HARVEY DISCOVERY

Ships shall be designed, constructed, and maintained in compliance with the structural requirements of a classification society. Following a collision with another vessel, the HARVEY DISCOVERY sustained damage to the port stern above the waterline: 25 ft bulwarks, 5 bulwark stantions, upper port aft bit and paint. Rectify deficiency to satisfaction of ABS.

Measures shall be taken to the satisfaction of the Administration to ensure equipment is functioning in a reliable manner. Crew unable to prove operation of bilge suction. 74SOLAS(04)II-1/46.2

An alarm system shall be provided for any fault requiring attention. STBD Z-Drive Hydraulic Oil Filter clogged alarm and WT closure low air alarm were showing alarmed and could not be cleared. 74SOLAS(04)II-1/51

Once the non conformities technical deficiencies issued during the interim ISM Audit conducted by ABS on 10 February 2022 are cleared by class, provide the Coast Guard with proof of clearance. 46 CFR 8.130.a.4

The vessel is prohibited from sailing or operating until above deficiency is cleared and an in-service inspection is conducted by the Coast Guard in accordance with CVC Policy. 46 CFR 8

Machinery shall be of a design and construction adequate for service for which intended. Crew unable to parallel generators or perform reverse power relay. Additionally over speed alarm was alarming on panel. MI's unable to verify proper functionality of generators. 74SOLAS(04)II-1/26.1

The surveys referred to in paragraph (a) shall be carried out as follows: the renewal and periodical surveys shall include an inspection of the equipment referred to in paragraph (b)(i) to ensure it complies with the relevant requirements. Safety Equipment Survey notes life rafts for 120 people, but should read 150. Additionally vessel had five 25 man life rafts and one 20 man, but should have six 25 man life rafts. 74SOLAS(14)I-8(b)(ii)

Before the ship leaves port and at all times during the voyage, all life-saving appliances shall be in working order and ready for immediate use; rescue boat was unable to be launched due to winch brake failure. 74 SOLAS (14) III/20.

In no case shall the upper range of the gauge be less than 1.2 times nor more than 2 times the pressure at which the relieving device is set to function; pressure indicating gauges on air receivers do not have an upper range of 1.2 to 2 times the relief device pressure.

Performance of crew members assigned fire-fighting duties shall be periodically evaluated by conducting drills. Multiple deficiencies were noted during the fire drill on the inspection. Vessel shall conduct a successful fire drill to the satisfaction of the Marine Inspector. 74 SOLAS (14) II-2/15.2.2.3

Exposed metal parts of electrical machines or equipment which are not intended to be live b ut which are liable under fault conditions to become live shall be earthed. Vessel ground detection system displayed a ground fault on both Bus A & Bus B. 74 SOLAS (14) II-1/45.1.1

The vessel company shall establish procedures to ensure that the ship is maintained and should ensure that any non-conformity is reported, appropriate action is taken, and records of these activities are maintained. This should be integrated into the ship's operational maintenance plan. Observed three vessel work items that were not reported to the vessel's SMS nor were there replacement parts on order. These included an inoperable reduction gear pressure sensor due to faulty tubing, a faulty hydraulic oil sensor on the stbd azipod, and a damaged gauge for a dry bulk tank. ISM Code Part A/10

All electrical apparatus shall be so constructed and so installed as not to cause injury when handled or touched in the normal manner. Circuit breaker in galley did not have appropriate "blanks" installed. 74 SOLAS (14) II-1/45.1.3

Where penetrations of watertight bulkheads and internal decks are necessary for access, arrangements are to be made to maintain watertight integrity. The stbd E/R escape hatch was not watertight. 74 SOLAS (14) II-1/13-1.1

Watertight doors and hatches in machinery spaces were found not to be watertight. Vessel shall install satisfactory gaskets and make door adjustments to ensure that watertight integrity is maintained in the spaces identified.

Multiple deficiencies noted during initial galley fire drill with crew. Attack team struggled to correctly don fireman's outfits and use SCBAs. Inspectors helped instruct and train crew following initial failed drill. Crew repeated galley fire drill and successfully passed.

Fuel return hoses were found wasted. Replace wasted fuel hoses on the MDE fuel return line.

02 portable extinguisher hoses were found wasted. Replace wasted portable CO2 extinguisher hoses on the deck locker and starboard side aft tank farm extinguishers.

Galley door was not self-closing. Make necessary door adjustments to ensure Class A fire boundaries are maintained for the galley.

The arrangements shall ensure that the watertight doors are watertight and weathertight and hatches remain weathertight in any sea condition. Examined all watertight doors and weathertight hatches, found six (06) watertight doors and three (03) weathertight hatchs that did not seal properly to ensre watertightness or weathertightness as reuiqred. Repair/renew to original condition prior to departure from port. ABS Steel Vessel Rules 3.2.9.9.3 & 3.2.15.9.5; 17c.

Deep-fat cooking equipment installed in enclosed spaces or on open decks shall be fitted with the following: an automatic or manual fire-extinguishing system tested to an international standard acceptable to the organization; a primary and back-up thermostat with an alarm; arrangements for automatically shutting off the electrical power upon activation of the fire-extinguishing system; an alarm indicating operation of the fire-extinguishing system in the galley; and controls for manual operation of the fire-extinguishing system which are clearly labeled for the crew. Vessel had installed a standalone deep-fat fryer in the galley without a fire-extinguishing system. SOLAS (14) II-2/10.6.4.

Provide heat shield to piping exceeding 150 degrees from air compressor to the air receivers, to the satisfaction of the class surveor.

Provide proper operation of the rescue boat to the class surveyor.
