in addition to the foregoing. As per crew interviews, vessel has discharged cargo residues and tank cleaning chemicals without the discharge in Part II of Garbage Record Book. In addition, crew is unfamiliar with ship's Garbage Management Plan and reporting requirements. Cites: MARPOL V/Reg. 9.1, MARPOL V/Reg. 19. Code: 30ABC.

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use. Vessel's forward liferaft was found stowed in forward Bosun's store, not in the assigned location on the bow. As per crew member, liferaft was removed due to heavy weather approximately one week before PSC exam. In addition, vessel's fitter is not provided with an immersion suit that fits. Crew member has been onboard since July and vessel has traveled to cold water ports without corrective action. 74 SOLAS 14, Ch. III/20.2. Code: 30ABC.

Resolution: Vessel purchased a immersion suit that fits the vessel's fitter. In addition, crew returned liferaft to its orignal location and did training on the importance of having operational lifesaving equipment.

Vessel's fitter is not provided with an immersion suit that fits. Crew member has been on board since July. Vessel's route requires all crew to be provided with immersion suits. No corrective actions have been taken in accordance with requirements by SOLAS.Ch. III/7.3

The function of fixed fire detection and fire alarm system shall be periodically tested to the satisfaction of the administration by means of equipment producing hot air at appropriate temperature to which the detector is designed to respond. The Vessel's crew does not have appropriate testing device available to test heat detection system. In addition, the vessel's crew is using lighter(s) to test flame detectors, but the crew is not able to confirm whether this method is the vessel's approved method of testing flame detectors.

Plans (fire control plans) and booklets shall be kept up to date; any alteration thereto shall be recorded as soon as practicable. Vessel's has reassigned space formerly known as "ship's laundry" to be used as storage for linens and other paper products. In addition, ship's laundry is no co-located with "engine changing room." Current fire plan does not reflect vessel's current arrangement.

Resolution: Fire plan has been updated. During a conversation onboard, class also mentioned that exhaust venting for the new dryer in the engine changing room has been fixed. Venting was found in substandard and hazardous condition as per class discussion.

The opening below the lowest course of guard rails shall not exceed 230 mm (9inches). The other courses shall be not more than 380 mm (15 inches) apart. In the case of ships with rounded gunwales the guard rail supports shall be placed on the flat deck. On vessel's upper deck, starboard side, two chains acting as railings have rusted away, leaving a gap greater than 15 inches between the uppermost railing and the deck, creating hazardous working condition for crew members on deck.

Bulk carriers shall be fitted with water level detectors: in each cargo hold, giving audible and visual alarms. Vessel's water ingress alarm was switched off for approximately 2 weeks as per crew statements following cargo hold cleaning. PSCO's on-board found the alarms still in the disabled position, only the vessel's electrician knew the system was off.

Before accepting life-saving appliances and arrangements that have not been previously approved by the Administration, the Administration shall be satisfied that life-saving appliances and arrangements comply with the requirements of this code. ;be clearly marked with approval information including the Administration which approved it and any operational restrictions; Vessel's starboard quick-release life-buoy was not marked with IMO approval or its required weight. Crew was not able to provided documentation for the life-buoy. While on-board, crew was able to switch lifebuoy out for one with required markings.

General arrangement plans shall be permanently exhibited for the guidance of ship's officers, showing clearly for each deck the control stations, the various fire sections enclosed by "A" class divisions, the sections enclosed by "B" class divisions together with particulars of the fire detection and fire alarm system, Vessel had a deteriorated, "training suit" in a space designated by the fire plan requiring a functional fire suit. While on-board, crew replaced the fire suit with a new one.

Nautical charts and nautical publications such as sailing directions, lists of lights, notices to mariners, tide tables and all other nautical publications necessary for the intended voyage shall be adequate and up to date. Vessel was outfitted with new operating system for ECDIS. Crew was unfamiliar with the system and how to update charts. PSCOs boarded vessel and charts had not been updated for approximately 2 weeks. While onboard the vessel contacted the company and charts were updated.
